1.Comparative Study between Ultrahigh Spatial Frequency Algorithm and High Spatial Frequency Algorithm in High-Resolution CT of the Lungs.

PURPOSE: To date, the high spatial frequency algorithm (HSFA) which reduces image smoothing and increases spatial resolution has been used for the evaluation of parenchymal lung diseases in thin-section high-resolution CT. In this study, we compared the ultrahigh spatial frequency algorithm (UHSFA) with the high spatial frequency algorithm in the assessment of thin section images of the lung parenchyma. METHODS AND MATERIALS: Three radiologists compared the UHSFA and HSFA on identical CT images in a line-pair resolution phantom, one lung specimen, 2 patients with normal lung and 18 patients with abnormal lung parenchyma. RESULTS: Scanning of a line-pair resolution phantom demonstrated no difference in resolution between two techniques but it showed that outer lines of the line pairs with maximal resolution looked thicker on UHSFA than those on HSFA. Lung parenchymal detail with UHSFA was judged equal or superior to HSFA in 95% of images. Lung parenchymal sharpness was improved with UHSFA in all images. Although UHSFA resulted in an increase in visible noise, observers did not found that image noise interfered with image interpretation. The visual CT attenuation of normal lung parenchyma is minimally increased in images with HSFA. The overall visual preference of the images reconstructed on UHSFA was considered equal to or greater than that of those reconstructed on HSFA in 78% of images. CONCLUSION: The ultrahigh spatial frequency algorithm improved the overall visual quality of the images in pulmonary parenchymal high-resolution CT.

2.The Morphology of the Mandibular Canal in Korean.

Ninety-three dry mandibles in Korean adults were radiographed and fifty-two hemimandibles were sectioned in an attempt to analyze the intramandibular courses of the mandibular canal and the internal structure of the mandibles. On radiographs, the intramandibular course of the mandibular canals was most in which located in the middle portion of the mandible, the shape of the mandibular canals was almost round and the average diameter of the mandibular canals was 2.0 +/- 0.4mm, encapsulated by thin cortical plates of 0.5mm thickness on sectioned specimens. In regard of facio-lingual relation, the prevalence of pattern which the mandibular canal ran nearly to the lingual cortical plate, and turned abruptly towards the buccal side was 70% of observed specimens. According to superior-inferior relation, the prevalence in which mandibular canals ran inferiorly, anteriorly and then, had an angle superiorly in the second mandibular molar region was 39.0%. Taken all together, in most of the case, the anterior border of the mandibular canal is located around the premolar region, and the mental foramen is opened nearly the second premolar. Therefore, it may be suggested that osteotomy has to be taken at the anterior place next to the canine region, at the 2.4mm below to the mental foramen on genioplasty.

3.Functions of Ich-1(L). and Ich-1(S) in Apoptotic Signaling Pathway of jurkat T Cells.

Human caspase-2, Ich-1 (Ice and Ced-3 homolog), has two different forms of mRNA species derived from alternative splicing, which encodes Ich-1 and Ich-1s. Ich-1v which induces apoptosis is antagonist of Ich-1s which suppresses Rat-1 cell death by serum deprivation. To investigate functions of Ich-1 and Ich-1s in T celi apoptosis, the fusion DNA constructs were made with the ecto and transmembrane of CDB and Ich-lv or Ich-1s and CDS-Ich-1 or CD8-Ich-1s chimeric protein was transiently expressed on Jurkat T cells. Tyrosine phosphorylation of intracellular proteins was induced in these transfectans when activated shortly by anti-CDB Ab. CDB-Ich-li transfectant in serum-rich condition and CDB-Ich-ls transfectant in serum-deprived condition underwent apoptosis when treated with anti-CDS Ab or incubated with NIH3T3 cells expressing stably Fas-L on their surface. We also made six antisense DNA constructs which could specifically inhibit the expression of Ich-1v, Ich- 1s, and then they were transiently transfected into Jurkat T cell. The overexpression of both of the antisese- Ich-1 against N-terminal 42 bp and against C-terminal 366 bp inhibited apoptosis through Fas signalling. But, when three different forms of antisense-Ich-1s were overexpressed in their transfectants, antisense-DNA against N-terminal 197 bp increased knd the one against C-terminal 66 bp inhibited apoptosis, instead the full size of antisense-DNA did not give any effects on apoptosis through Fas pathway.

5.Evaluation of Right Ventricular Function with Quantitative Radionuclide Ventriculography in Chronic Obstructive Lung Diseases.

A reproducible noninvasive technique for measuring right ventricular ejection fraction (RVEF) was developed using first pass quantitative radionuclide angiocardiography. Tests were carried out in the right anterior oblique position with a computerized multicrystal scintillation camera with high count rate capabilities. RVEF was calculated on beat to beat basis from the high frequency components of the background-corrected right ventricular time-activity curve. The following results were obtained; 1) In 10 normal adults, RVEF averaged 50.9+/-8.2. In 20 patients with chronic obstructive lung diseases (COPD), RVEF was 37.9+/-6.1% and significantly lower than that of normal persons(P<0.005). 2) There was meaningful correlation between RVEF and forced expiratory volume (FEVI) in patients with COPD (r=0.51). And there was significant difference of RVEF between 13 patients with FEVI less than IL/min (6 patients with right ventricular failure, 7 patients with patients without that) and 7 patients with FEVI IL/min or more (35.7+/-6.0%, 42.1+/-3.2% respectively. P<0.005). 3) All 6 patients with clinical manifestation of right heart failure had abnormal RVEF and had FEVI less than IL/min, which was significantly lower than that of 14 patients without clinical manifestation of right heart failure (33.6+/-4.8%, 39.8+/-5.6%, respectively. P<0.005). 4) In 14 patients without clinical manifestation of right heart failure, 11 patients among whom 7 patients had FEVI less than IL/min, had abnormal RVEF.

6.Cloning of Chromosomal Band Specific cDNA - cDNA related with neural development-.

Recently, surmountable amounts of genes are being cloned without information about them and it has become neccessary to develop new techniques for discovering genes with more informaiion like as chromosomal location and possible functions. We have developed one such a method and applied it to search for genes that may be related with the neural development. The mRNAs were extracted from cerebral cortex of 18 week old human fetus, cDNAs were made by reverse transcription from these mRNAs and Uni-amp cDNAs having Uni-amp adapters at both ends were made for subsequent PCR. To observe the distribution of the Uni-amp cDNAs on the chromosome, fluorescent in situ hybridization was performed with biotin labeled Uni-amp cDNAs. Among the chromosome bands showing strong hybridization with the cDNAs, 7q22 was microdissected from the chromosome hybridized with unlabeled Uni-amp cDNAs and amplified by PCR with Uni-amp primers. These amplified cDNA fragment were subcloned to vectors and the nucleic acid sequences were analysed. As a result, 46 different clones were obtained. They were categorized as 12 clones of well characterized genes, 14 clones showing low homology with known genes, 13 clones of simply registered uncharacterized human cDNAs, 7 clones of unknown genes. In situ hybridization histochemistry of 34 novel genes, except 12 known genes, were performed on developing and adult rat tissue sections to see the tissue specificity and developmental expression of these genes. The expression of several novel genes were restricted to the nervous system. From these results, it may be suggested that our technique is very useful to clone the genes expressed in the developing human braine with confirmed chromosomal location. In addition, this cloning technique can be used to discover the new genes related with neural development in combination with functional screening methods.

7.Surgical Treatment of Funnel Chest.

BACKGROUND: Funnel chest is one of the most common anomaly of chest wall, which is manifested by depression of sternum and costal cartilage. Popular operative methods were Ravitch operation and Wada operation. MATERIAL AND METHOD: From 1983 to 1996, 21 cases of funnel chest were corrected surgically in the department of thoracic surgery, National Medical Center. Investigated age and sex distribution, combined anomaly,clinical symptom, degree of correction and complication, postoperative satisfaction. We used 2 different surgical methods, one was Wada & its variants(17 cases), the other was Ravitch and it variants(4 cases). Most of operative indications were cosmetic problems. RESULT: The pre-operative Welch index was 4.188, but this index decreased to 3.46 after the operations.(p=0.046) The degree of correction was higher in Wada & it variant operation than the modified Ravitch operation.(p=0.54) Their results were satisfactory in 20 patients, while unsatisfactory in 1 patient because of a k-wire fracture. There was no recurrence of chest wall depression or postoperative death during the OPD follow up period. CONCLUSION: We recommend Wada operation in symmetric and small degree of depressive chest wall deformity in preand post school age.

9.Artifacts by dental materials on magnetic resonance imaging.

Magnetic resonance imaging(MRI)has proved to be a valuable method for evaluation of the head and neck. Unfortunately, metallic devices associated with certain dental fillings and applicances often cause variable artifacts that can obscure normal or pathologic conditions on MR and computed tomography. In this work, we assessed the MR appearance of dental prosthetic materials in vitro and in vivo including precious alloys. Nonprecions alloys, resin, amalgam and titanium alloy, For in vivo studies, these materials were placed in healthy volunteer's mouths and then images were assesed. Analyis of the appearance of shape and extent of artifact,and observed influence of these artifacts on the image interpretation at 0.2 Tesla permanent type MR scanner were valuated. Material used as temporary or permanent filling of crowns such as amalgam. precious alloy and, microfilled resin did not cause artifact on the image. The size of the artifact produced by the nonprecious alloys was influenced by the ferromagnetism of the object and the volume prosthesis, and was related to the scanning sequence. Nonprecious alloys produced minimal local signal distortions, where precious alloys, and dental resin had no effect on the MR image in vivo. These results were mainly from a low field strength MR scanner used in this study.

10.Evaluation of DNA Ploidy of Bronchogenic Carcinomas by Image Analysis.

In order to extract useful tumor cell-specific information. DNA contents and other morphological parameters were measured by image analysis. Single cell preparation was made from archived paraffin blocks of 14 cases of bronchogenic squamous cell carcinoma, poorly differentiated, by protease treatment. The cells were Feulgen stained, and DNA content, area, perimeter, and major axis of the tumor cell nuclei were measured. Inflammatory lymphocytes concurrent with the tumor cells were used as an internal standard. DNA ploidies of the lymphocytes and 2C tumor cells showed simple peaks with Gaussian distribution and mean coefficients of variation of 10% and 14% respectively. By the location and proportion of the tumor cells other than 2C cells, DNA ploidies could be classified into diploidy(1 case), polyploidy(2 cases), and aneuploidy(11 cases). The mean proportion of DNA aneuploidal tumor cells relative to the total tumor cells was 82.8%. In 8 cases, nuclear areas showed more or less overlapped distribution, whereas DNA contents showed discrete peaks. THes results suggest that many bronchogenic squamous cell carcinomas, poorly differentiated, have DNA aneuploidy and high proportion of aneuploidal cells, and that nuclear size and DNA content are more or less independent parameters.
