Fifteen cases of genital warts were examined for the presence of human papillomavirus (HPV) using polymerise chain reaction (PCR). HPV6/11 DNA were found in all cases of genital warts. The PCR based methods described here provide a sensitive, accurate means of detecting genital HPVs.

4.Report of A Case of Renal Actinomycosis.

Actinomycosis, in which the principal causative agent in man is known to Actinomyces israelii, is a chronic, suppurative diseases characterized by extensive fibrosis, multiple abscesses, and formation of sinus tracts that drain suppurative exudates. On the basis of the anatomical sites involved; it can be subclassified into the cervicofacial form, which is the most common form, pulmonary form and abdominal form. Kidneys are rarely affected. Clinically, radiologically, and at operation it is difficult to differentiate the renal actinomycosis from renal tuberculosis and renal carcinoma. The prognosis is excellent after nephrectomy followed by appropriate antibiotic therapy. We presented a case of renal actinomycosis with a brief review of the literatures.

5.Effects of Etomidate and Midazolam on the Isolated Rabbit Abdominal Aorta and Pulmonary Artery.

Etomidate and midazolam are newly developed and used in clinical trials. Etmoidate, a carboxylated imidazole derivative, decreases systemic vascular resistance and increases the pulmonary artery pressure in vivo. Midazolam, a water soluble derivative of benzodiazepine, decreases pulmonary artery pressure and is useful for pulmonary hypertensive patients. This study was designed to investigate the direet effects of etomidate and midazolam on vascular tension of the rabbit abdominal aorta and the pulmonary artery in vitro. In the vascular preparations with or without endothelium, changes in tension were measured following cumulative administration of etomidate (10(-6)M, 10(-5) M, 5X10(-4) M) and midazolam (10(-6)M, 10(-5)M, 10(-4)M). Vascular effects of these drugs were also studied in the preparations pretreated with indomethacin, nitro(w)-L-arginine methyl ester (L-NAME) and methylene blue. The results wer as follows; 1) Etomidate and midazolam induced vasorelaxation and the degree of relaxation depended on the concentration. 2) After denudation of the endothelium, vasorelaxant effect of etomidate and midazolam was efficiently decreased in abdominal aorta but not in pulmonary artery. 3) Indomethacin reduced vasorelaxing effect of etomidate efficiently, but didn't affect vasorelaxing effect of midazolam. 4) Following pretreatment of vascular preparations respectively with L-NAME and methylene blue, the relaxing responses to etomidate (10(-5) and 5X10(-5) M) of both abdominal aorta and pulmonary artery were depressed. Also, depressed was the relaxing response of abdominal aorta to midazolam (10(-5) M). The results of present study suggest that etomidate and midazolam possess vasorelaxing effects in both rabbit aMominal aorta and pulmonary artery. The vascular effect of etomidate is mediated via the nitric oxide pathway and also in part, by PGI2, whereas part of the vascular effect of midazolam is associated with the nitric oxide pathway.

7.The Effect of the Valsalva Maneuver on the External Jugular Vein.

BACKGROUND: The external jugular vein (EJV) is a useful intravenous (IV) cannulation site for anesthesiologists, although it has a relatively high failure rate. Unlike other central veins, visualization of the EJV is important during IV cannulation, and the Valsalva maneuver distends the jugular venous system. However, the relationship between the maneuver and EJV visibility remains unknown. This study compared EJV visibility before and after the Valsalva maneuver. METHODS: This was a prospective observational study that included 200 participants. After the induction of anesthesia, EJV visibility grade, depth from the skin to the EJV superficial surface (EJV depth), and EJV cross-sectional area (CSA) before the Valsalva maneuver were measured. The same parameters were measured after the Valsalva maneuver was performed. The EJV visibility grade was defined as grade A: good appearance and good palpation, grade B: poor appearance and good palpation, and grade C: poor appearance and poor palpation. RESULTS: Patient body mass index and EJV depth affected the EJV visibility grade before the Valsalva maneuver (p < 0.05), although EJV CSA did not. The Valsalva maneuver distended EJV CSA and reduced EJV depth, although these changes were not correlated with EJV visibility grade. With regard to EJV visibility, 34.0% of grade B cases and 20.0% of grade C cases were improved by the Valsalva maneuver. CONCLUSIONS: Although the Valsalva maneuver improved EJV CSA and EJV depth, it did not greatly affect EJV visibility grade.

8.A Clinical Study of Buoy Flap

Buoy flap was useful method for soft tissue defect, bone defect or bone and soft tissue defect. Also the buoy flap could use monitoring for blood flow. The advantages of the buoy flap were long length of perforating cutaneous branch, large diameter of Peroneal vessel, transposition in same leg and the another advantage was that the fibula is the long, straight and mainly cortical bone. To obtain successful result of buoy flap, one must have the anatomic concept for vascular pattern (straight branch, proximal oblique branch, branch from muscular branch, distal oblique branch), relationships with surrounding muscles (between Peroneus m. and Soleus m., Through Peroneus m., through Soleus m.) and length of perforating branch. So, we studied total 19 person, 22 cases of buoy flap, who were admitted and took the operation at Kyung Hee University College hospital, from October 1985 to March 1987.

9.Cortical Dysplasia of Brain: 2 Autopsy Cases.

Cortical dysplasia is used to designate any cerebral developmental malformation, and encompass a spectrum of pathologic changes from mild cortical disruption to more severe pattern of cortical dyslamination with large bizarre neurons, balloon cells, and astrocytosis. The gross appearance of the brian reveals focal or diffuse cortical thickening with no clear cortical-white matter junction due to excessive spillover of abnormal neurons. Cerebellar cortical dysplasias are uncommon but has been reported in cases with more widespread cerebral malformations. We present two autopsy cases of cortical dysplasia, one of which involves both cerebrum and cerebellum. Case 1 was a 4-year-old boy and presented with severe intractable epilepsy of neonatal onset , cerebral palsy, microcephaly, and severe neurologic deficits. Both cerebrum and cerebellum were markedly reduced in size. Microscopic examination of cerebrum showed disorganized cortical architecture with abnormal neurons in the cortex and white matter. The cerebellum disclosed massive spillover of Purkinje cells into white matter. Case 2 was a 51-year-old woman and found in dead in her house. No previous clinical history was informed. At autopsy, no gross abnormality of the brain was observed, but microscopic examination showed giant, dysmorphic neurons throughout the cerebral cortex.

10.One Case of Benign Lichenoid Keratosis.

We present a case of BLK developed in a 43-year-old Caucasian female. She had an asymptomatic, 4mm-sized, scaly erythematous papule on her left forearm. On histologic examination, this case showed a focal parakeratosis, hyperkeratosis, focal hypergranulosis and basal vacuolar degeneration in the epidermis, as well as band-like lymphohistiocytic infiltration with melanophages and solar elastosis in the dermis. The skin lesion was totally excised and no recurrence was noted.
