1.Endoscopy-assisted Percutaneous Carpal Tunnel Release through Single Small Incision.

PURPOSE: To avoid prolonged recovery time and scar tenderness in open carpal tunnel release and possible nerve or tendon injuries by bulky instruments in endoscopic techniques, we tried to percutaneously release the transverse carpal ligament using simple, thin instruments. MATERIALS AND METHODS: We performed carpal tunnel release percutaneously through a single small incision on the wrist with curved mosquito and meniscectomy hook knife under general or regional anesthesia in 41 wrists of 27 patients during the last 5 years and followed up for at least one year. Before and after release, endoscopic examinations were done to rule out space occupying lesions or other anormalies and to confirm complete release. RESULTS: According to the Cseuz's criteria, 38 cases (93%) showed excellent or good results, which was compatible to open or endoscopic technique with special instruments and there was no major neurovascular injury except one superficial palmar arch laceration associated with the second palmar incision in complicated case. CONCLUSIONS: From earlier experience, we learned that this simple technique may be used successfully with low cost without any fear of complication.

4.Minimal Change Disease Associated with Gastric Adenocarcinoma and Multiple Liver Metastasis.

A 35-years-old man was hospitalized with gene-ralized edema. 6 months ago, the patient was operated by radical subtotal gastrectomy for advanced gastric adenocarcinoma. The patient presented with multiple liver metastasis and the massive proteinuria. The patients renal biopsy revealed minimal change nephrotic syndrome and treated with prednisolone and diuretics. The patient was improved clinical symptome and decreased 24 hours urine protein. In literature reviewed, gastric carcinoma was not associated with minimal change nephrotic syndrome. We experienced advanced gastric carcinoma associated with minimal change nephrotic syndrome, thus we report it.

5.Metastatic carcinoma of the thyroid to the skull: report of 2 cases

Two cases of skull metastasis from thyroid carcinoma are presented. The one case shows multiple involvement inthe skull base with sphenoid sinus, frontal bone, and rib. The other case is solitary extensive involvement to theright parietal bone. The follicular caroinoma of the thyroid shows a strong tendency toward vascualr invasion and dissemination through blood stream to the lungs, bones, liver, brain, and other distant sites. The sternum, thevetebrae, and the pelvis are the most common sites in bone metastasis of thyroid carcinoma, followed by the femur,shoulder girdle, skull, and rib. Metastatic caroinoma to the skull base, including sphenoid sinus s extremelyrare. Authors experienced recently 2 cases of metastatic carcinoma of the thyroid to the skull and report withreview of literature.

6.Intra-abdominal Angiosarcomatosis Induced by Irradiation.

Therapeutic irradiation can induce angiosarcoma. Radiation-induced angiosarcoma constitutes 20% of all angiosarcomas. Although its common site of origin is the skin and subcutaneous tissue, it rarely arises in small or large bowels with a presentation as multifocal abdominal angiosarcomatosis. We report a case of intra-abdominal angiosarcomatosis involving the jejunum, ileum, transverse colon, mesentery and right ovary in a 63-year-old female. It developed 10 years after therapeutic irradiation for squamous cell carcinoma of uterine cervix. She developed panperitonitis due to intestinal perforation. She died from sepsis 3 days after segmental resection of the small bowel and right oophorectomy. We reviewed the previously reported cases and describe the clinicopathologic features of this tumor.

7.A Clinical Study of Pelviscopic Surgery 219 Cases.

From January 1995 to December 1997, a total of 219 cases pelviscopic surgery was performed at Il Sin Christian Hospital. To evaluate the safty and efficacy of pelviscopic surgery, we reviewed indication of operation, mean age, parity, history of abdominal operation, type of operation, duration of hospitalization and complication retrospectively. The number of pelviscopic surgery have risen from 33 cases in 1995 to 53 cases in 1996 and 133 cases in 1997. The most common indications were 118 cases for ectopic pregnancy (53.9%) and 42 cases for ovarian cyst (19.2%), 20 cases for infertility (9.1%) in order. Among the ovarian cyst, endometrioma was the most common. The mean age of patients was 31+/-6.7 years old and mean parity was 0.77. In most cases, mean blood loss was lesser than 100 cc except 500 cc in LAVH, 150 cc in CISH, 106.7 cc in ectopic mass removal. The postoperative hospital stay was varied from 0 day to 10 days, but usually 2 days. The 26 cases had complicated and the most common complication was fever above 38degrees C. In conclusion, it is evident that pelviscopic surgery is lesser invasive technique, has lower complication rate and shorter the length of hospitalization. So, we expect the number and indication of pelviscopic surgery will be increased in future.

8.Assessment of Factors Affecting Plate Waste and Its Effects in Normal & Soft Diets Provided from Hospital Foodservice.

The purposes of this study were to : (a) analyze the portion size & plate waste of normal & soft diets provided by dietetic departments in hospital, (b) investigate the factors affecting plate waste, and (c) determine the nutritional & monetary value of plate waste. A questionnaire for food preference and sensory evaluation was developed and a survey was carried out for the 46 normal diet and 56 soft diet patients in Sanggye Paik hospital in Seoul. Serving size and plate waste were weighed by the electric scale, and CAN-Pro program was used to evaluate the nutrition value. The data were analyzed using the SAS package program for descriptive analysis, t-test, ANOVA, and Pearson correlation. The average plate waste rate for normal diet and soft diet were 30.3% and 49.6%. More plate waste amount occurred on female patients' diet than male patients' diets regardless of the kind of diet. The plate waste percentage of male patients were higher than those of female on normal diet. On soft diet, patients who hospitalized in surgery or pediatrics department were higher than in other wards. Plate waste of normal diet and amount were correlated positively, but wastage and preference were correlated negatively Negative correlation was between taste and plate waste, and between temperature and plate waste, too. On the other hand, plate waste caused the deficiency of some nutrients such as Ca, Fe, Vit. B2 and a great monetary loss.

9.A Clinical Observation on the Surgical Abdmen in Children.

The clinical observation was made in 220 children who was admitted to department of general surgery of St. Paul's Hospital from 1972 to 1977 and the following results were obtained. 1. The age incidence was the most common in 10~15 years age group (40.0%). 2. Distribution of the operated cases in order of frequency was acute appendicitis(44.6%), inguinal hernia(23.2%) and intussusception)11.4%). 3. 60cases(26.3%) had operation due to intestinal obstruction. The common causes of intestinal obstruction was intussusception(41.7%), congenital anomalies(36.7%) and incarcerated hernia(11.7%). 4. The highist incidence of the cases according to the age were as follows: newborn infant; intestinal obstruction due to congenital origin(all cases), 4wks-1year; intussusception(41.5%), 1-2years and 2-6years; inguinal hernia(75.0% and 44.7%), 6-10years and 10-15years; acute appendicitis(40.6% and 86.4%). 5. 7 cases were injured abdominal visera by trauma. Among them, spleen and liver was injured in 3cases, respectively, and ileum in 1case. 6. Overall mortality rate was 0.9%.

10.A Clinical Observation on Hydrocele.

A clinical observation was made on 43 patients of hydrocele who were admitted to the Department of Urology, Chonnam University Medical School during the 6 years period from January, 1976 to December, 1981. The results were as follows. 1. The incidence of hydrocele in 43 patients was 2.4% of 1,809 inpatients and 21.9% of diseases of scrotal contents in 196 patients. 2. The most common population was 0-9 years of age giving a rate of 39.4% (17 patients). 3. There were 28 patients in the right and 12 patients in the left with a ratio of 2.3:1. 4. Combined diseases of external genitalia were observed in 22 cases and the most common one was inguinal hernia in 13 cases. 5. There were 20 cases of hydrocele communicated with the peritoneal cavity, of which inguinal hernia was observed in 13 cases. 6. Treatment consisted of hernioplasty in 19 cases (41.3%), Winkelmann's technique in 16 cases (34.8%)and Von Bergman's method in 8 cases (17.4%).
