1.A Clinical Observation on Injuries of External Genitalia.

A clinical observation was made on 27 cases of injuries of external genitalia of the in-patient in the department of uro1ogy, Capital Armed Forces General Hospital during the period from January. 1977 to December, 1978. The results were as follows: 1) The rate of genitourinary injuries accounted for 19.5 per cent to the total admission patients during Z years. Twenty-nine per cent of all injuries were in patients with injuries of externa1 genitalia and they accounted for 5. 7 per cent to the total admission. 2) Most favorable age was in from 20 to 30 years for 96 per cent. 3) Kick or blow was the most frequent cause of the injuries (66.7%) and the next iatrogenic. Straddle fa11, automobile accident. gun shot and suicidal attempt were listed not less frequently. 4) Majority of injuries of external genitalia involved scrotum and testes. 5) 5cases (18.5%) of injuries of external genitalia were associated with injuries of other organs, especially with urethral injuries in 3 cases. 6) Surgical treatment was done on 20 of 27 cases of injuries of external genitalia and conservative treatment was done on others. In all scrotal explorations, drainage and compression dressing were utilized. These were resulted in a decrease of edema and hematoma formation. 7) Complications, such as chronic nonspecific epididymitis, hydrocele or hematocele etc., resulted from conservative treatment that had been done in 5 (75%) of 7 cases.

2.6 Cases of Modified Dismembered Pyeloplasty in Hydronephrosis.

The dismembered Foley Y-plasty and Anderson-Hynes pyeloplasty are in principle the same operation for the correction of hydronephrosis secondary to obstruction at ureteropelvic junction. The differences between the former and the letter are that Anderson used a downward and continuous 0000 suture for a watertight closure, while Foley proposed upward and interrupted 0000 sutures about 2mm. apart for accurate tissue approximation and minimal interference with wound margin blood supply. We used downward and interrupted 0000 suture so-called modified dismembered pyeloplasty as mixed type, with a use of nephrostomy and stenting in 6 cases of hydronephrosis secondary to ureteropelvic obstruction during the period from January, l978 to June, l979. They were observed for 6 to l8 months and the results were as follows: 1) Hydronephrosis secondary to ureteropelvic obstruction was caused by congenital stricture in 4 kidneys, aberrant vessel in 1 kidney and congenital stricture with aberrant vessel in 1 kidney. 2) Of 2 cases of aberrant vessel, one was cut and the other not cut because of ischemia of corresponding lower pole. 3) In all case, Ureteral stenting was done with nephrostomy. But complication, such as urinary 1eakage, did not occurred in any cases. 4) On the follow up, the result was fair in 1 case and good in others.

3.Histopathological Studies of 300 Cases of Non-Hodgkin's Lymphoma in Korean Patients.

Non-Hodgkin's malignant lymphoma is a relatively frequent lymphoreticular malignancy, and has been reported to constitute up to 5.2% of all malignant tumors in Korean patients. Various morphologic classifications of non-Hodgkin's lymphoma have been proposed, and among them, the Rappaport's classification has been most widely accepted. In 1982, a National Cancer Institute sponsored study on classification led to the creation of the Working Formulation in an attempt to resolve the controversy anddebate regarding the various classifications of non-Hodgkin's lymphoma. Angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosis are lymphoreticular proliferative disorders which have reported to transform to malignant lymphoma. The purpose of the present study is to reclassify non-Hodgkin's lymphomas according to the Working Formulation and to investigate the histopathological and immunocytochemical characteristics of angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosis. This study reviewed 300 cases of nodal and extranodal non-Hodgkin's lymphoma, 26 cases of polymorphic reticulosis, and 7 cases of angioimmunoblastic lymphadenopathy with dysproteinemia examined in the Departments of Pathology, Yonsei University College of Medicine, Youngdong Severance Hospital and Yonsei University Wonju College of Medicine from January 1977 to December 1986. In non-Hodgkin's lymphoma, each case was classified according to the Working Formulation and the Rappaport classification. All angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosis cases were subjected to histopathological analysis and a review of the clinical records. Immunocytochemical studies were done using kappa and lambda chains for B-cell markers and alpha-1-antichymotrypsin for histiocytic marker. The results obtained were as follows; 1) Among 300 cases of non-Hodgkin's lymphoma, the primarily involved tumor sites were the lymph nodes (141 cases), the gastrointestinal tract (67 cases), and the tonsils (32 cases) in descending order of frequency. 2) Using the Working Formulation, intermediategrade lymphomas occurred in 66.4% of the patients, and the most common subtype was "diffuse, large cell" (32.7%). By the Rappaport classification, 3 patients had nodular lymphomas, and "diffuse, histiocytic" was the most common subtype. 3) Infarction was present in 32 cases in which the "diffuse, lagre cell" type was most frequently associated. 4) In immunoperoxidase stains of 7 cases of angioimmunoblastic lymphadenopathy with dysproteinemia, proliferating immunoblasts revealed a polyclonal positivity for kappa and lambda chains. Atypical reticulocytes present in 26 cases of polymorphic reticulosis revealed a negativity for kappa, lambda and alpha-1-antichymotrypsin.

5.Effects of Repeated Citalopram Treatments on Chronic Mild Stress- Induced Growth Associated Protein-43 mRNA Expression in Rat Hippocampus.

Although growth associated protein-43 (GAP-43) is known to play a significant role in the regulation of axonal growth and the formation of new neuronal connections in the hippocampus, there is only a few studies on the effects of acute stress on GAP-43 mRNA expression in the hippocampus. Moreover, the effects of repeated citalopram treatment on chronic mild stress (CMS)-induced changes in GAP-43 mRNA expression in the hippocampus have not been explored before. To explore this question, male rats were exposed to acute immobilization stress or CMS. Also, citalopram was given prior to stress everyday during CMS procedures. Acute immobilization stress significantly increased GAP-43 mRNA expression in all subfields of the hippocampus, while CMS significantly decreased GAP-43 mRNA expression in the dentate granule cell layer (GCL). Repeated citalopram treatment decreased GAP-43 mRNA expression in the GCL compared with unstressed controls, but this decrease was not further potentiated by CMS exposure. Similar decreases in GAP-43 mRNA expression were observed in CA1, CA3 and CA4 areas of the hippocampus only after repeated citalopram treatment in CMS-exposed rats. This result indicates that GAP-43 mRNA expression in the hippocampus may differently respond to acute and chronic stress, and that repeated citalopram treatment does not change CMS-induced decreases in GAP-43 mRNA expression in the GCL.

6.Imaging of the Jaw Cysts with a Dental CT Software Program: Distinction of Odontogenic Keratocysts from Other Cysts.

PURPOSE: To evaluate the usefulness of a dental CT software program in the assessment of jaw cysts and in the differentiation of odontogenic keratocysts and other cysts. MATERIALS AND METHODS: Seventeen patients with proven jaw cysts (8 maxillae & 9 mandibles) were evaluated with a dental CT software program for location, locularity, the presence or absence of marginal scalloping, and height to length ratio. For the delineation of involvement or displacement of neurovascular bundles, cortical erosion, perforation or expansion, and tooth root resorption by the jaw cysts, images from this program were compared to conventional images. RESULTS: Seventeen lesions icomprised 15 odontogenic cysts (five odontogenic keratocysts, five radicular, three residual and two dentigerous cysts) and two non-odontogenic cysts (one nasopalatine duct cyst and one postoperative maxillary cyst). Images of jaw cysts obtained with the dental CT software program delineated much more clearly than conventional images the status of neurovascular bundle and cortical bone, but there was no clear difference between the two modalities in delineating tooth root erosion. Dental CT findings of five mandibular odontogenic keratocysts were scalloped margin in all, mandibular ramus involvement in four, height to length ratio below 60% in four, and multilocularity in two. The findings of the other 12 cysts (eight maxillae and four mandibles) were unilocularity in all, smooth inner margin in ten, height to length ratio below 60% in only two, and ramus involvement in none. CONCLUSION: Adental CT software program is an improved imaging modality for assessing jaw cysts ; and findings which tend to indicate odontogenic keratocysts are marginal scalloping, mandibular ramus involvement, prominent spread along the marrow space and multilocularity.

7.Evaluation of a Colorimetric Antifungal Susceptibility Test by 2,3-Diphenyl-5-Thienyl-(2)-Tetrazolium Chloride for Fluconazole in Candida Species Isolated from Clinical Specimens.

BACKGROUND: The aims of this study were to evaluate the colorimetric antifungal susceptibility test to fluconazole using 2,3-diphenyl-5-thienyl-(2)-tetrazolium chloride (STC) for various Candida species isolated from clinical specimens and to compare the results with those of the CLSI M27-A2 standard method. METHODS: The fluconazole MICs for 204 clinical Candida isolates consisting of 100 C. albicans, 45 C. glabrata, 28 C. tropicalis, 22 C. parapsilosis, and 9 other Candida species were determined by the CLSI and STC colorimetric methods. RESULTS: All 204 Candida strains were grown on the growth control wells of CLSI standard plates, but 26 Candida strains (6 C. albicans and 20 C. tropicalis) were not grown on those containing STC. Therefore, those 26 Candida strains were excluded from the comparison of MICs in this report. Overall, the STC visual and spectrophotometric readings of fluconazole MICs showed 96.1% (N=171) and 89.9% (N=160) accordance with those obtained by the CLSI standard method within 2 dilutions, respectively. The STC visual reading of C. albicans showed 76.6, 92.6, and 95.8% accordance with the CLSI standard method within 1, 2, and 3 dilutions, respectively. The agreement between the two endpoint determinations of the STC colorimetric method (visual and spectrophotometric readings) was excellent, with 170 of the 178 MICs within 2 dilutions. CONCLUSION: The STC colorimetric method to determine the MIC for Candida species except C. tropicalis showed high levels of agreement with CLSI method. And also, it is useful with objective and easy interpretation.

8.Prediction of the severity and length of hospital stay in infants with acute bronchiolitis using the severity score.

PURPOSE: This study aimed to evaluate the clinical factors that could influence the length of hospital stay (LHS) of infants with acute bronchiolitis using the bronchiolitis severity score (BSS). METHODS: The medical records of 105 infants aged 0–12 months with acute bronchiolitis who were admitted to a tertiary hospital between December 2014 and May 2015 were reviewed. The BSS was composed 5 factors, namely general condition, wheezing, chest retraction, respiration rate, and peripheral oxygen saturation, which were evaluated at admission. In addition, the age of the infants, LHS, body weight, birth history, familial history, laboratory test result, chest X-ray, and treatment modalities were reviewed. Subjects were classified into 3 groups according to their BSS, and logistic regression analysis was used to identify the association of the BSS with longer LHS. RESULTS: Among subjects, 62 were studied. Their mean LHS and age were 5.5±2.0 days and 3.9±2.7 months, respectively. Twelve infants were classified as mild (20.3%), 43 as moderate (68.8%), and 7 as severe (10.9%). Venous blood gas pH value and white blood cell count were the best predictors of disease severity. The LHS was associated with the BSS, age, body weight and pCO2 by venous blood gas analysis (P<0.05). CONCLUSION: The LHS was associated with the BSS, age, body weight, and pCO₂ by venous blood gas analysis at admission. The BSS could be a useful tool to predict disease severity and decide treatment strategies for infants with acute bronchiolitis who have no known risk factors.

9.Analysis of vitamin D-binding protein (VDBP) gene polymorphisms in Korean women with and without endometriosis

OBJECTIVE: Vitamin D-binding protein (VDBP) mediates various biological processes in humans. The goal of this study was to investigate whether VDBP gene polymorphisms could predispose Korean women to endometriosis. METHODS: We prospectively enrolled women with endometriosis (n = 16) and healthy controls (n = 16). Total serum 25-hydroxyl vitamin D (25(OH)D) concentrations were measured using an Elecsys vitamin D total kit. Levels of bioavailable and free 25(OH)D were calculated. Concentrations of VDBP were measured using a vitamin D BP Quantikine ELISA kit. DNA was extracted using a DNeasy blood & tissue kit. Two single-nucleotide polymorphisms (SNPs; rs4588 and rs7041) in GC, the gene that codes for VDBP, were analyzed using a TaqMan SNP genotyping assay kit. The functional variant of VDBP was determined based on the results of the two SNPs. RESULTS: Gravidity and parity were significantly lower in the endometriosis patients than in the control group, but serum CA-125 levels and the erythrocyte sedimentation rate were significantly higher. Total serum 25(OH)D levels in the endometriosis patients were significantly lower than in the control group. However, serum bioavailable 25(OH)D, free 25(OH)D, and VDBP levels did not differ significantly between the endometriosis and control groups. The genotypes and allele frequencies of GC were similar in both groups. CONCLUSION: Korean women with endometriosis had lower total serum 25(OH)D concentrations than controls. Neither serum VDBP concentrations nor polymorphisms in the gene coding for VDBP were associated with endometriosis. Further studies are needed to investigate the pathophysiology and clinical implications of 25(OH)D and VDBP in endometriosis.

10.2 cases of idiopathic BOOP associated with rare radiologic finding.

Idiopathic bronchiolitis obliterans with organizing pneumonia (BOOP) is a specific clinicopathologic syndrome characterized by a pneumonia-like illness, with excessive proliferation of granulation tissue within bronchioles, alveolar ducts and alveoli. The changes are most numerous in alveolar ducts. The presence of intraluminal tufts of organizing connective tissue in alveolar ducts and more distal airspaces has been termed organizing pneumonia The radiologic manifestations are distinctive with bilateral, diffuse alveolar opacities predominantly in the subpleural and lower lung zone. Patchy migratory pneumonic foci or solely nodular infiltrates are rarely present in BOOP. BOOP is a diagnosis of importance because of its dramatic response to steroids.
