2.Anuria in a Infant due to Bilateral Ureteropelvic Fungus Balls.

Fungal infection has been observed with increasing frequency in recent years because the use of combinations of broad spectrum antibiotics, immunosuppressive agents, and antineoplastic agents is increasing and the survival rate of premature baby is increasing. We experienced a 3 month old male infant with anuria due to bilateral ureteropelvic fungus balls. He was born at 31 weeks gestation period and had been treated with broad spectrum antibiotics for 5 weeks after birth. We removed fungus balls surgically and made nephrostomy bilaterally. And then irrigation of amphotericn B through nephrostomy and systemic amphotercin B injection had performed for 3 weeks. Thereafter fungus balls completely disappeared.

3.In Thyroid Cancer Patients, Is Preoperative FNAB-C Reliable for Prediction of Lateral Cervical LN Metastasis?.

PURPOSE: The goal of this study was to evaluate the diagnostic accuracy of preoperative fine needle aspiration biopsy cytology (FNAB-C) in predicting lateral lymph node metastasis in papillary thyroid cancer patients. METHODS: A total of 592 patients who underwent thyroid cancer surgery and intra-operative lateral cervical LN frozen section or RND, from January 2002 to December 2011, were evaluated retrospectively. Among them, 228 cases had suspicious findings in FNAB-C of lateral nodes. We reviewed their radiological and pathological reports. RESULTS: Intra-operative frozen section examination was performed in 540 cases and RND was performed in 314 cases. This study included 534 women (83.4%) and 106 men (16.6%). Patients' ranged in age from 9 to 83 years (mean, 45.65 years). FNAB-C was performed in 228 cases. The sensitivity and specificity of FNAB-C was 71.5% and 78.6%, respectively; 35.9% of cases had a false negative result. The combination of FNAB-C and intra-operative frozen section test sensitivity and specificity was 87.2% and 93.6%, respectively. CONCLUSION: The results for sensitivity in FNAB-C actually appear low, and false negative results were very high. In papillary thyroid cancer in patients with FNAB-C, even if the result is negative, if lymph node metastasis is suspected based on radiologic evidence, frozen section examination should be performed for determination of metastasis.

The importance of tick-borne diseases is increasing because of climate change, with a lack of long-term studies on tick-borne pathogens in South Korea. To understand the epidemiological characteristics of tick-borne diseases, the monthly distribution of field ticks throughout the year was studied in South Korea between May 2014 and April 2018 in a cross sectional study. The presence of various tick-borne pathogens ( During the study period there were 11,717 ticks collected and 4 species identified. In conclusion, due attention should be paid to preventing tick-borne infections in humans whilst engaged in outdoor activities in Spring and Autumn, particularly in places where there is a high prevalence of ticks.
5.Surgical Methods for Radioiodine Refractory Thyroid Cancer.

Most patients with thyroid cancer (of follicular cell origin) are successfully managed with a combination of surgery, radioactive iodine (131I-RAI), and suppression of thyroid-stimulating hormone with thyroid hormone replacement, obtaining survival rates approaching 90% at 20 years. Although the prognosis of patients with differentiated thyroid carcinoma (DTC) is favorable, recurrence occurs in up to 30% patients. In addition, many patients with recurrent or metastatic disease, as well as those with less differentiated tumors, will have a much poorer prognosis and lose their ability to concentrate functional iodine and are therefore not targeted by 131I-RAI therapy. There are many treatment options but no definitive treatment for radioiodine refractory thyroid cancer. This paper will discuss the roles of surgical treatment for patients with radioiodine refractory thyroid cancer.

6.Use of Polyurethane with Sustained Release Dexamethasone in Delayed Adjustable Strabismus Surgery.

PURPOSE: To determine the effect of polyurethane with sustained release dexamethasone in delayed adjustable strabismus surgery in rabbits. METHODS: After recession of the superior rectus muscle(SRM), balanced salt solution or polyurethane film with/without sustained release dexamethasone was applied beneath and over the SRM in the control group(group C), polyurethane-steroid group(group P-S) and polyurethane group(group P), respectively. Delayed adjustment was performed once on each SRM at four and six weeks postoperatively by a masked observer. The possible length to adjust and the necessary force required for the adjustment, as well as the degree of any adhesions, were also evaluated. RESULTS: In the control group, adjustment was impossible in all of the eyes at four and six weeks postoperatively. In group P-S, adjustment was possible in 11 out of 11(11/11) eyes four weeks postoperatively and in 10/11 eyes six weeks postoperatively. In group P, adjustment was possible in 9/11 eyes four weeks postoperatively and in 10/12 eyes six weeks postoperatively. In addition, the degree of adhesion was significantly less in group P and in group P-S compared to the control group. The degree of adhesion between the SRM and the sclera was marginally better(p=0.051) in group P-S four weeks postoperatively than in group P. CONCLUSIONS: Use of polyurethane film could delay adjustment in most of the rabbit eyes for up to six weeks postoperatively. Sustained release dexamethasone is helpful for delaying adjustment in rabbit eyes until six week postoperatively without the need for frequent topical instillation of steroids.

7.Surgical Treatment of Complete Vascular Ring Associated with Kommerell's Diverticulum.

Forty year old woman with dysphagia underwent surgical correction of complete vascular ring formed by right aortic arch, Kommerell's diverticulum and ligamentum arteriosum. Operation consisted of ligamentum division, reduction diverticuloplasty and posterior diverticulopexy via left posterolateral thoracotomy. Dysphagia was relieved postoperatively, and concentic narrowing of esophagus in the level of aortic arch disappeared on postoperative esophagography.

8.Capillary leak syndrome and disseminated intravascular coagulation after kidney transplantation in a patient with hereditary angioedema - A case report -

Background:
Hereditary angioedema (HAE) is a rare disease caused by the deficiency of C1 esterase inhibitor. HAE has a risk of life-threatening complications such as capillary leak syndrome (CLS) and disseminated intravascular coagulation (DIC).Case: A 42-year-old man with HAE presented for deceased-donor kidney transplantation. Prophylactic fresh frozen plasma (FFP) was given before surgery because of the risk of edema development. With careful management during anesthesia, there were no problems during surgery. However, generalized edema, hypotension, hypoalbuminemia, massive drainage of serosanguineous fluids from the intraabdominal space, and DIC occurred on the day after surgery. CLS was suspected and sustained hypotension with generalized edema became worse despite treatment with albumin, danazol, FFP, and vasoactive drugs. The patient’s condition worsened despite intensive care and he died due to shock.
Conclusions
The anesthesiologist should prepare for the critical complications of HAE and prepare the appropriate treatment options.
9.Treatment of Unstable Osteochondral Dissecans Lesion of the Knee Joint Using Autologous Osteochondral Plug.

PURPOSE: The purpose of this study was to evaluate the clinical results of fixation of the unstable osteochondral dissecans (OCD) lesion with autologous osteochondral plugs. MATERIALS AND METHODS: This study was conducted in 19 patients who were relevant to the International Cartilage Repair Society 2, 3, or 4 who were treated with autologous osteochondral plugs and followed-up for more than 2 years from January 2004 to January 2012. Clinical evaluation was performed by comparing the preoperative and last follow-up scores of Lysholm score and subjective International Knee Documentation Committee (IKDC) score. RESULTS: All patients were male and the average age was 19.1 years. Of the 19 cases, there were 16 cases of medial femoral condyle lateral side lesion, 2 cases of lateral femoral condyle articular surface, and 1 case of femoral intercondylar notch lesions. The average size of the lesion was 5.68 mm2, and average use of osteochodral plugs were 4.3. Average follow-up period was 38 months. Preoperative Lysholm score, IKDC subjective score showed significant improvement. CONCLUSION: Fixation with autologous osteochondral plugs for unstable OCD uses the remnant tissues therefore conserving it, which is thought to be the positive aspect of this type of operation.

10.Isovolumic Phase Indices of Myocardial Contractility in 16 Korean Adults with Normal Physical Activities.

Assessment of Myocardial contractility is critically important task in the evaluation of patients with heart disease. In recent years many indices have been studied to evaluate myocardial inotropic state, applying the skeletal muscle mechanics to intact heart. Among such indices, Vmax(maximum velocity of shortening of the unloaded contractile elements) and Vpm(physiologic maximum observed velocity of myocardial shortening) provide a measure of myocardial contractility independent of preload or afterload. To obtain normal values in Korean adults, the left ventricular pressure data of 16 patients with normal physical activities were analyzed, using the digital computer. Seven patients had ventricular septal defect with Qp/Qs less than 2.0, one patient had mitral stenosis, another one patient had mitral stenosis and aortic regurgitation (grade I/IV and the remainder seven patients had no intrinsic cardiac disorder. But, the cardiac performances of all patients were apparently normal. The results were as follows: 1) Cardiac index was greater than 3.0 l/min/m2 in all patinets: 4.9+/-1.32(+/-S.D.)l.min/m2. 2) Ejection fraction was greater than 55% in all patients: 71.2+/-8.04(+/-S.D.)%. 3) Left ventricular end diastolic pressure(LVEDP) ranged from 3 mmHg to 12 mmHg. 4) Vmax: 48.1+/-9.41(+/-S.D.) sec(-1). 5) Vpm: 39.3+/-8.13(+/-S.D.) sec(-1).
