OBJECTIVES: To confirm the HAVS in workers exposed to hand-arm vibration with using the objective diagnostic methods METHODS: Several tests were used to help substantiate a clinical diagnosis of HAVS. Some other diseases were excluded by a medical interview, hematologic assessment, urinalysis, X-rays. The hand-arm vibration acceleration level of the grinder was evaluated for considering HAVS. Peripheral vascular changes were examined by a cold provocation test. An electromyography was implemented to ascertain the peripheral neural changes. RESULTS: 8 workers with symptoms in hands & fingers, exposed to hand-arm vibration were examined. Some other diseases(primary Raynauds disease, secondary Raynauds disease in not occupational origin) were excluded by a medical interview, hematologic assessment, urinalysis, X-rays. The hand-arm vibration acceleration level of the grinder was 7. 9 m/sec2. The recovery time of a skin temperature followed finger cooling was significantly prolonged in 6 workers with HAVS. The nerve conduction velocity was reduced in 6 workers with HAVS. CONCLUSIONS: So we report 6 cases of HAVS in grinding workers exposed to hand-arm vibration diagnosed objectively by an exclusion of some other disease, handarm vibration acceleration level, cold provocation test, electromyography.

The Varicella zoster virus is responsible for two common infectious diseases: chicken pox(Varicella) and shingles(Herpes zoster). Chicken pox is the primary infection. After the initial infection, the virus remains dormant in sensory ganglia until reactivation may occur decades later. The subsequent reactivation is Herpes zoster. Herpes zoster of the trigeminal nerve distribution manifests as painful, vesicle eruptions of the skin and mucosa innervated by the affected nerve. Oral vesicles usually appear after the skin manifestrations. Reports of osteomyelitis of jaw after trigeminal herpes zoster are extremely rare. We report a case of osteomyelitis on mandible caused by herpes zoster infection which was treated by antiviral drug, curettage. At 1 year post-operatively, mandibular mucosa had healed without recurrent sign. But post-herpetic neuralgia is remained.

Forty adult patients who received lumbar epidural anesthesia and axillary nerve block were assigned to four groups. Of these, 15 patients in group 1 (lumbar epidural anesthesia group) were given 1.5% lidocaine 400 mg (27 ml) and 15 patients in group 2 (lumbar epidural anesthesia group) were given lidocaine with epinephrine (200,000:1) in the same volume and concentration as in group l. 5 patients in group 3 (axillary nerve block group) were given lidocaine in the same volume and concentration and 5 patients in group 4 (axillary nerve block group) were given lidocaine with epinephrine (200,000:1). The plasma concentration of lidocaine were measured by immunofluorescence assay and the peak concentration were achieved 15 and 30 min, after injection in group 1, 2, respectively. Similar results were observed in group 3 and 4. The peak plasma concentration in group 1, 2,3 and 4 were 2.85+/-1.17, 1.81+/-0.60, 2.35+/-0.97 and 1.62+/- 0.67 ug/ml (Mean+/-SD), respectively. and the time course of plasma levels of lidocaine in group 2 and 4 lower than those in group 1 and 2. These results indicate the mixed injection of 1.5% lidocaine 400 mg with epinephrine is considered safe when there is no contraindication because plasma concentration is reduced significantly.

PURPOSE: Febrile seizure (FS) is the most common type of seizure in children between 6 months to 5 years of age. A family history of febrile seizures can increase the risk a child will have a FS. Yet, prevalence of FS regarding external environment has not been clearly proved. This study attempts to determine the association between prevalence of FS and weather. METHODS: This study included medical records from the Korea National Health Insurance Review and Assessment Service. Data were collected from 29,240 children, born after 2004, diagnosed with FS who were admitted to one of the hospitals in Seoul, Korea, between January 2009 and December 2013. During the corresponding time period, data from the Korea Meteorological Administration on daily monitoring of four meteorological factors (sea-level pressure, amount of precipitation, humidity and temperature) were collected. The relationships of FS prevalence and each meteorological factor will be designed using Poisson generalized additive model (GAM). Also, the contributory effect of viral infections on FS prevalence and weather will be discussed. RESULTS: The amount of precipitation was divided into two groups for comparison: one with less than 5 mm and the other with equal to or more than 5 mm. As a result of Poisson GAM, higher prevalence of FS showed a correlation with smaller amount of precipitation. Smoothing function was used to classify the relationships between three variables (sea-level pressure, humidity, and temperature) and prevalence of FS. FS prevalence was correlated with lower sea-level pressure and lower humidity. FS prevalence was high in two temperature ranges (-7 to -1℃ and 18–21℃). CONCLUSION: Low sea-level pressure, small amount of precipitation, and low relative air humidity may increase FS prevalence risk.

An epidermal cyst, presenting as an intrascrotal tumor, with no relation to the testicles or cord structures, is a rare benign tumor. Their etiology is unknown, but the most widely accepted theory on the histogenesis of testicular epidermal cysts is that they are the end result of a monolayer teratoma arising from germ cells; however, an extratesticular epidermoid cyst is believed to be an abnormal closure of the median raphe and urethral groove. An extratesticular epidermal cyst is normally subcutaneous, and can be localized anywhere along the median raphe, from the distal penis to the anus. To our knowledge, there has been no case reported in the Korean literature. A case of an intrascrotal epidermal cyst is reported in a 44-year-old man.

The purpose of this study is to investigate the relationship between the self-reported musculoskeletal symptoms and related factors among VDT operators in bank. The subjects were 950 women workers. The characteristics of subjects were 26.6 years of average age, 5.7 years of average VDT use duration and 6.5 hours of daily VDT use time. The results were as follows. The prevalence of self-reported musculoskeletal symptoms more than severe in Likert scale were 51.4%, 43.9% at the shoulder, 38.3%, 31.8% at the lower back, 38.0%, 31.3% at the neck, 31.2%, 25.2% at the upper back, 21.7%, 16.5% at the wrist and 13.6%, 8.8% at the fingers (while on duty, after home-coming). General characteristic variables affecting the musculoskeletal symptoms were married, number of child and daily house-working time. Work-related factors affecting the musculoskeletal symptoms were VDT working career, daily working time, daily VDF use time, VDF use time without rest, daily work-load and regular rest. As the result of the multiple logistic regression analysis, daily working time, daily VDT use time and VDT working career were significant variables accounting for the musculoskeletal symptoms.

Herein, a new left renal vein stenting procedure (expandable metallic stent placement), which provided relief for an 18-year-old man incapacitated by an intermittent gross hematuria, is reported. The placement of an expandable metallic stent is a minimally invasive therapy, and a simpler and more physiological therapy than previous approaches to Nutcracker syndrome.

OBJECTIVE: To evaluate the findings of brain MRI in patients with carbon disulfide poisoning. MATERIALS AND METHODS: Ninety-one patients who had suffered carbon disulfide poisoning [male:female=87:4; age, 32-74 (mean 53.3) years] were included in this study. To determine the extent of white matter hyperintensity (Grade 0-V) and lacunar infarction, T2-weighted MR imaging of the brain was performed. RESULTS: T2-weighted images depicted white matter hyperintensity in 70 patients (76.9%) and lacunar infarcts in 27 (29.7%). CONCLUSION: In these patients, the prevalent findings at T2-weighted MR imaging of the brain were white matter hyperintensity and lacunar infarcts. Disturbance of the cardiovascular system by carbon disulfide might account for these results.

Contrast-induced encephalopathy (CIE) is a rare complication of angiography and endovascular intervention following administration of iodinated intravenous contrast agents. Neuroimaging findings of CIE usually show cerebral edema, leptomeningeal enhancement, and parenchymal signal abnormality on fluid-attenuated inverse recovery (FLAIR). Hyperintense acute reperfusion marker (HARM) generally implies an enhancement of the subarachnoid cerebrospinal fluid space on brain FLAIR imaging in cases of acute ischemic stroke or hyperperfusion syndrome. We report a case of possible CIE following cerebral angiography, accompanied by HARM sign.
