2.Morphometric Study on Mucinous Tumors of the Ovary.

Mucinous tumors of the ovary are the most common tumors arising from the common epithelium of the ovary in Korean. Distinguishing the tumor with borderline malignancy from mucinous cystadenocarcinoma is very important in determining proper therapeutic modalities and prognosis. Authors have undertaken morphometric analysis of various parameters from both borderline lesions and carcinomas of mucinous nature of the ovary. In each, five cases of the borderline and malignant tumors were subjected to be evaluated. Various cytologic and histologic parameters were analyzed using Kontron IBAS-I. 1) The most helpful parameter-for differentiation between borderline and malignant mucinous tumors is cell concentration (sensitivity 80%, specificity 80%). The discrimination value is more than 35 cells per 100 micrometer of the basement membrane length. If the cellular concentration is higher than the discriminating value, that indicates malignancy. 2) Tumor cell height, though it is other parameter of stratification, is not helpful for differentiation of the two lesions. 3) Cytologic atypia, either in size or in form, can not be a criterion distinguishing the borderline from malignancy. 4) Papillary growths can not be a criterion of either borderline and malignant lesions. 5) The degree of irregularity at tumor-stroma interface is not helpful for differentiation between borderline and malignancy.
3.TCA chemical peeling.

Tricholroacetic acid (TCA) chemical peeling is an effective, versatle and safe therapeutic method for patients with pigmentary disorders, acne scars and aging skin. With an experienced and skilled operator, the associated complications are rare and tend to be mild. No. ystemic toxicity is known to be induced by TCA. We observed the effect of TCA chemical peeling, evaluated by 242 patients themselves with melasma, freckles, senile lentgines, pigmented nevi, seborrheic keratosis, skin tags or acne scars. The patients evaluated TCA peeling as excellent for the treatment of seboirheic keratosis, pigmented nevi, skin tags and acne scars, and as selectively recommendable for the treatment of melasma, freckles and senile lentigines.

5.Relationship between Sleep Disturbances and Cognitive Impairments in Older Adults with Depression.

OBJECTIVES: Depression, sleep complaints and cognitive impairments are commonly observed in the elderly. Elderly subjects with depressive symptoms have been found to show both poor cognitive performances and sleep disturbances. However, the relationship between sleep complaints and cognitive dysfunction in elderly depression is not clear. The aim of this study is to identify the association between sleep disturbances and cognitive decline in late-life depression. METHODS: A total of 282 elderly people who underwent nocturnal polysomnography in a sleep laboratory were enrolled in the study. The Korean version of the Neuropsychological Assessment Battery developed by the Consortium to Establish a Registry for Alzheimer's Disease (CERAD-K) was applied to evaluate cognitive function. Depressive symptoms were assessed with the geriatric depression scale (GDS) and subjective sleep quality was measured using the Pittsburg sleep quality index (PSQI). RESULTS: The control group (GDS< or =9) when compared with mild (10< or =GDS< or =16) and severe (17< or =GDS) depression groups, had significantly different scores in the Trail making test part B (TMT-B), Benton visual retention test part A (BVRT-A), and Stroop color and word test (SCWT)(all tests p<0.05). The PSQI score, REM sleep duration, apnea-hypopnea index and oxygen desaturation index were significantly different across the three groups (all indices, p<0.05). A stepwise multiple regression model showed that educational level, age and GDS score were predictive for both TMT-B time (adjusted R2=35.6%, p<0.001) and BVRT-A score (adjusted R2=28.3%, p<0.001). SCWT score was predicted by educational level, age, apnea-hypopnea index (AHI) and GDS score (adjusted R2=20.6%, p<0.001). Poor sleep quality and sleep structure alterations observed in depression did not have any significant effects on cognitive deterioration. CONCLUSION: Older adults with depressive symptoms showed mild sleep alterations and poor cognitive performances. However, we found no association between sleep disturbances (except sleep apnea) and cognitive difficulties in elderly subjects with depressive symptoms. It is possible that the impact of sleep disruptions on cognitive abilities was hindered by the confounding effect of age, education and depressive symptoms.

6.Two Cases of Pseudohypoaldosteronism Type I.

Pseudohypoaldosteronism type 1 is a genetic renal tubular disease of salt wasting, presenting in young infants. Tubular unresponsiveness to elevated endogenous and exogenous aldosterone is the suggested pathogenetic mechanism. Oral sodium chloride supplementation relieve the clinical symptoms and electrolyte distrubances. We experienced 2 cases of PHA type 1 in 38-day and 45-day old male infants who were presented with failure to thrive, vomiting and/or dehydration. Laboratory data showed hyponatremia, hyperkalemia, hypochloremia and metabolic acidosis. Renal and adrenal functions were normal. Plasma renin activity and plasma aldosterone concentration were markedly elevated. Under the diagnosis of pseudohypoaldosteronism type 1, oral supplementation of NaCl and/or kayexalate improved the clinical states of the patients.

7.Three Cases of Secondary Syphilis with Simultaneous Primary Syphilitic Lesions.

We present herein 3 cases of early secondary syphilis with simultaneous primary syphilitic lesions. These cases developed primary syphilitic lesions four to six weeks after sexual contact and lasted six to ten weeks thereafter until the secondary syphilitic lesions appeared. All three patients showed a strong positive for the STS. Spirochetes were seen under dark field examination of the primary syphilitic lesions, Histopathologically, numerous plasma cells around capillaries and endothelial hyperplasia were seen in the dermis.
