2.Studies on Plasma Renin and Aldosterone in Essential Hypertension.

Plasma renin activity (PRA) and aldosterone were measured to evaluate various status of clinical conditions in normal controls and patients with essential hypertension. Fourteen controls and 45 essential hypertensive patients for PRA, and 10 controls and 28 patients with essential hypertension for plasma aldosterone were studied. The results obtained are summarizing as follows. 1. Mean value of PRA of normal subjects was 2.9+/-1.4ng/ml/hr at rest, and 5.9+/-2.3ng/ml/hr by double stimulation, respectively, the latter elevated up to twice the former. 2. Mean value of PRA in the patients with essential hypertension was 3.37+/-3.0ng/ml/hr at rest, and 5.99+/-4.9ng/ml/hr by double stimulation. The latter have increased to 1.8 times the former. 3. The patients with low PRA observed in 33.3%, the normal in 42.2%, and the high in 22.5%, at rest. After double stimulation, the low group was observed in 22.2%, the normal in 60.0%, and the high in 17.8%. 4. Mean age of the patietns with low PRA was 57.2+/-2.6 years. 4.THE SHEAR BOND STRENGTH OF RESIN TEETH TO THE DENTURE BASE RESIN.

The bond failure of resin teeth to denture base resin is one of the failure in prosthodontic treatment. The purpose of this study was to evaluate the bond strength of artificial resin teeth to the denture base resins. Specimens were made with heat curing acrylic resins (Vertex Rs, Lucitone 199) and artificial resin teeth (Tiger, Trubyte biotone, Endura, Orthosit, Trubyte bioform IPN) and the bond strength were measured with testing machine (Zwick, Germany) and the mode of bond failure were observed. The results were as follows; 1. The bond strength of Vertex Rs to artificial resin teeth was the highest in Trubyte biotone, and It was the lowest in Tiger and Trubyte bioform IPN 2. The bond strength of Lucitone 199 to artificial resin teeth were higher in Orthosit and Trubyte bioform IPN than in Tiger and Trubyte biotone. 3. The bond strength of Trubyte biotone to Vertex Rs was higher than to Lucitone 199 and that of the Trubyte bioform IPN to Lucitone 199 was higher than to Vertex Rs. 4. Nearly all bond failures of specimens occured cohesively within the resin teeth.

5.The Amount of Physical Activity According to the Level of High Density Lipoprotein Cholesterol in Korean Adults.

BACKGROUND: A low level of high-density lipoprotein-cholesterol (HDL-C) is well known as an important cardiovascular risk factor which is closely associated with physical activity levels. However, there is a very few studies conducted on relationship between physical activity and HDL-C status among Korean adults. METHODS: We selected total 22,294 people aged 19 to 69 years old who had not taken lipid lowering agents from the 2005 Korea National Health and Nutrition Examination Survey (NHANES) data. The population was classified into three groups using the criteria for HDL-C of Adult Treatment Panel III. The relationship between the physical activity and HDL-C was investigated and the amount of physical activity levels of the three HDL-C groups were obtained with analysis of covariance with adjustments for various factors influencing the physical activity levels. RESULTS: After adjusted for various factors influencing HDL-C, physical activity is significantly associated with HDL-C. As physical activity is increased, HDL-C is much higher (P<0.001).The physical activity levels were significantly different among the three HDL-C groups. The amount of physical activity in low HDL-C group was significantly low compared to high HDL-C group (1653.08+/-65.31 MET-min/wk vs. 1916.70+/-102.86 MET-min/wk, P<0.001). CONCLUSIONS: We confirmed that the amount of physical activity level in low HDL-C group was significantly lower compared with that of high HDL-C group in Korean adults.

6.Computerization of Reporting and Data Storage Using Automatic Coding Method in the Gastrointestinal Endoscopy.

The authors developed a computer program for use in report printing as well as data storage and retrieval for the gastrointestinal endoscopy service. This program used IBM PC XT and was written in dBASE III plus language. We applied the automatic SNOMED coding method, which is one of the most efficient and accurate method of computerization of medical data. The working sheet which contained the results of previous endoscopic studies could be printed during registration. The dBASE word processor enabled issuing of the formal report of endoscopic result, and the data storage was carried out during the typewriting of the report. Two kinds of data files were stored in the hard disk; the temporary file contained full informations and the permanent file contained patients identification data and SNOMED code. Searching of a specific case was performed by chart number, patients name, date of study, or SNOMED code within a second. All the cases were arranged by SNOMED codes of procedure, topography and morphology codes. Every new data was copied to the diskette automatically. with which data could be restored in case of hard disk failure. The main advantages of this program in comparison to the large main frame computer system are low price, flexibility and easy accessibility. Based on our experience (including surgical pathology department, radiology, clinical pathology), we assume that this program may fit every endoscopy room where there are less than 20,000 cases per year.

7.Remodelling of Patellar Ligament Autograft after ACL Reconstruction: A Histological and Electron Microscopic Study

A histological and electron microscopic study were conducted in order to observe a morphological changing pattern of patellar ligament autograft after endoscopic ACL reconstruction. Seventeen patients among 117 consecutive series which underwent endoscopic ACL reconstruction from January 1990 to March 1995, were involved in this study according to the various follow-up periods. In second-look arthroscopic finding at 6 months follow-up after ACL reconstruction, reconstructed ACL was covered with synovium and round-shaped, and after the biopsy active bleeding could be seen. In histological study, 6 months follow-up, spindle or ovoid shaped fibroblast with hypercellularity, high crmp patterns of collagen fiber, and parallel arrangement of fibroblast to collagen fiber were noted. The cellularity of fibroblast decreased in according to periods of follow up. At 12 months follow up, spindle or ovoid fibroblast, its cellularity and arrangement pattern and crimp pattern of collagen fiber seemed to be almost similar to those of normal ACL. This similaryity continued up to 2 years and 2 months after reconstruction. In EM study, until 6 months follow-up after reconstruction, the graft showed randomly arranged fibroblasts with hypercellularity, which had abundant cytoplasm with marked irregular cytoplasmic borders and prominent nuclear indentation, Collagen fibrils were immature and showed a unimodel distribution with small diameter. At 12 months follow-up, elongated fibroblasts appeared, but these cells contained folded nuclei and abundant cytoplasm. Collagen fibrils had parallel arrangement and sparse cross striation, and comprised of mainly small fibrils and scattered large fibrils. After 24 months follow-up, the cellularity decreased more. The regular arrangement of collagen fibrils with wavy pattern similar to normal anterior cruciate ligament was noted. Two distinctive population of the small and large diameter of fibrils was also observed (bimodal distribution). At 26 months, the large and small collagen fibrils compresed a characteric bimodal pattern with parallel arrangement, but the majority of fibroblasts had irregular cytoplasmic contour, and abundant cytoplasm with increased organelles, which indicated the active function. These findings suggest that the graft are still immature even at 26 months in postoperative follow up despite considerable maturation of collagen fibril.

10.Clinical Observation on the Effect of Parenteral Reserpine.

Parenteral reserpine was given intramuscularly to 32 hospitalized hypertensive patients: 10 hypertensive patients without renal insufficiency, 3 hypertensive patients with heart failure, 10 hypertensive patients of malignant phase or with uremia, and 9 hypertensive patients with cerebrovascular accident. Follwoings were the result. 1. In the majority of patients, the effective dose of reserpine was 2 to 3 mg. 2. Reserpine given intramuscularly lowered blood pressure in 2 to 4 hours, had its maximum effect in 3 to 6 hours and had a duration of 3 to more than 24 hours (average 9 hours). 3. When effective dose of reserpine was given, blood pressure was lowered significantly (more than 30mmHg in mean blood pressure) in 18 patients (81.7%) of 22 hypertensive patients without renal insufficiency, and in 4 patients (40%) of 10 hypertensive patients with renal insufficiency. 4. Major side effect was drowsiness which was more evident in the patients with renal insufficiency. 5. Reserpine administered parenterally is an effective and safe agent for the treatment of hypertensive emergencies on a short term basis especially in the patient without renal insufficiency.
