1.A Case of Spontaneous Perforation of the Right Intrahepatic Duct - An ERCP Diagnosis.

Spontaneous perforation of bile duct in adults is very rare, with less than 30 cases described in the literature to date. We repoit a case of a 65-year-old man who presented with severe colicky abdominal pain and fever, just like symptoms of peritonitis. ERCP provided a preoperative noninvasive confirmation of the diagnosis of the rupture of right intrahepatic duct. The patient was performed T-tube choledochotomy and drainage of retroperitoneal bile collection. The etiolgy, diagnosis, and treatment of spontaneous perforation of bile duct is discussed.

4.Plasma Carotenoid Levels in Healthy men and Acute Cardiovascular Disease Patients in Taegu.

Plasma carotenoid levels were compared among 64 healthy male subjects (control) and 38 patients of ischemic heart disease(IHD) and 20 ones of cerebral infarction(CI) all of whom were over 50years of age. Another 98 healthy male subjects aged 23 to 58 were selected to compare their plasma carotenoid levels by age groups, Levels of lutein, zeaxanthin and crpytoxanthin were lower in IHD(34+/-2, 13+/-1 and 62+/-7 microgram/dl)and CI(36+/-3, 12+/-2 and 41+/-6 microgram/dl)patient groups than in control group (84+/-5, 16+/-2 and 69+/-3 microgram/dl) while those of lycopene, alpha-and beta-carotene varied little among the three groups. The sum of the six carotenoid levels were levels were, therefore,highest(205+/-14 microgram/dl) in the control group followed by IHD(155+/-15 microgram/dl) and CI(128+/-17 microgram/dl) patient groups, Among the 98 healthy male subject for the age group study, levels of the three major carotenoids increased with age from the twenties to the fifities ; lutein, from 64+/-6 to 89+/-8 microgram/dl, cryptoxanthin, 57+/-8 to 73+/-4 microgram/dl and beta-carotene were more significantly correlated(r=0.30 to 0.61, p<0.01), whereas levels of lycopene and alpha-caroteme were significantly(r=0.21 - 0.23, p<0.05) correlated.

5.The Expression of CD10 and CD15 Is Progressively Increased during Colorectal Cancer Development.

BACKGROUND: The aim of this study was to examine the expression of CD10 and CD15 in tumor cells, stromal cells and infiltrating inflammatory cells during colorectal carcinoma (CRC) development and to investigate their expression levels between the tumor center and invasive front and compare them to clinicopathological parameters in invasive CRC. METHODS: We performed immunohistochemical staining for CD10, CD15, and E-cadherin in 42 cases of CRC, 49 of tubular adenoma, 15 of hyperplastic polyp, and 17 of non-neoplastic colon. RESULTS: CD10 was expressed in tumor cells (tCD10), stromal cells (sCD10) and infiltrating inflammatory cells (iCD10), and CD15 was expressed in tumor cells (tCD15) and infiltrating inflammatory cells (iCD15). Their expressions were progressively increased during CRC development and the iCD10 expression level was significantly correlated with the iCD15 expression level in invasive CRC. Invasive front revealed a higher expression level of iCD10 and iCD15 than the tumor center. Moreover, the iCD15 expression level of invasive front was significantly correlated with the degree of tumor budding and tCD15 in whole tissue sections was closely associated with tumor depth. CONCLUSIONS: The present study suggests that the expression of CD10 and CD15 is associated with the development and progression of CRC.

6.Ultrasonographic Findings of the Normal Nerves in Common Entrapment Site; Cross-Sectional Area Reference Value and Normal Variant.

BACKGROUND: Neuromuscular ultrasound has emerged over the last decade as a useful tool for diagnosing peripheral neuropathy. Because nerve enlargement is the most important diagnostic marker of an abnormal nerve, quantification of nerve size is essential. METHODS: We included 80 healthy volunteers aged 21-60 years. The nerve cross-sectional area (CSA) was measured in all participants at the following common compressive sites: bilateral median nerve at the carpal tunnel and forearm, ulnar nerve at the wrist and medial epicondyle, radial nerve at the spiral groove, peroneal nerve at the fibular head and popliteal fossa, and tibial nerve at the popliteal fossa. Anatomical variants were also evaluated, including a bifid median nerve and persistent median artery. RESULTS: The CSAs were 9.58+/-1.55, 6.87+/-1.61, 4.72+/-0.91, 6.64+/-1.33, 6.48+/-1.68, 12.35+/-3.55, and 26.98+/-6.92 mm2 (mean+/-SD) for the median nerve at the carpal tunnel and forearm, ulnar nerve at the wrist and medial epicondyle, radial nerve at the spiral groove, peroneal nerve at the fibular head, and tibial nerve at the popliteal fossa, respectively. The nerve CSA was significantly larger in men than in women and was correlated with body mass index, weight, and height. A bifid median nerve and persistent median artery were seen in 12 (7.5%) and 4 (2.5%) of 160 hands, respectively. CONCLUSIONS: The information produced in this study can serve as reference data when evaluating these nerve sites using ultrasound.

PURPOSE: The purpose of this study is to examine the relationship among body image, depression and sexual function in Korean postmenopausal women. METHODS: Subjects were 96 postmenopausal women who have lived in Korea. Data was collected using Semantic Differential scale, CES-D, and FSFI. RESULTS: The level of body image was positive, depression was mild, and sexual function was moderate. There were no significant correlation between depression and sexual function. The subjects who had more positive body image experienced higher sexual function and less depressed mood. CONCLUSION: These findings showed the need for a knowledge development program for nurses regarding women's sexual function. Also, nurses must do counseling with sexual partner's and consider patients' body image when counseling those who complain of sexual dysfunction

Heterotopic pregnancy, simultaneous intrauterine and extrauterine pregnancy, is an extremely rare case. The incidence of heterotopic pregnancy was about 1 to 30,000 pregnancies, but it has increased after wider use of assisted reproductive technology. The rising incidence presents a serious problem as the diagnosis of this potentially fatal condition is often missed. Careful pelvic examination combined with serial beta-hCG determinations, and transvaginal sonography to evaluate the adnexal region are necessary prerequisites for early diagnosis. We report a case of heterotopic pregnancy following in vitro fertilization and embryo transfer(IVF-ET) with a brief review of literature.

10.Subacute Combined Degeneration with Motor Neuropathy Following Nitrous Oxide Abuse

The neurotoxicity of nitrous oxide (N2O) is known to be mainly occurred by interference with action of vitamin B12. A 38-year-old male presented with progressive gait instability after recreational inhalational use of N2O for 2 months. Although spine magnetic resonance image showed typical subacute combined degeneration, nerve conduction study showed motor neuropathy that is atypical findings of vitamin B12 deficiency. This case suggests N2O-induced neuropathy can develop separately from a vitamin B12 deficiency.
