1.Morphometric Study on Histologic Components of the Spleen.

The partial splenectomy was performed on the basis of the arterial distribution in order to reduce complications. The spleen consists of the various histologic components that have different functions. But we can not find the reports whether the distribution of red pulp, white pulp and trabecula is uniform or not according to the regions of the spleen. We used 15 spleens from Korean[male 8, female 7 and age 17-74]. The volume was measured with the mass cylinder. The 1cm3 blocks were selected in 5 different regions. Whole splenic slices by celloidin embedding were made from two spleens. The point counting method with eyepeice reticule was used for the measurement of the distribution of red pulp, white pulp and trabecula. The results were as follows : 1. The volume of the spleen was ranged 45-158ml and the individual difference was marked. The male spleen was larger than female`s [p<0.05]. 2. The ratio of red pulp, white pulp and trabecula of the paraffin embedding preparations was 83.2, 10.9, and 5.9% respectively. There was no significant difference in the distribution of the histologic components among five regions[upper, hilum, middle, outer, and lower]. 3. The distribution of red and white pulps in whole splenic slices was not uniformity. The ratio of red pulp, white pulp and trabecula in whole splenic slices was 80.1, 13.4 and 6.5% respectively. These results showed no significant difference with those of five regions of the spleen.

3.A Study on the Precancerous Lesion of Breast Carcinoma in 9, 10-Dimethyl-1, 2-Benzanthracene-treated Rats.

Following results were obtained from the light microscopic and stereomicroscopic observations of the breasts of rats treated with 9, 10-Dimethyl-1,2-Benzanthracene(DMBA). 1) Adenocarcinomas developed in 17 rats (24%) among 70 DMBA-treated rats. 2) Terminal and buds (TEB) were observed longer in DMBA-treated rats than in control group, but they finally disppeared 4 monthes after treatment. 3) Many hyperplastic alveolar nodules (HAN) developed in DMBA-treated rats. 4) There were no transitional lesions between TEB and adenocarcinoma or HAN and adenocarcinoma. 5) The number of lobules was decreased in DMBA-treated rats. On the other hand, terminal ducts were increased in number. These findings suggest that DMBA stimulate the regression of lobules and induce to form terminal ducts from which adenocarcinomas and HAN develop independently.

5.Artifacts by dental materials on magnetic resonance imaging.

Magnetic resonance imaging(MRI)has proved to be a valuable method for evaluation of the head and neck. Unfortunately, metallic devices associated with certain dental fillings and applicances often cause variable artifacts that can obscure normal or pathologic conditions on MR and computed tomography. In this work, we assessed the MR appearance of dental prosthetic materials in vitro and in vivo including precious alloys. Nonprecions alloys, resin, amalgam and titanium alloy, For in vivo studies, these materials were placed in healthy volunteer's mouths and then images were assesed. Analyis of the appearance of shape and extent of artifact,and observed influence of these artifacts on the image interpretation at 0.2 Tesla permanent type MR scanner were valuated. Material used as temporary or permanent filling of crowns such as amalgam. precious alloy and, microfilled resin did not cause artifact on the image. The size of the artifact produced by the nonprecious alloys was influenced by the ferromagnetism of the object and the volume prosthesis, and was related to the scanning sequence. Nonprecious alloys produced minimal local signal distortions, where precious alloys, and dental resin had no effect on the MR image in vivo. These results were mainly from a low field strength MR scanner used in this study.

6.Prevalence of Tardive Dyskinesia in Inpatients Using Antipsychotics.

OBJECTIVE: This study was to investigate the prevalence of tardive dyskinesia (TD) in Korean inpatients using antipsychotics and the relationship between TD and sociodemographic, clinical variables. METHODS: This study was done from January to Feburary, 2003, at the Seoul National Mental Hospital. A cross-sectional assessment of randomly selected inpatients (n=324;mean age=39.73+/-9.63years) with DSM-IV schizophrenia, schizoaffective, bipolar disorder was performed with standard rating instrument for TD and extrapyramidal symptoms. The relations between the prevalence of TD and sex, age, length of medication and dosage of antipsychotics, clozapine, and other psychotropic medication, EPS were analyzed with Chi-square test or t-test. RESULTS: Using Schooler and Kane's criteria, 20 subjects (6.17%) had TD. TD was significantly less prevalent in patients receiving clozapine. There was no other significant difference between the TD and without TD groups with respect to type of antipsychotics, other psychotropic drug, antiparkinsonian drug, mood stabilizer augmentation. CONCLUSION: The result of this study have confirmed the lower prevalence of TD among inpatients using antipsychotics compared to previous investigations. The study has also replicated the association of TD with older age. Clozapine use was associated with lower prevalence rate of TD.

8.A Case of Nasolacrimal Duct Obstruction Caused by a Lacrimal Sac Retention Cyst

Acquired nasolacrimal duct obstruction may result from chronic infection, lacrimal stones, anatomical variations such as aberrant ethmoid cells, facial fractures, or complications following nasal surgery. In Korea, there has been no reported case of secondary nasolacrimal duct obstruction due to a retention cyst in the lacrimal sac. Recently, the authors encountered a 65-year-old female patient who presented with epiphora, was diagnosed with a lacrimal sac retention cyst, and was successfully treated with endoscopic marsupialization.
9.Nonimmune Hydrops Fetalis in One Identical Twin: A Case of Spontaneous Reversal in Utero.

Nonimmune hydrops fetals is defined as generalized edema of the fetal soft tissue in utero and may be associated with effusion in the serous cavities without hematologic evidence of isoimmunization. The most common demonstrable causes are cardiac anomalies, followed by chromosomal disorders, congenital malformations, alpha-thalassemia, and the twin-twin transfu-sion syndrome. We have experienced a case of nonimmune hydrops fetalis in identical twin which caused by congenital mitral valve insufficiency and disappeared spontaneously at 35 wks. Now we report a case of nonimmune hydrops fetalis that slowly and spontaneously imp-roved without interventions with a brief review of related literatures.

10.A Case of Proximal Renal Tubular Acidosis Accompanied by Vitamin D Deficient Rickets.

In the clinical state of vitamin D deficiency, it is possible that associated phosphate depletion, parathyroid hormone excess, and hypocalcemia may all depress the proximal tubular reabsorption of bicarbonate, in addition to abnormal skeletal modeling or remodeling. Although nutritional rickets is considered a rare disease in developed countries nowadays, cases of vitamin D deficient rickets caused by various unhealthy lifestyles such as insufficient exposure to sunlight, breast feeding infants without giving vitamin D supplements, unbalanced vegetarian diets of breast feeding mothers, low-birth weight, and maternal deficiency of vitamin D or calcium are increasing. Here, we present the case of an 8 month old girl, who was completely breastfed without any weaning diet or infant vitamin supplements. She visited our emergency room with hypocalcemic seizure and subsequently was diagnosed with vitamin D deficient rickets accompanied by overt bone changes and proximal renal tubular acidosis. After intravenous(IV) and oral calcium replacement therapy(IV calcium gluconate injection 1 mEq/kg/day for 6 days, 2 mEq/kg/day for 4 days followed by oral calcium gluconate administration 4 g/day for 3 days) with vitamin D supplement(Alfacalcidol 0.5 mcg/day) during admission, serum calcium level was normalized with clinical improvement. Oral sodium bicarbonate(0.6 g/day) was administered from the 2nd hospital day for 2 weeks, which normalized the serum bicarbonate(measured by tCO2) level. Calcium and vitamin D replacement were continued for 2 weeks and 3 months each. After discontinuing medications, follow up laboratory findings showed good maintenance of serum calcium, alkaline phosphate and bicarbonate levels with complete improvement of bone X-ray findings.
