4.Comparison of Diagnostic Accuracy in Uterine Pathology among HSG, Hysteroscopy, and Sono-Hysterography.

Hysterosalpingography and Hysteroscopy have been used for the detection of intraute-rine pathology such as polyps, submucous myomas, intrauterine adhesion and endometrial hyperplasia or cancer. Recently the ultrasound has also been utilized for the detection of uterine pathology. Therefore the purpose of this study was to evaluate the diagnostic efficacy of SonoHysterography in detection of intrauterine pathology compared with HSG and Hysteroscopy. 32 patients underwent Sono-Hysterography and Hysteroscopy for the evaluation of the uterine pathology from september 1995 to January 1996. Nine of 32 patients had infertility problem and HSG performed prior to Sono-Hysterography and Hysteroscopy. The results are as follows : 1. The patients' ages ranged from 20 to 50 years(median 37.9). 2. All 9 patients with infertility who had positive HSG findings in uterine cavity showed the intrauterine pathology in Sono-Hysterography as well as Hysteroscopy. The detail findings are as follows :septated uterus(n=2), intrauterine adhesion :IUA(n=3), endometrial polyp(n=3), and IUA combined endmetrial hyperplasia(n=1). 3. Twenty two of 23 patients with abnormal uterine bleeding showed the intrauterne pathology and one patient had negative finding in Sono-Hysterography. However, Hysteroscopy revealed positive intrauterine pathology in 22 patients who had abnormal uterine bleeding. One patients who had positive finding in Sono-Hysterograply showed negative by Hysterography. In contrast, one patient who had negative finding in Sono-Hysterography had positive uterine pathology with polyp in Hysteroscopy. The histologic pathology in all 23 patients reported endometrial polyp(n=12), placental polyp(n=2), submucous myoma(n=1), endometrial hyperplasia(n=5), endometrial cancer(n=1), normal endometrial finding(n=2). 4. Sono-Hysterography, therefore, has a sensitivity and positive predictive value of 96.6%, 93.5% respectively. Our study showed a positive Sono-Hysterography is very predictive of the intrauterine pathology. Sono-Hysterography is safe, quick and minimal invasive procedure. So it is an invaluable technique in the evaluation of uterine cavity.

5.Low-Level Light Therapy with 410 nm Light Emitting Diode Suppresses Collagen Synthesis in Human Keloid Fibroblasts: An In Vitro Study.

BACKGROUND: Keloids are characterized by excessive collagen deposition in the dermis, in which transforming growth factor β (TGF-β)/Smad signaling plays an important role. Low-level light therapy (LLLT) is reported as effective in preventing keloids in clinical reports, recently. To date, studies investigating the effect of LLLT on keloid fibroblasts are extremely rare. OBJECTIVE: We investigated the effect of LLLT with blue (410 nm), red (630 nm), and infrared (830 nm) light on the collagen synthesis in keloid fibroblasts. METHODS: Keloid fibroblasts were isolated from keloid-revision surgery samples and irradiated using 410-, 630-, 830-nm light emitting diode twice, with a 24-hour interval at 10 J/cm². After irradiation, cells were incubated for 24 and 48 hours and real-time quantitative reverse transcription polymerase chain reaction was performed. Western blot analysis was also performed in 48 hours after last irradiation. The genes and proteins of collagen type I, TGF-β1, Smad3, and Smad7 were analyzed. RESULTS: We observed no statistically significant change in the viability of keloid fibroblasts after irradiation. Collagen type I was the only gene whose expression significantly decreased after irradiation at 410 nm when compared to the non-irradiated control. Western blot analysis showed that LLLT at 410 nm lowered the protein levels of collagen type I compared to the control. CONCLUSION: LLLT at 410 nm decreased the expression of collagen type I in keloid fibroblasts and might be effective in preventing keloid formation in their initial stage.

6.DIAGNOSTIC METHOD OF UROLITHIASIS PATIENTS IN EMERGENCY DEPARTMENT.

Nephrolithiasis is one of the most common disease seen in the Emergency Department. Among the patients who visited in the Emergency Department with acute abdominal pain, many people were suspected urolithiasis. Because of limitation of diagnostic method on Emergency Department we planned this study in order to confirm the our accuracy of diagnosis and the possibility of misdiagnosis. During 5 months from April 1996 to August 1996, the 189 patients was suspected the urolithiasis. Among this patient, 51 cases was excluded due to incomplete study. After performing P/ex, U/A KUB in the Emergency Department, they were followed out patients department of Urology and performed Intravenous Pyelogram. The sensitivity and specificity for KUB were 84.5% and 94.0%, respectively. The positive predictive value was 93.8%, the negative predictive value was 93.8%. Diagnostic accuracy of urolithiasis in our Emergency Department is 89.1%. There was no relationship between urinary occult blood and KUB or IVP. There was no relationship between degree of occult blood and diagnosis of urolithiasis. For differential diagnosis of urolithisis, emergency ultrasound and IVP will be available. So combination of this diagnostic methods will improve accuracy and safety of diagnosis and treatment of urolithiasis.

7.The relevance of subjective health recognition and happiness index of local community residents (Using community health survey data 2014).

OBJECTIVES: Health is closely related to an individual's assessment of their quality of life and their subjective level of happiness. The aim of this study was to investigate the association between happiness index and self-rated health by utilizing a community health survey in Korea. METHODS: The study participants were 17,937 people living in Gyeongsangnam-do, Korea, who completed a community health survey in 2014. The non-subjective variables included in the statistical analysis were sex, age, region, education, income, and economic activity. The subjective awareness variables included self-rated health, self-rated oral health, chewing difficulty, pronunciation difficulty, and happiness index. Statistical analysis of the data was done using the SPSS 21.0 program. RESULTS: Evaluation of the subjective awareness levels revealed that the respondents that were better educated, whose income levels were higher, and those who engaged in an economic activity found themselves to be in better physical health and oral health. In contrast, the respondents that were female, older, and with residence in the counties reported a higher incidence of chewing difficulty. There was a positive correlation between happiness index and subjective awareness level, and a negative correlation between chewing difficulty and pronunciation difficulty. Happiness index was influenced the most by subjective health levels, followed by income, age, pronunciation difficulty, subjective oral health, chewing difficulty, academic credential, region, and sex. CONCLUSIONS: Therefore, in order to improve the happiness index of community residents, regular health checkups and systematic oral health education should be prepared in order to improve awareness of subjective health and reduce discomfort in the oral cavity, and efforts should be made to reduce the gap between regions.

9.Cross Training Effect Following Unilateral Leg Strengthening Exercise.

OBJECTIVE: To examine the effects of unilateral leg exercise on the contralateral leg and the cross training effect according to the training intensity. METHOD: Nineteen healthy males volunteered to be subjects for this investigation and were divided into a training group (N=13) and a control group (N=6). One leg of each subject in the training group was randomly assigned to a six week, three day/week isokinetic strengthening training program for concentric knee extension-flexion performed at 60 degrees/second (group A, N=6) and 180 degrees/second (group B, N=7). The control group did not train for six weeks. The strength of the knee extensor and flexor was tested before and after the six week period training by Cybex 770 dynamometer. RESULTS: In both groups A and B, the training resulted in significant increase of knee extensor strength in trained limb compared to control group. However, the strength increment of untrained limb was not significant compared to control group. CONCLUSION: There was no significant cross training effect following unilateral leg strength exercise.

10.Evaluation of hematologic profile may be needed for patients treated with oxcarbazepine

PURPOSE: The major side effects of treatment with oxcarbazepine (OXC) are skin rash and hyponatremia. Hematologic side effects are reported rarely. The aim of this study was to investigate the rate and types of the hematologic side effects of OXC. METHODS: The medical records of 184 patients diagnosed with epilepsy or movement disorder and on OXC monotherapy, at the Department of Pediatrics of Inje University Sanggye Paik Hospital from July 2001 to July 2018, were retrospectively reviewed. RESULTS: Of the 184 patients, 10 (5.4%) developed leukopenia in addition to pancytopenia and 2 (1.0%) developed pancytopenia. Leukopenia developed in 11 days to 14 years after OXC administration and was more frequent in males than in females (male vs. female, 9 vs. 1; Fisher exact test, P<0.05). Of the eight patients with leukopenia alone, 7 continued OXC treatment; 6 improved without intervention; 1 was lost to follow-up; and 1 received a reduced OXC dose, who improved after intervention. Pancytopenia developed within 2 months of initiation of OXC treatment. Both patients initially continued OXC. One improved within 1 month and continued treatment with OXC, but the other showed progression of the side effect, leading to the discontinuation of OXC and subsequent improvement within 1 month. There were no significant differences in the ages of the patients, OXC dose, and duration of OXC treatment between patients with and without these side effects of OXC (P >0.05, t-test). CONCLUSION: OXC-induced leukopenia is not rare and may result in pancytopenia. Patients being treated with OXC should be regularly monitored for abnormal complete blood count profiles.
