4.A Case fo Angiosarcoma.

Angiosarcoma is rare disease not only in Korea but throughout the world; it is all the more so on the skin. We made an observation on a case of angiosarcoma on the scalp of a 57 year-old man. Noteworthy in this case are: (1) the patient was suffered from a head trauma 6 years prior to onset of the disease, (2) the progress of the disease showed an extreme rapidity accompanied by severe headache intolerable by any medication, (3) no effect was resulted from X-ray irradiation, and (4) the patient survived for 7 months with the disease.

5.Role of Toe - Flexors in Ankle Plantar Flexion during Normal and Rapid Walking - 3 Dimensional Kinetic Study.

It is known that the toe-flexors exert some power generation in ankle plantar flexion. However, there has been no paper published in which the power generation was quantified. The purpose of this paper, therefore, is to quantify the amount of contribution of the toe-flexors to the ankle plantar-flexion in normal and rapid walking using the kinetic data of three dimensional gait analysis system. In order to restrict the action of the toe-flexors, we designed special braces which can be applied to the forefeet of the examinee during walking. We performed the gait analysis in ten normal adult volunteers with and without braces, and evaluated the moment and power of toe-flexors during terminal stance and pre-swing phase of gait cycle. Gait analysis was done with the VICON 3-dimensional motion analysis system (VICON, Oxford Metrics, Oxford, England) and 2 force plates (AMTI, Advanced Mechanical Technoiogy, Newton, MA, U.S.A,). The kinetic results are as follows: l. Average speeds of normal and rapid walking were 1.12m/sec and 1.41m/sec respectiveIy. 2. In normal walking, peak ankle plantar-tlexion moment decreased 5.5% with braces, and sum of ankle plantar-flexion moment decreased 12.3% with braces. Both of the results were not significant statistically (p>0.05). Peak ankle power generation decreased 11.0% with braces, and sum of ankle power generation decreased 10.4% with braces. These decreases were also insignificant statistically (p>0.05). 3. In rapid walking, peak ankle plantarflexion moment decreased 26.7% with braces. The decrease was horderline significant statistically (p=0.062). The sum of ankle plantar-tlexion moment decreased 26.6% with braces, but the decrease was not significant statistically (p>0.05). Peak ankle power generation decreased 40.2% with braces, and sum of ankle power generation decreased 37.9% with braces. These decreases showed borderline significance statistically (p=0.062). In conclusion, toe-flexors may contribute about 10% of the total ankle plantar-flexion power generation, and the contribution will be increased with increase of walking velocity. We must be very careful to sacrifice the toe-flexors in cases with weak triceps power.

7.A study on Purification of Water Works by Multi-filter Bed Method with Aeration: Application of Biological Oxidation by Aerobic Microorganism.

5 bed sand filter, applying biological oxidation, was designed and studied on the treatment of water works. Never using any coagulant agent (drugs), which may cause water pollution in pre-treatment of head water, the author attempt a high rate filtration by the microorganism (nitrofication bacteria) end plant which populate in multi layer sand beds. The result are as follows ; In order to evaluate the oxygen effect on filtration, oxygen was injected in aeration tank attached to each filter tank while filtration, and NH3 was tested as a representative ingredient. It was found out that the aeration method was more effective, with over 33% of NH3 removal capacity, than the anerobic and this 5 bed filter showed double removal capacity of NH3 by comparing with conventional sand bed (2 stage bed). According to the examination of two kind of head water, pre-treated with coagulant agent and activated carben, the filtration capacity was affected by the polluted condition of head water, resulting that lower value of pollution and slower velocity of filtration showed more efficiency of NH3 removal. In this experiment NH3 content tested in treated water had a fairly good correlation with others.

9.A Case of letterer-Siwe Disease.

Letterer-Siwe disease occurred in a 3 year 7 month old male child showing severe emaciation, hepatosplenomegaly, otitis media, hemorrhagic and impetiginous crusted patches on the body, and seborrheic dermatitis like skin rashes in the scalp. Also showed are severe anemia, thrombocytopenia, bronchopneumonia and multiple bony defects in skull. The child expired in a month after admission.

10.A case of doxycycline-resistant tsutsugamushi meningoencephalitis

Here we report a case of doxycycline-resistant tsutsugamushi meningoencephalitis. A 63-year-old man with altered mental status was diagnosed with a tsutsugamushi infection by a local clinic and administered standard doxycycline treatment for 5 days without improvement. Azithromycin treatment for 3 days dramatically improved his clinical status.
