The immunocompetence is important not only to kill the neoplastic cells but also to keep the neoplastic cells from growing further. T lymphocyte is plays the most important role in maintaining the tumor immunity efficiently. T lymphocyte has its specific functions depending in the subset of T lymphocytes. The author analyzed the T lymphocyte subsets in 31 brain tumor patients using anti-CD3, anti-CD4, anti-CD8 monoclonal antibodies and flow cytometry to determine the immunological status of brain tumor patients. All CD3, CD4 and CD8 subsets were reduced in both benign and malignant brain tumor patients but more signigicantly reduced in malignant tumor group. But in benign tumor group, the subtypes of T lymphocytes were not so different from those of normal healthy controls except the pituitary tumor patients, who showed the significant decrease in all the subtypes. In malignant tumor group, each subtype was signigicantly reduced and CD8 subtypes was markedly reduced in metastatic tumor patients, These analyses were considered to have the possibility to be contributable to planning the further immunotherapy and also the possibility to moniter the brain tumor patients clinically.

4.Distraction Osteogenesis after Membranous Onlay Bone Graft in a Dog Model.

The purpose of this study was to investigate the possibility of distraction osteogenesis in membranous onlay bone graft on the mandible and to clarify the histology of bone repair during distraction osteogenesis in the membranous onlay bone in a dog model. Four dogs, 5 months of age at the beginning of the experiment, were used for this study. The zygomatic arch was exposed in the subperiosteal plane and the full-thickness zygomatic arch was harvested to 3 centimeters in length. The lateral surface of the mandibular body was exposed in the subperiosteal plane and the membranous onlay bone graft was performed with firm contact using screws. The osteotomy on the membranous onlay bone graft and underlying mandibular body was carried down week 1 in dog 1, week 2 in dog 2, week 3 in dog 3, and week 4 in dog 4 after membranous onlay bone graft. The external distraction device was applied to the mandibular body. Mandibular distraction was started 7 days after the operation at a rate of 1mm per day for a total of 10 mm distraction over 10 days. After completion of distraction, the distraction device was left in place for 6 weeks bony consolidation of the distracted area. Radiographs were carried out at 2 weeks, 4 weeks and 6 weeks after distraction. New bone between the native underlying mandibular segments was generated in the distracted zone in all dogs. The new bone between the native underlying mandibular segments was generated in the distracted zone in all dogs. The new bone between segments of membranous onlay bone graft was not generated in dog 1, but it was generated in dogs 2, 3 and 4. However, in dog 2 and 3, the new bone between segments of the distracted membranous onlay bone graft presented less firmness with fibrous tissue than that of the native underlying mandibular segment. Histologically, the distracted gap between segments of the membranous onlay bone graft was composed of much fibrous tissue in the central zone while activated osteoblastic cells formed new bone in the margins of the distracted gap in dogs 2 and 3. In dog 4, there were abundant osteoblastic activities in the distracted gap and the new bone appeared as nearly-normal cortical bone. In conclusion, these findings suggested that membranous onlay bone graft had an osteogenic capacity and that distraction osteogenesis was possible in membranous onlay bone graft.

5.Clinical Results of 500 Cases of Intraocular Lens Implantation(I).

From May, 1985 to July, 1986, 500 cases of intraocular lenses were implanted, and all 500 cases had been followed up for more than 2 months. The results were as follows: 1. In the 500 cases of intraocular lens implantation, the number of patients were 318 men and 115 women. Patient's age was largely distributed to fifth decade in men and fourth decade in women. 2. IOL power ranged from +13.5D to +23.0D in PCL and ranged from +12.5D to +20.0D in ACL. The most frequently used IOL poeer was +19.5D in PCL and +18.0D in ACL. 3. Postoperative corrective vision with IOL was improved up to 0.8 or better in 446 cases(89.2%). 4. During operation, some complications were developed and those were posterior capsular rupture in 19 cases(3.8%), hyphema in 15 cases(3.0%), Descemet's membrane stripping in 2 cases(0.4%), and corneal edema in 1 case(0.2%). More frequently developed postoperative complications were clinical cystoid macular edema in 26 cases(5.2%), secondary glaucoma in 18 cases(3.6%), and distorted pupil in 13 cases(2.6%). 5. Postoperative corrective vision lower than 0.4 was observed in 22 cases(4.4 %), and the causes were diabetic retinopathy, cystoid macular edema, vitreous opacity, congenital cataract, phacoanaphylactic uveitis, endophthalmitis, amblyopia, optic nerve atrophy, corneal opacity, glaucoma, hypertensive retinopathy, and others.

6.A Case of Brown's Syndrome Following Tucking of Superior Oblique Muscle.

Brown's syndrome or the superior oblique tendon sheath syndrome is motility befect of the eye ball manifest by an inability to raise the adducted eye above the midhorizontal plane. The authors have operated on one patient, 5 years old female, who developad Brown's syndrome secondary to tucking of the superior oblique tendon. The surgery for relieving of the superior oblique tendon tucking and steroid injection on the tendon region were performed on the fourth day after the superior oblique tendon tucking. The symptoms of Brown's syndrome have been improved since the relieving of the superior oblique tendon. At 5 months after this surgery, the patient sbowed only mild hypotropia with A pattern and orthotropic state in the primary position.

7.Speckled Lentiginous Nevus.

Speckled lentiginous nevus is a clinical variant of nevus-cell nevus first described by Stewart et al. in 1978. It is characterized by small, dark hyperpigmentated speckles superimposed on a tannish-brown background. The speckled areas show varying histologic patterns ranging from nevus incipiens to junctional or compound nevus. The back ground shows histologic features of Ientigo simplex. Recently, we observed clinical and histological features of 5 cases of speckled lentiginous nevi. The age of onset ranges from birth to infancy, The locations are face, thigh, back and scapula. One has zosteriform distribution in upper extrernity, chest and back. Histologic features of speckles are junctional or compound nevus. We suggest that the origin of dark speckles may be from the tannish-brown background, lentigo simplex.

8.Effects of Halothane Anesthesia and Surgical Procedure on Human Growth Hormone , Insulin and Glucose Level in Plasma .

This study was undertaken to investigate the effects of halothane anesthesia alone on blood glucose metabolism in ten surgical patients by determining plasma growth hormone ,(HGH), insulin and glucose, and to compare these with the effects of halothane anesthesia plus surgery. Each patient was premedicated with diazepam, 5 mg and atropine sulfate, 0. 5mg intramuscu Jarly one hour before the induction of halothane anesthesia. After endotracheal intubatien had been carried out following SCC 30-40mg, anesthesia was mairrtained with halothane and oxygen. The muscle relaxant (gallamine) in divided doses was injected during surgery when needed and ventilation was controlled or assisted intermittently throughout the procedure. A moderate depth of anesthesia was maintained according to clinical judgement, based on signs including blood pressure, pulse rate and somatie reflex responses to the surgical xtimulation. Eight blood samples were obtained from each patient:( I ) at 09.00 a.m. immediately before :induction of anesthesia (this sample served as acoatrol value) (2) the next three were after 15. 30 and 45 minutes of halothane anesthesia but before the start of the operation; (3) further three, 15,30 and 45 minutes, after the start of the operation;(4) in the recovery room after fully awaking. HGH RIAKIT was utilized as a kit for determination of human growth hormoae by radioimmunoassay and also lNSULIN-RIAKlT for insulin levels in the blood, For the determination of blood glucose level, OTB method was utilized. The ratio between male and female pgtients was 5:5, mean age 39. 2+/-12. 1 and average body weights 55. 5+/-8 .5 kg. The mean control plasma growth hormone level(HGH) in 10 surgical patients after premedication, and immediately before induction of anesthesia was 2. 03+/-0. 28ng/ml. The mean yre-surgical concentration of HGH in plasma after 15, 30 and 45 minutes of halothane anesthesia rose to 2.63+/-0.41 ng/ml, 2.76+/-0.35 ng/ml, and 2.95+/-0.42 ng/ml, respectively but these values were not significantly elevated above the control value. The plasma levols significantly increased to 3. 42+/-0. 5 mg/ml (P <0. 05), 6. 64+/-0. 6 mg/ml (p <0.001) and 7.83+/-0,75 ng/ml. (p<0.001) 15 minutes, 30 minutes and 45 minutes respectively after the start of the operation. It decreased to 3. 58+/-0. 68 ng/ml. (p<0. 05) in the recovery room when the patient awoke fully. The mean pre-anesthetic control insulin level in plasma. in 10 surgical patients was 12. 6+/-2. 0 ng/ml which was within the normal range. This level. did not vary appreciably during halothane anesthesia alone, during operation or in the post-operative period. The mean control pre-anesthetic blood glucose level was 78. .7+/-3.9 mg/100ml. It rose to 80. 6+/-2. 9 mg/100ml. 83. 2+/-3. 6mg/100 ml 30minutes, 45minutes and of halothane anesthesia alone and 15 minutes of the start of the operation but these changes were not significant. It increased significanty to 98. 2+/-4. 4 mg/100ml(p<0. 05) and 103. 9+/-4. 0 mg/100ml (p<0. 001) 30 minutes and 45 minutes respectively after the start of the surgery but it rose to 95.7+/-3.1mg/ 100ml (p <0. 01) in the post-operative period. Consequently the plasma HGH level during halothane anesthesia alone for 45 minutes was slightly, increased and rose significantly during operation and in the post-operative period. The peak level was achieved 45 minutes after the start of surgery. Plasma insulin levels did not change appreciably during anesthesia alone or during surgery. Blood glucose levels increased slightly during anesthesia alone but rose significantly during operation and in the post-operative period.

9.Analysis of Urinary Calculi in Voided Urine after ESWL.

Accurate analysis of urinary calculi is fundamental for study of the etiology of stone formation and essential for treatment of urinary stone and its prevention. A total of 600 patients underwent treatments with ESWL for urinary tract calculi between May, 1987 and June, 1989. The stone fragments that were passed with 99 patient's urine and 21 stone calculi required surgical removal, were analyzed by infrared spectrometer. The following results were obtained : 1. Mixed calculus (58.3%) in much more than single calculus (41.7% ), The most common type of calculi is calcium oxalate-calcium phosphate (69 cases-57.5% ). The most common type of components is calcium oxalate (105 cases-87.5%). 2. Uric acid stones are found in acid urine, and most of magnesium ammonium phosphate stones are found in alkaline urine. 3. 17 of 120 cases have bacteriuria on culture and most of them are ureasplitting organisms. 4. 21 calculi of failed ESWL were analyzed and the components were calcium oxalate-calcium phosphate in 14, calcium oxalate in 4, magnesium ammonium phosphate in 2, uric acid in 1. 5. The causes of failed ESWL were stone components in 5 cases, impacted stone in 10 cases and poor stone localization in 6 cases.

10.Effect of the Early Traumatic Experience on the Mental Health of the Elderly.

This study was intended to analyze the relation between the psychic traumatic experience and the psychological health of the aged. The authors carried out this study by means of the combined anxiety-depression scale (CADS) and the preadolescence traumatic experience scale (PTES) with 278 aged men and women residing in Taegu from September to October 1988. The results were as follows: 1. Based on the scores evaluated by CADS, the scores of the both groups showed that comparative group was accounted for 40.15±6.19, while the experimental group for 57.75±6.37, which showed significantly higher score in the experimental group (p<0.001). 2. The experimental group showed significantly higher early experience score than the comparative group in the dietary difficulty, alcoholism among family members, disunion between husband and wife, trouble between mother and children, early mother loss, parent's indifference and unwanted birth (p<0.001). 3. The experimental group showed higher early experience score than the comparative group by sex, age, marital status and grown location (p<0.001). 4. When the subjects were included in the unemployed and in the middle or low classes and their parents were engaged in agriculture and commercial business and believing in buddhism or no-religion, showed higher experience score (p<0.001).
