2.AgNOR Counts in S-phase Human Cells.

The nucleolus of human cell is a morphologically well recognizable nuclear organelle and the argyrophilic NORs (AgNORs) are nucleic acid-argyrophilic nonhistone protein complex in the nucleoli and the silver staining allows their identification and enumeration at the light microscopic level. The AgNOR counts are in parallel with mitotic activity and vary in different phase of cell cycle. It has been reported that human cells have one AgNOR during interphase and S-phase. However, the correlation between the number of AgNORs and S-phase markers is still controversial and they have never been studied simultaneously. In this study, AgNOR and PCNA were stained simultaneously to find out the relationship of AgNOR counts with cell cycle (S-phase) in human palatine tonsil, gastric carcinoma, liver and brain tissues. S-phase cells (PCNA-positive) were found predominantly in lymphoid follicles in palatine tonsil but gastric carcinoma showed diffuse immunoreactivity for PCNA. The AgNOR counts varied according to the type and locus of tissue. More than one AgNOR were identified in S-phase cells and some of hepatocytes and neurons in the brain which were not in S-phase contained two or more AgNORs. The above results suggest that the number of AgNOR is a characteristic feature of each type of cells and can be more than one even in S-phase.

3.A Clinical Study of 12 Cases of Sarcoma of the Uterus.

OBJECTIVE: Sarcoma of uterus is originated from uterine muscles and/or connective tissues and rare and most lethal of all primary uterine tumors. The purpose of this study was to evaluate the clinical outcome of 12 patients diagnosed primary uterine sarcoma at the Department of Obstetrics and Gynecology in Kyungpook National University Hospital between 1984 and 1998 for 15 years. METHODS: After review of chart in twelve patients, data including clinical and histologic findings, treatment and outcome were analysed. RESULTS: The most common histologic finding was leiomyosarcoma(6cases, 50%) and the age of patients ranged 24 to 69 years and the average age was 43.25 years and two patients were nulliparous. The most common sign was abnormal uterine or vaginal bleeding(48%), and pelvic cavity or vaginal mass(16%), pelvic pain(16%), no symptom(16%), in order of frequency. The distribution by FIGO clinical stage was four cases(33%) for stage I, one case (8%) for stageII, one case(8%) for stage III, six cases (50%) for stage N. The nine cases received hysterectomy with or without BSO. Two cases received myomectomy to preserve fertility. The mean follow-up duration was 39.6 months. CONCLUSION: Uterine sarcoma is an uncommon and aggressive tumors with a poor prognosis. Leiomyosarcoma is the most frequent histologic type(50%). Stage I uterine sarcoma with or without adjuvant chemotherapy has better prognosis than other stages.

4.Association of Hypertension with Cluster of Obesity, Abnormal glucose and Dyslipidemia in Korean Urban Population.

To examine the association of hypertension with cluster of obesity, abnormal glucose and dyslipidemia in Korean urban population, we conducted this cross-sectional study among 3027 men and 2127 women age 20-85 years who visited a prevention center between May 1991 and June 1995 for a multiphasic health check at St. Mary's Hospital, Seoul. By the self-administered questionnaire, the informations of educational attainments, monthly income, alcohol consumption, cigarette smoking, and physical excercise level were obtained. Height, weight, and blood pressure were measured by a trained nurse. The fasting blood sugar(FBS), total cholesterol, high density lipoprotein (HDL) cholesterol and triglyceride were tested by enzyme method. Low density lipoprotein (LDL) cholesterol was calculated by 'total cholesterol - HDL cholesterol - triglyceride/5'. For testing the differences of cardiovascular risk factors between hypertension and normotension group, t-test and x2 test were performed and for the age adjusted odds ratios of hypertension in persons with obesity, abnormal glucose, and dyslipidemia compared with normal, logistic regression was performed by using SAS pakage programme. The results obtained were as follows: 1. Age, weight, body mass index, blood glucose, total cholesterol, low density lipoprotein, triglyceride of hypertension group in men and women were significantly higher than normotension group, but height and high density lipoprotein of hypertension group only in women significantly lower than normotension group. The frequency of obesity (body mass index > or =25 kg/m2), abnormal glucose (> or = 120 mg/dl), hypercholesterolemia (> or =240 mg/dl), lower HDL cholesterol (<45 mg/dl in women only), higher LDL cholesterol (> or =160mg/dl), and hypertriglyceridemia (> or =250 mg/dl) in hypertension group of men and women were significantly higher than normotension group. 2. Systolic and diastolic blood pressure were negatively correlated with hight, but positively with age, weight, BMI, total cholesterol, LDL cholesterol, and triglyceride in men and women. And BMI was positively correlated with fasting blood sugar, total cholesterol, LDL cholesterol and triglyceride but negatively with HDL cholesterol. 3. The odds ratios of hypertension were as follows in men and women : among persons who were obese compared with those nonobese, 2.53 (95% Confedence Intervals [C.I.] 2.08-3.07) and 2.22 (95%C.I. 1.71-2.87); among persons who were abnormal glucose compared with those normoglycemic, 1.43 (95%C.I 1.13-1.82) and 2.01 (95%C.I 1.36-2.94); and among persons who were dyslipidemia (hypercholesterolemia or lower HDL cholesterol or higher LDL cholesterol or hypertriglyceridemia) compared with those normal lipid, 1.59 (95%C.I 1.30-1.95) and 1.51 (95%C.I 1.16-1.96). After combined more than one risk factor, the odds ratios were increased. Among persons with cluster of obesity, abnormal glucose, and dyslipidemia, the odds ratio of hypertension was 2.25 (95%C.I 1.47-3.37) in men and 3.02 (95%C.I 1.71-5.30) in women. In conclusion, it was suggested that hypertension was associated with cluster of obesity, abnormal glucose, dyslipidemia in this Korean urban population.

5.Surgical Treatment for 3 Cases with Established Volkmann's Ischemic Contracture of the Upper Limb

Volkmanns ischemic contracture, despite a relatively small incidence, is one of the most devastating complications following a limb injury. The orthopedic surgeon must be equipped to recognize and effectively treat this complication. Recently the authors have managed 3 cases with V.I.C. with the following results. 1) The etiology showed CO poisoning, arterial injury, wringer injury around the elbow in each case. 2) The method of surgical treatment depended upon deformities and functional loss of individual patient. 3) The end results seemed to be proportionate to the previous deformities and functional less. 4) Early postoperative passive exercises of the digits seemed to be helpful in restoring further range of motion.

7.Ultrastructural Findings of Bowenoid Papulosis of the Genitalia.

A 41 year-old man had dark pigmented papules and plaques, 4 to 15 mm in size, on the proximal portion of the dorsum of the penile shaft and pubic area of 8 months duration. Over next several months, flesh colored, macerated papules, 2 to 4 mm in size, appeared on the left side of the scrotum Light microscopic examination from a pigmented lesion of the penile shaft showed typical findings of Bowen's disease. Ultrastructural findings included: widening of intercellular space, a decrease in the number of desmosomes with microvillous projections, aggregation of tonofilaments, an increase in the mitotic keratinocytes, an increase in the number of melanin granules, and disorganized bundles of tonofilaments surrounding chromosomes. Virus-like particles were not observed. Based on the ultrastructural findings and review of literatures, it seems to be probable that bowenoid papulosis is a clinical variant of Bowens disease.

Two cases of neurofibrossrcoma involving cutaneous tissues of the patients with multiple neurofibromatosis were presented. One case, 49-year-old man, showed highly malignant histopathological feature with lethal outcome and the other case, 56-year-old man, showed less malignant histopathology with good recovery from local excision.

9.Tracheobronchial Aspergillosis An autopsy case report.

Tracheobronchial aspergillosis is an unusual form of invasive aspergillosis characterized by noninvasive or only superficially invasive tracheobronchitis with a propensity for dissemination. We report a two-year-old male who suddenly died of respiratory failure. Postmortem examination revealed a pseudomembrane covering the mucosa of larynx, trachea and bronchial tree of both lungs. This pseudomembrane was composed predominantly of Aspergillus hyphae. There was transmural necrotizing bronchitis with fungal invasion to the narrow zone of peribronchial tissue, and dissemination to the stomach and kidney. This form of pulmonary aspergillosis had not been reported in this country.

10.A Case of Chronic Persistent Photosensitivity.

Persistent light reaction, actinic reticuloid, photosensitive eczema are three chronic photodermatoses with a feature of clinical, histological, and photobiologieal characteristics in common. These conditions were given the name of chronic persistent photosensitivity. Chronic persistent photosensitivity with the feature of photosensitive eczema developed in a 68-year-old male patient who had experienced persisited dermatitis. On phototesting the patient showed extremely severe photosensitivity to the short wave UVB. A skin biopsy showed microscopic finding of eczematous dermatitis.
