1.A Case of Bilateral Verrucous Hemangioma Combined with Tuberous Sclerosis.

We report a 37-year-old man who had a cutaneous marker of tuberous sclerosis with angiofibromas, periungal fibromas and who also showed bilateral verrucous hemangiomas on buttock. It is not known whether there is association between verrucous hemangioma and tuberous sclerosis, but there are some evidence suggest a more than chance association. We describe a case of bilateral verrucous hemangiomas combined with tuberous sclerosis.

2.The Relationship between Serum Total Testosterone and Clinical Symptoms of Late-onset Hypogonadism in Aging Males.

PURPOSE: Late-onset hypogonadism(LOH) in the aging male is a clinical and biochemical syndrome characterized by a decline in levels of serum testosterone, and results in various physical and mental disabilities. We aim to evaluate the relationship between serum testostosterone levels and symtpoms of LOH. MATERIALS AND METHODS: As part of an andropause screening program, we examined ADAM questionnaires, IIEF-5 scores, and total serum testosterone levels among healthy volunteers aged over 40 years. RESULTS: The mean age of the 409 men was 59.8 years(range 40~89), and the mean serum testosterone level was 471.5+/-180.2 ng/dl. The serum testosterone level decreased significantly in correlation with age. There was a 92.5% positive response on the ADAM questionnaire. Among patients whose serum testosterone was less than 350 ng/dl, the ADAM questionnaire was positive in 23.7%(n=97). The mean serum testosterone level for men with a positive or negative ADAM questionnaire was 469.2+/-181.8 or 489.2+/-170.5 ng/dl, respectively; this difference was not statistically significant (p=xxx). There was also no significant correlation between IIEF-5 scores and serum testosterone levels. CONCLUSIONS: Among men over 40 years of age, 23.7% met the clinical and biochemical diagnostic criteria for late-onset hypogonadism, and there was no relationship between serum testosterone levels and symptoms of late-onset hypogonadism.

3.Measurement of Penile Sensitivity by Digital Vibrometer, MMD-1000A(R).

PURPOSE: The measurement of penile sensitivity is a noninvasive and economical screening method to detect various neurologic disorders including premature ejaculation. We measured the normal range of penile sensitivity using a conventional analogue vibrometer and a new type of digital vibrometer and compared the clinical utility of the two instruments. MATERIALS AND METHODS: The vibration threshold of 50 males who had normal erectile function and ejaculation was measured 3 times with both an analogue and a digital vibrometer. The sites of measurement included the plantar side of the 2nd finger, a distal site on the calf, the penile shaft, and glans. The confidence interval was analyzed by the Kernel density estimation method. RESULTS: The ranges measured by the digital vibrometer were narrower than those measured by the analogue instrument. The normal ranges in the 90% confidence interval by the digital vibrometer were 1.03~.16, 2.49~10.13, 1.54~3.41, 1.61~3.53, 1.91~3.80, 1.55~4.03 Hz/mv in finger tip, calf, right penile shaft, left penile shaft, right glans penis, and left glans penis, respectively. The normal ranges in the 95% confidence interval by digital vibrometer were 1.07~2.15, 3.23~9.50, 1.58~3.33, 1.65~3.48, 1.93~3.38, 1.83~3.73 Hz/mv in finger tip, calf, right penile shaft, left penile shaft, right glans penis, and left glans penis, respectively. The results from the digital vibrometer were more consistent than those by the analogue vibrometer. CONCLUSIONS: The digital vibrometer, MMD-1000A(R) can substitute for the analogue type and has the advantages of greater convenience and more precise measurement of penile sensitivity.

Lung transplantation has been successfully employed for variety of obstructive lung disease. Single lung transplantation has become a therapeutic option for end-stage obstructive lung disease. The patient, a 57 year old man with emphysema, suffered from severe dyspnea, which progressively aggravated him for the last three years. A single lung transplantation was performed from a young brain-dead donor on April 7th, 1999 in the department of thoracic surgery, Respiratory Center, Yongdong Severance hospital, yonsei University. The immunosuppressive regimen was based on cyclosporine A and azathioprine from beginning, adding steroid. Single lung transplantation was feasible and beneficial in patients with end-stage emphysema.

5.Microsporum Canis Infections Transmitted from a Tiger in a Group .

Microsporum canis is one of the zoophiles and pets(especially dogs and cats) play a role as a source of this organism. M. canis is most likely transmitted by contact of these animal hosts with humans. We report on 16 foreigners who are infected with M. canis transmitted from a show tiger. Fourteen of the patients had directly contact with a show tiger on occasion. Patients showed typical tinea corporis lesions on the extremities, and the tiger showed hairless patches on the abdomen. The culture and microscopic findings from patients' lesions revealed M. canis as the causative fungus.

6.An analytical on the periodontal conditions and treatment in leprous patients.

The purpose of this study was to investigate the oral health periodontal conditions (CPITN(0), CPITN(1) CPITN(2) CPITN(3)) and periodontal treatment (non-surgical periodontal treatment and surgical periodontal treatment) of patients who have leprous disease. The objects for this study were 754patients (male: 453, female: 301, 60-69ages: 340, 70-79ages: 414) treated in the oral health center among Leprous disease patients with gingivitis and periodontal disease, residents in the Sirocco from 2003, May 1st to 2005, April 30th. The subdivided result carrying out periodontal conditions and treatment were as follows: 1. 7.The Hemodynamic Effect of the Elevation of Lower Extemities during Hyperbaric Spinal Anesthesia .

Hypotension is one of the well-known complications following spinal anesthesia, and Trendelenburg position seems to have been used as one of the management of it. But, Trendelenburg position generally raises spinal anesthetic level with the use of hyperbaric tetracaine, and blood pressure may even further decrease. If simple elevation of lower extemities can prevent hypotension successfully without raising spinal anesthetic level following hyperbaric spinal anesthesia, there will be a possibility for us to replace Trendelenburg position with the elevation of lower extremities. We examined 50 cases of hyperbaric spinal anesthesia followed by elevation of lower extremities with the angles of l5 and 30 degrees. At each case, hydration with 10 ml per Kg was performed during the fixation time (about 30 minutes) and then, blood pressure(systolic and diastolic), heart rate were checked with the time-interval of 1 and 2 minutes for each angle. The paired T-test showed that systolic and diastolic blood pressure after spinal anesthesia was increased in all cases with mean values of 2.74~5.68 mmHg for systolic blood pressure(p<0.05) and 3.26~7.22 mmHg for diastolic blood pressure(p<0.05), and that heart rate was decreased by 3.54-5.82 beats per minute(p<0.05). In conclusion, simple elevation of lower extremities and routine hydration therapy seemed to elevate blood pressure, making it possible for us to consider elevation of lower extremities to prevent hypotension without raising spinal anesthetic level in routine hyperbaric spinal anesthesia.

8.The Interface between Cytoskeletal Aberrations and Mitochondrial Dysfunction in Alzheimer's Disease and Related Disorders.

The major defining pathological hallmarks of Alzheimer's disease (AD) are the accumulations of Abeta in senile plaques and hyperphosphorylated tau in neurofibrillary tangles and neuropil threads. Recent studies indicate that rather than these insoluble lesions, the soluble Abeta oligomers and hyperphosphorylated tau are the toxic agents of AD pathology. Such pathological protein species are accompanied by cytoskeletal changes, mitochondrial dysfunction, Ca2+ dysregulation, and oxidative stress. In this review, we discuss how the binding of Abeta to various integrins, defects in downstream focal adhesion signaling, and activation of cofilin can impact mitochondrial dysfunction, cytoskeletal changes, and tau pathology induced by Abeta oligomers. Such pathological consequences can also feedback to further activate cofilin to promote cofilin pathology. We also suggest that the mechanism of Abeta generation by the endocytosis of APP is mechanistically linked with perturbations in integrin-based focal adhesion signaling, as APP, LRP, and beta-integrins are physically associated with each other.

9.Association between the Blautia/Bacteroides Ratio and Altered Body Mass Index after Bariatric Surgery

Background:
Current evidence support that the gut microbiota plays a potential role in obesity. Bariatric surgery can reduce excess weight and decrease the risk of life-threatening weight-related health problems and may also influence gut microbiota. In this study, we aimed to investigate the changes in gut microbiota before and after bariatric surgery and evaluate the association of the gut microbial shift and altered body mass index (BMI) after bariatric surgery.
Methods:
Between January 2019 and July 2020, stools from 58 patients scheduled for bariatric surgery were collected. Six months after bariatric surgery, stools from 22 of these patients were re-collected, and the changes in gut microbiota before and after bariatric surgery were evaluated. In addition, the differences in gut microbiota between patients with severe obesity (BMI >35 kg/m2, n=42) and healthy volunteers with normal BMI (18.8 to 22.8 kg/m2, n=41) were investigated.
Results:
The gut microbiota of patients who underwent bariatric surgery showed increased α-diversity and differed β-diversity compared with those before surgery. Interestingly, Blautia was decreased and Bacteriodes was increased at the genus level after bariatric surgery. Further, the Blautia/Bacteroides ratio showed a positive correlation with BMI. To validate these results, we compared the gut microbiota from severely obese patients with high BMI with those from healthy volunteers and demonstrated that the Blautia/Bacteroides ratio correlated positively with BMI.
Conclusion
In the gut microbial analysis of patients who underwent bariatric surgery, we presented that the Blautia/Bacteroides ratio had changed after bariatric surgery and showed a positive correlation with BMI.
