Mucocuaneous Lymph Node Syndrome(Kawasaki disease) is a new diseae entity that was first described by Kawasakiin 1967. It occures predominantly in children less than 5yrs old and acute febrile illness, which is mucocutaneousinvolvment associated with swelling of cervical lymph nodes. The coronary artery aneurysms have been revealed 20–30% of patients with Kawsaki disease. The authors report a case of multiple coronary artery aneurysms inKawasaki disease which was diagnosed by a coronary arteriography.

Coronary artery steonosis can be effectively treated by non-operative percutaneous tansluminal coronaryangioplasty(PTCA). We performed PTCA in three patients with coronary artery stenosis, who were referred to thedepartment of radiology, from January to Dec. in 1985. The results were as follows; 1. The locations of coronaryartery stenosis were the proximal anterior descending branch of left coronary artery. 2. The number of stenoticlesions was single without calcium plaque in all three cases. 3. The extent of coronary artery disease is focal,under 2cm in length. 4. PTCA was performed successfully with satisfactory post-dilatation results in all cases. 5.Severe complications such as arterial intimal dissection, acute myocardial infarction, did not occurred. PTCA hasmany advantages over vascular surgery on surgical, economical, and psychological aspects.

BACKGROUND: In the United States, the Centers for Disease Control and Prevention recorded a 20-fold increase in the incidence of vancomycin-resistant enterococci (VRE) associated with nosocomial infections between 1989 and 1991. Although VRE has been reported in Korea since 1992, infections caused by these organisms are still extremely rare in Pusan, Korea. Therefore, a point prevalence culture survey was carried out to investigate the prevalence of intestinal colonization with VRE among patients admitted to Kosin Medical Center, which can predict the appearance of clinical infections with VRE. METHODS: Between July 1997 and August 1997, stool specimens were obtained from 303 patients. Specimens were placed in bile esculin azide broth containing vancomycin (64 microgram/mL) and aatreonam (60 microgram/mL). Cultures were done for 48 hours at 37degrees C, and turbid solutions were subcultured on blood agar. Minimal inhibitory concentrations (MIC) of vancomycin and teicoplanin to Enterococcus isolates were determined by Etest on Mueller-Hinton agar. For amplification of the vanA, vanB, and vanC genes, polymerase chain reactions were performed. RESULTS: VRE isolates were isolated from 6 of the patients (2%). Four of them were identified as E. faecium, and 1 was identified as E. avium, and 1 was identified as Enterococcus spp. All of them were highly resistant to vancomycin (MICs >256 microgram/mL), and they were also resistant to teicoplanin (MICs 32-->256 microgram/mL). All of 6 VRE strains carried vanA gene. CONCLUSION: The colonization of VRE was not infrequent among the patients of a university hospital in Pusan, Korea. Moreover, a large proportion of the colonizing VRE was revealed Enterococcus faecium with vanA gene, which implies quite a possibility of a sudden rising of infections by this organism in the near future. So we propose that the vancomycin susceptibility test should be done for every enterococcal isolate from clinical specimens and the intestinal colonization rate of VRE should be closely monitored at regular intervals for the purpose of surveillance 50 that proper establishment of plans for the prevention of this troublesome pathogen's spread can be promptly made.

Recently, the efficacy of immuno-oncologic agents for advanced hepatocellular carcinoma (HCC) has been proven in several trials. In particular, atezolizumab with bevacizumab (AteBeva), as a first-line therapy for advanced HCC, has shown tremendous advances in the IMBrave150 study. However, second or third-line therapy after treatment failure with AteBeva has not been firmly established. Moreover, clinicians have continued their attempts at multidisciplinary treatment that includes other systemic therapy and radiotherapy (RT). Here, we report a case that showed a near complete response (CR) of lung metastasis to nivolumab with ipilimumab therapy after achieving a near CR of intrahepatic tumor using sorafenib and RT in a patient with advanced HCC who had experienced treatment failure of AteBeva.

The purpose of this study was to investigate the effects of investing conditions on the marginal discrepancy of conical telescope outer crown with Co-Cr-Ti alloy(Dentitan) and to compare the marginal discrepancy and the retention force of outer crowns using different pattern materials (plastic foil, casting wax, pattern resin). To evaluate the effects of investing conditions on the marginal discrepancy, patterns with plastic foil were invested under three different liquid/powder ratio conditions using phosphate bonded investment (Univest-nonprecious): standard, 10% decreased and 10% increased. At each liquid/powder ratio condition, metal ring was lined with single or double layers of ceramic ring liner. The marginal discrepancy of outer crown at different investing conditions was measured by x 100 compact measuring microscope(STM5, Olympus, Japan). For measurement of the marginal discrepancy and the retention force of outer crown using different pattern materials, the investing condition of 10% decreased liquid/powder ratio and double layers of ring liner was selected because this investing condition resulted in the best fit of outer crown. Marginal discrepancy was measured in the same way above and retention force on universal testing machine. Under the conditions of this study, the following conclusions were drawn: 1. The thickness of ring liner had more influence on the marginal discrepancy of outer crown than the liquid/powder ratio, and the acceptable marginal fitness could not be expected at the investing condition directed by investment manufacturer. 2. There were no differences in the marginal discrepancy of outer crown among three different pattern materials 00.05). 3. Casting wax showed the greatest retention force(1640g) of outer crown, followed by pattern resin(1110g), plastic foil(820g). However, there was no significant difference between plastic foil and pattern resin(p>0.05). 4. Plastic foil showed the least variation in marginal discrepancy and retention force.

PURPOSE: The purpose of this study was to investigate the effect of size and shape of retention element on the bond strength of indirect composite resin and metal. MATERIAL AND METHOD: The metal disk specimens, each 6mm in diameter, were cast from Cr-Co alloy. They were divided into 8 groups by applied retention element: retention bead group B2 (phi 0.2mm), B4 (phi 0.4mm), B6 (phi 0.6mm), B8 (phi 0.8mm), retention crystal group C2 (0.2mm), C5 (0.5mm), C8 (0.8mm), and sandblasting group SB (110micrometer Al2O3 blasting) as control. Eighty-eight metal specimens were veneered with TESCERA(R) Indirect resin system. One specimen of each group was sectioned and the resin-metal bonding pattern at the interface was observed under measuring microscope. Other specimens were then tested for tensile bond strength on an Instron universal testing machine at a crosshead speed of 2mm/min. RESULTS: 1. Compared to sandblasting, beads or crystals increased the resin-metal bond strength (P<.05). 2. 0.2mm retention crystals were most effective in improving the resin-metal bond strength (P<.05). 3. 0.2mm beads showed the highest bond strength among retention bead groups, but there was no statistically significant difference (P>.05). 4. Retention crystals tend to be higher in bond strength than retention beads due to wider surface area. 5. The larger retention element, the larger the undercut for the mechanical retention, but the gap at resin-metal interface was also increased. CONCLUSION: Within the limitations of this study, 0.2mm retention crystals were most effective in improving the resin-metal bond strength.
