A 25-year-old female was seen because of rnultiple, asymptornatic papular eruptions on the vulvar area since 1 year ago. She had been suffered from palmar hyperhidrosis for 3 years. A skin biopsy specimen from the vulval lesion revealed characteristic findings of syringoma.

3.A case of Solitary Glomus Tumor.

A 72 years old man had an 1 x 1cm sized, solitary glomus tumor on the right knee joint and had suffered from sharp, episodic pain and tenderness, radiating to the right thigh and the buttock for about 20 years. Simple surgical excision was performed for removal of the tumor mass and the subjective symtoms. There is no recurrence until now.

5.Comparison of portal CT and indirect portography in hepatic masses.

We compared 87 portographic filling defects detected by portal CT in 64 patients were compared with those obtained by indirect portography. The indirect portography could visualize portogram only in anterior-posterior view. But the portal portal CT could visualize both portogram and hepatogram. We examined the portal CT and indirect portography and compared the accuracy of the both methods to evaluate the limitation and significance of the indirect protography. The mass shape lesions were seen on the portal CT which means portal flow defects of the mass lesions only could not depict totally in indirect portography (0%, 0/41). And the larger defects than real mass lesion were seen in portal CT means mass with associated portal flow defect and find portal vein invasion around the mass in 52% (24/46) of the indirect portography. Among them, only 66% of mass were detected correctly in the indirect portography comparing with mass lesion in portal CT. In summary, indirect portogram could not detect small filling defects which detected in portal CT and could not depict the extent of large filling defects. It also could not visualize correctly the protal flow in non-lesion side of the liver parenchyma.

6.Fingertip Dermatoglyphic Patterns in Patients with Atopic Dermatitis and their Families.

Although we made a comparision of fingertip dermatoglyphic patterns between R7 patients with atopic dermatitis(Femals: 54, Male: 35) and 73 their families without atopic dermatitis, no significant results were obtained. So, we compared secondly our results of 2 groups with those of two thousand nonatopic, normal Koreans, as control. Results are as follows: ] In comparison with control, a significant increase in the ulnar loop pattern was detected in families(p<0, ()5), not atopic dermatitis. p. No significant differ ence between right hands and left hands in all three g f OUpS. In atopic dermatitis and control groups, 1 st and 4 th digits showed mostly whorl pattern, and 3 rd and 5 th digits had more ulnar loop pattern than others. Rut families had mostly ulnar loop pattern at all digits except 4 th digit, wl..orl pattern. 4. Each of atopic dermatitis, family and control group had 13, 79% (whor1: 8. 05% ulnar loop: 5 75%), 15. 49p; (whorl: 4. 23%, ulnar loop: 11. 27%), 15. 78%(whorl: 8, 52% ulnar loop: 7. 1 %, arch: 0. 05%), of same dermatoglyphic pattern on all ten digits. Especially, ulnar loop pattern in fanilies was detected significantly higher than other two groups(p<0. 05). Average incidences of bilateral symmetry of pattern type between right and left digits were 78.57%, 74. 67% and 78.74% respectively in atopic dermatitis, family and control group.

9.Comparison of the Clinical Effects of the Different Ventilatory Care Strategies in the Neonates with Acute Respiratory Failure: High Flow Rate - Constant Flow Rate.

PURPOSE: High flow rate (FR) and pressure limit (PL) strategy with time-cycled pressure-limited (TCPL) ventilator is employed routinely in the neonates. Theoretical basis of this strategy is the two-compartment theory that the lung with acute respiratory failure consists of units with different compliance and resistance. But such constant pressure strategy has the risk of ventilator induced lung injury. We compared the ventilatory indices and clinical outcomes of two different strategies, high FR-constant pressure and low FR-constant FR in the ventilator care of the neonates with acute respiratory failure. METHODS: For the neonates born in our hospital and treated with mechanical ventilation from March to August in 1997, two different ventilator strategies were employed randomly with flow control ventilator. In the high-FR group, the FR was fixed at 10 L/ min and the PL was adjusted according to the arterial blood gas analysis (ABGA) results. In the low-FR group, the FR was adjusted to 10 mL/kg of tidal volume. Sixty neonates were enrolled, 32 in high-FR and 28 in low-FR group. Ventilatory indices and clinical outcomes were statistically cornpared in the two groups. RESULTS: Perinatal factors were not different in the two groups. Initial ventilator settings, ABGA results and ventilatory indices were not different. The tidal volume, fraction of inspired oxygen, peak inspired pressure and oxygenation index were higher and dynamic compliance was lower in the high-FR group compared to the low-FR group after 3 to 72 hours of ventilator care. In clinical outcomes, incidences of pulmonary interstitial emphysema, pneumothorax and chronic lung disease were significantly lower in the low-FR group. CONCLUSION: Low-FR with constant FR strategy resulted in better clinical outcomes in the ventilator care of neonates. We conclude that constant FR strategy prevents damage of the better compliant lung units and decreases the incidence of acute and chronic complications of ventilator care.
