1.Tuberculous Uveitis in a Hemodialysis Patient.

Tuberculous uveitis is a rare manifestation of tuberculosis infection. Although early diagnosis is important to saving the sight of patients, it is difficult. In dialysis patient, the diagnosis of tuberculosis is often delayed because of non-specific symptoms and extrapulmonary involvements. We report a case involving a 37-year-old man receiving hemodialysis who was presented with persistent fever and left- sided blurred vision. At ophthalmic examination, active papillitis, vitritis, and peripapillary retinal detachment were observed. Neck computed tomography showed multiple necrotic lymphadenopathies (LAPs) in right lateral neck, which was proven as tuberculous lymphadenitis on histological examination. After anti-tuberculosis therapy, fever was subsided and LAPs were improved. Visual acuity was also improved.

2.Laser and Light Sources in the Treatment of Onychomycosis.

Onychomycosis is a common fungal infection of the nails and there is wide distribution across the continent. Aside from Trichophyton rubrum (88%) being the main causative agent, other dermatophytes, yeasts or non-dermatophyte molds can affect nails and adjacent tissues and cause sundry symptoms: subungual hyperkeratosis, onycholysis, nail thickening, discoloration, malodorous condition. Oral antifungal agents are the mainstay treatment for onychomycosis but undesirable side effects, such as hepatotoxicity, remain a main reason for drug withdrawal. Moreover, other modalities as a reliable treatment option are disappointing with a low cure rate. The advancement in laser and light technology has opened a new field of opportunity for the treatment of refractory skin disease. As supported in clinical studies, neodymium-doped yttrium aluminium garnet laser, diode laser, fractional carbon dioxide laser, and erbium:glass laser have produced a favorable outcome, with or without additional agents, for the treatment of onychomycosis. But the mechanism is still unclear and reliable data are relatively scarce despite the potential as an alternative armamentarium. To date, as with laser therapy, there have been only a few studies conducted concerning the role of light therapy in the superficial fungal infection. Herein we aimed to review the medical literature on laser and light therapy of onychomycosis and provide our point of view for health providers and researchers.

Although left atrial appendage closure is an alternative to warfarin use in atrial fibrillation patients with a high bleeding risk, long-term outcome data in real-world practice are still limited. We describe a delayed shunt on the WATCHMAN fabric membrane and associated stroke at 7 months after the procedure. This shunt on the device surface disappeared at 15 months on the follow-up transesophageal echocardiogram.

7.Incidence Rate and Characteristics of Falls in Patients Using Comprehensive Nursing Care Service

BACKGROUND: Falls are the most frequent adverse events reported in hospitals. The aim of this study was to investigate the incidence rate and characteristics of falls in patients who used comprehensive nursing care service in National Health Insurance Service Ilsan Hospital. METHODS: Incidence rate of falls was investigated in patients using comprehensive nursing care service, from July 2013 to Jun 2017 and compared with those not using this service. The characteristics and risk factors for falls, and fall-related injuries were obtained. RESULTS: Among the 62,445 patients who used the comprehensive nursing care service for 4 years, total of 672 falls were reported. The incidence rate of falls per 1,000 patients-day was 1.15. The percentage of fall-related injuries was 26.9% and that of major injury was 2.2%. Although the incidence rate of all falls was slightly higher in patients using comprehensive nursing care service than those not using this service, falls-related injuries were not correlated with the implementation of this service. CONCLUSION: The falls could be more frequently detected and reported in comprehensive nursing care service, but there was no difference in fall-related injuries.

8.A Case of Pulmonary Capillary Hemangiomatosis-Like Lesion Followed for Seven Years.

Pulmonary capillary hemangiomatosis (PCH) is a rare disease of unknown etiology that is characterized by nodules composed of infiltrating capillary blood vessels. Herein, we describe a case of a PCH-like lesion that was detected by chest computed tomography. Transthoracic needle aspiration resulted in life-threatening hemorrhage. The patient was followed for seven years. He remained in good health and a follow up image showed little interval change.

9.Nutritional Risk and Physical Activity on Quality of Life in Patients with Colorectal Cancer.

PURPOSE: This study aimed to investigate nutritional risk, physical activity, and quality of life (QoL) and the interaction of these factors in colorectal cancer patients. METHODS: Participants were colorectal cancer patients receiving follow-up treatment at the colorectal cancer outpatient clinic of Severance Hospital in Seoul. Participants were recruited from October 25 to November 30, 2010. The survey questionnaire consisted of Nutritional Risk Score (NRS), International Physical Activity Questionnaire (IPAQ) and Functional Assessment of Cancer Therapy - scales for Colorectal Cancer (FACT-C). RESULTS: NRSs were significantly higher in patients with low BMI (F=18.63, p<.001) and those who were underwent colorectal surgery within 1 month (F=5.96, p<.001). Physical activity level of the participants was 28.96METs. Lower nutritional risk was associated with a higher QoL (r= - .34, p<.001). In a multiple regression analysis, the predictors of QoL were the age, economic status, and nutritional risk of the patient (R2=40.0%). CONCLUSIONS: Targeted tailored nursing interventions are needed to assess nutritional risks in order to improve colorectal cancer patients' QoL.

10.Role of KATP Channel During Sustained Ventricular Fibrillation.

OBJECTIVES: To investigate the role of ATP-sensitive potassium channel (KATP) during sustained ventricular fibrillation (VF), the effects of gliburide, a specific blocker of KATP channel and PCO400, an KATP opener, were studied in isolated and perfused swine right ventricular free walls (n=). METHODS: Recording of single cell transmembrane potentials was performed and constructed action potential duration restitution (APDR) curve by plotting APD 90%(APD90) versus preceding diastolic interval (DI). RESULTS: All isolated tissues fibrillated spontaneously. In this preparation, stable VF could persist over a 4-hour period if it was allowed to continue undisturbed (n=). Gliburide (1-5 uM) increased DI without significant changes in APD90 during VF, resulting in more regularization of VF. Higher concentration (10-20 uM) increased both APD90 and DI, and converted to monomorphic ventricular tachycardia (MVT) through the transitional period characterized by APD alternans. PCO400 (1-2.5 uM) caused a significant shortening of APD during MVT and a period of APD alternans became more evident before conversion from MVT to VF. Gliburide eliminated profibrillatory effect of PCO400. This antifibrillatory action of gliburide was accompanied by gradual decrease in the maximum slope of APDR curve during VF. CONCLUSION: KATP channel blockade causes a transition from VF to MVT via lengthening of DI and APD alternans, concomitantly with a reduction of the slope of APD restitution curve.
