2.Utility of 99mTc-MAG3 Perfusion Indices in the Evaluation of Renal Transplant Function During Early Post-transplantation Period.

PURPOSE: We have examined the utility of 99mTc-MAG3 perfusion indices for assessing renal graft function in early post-transplantation period. MATERIALS AND METHODS: Our study included 80 renal transplant recipients (48 men and 32 women, mean age: 40.3 years). Diagnosis was based on biopsy, laboratory data and clinical course. Renal scintigraphy (RS) was obtained using 100 MBq of 99mTc-MAG3 from 11 days to 23 days of kidney transplantation. We measured 5 indices in whole-kidney (WK) and cortical (C) renograms; Hilson's perfusion index (PI), transplant perfusion index (TP) and transplant function index (TF) as perfusion parameter, and the time to peak activity (Tmax) and the ratio of renal counts at 20 min to that at 3 min (K20/3) as functional parameter. RESULTS: The diagnoses at the day of RS were normal graft (NG) in 44, acute rejection (AR) in 14, acute tubular necrosis (ATN) in 10, and Cyclosporine A nephrotoxicity (CsA) in 12. TP and TF were significantly decreased in AR, ATN and CsA, compared to those in NG. K20/3 of AR and ATN were significantly greater than that of NG. WK-Tmax of AR was significantly longer than that of NG. K20/3 of AR and C-K20/3 of ATN were significantly prolonged relative to those of CsA. There were no statistically significant perfusion indices among complication groups. CONCLUSION: TP and TF reflecting microperfusion and initial tubular extraction are reliable in assessing graft function. However, it is required to correlate perfusion indices with functional indices and clinical course in differentiating from one another among complication groups.

3.3-dimensional one point foxation with x-shaped miniplate in the cases of the zygomatic fracture.

Recently the internal rigid fixation with plates and screws became popular procedure. In the cases of the zygomatic fracture, it is sufficient to fix more than 2 point in many studies. Therefore, the internal rigid fixation requires the aggressive incision to expose the zygoma, such as coronal incision. Since many patients are afraid of this aggressive operation, we have provided a simple and effective method which is a 3-dimensional one point fixation on the zygomaticofrontal suture with X-shaped(6-hole) miniplate for the treatment of zygomatic fractures. From March 1995 to september 1995, we treated 13 cases of zygomatic fractures by open reduction and X-shaped miniplate fixation on the zygomaticofrontal suture. The results were as follows: 1) plane to plane contact on each side of the fracture with one point X-shaped miniplate. 2) 91.8% correction on Water's views, and 93.3% correction on zygomatic arch view with one point fixation. 3) patients were satisfied with these procedures because of minimal incision, mild edema and low operation price (short operation & anesthetic time, one plate & 6 screws cost).

4.A Study on Psychoticism in College Students.

The authors studied Psychoticism, using SCL-90, in the subjects of 3,893 male college students and 1,976 female college students of Yeung Nam University, collecting data during the periods from October to November, 1984. The results could be summarized as follows: 1. There was significant difference in the mean averages of total psychoticism scores between male and female students; male students scored 6.81±5.13 female students scored 8.14±5.05. 2. Eighty eight male students (2.4%) showed seriously high psychoticism scores of 20 or higher, while 63 female students (3.2%) showed the same scores. 3. In a comparison between male students in different school grades, freshmen showed the highest level of psychoticism scores, and as the grades of students became higher, the levels of psychoticism scores were lower. 4. Among the psychoticism factors, there were strong tendencies toward higher psychoticism scores in the students who were dissatisfied with their college, and had pessimistic views of self-image in the past, present or future, in both groups. 5. The male students whose educational fees were paid by their parents, and whose spending money per month was under 10,000 won, showed higher level of psychoticism scores. 6. The female students whose maturation locations were city, who were dissatisfied with their departments, who resided in dormitory of other residencies, and whose educational fees were paid by their brothers or sisters, showed higher level of psychoticism scores. 7. The items relating to 5 (Feeling lonely with people) 9 (Never feeling close to another) 10 (Idea something writh mind) in psychoticism and the item of depressed affect in SDS were significantly correlated over 0.40 of correlation coefficient.

The authors studied neuroticism, using Eysenck's Maudsley Personality Inventory (MPI), in 3,893 male college students and 1,976 female college students of Yeungnam University. There was significant difference in the mean averages of total neuroticism scores between male and female students, male students scored 13.45±.44, female students scored 16.89±7.19. The items relating to (16) Do you sometimes feel happy, sometimes depressed, without any apparent reason? (38) Do you daydream a lot? (1) Do you have dizzy turns? (2) Do you get palpitations or thumping in your heart? And (35) Do you worry about your health? were scored highly in male, and the items relating to (16) Do you sometimes feel happy, sometimes depressed, without any apparent reason? (1) Do you have dizzy turns? (38) Do you daydream a lot? (8) Do you worry too long over humiliating experiences? (9) Do you consider yourself rather a nervous person? Were scored highly in female. Three hundreds and ninety eight male students (10.3%) showed seriously high neuroticism scores of 24 or higher, while 256 female student (13.0%) showed high neuroticism scores of 26 or higher. In a comparison between male students in different school grades, freshman showed the highest level of neuroticism scores. Among the psychosocial factors, there were strong tendencies toward higher neuroticism scores in the students who were dissatisfied with their colleges or departments, and who had pessimistic views of self-image in the past, present, or future in both groups. The male students whose educational fees were paid by their parents, and whose spending money per month was under 10,000 won, showed higher neuroticism scores. The female students whose educational fees were paid by their brothers or sisters, and whose maturation locations were U-P, and who resided in dormitory or other residencies, shoed higher neuroticism scores.

6.Primary Signet Ring Cell Carcinoma with Widespreand Metastasis.

We present an autopsy case of signet ring cell carcinoma of urinary bladder which underwent widespread metastsis through nearly entire organ. The patient, 45-year old male, suffered from hematuria, dysuria and flank pain for 6 months. Under the diagnosis of the signet ring cell carcinoma by the cystoscopic biopsy, radical cystectomy and ileocutaneous ureteral division were performed. But he died after 11-months after the diagnosis. The autopsy confirmed the widespread metastasis at the lung, kidney, adrenal gland, parathyroid gland, heart, stomach, ileum, spine, rib and meninges. The preveously resected urinary bladder discolosed the well defined fungating mass at the dome and anterio-lateral portion without apparent evidence of the remnant of the urachus. Histologically, the signet ring cells were predominent and focally mucinous pool floating the signet ring cells was noted. We could find several foci of the well differentiated adenocarcinoma and poorly differentiated adenocarcinomas. The above findings may reflect that the origin of carcinoma may be from the multipotential transitional cell epithelium.

9.CT diagnosis of the fat containing mediastinal masses

Fat containing masses, except mediastinal lipomatosis, of the thorax are uncommon. In spite of uncommonoccurences, as CT can detect not only the fat content but also the presence and character of the non-fattycomponent of the mass, accurate preoperative diagnosis can usually be made in most cases of fat containingmediastinal masses. Authors report 6 cases of fat containing mediastinal masses, that were diagnosedpreoperatively by CT scan, comprising 2 cases of limpoma(combined with plexiform neurofibroma in a case), 1liposarcoma, 1 thymolipoma, and 2 cases of omental hernia through Morgagni foramen.

10.Factors Influencing Self-Rated Oral Health in Elderly People Residing in the Community: Results from the Korea Community Health Survey, 2016

The aim of this study was to examine the factors influencing perceived oral health in elderly individuals residing in the community. This study used raw data from the Korea community health survey, 2016. Of the 64,223 participants that were elderly (aged ≥ 65 years), 61,280 (95.4%) were included for analysis. Self-rated oral health was the dependent variable and 6 independent variables including age, gender, type of area of residence (metropolitan or provincial), educational level, income, and living status with spouse were assessed. Oral function was studied based on mastication, pronunciation, and use of dentures, and oral health behavior included brushing teeth after breakfast, after lunch, after dinner, and before sleep). The EQ-5D questionnaire measured health-related quality of life (mobility, self-care, usual activities, pain/discomfort and anxiety/depression). Among the general characteristics, age, gender, educational level, income, and living status with spouse were the factors that affected self-rated oral health. Mastication, pronunciation, use of dentures, and brushing after lunch, dinner, and before sleep were the factors that influenced self-rated oral function. All domains of the EQ-5D (pain/discomfort, mobility, self-care, usual activities, and anxiety/depression) were factors that affected self-rated oral health. The results of the current investigation suggest that the development of management and education strategies for oral health promotion in the elderly, should focus on improving oral function and oral health behavior, taking into account the socio-economic and demographic characteristics that have been shown to be associated with poor self-rated oral function.
