4.A case of neuroblastoma with multiple skin metastases.

We report a case of neuroblastoma with multiple skin metastases as a chief complaint in a 2-month-old girl. the skin lesions were rnultiple, pea-sized, bluish, nontender, moable subcutaneous nodules on abdomen, back and scalp. Histopathology showed small round or poly gonal tumor cells which have deeply stained, basophilic, hyperchromatic nuclei with some mitoses. Th.se tumor cells showed clumping tendency which is one of early menifestations of rosette formation. Immunohistochemically positive reaction was demonstrated by anti-NSE(neuron specific enolase) antilody but negative reaction by anti-NFP (neurofilament proteiin ) antibody. She has been succesfully treated with combined chemotherapy for 10 months without relapse.

5.Changes of Oxygen Saturation Depend on the Tracheal Extubation Methods in Pediatric Patients.

This study was performed to observe the changes of oxygen saturation after tracheal extubation, which depends on the following tracheal extubation methods of the group 1. 2A. 2B, has done. One hundred twenty-five healthy patients that had been selected out of 1~15 year-old at random were divided as follows; Group 1; extubation performed in being awake (n=49) Group 2; anesthetized extubation (n=76) 2A: extubation done in 5 min after discontinuing N2O (n=38) 2B: extubation under anesthesia (GOE or GOF) (n=38) The grouping, which has at random been assigned to the patients, was done just before the end of operation. Oxygen saturation was measured continuously by pulse-oximeter (Minolta Pulsox TS-7, Japan) and was recorded in the process of operation, immediately after extubation and at 1.2.4.6.8.10.20.30 min after extubation while they were spontaneously breathng room air. In case oxygen saturation were less than 90%, supplementary oxygen was administered to them. The frequency of hypoxemia was higher in Group 2 (19.7%) than in Group 1 (10.2%). In group 2B, 2 patients developed severe hypoxemia and 1 patient developed PVC immediately after extubation. Changes of oxygen saturation were as follows; Oxygen saturation in Group 2B was higher than that in Group 1 at each 2 min and 4 min and was also higher than that in Group 2A at 4 min after extubation. As a result, Group 1 is the safest extubation method because of its low risk of hypoxemia. If anestltized extubation must be needed, Group 2A would be preferable to Group 2B method because the frequency of hypoxemia was higher in Group 2B than in Group 2A.In addition, It is suggested that monitoring oxygen saturation continuously by the patients should be safe.

7.Studies on the current epidemiological situation of brugian filariasis in endemic areas of Korea.

An epidemiological study on brugian filariasis was carried out in endemic areas including Cheju Island in Korea, with a brief review of literatures.The results showed that the incidence among residents has remarkably decreased in Cheju Island, which was the main endemic area. Reviewing available informations on the prevalence of filariasis reported in recent years and also judging from the present socio-economic conditions which enable people to practice personal protection against mosquitos, it can be said with confidence that filariasis has almost disappeared from Cheju Island and inland areas. The disease is considered to remain at a low level of endemicity in Hugsan Islands. Certainly mass diethylcarbamazine (DEC) treatment carried out in Cheju Island in the 1960s and 1970s and remarkable economic growth followed by improved living standard and altered life-style of inhabitants could all have combined effects on the disappearance of this mosquito-borne diseae in this island. If the present trends go on, the possibility of resurgence of filariasis in Cheju Island is hardly postulated.

9.A Case of Infantile Polycystic Kidney.

The infantile polycystic kidney disease is rare fetal urinary tract anomaly. It is inherited with an autosomal recessive pattern and recurrence rate is 25%. The gene locus is on chromosome 6p. The pathogenesis of infantile polycystic kidney is the primary defect of the collecting ducts. The ultrasonographic finding of infantile polycystic kidney is oligohydramnios, bilaterally symmetrical enlarged kidneys with maintenance of their reinform shape. The differential diagnosis with adult polycystic kidney disease is important. The massive enlargement of the kidneys is rarely seen in adult polycystic kidney disease and the examination of the parents and other members of the family is helpful to confirm the adult polycystic kidney disease. If there is severe renal involvements, stillbirth or neonatal death secondary to pulmonary hypoplasia would be developed. If it were diagnosed before viability, termination of pregnancy is recommended. In a fetus at risk, diagnosed after viability, pregnancy termination is also recommended since this condition is uniformly fatal. We present a case of infantile polycystic kidney.
