1.The Effects of Inhalation Method U sing Essential Oils on the Preoperative Anxiety of Hystrectomy Patients.

The purpose of this study was to elucidate the effects of inhalation method using essential oils on the preoperative anxiety of hystrectomy patients, and to provide effective and holistic nursing care to them. The research design was a nonequivalent control group non synchronized design. The data were collected from February 1 to March 31, 2002 at D Medical Center in Busan. The subjects were forty one patients that were operated on under general anesthesia for hystrectomy. They were assigned to two groups, twenty one subjects in the experimental group and twenty subjects in the control group. The tool of the Visual Analogue Scale(VAS) was used to anxiety on all patients the day before surgery and the preoperative period Then systolic and diastolic blood pressure. pulse rate levels were measured the day before surgery and the preoperative period. The experimental group received two treatments of inralation method using essential oils of with lavender, ylang ylang, and bergamot oil. The data were analyzed by be chi test and the independent t-test The results of this study were summarized as follows: 1. Hypothesis 1: It has been supported that the experimental group received inhalation method using essential ons might cause lower level of the preoperative VAS anxiety than that of the control group(t = -2. 93. p = .006) 2. Hypothesis 2: It has been rejected that the experimental group received inhalation method using essential oils might cause lower level in the preoperative systolic blood pressure than the control group(t=-.120, p= .905). It has been rejected that the experimental group received inhalation method using essential oils might cause lower level in the preoperative diastolic blood pressure than the control grouP.(t=1.766, p=.085). 3. Hypothesis 3. It has been supported that the experimental group received inhalation method using essential oils might cause lower level in preoperative pulse rate than the control group(t=5.853, p=.000) According to these results. inhalation method using essential oils can be considered an effective nursing intervention that relieves the preoperative anxiety of hysterectomy patients and stabilizes vital sign partially.

2.The Clinical Study of Congenital Syphilis in Low Birth Weight Infants.

The clinical study was made on 52 cases who had treated for congenital syphilis at Chonnam University Medical School Hospital. Of these, 39 cases were admitted to Pediatric Department of C.U.M.S.H. from Jan. 1971 to Dec. 1978 via out patient dispensary and 13 cases were born in C.U.M.S.H. form Jan. 1974 to Dec. 1978. The results were summarized as followings; 1) 8% of the total low birth weight infants and 0.5% of the infants who weighed over 2,500gm at birth were diagnosed as congenital syphilis. 2) In sex distribution 36 were male and rest of them were female. 3) 70% among congenital syphilitic patients in low birth weight infants had prematurity. 4) The first clinical sympton appeared 89.4% within the fourth week after birth, 53.9% in the first week and 9.6% after the fourth week. 5) The most frequent clinical manifestation was hepatomegaly(79%), next in order of skin manifestaion(72%), anemia(60%), splenomegaly(48%), rhinitis and snuffle(48%). 6) In V.D.R.L. test, 96% of the patient, 98% of the mother and 72% of the father revealed reactive response. 7) Bony changes of roentgenographic examination were detected in 47 cases(90%). 8) 16 cases of congenital syphilitic patients in low birth weight infants(52 cases) and 8 cases of congenital syphilitic patiets who weighed over 2,500gm at birth were died and the most common cause of death in the patients of low birth weight infants was prematurity.

4.Clinical Studies on Purpura in Children.

The following results were obtained through the clinical review of 182 cases who were admitted with purpuric manifestation at the Pediatric Department of Chonnam National University Hospital during the four-year period, January 1975 through Decemver 1978. 1. The incidence was in the order of leukemia 50 cases(27.5%), sepsis 33 cases(18.1%), meningococcemia 33 cases(18.1%), allergic purpura 29 cases(15.9%), aplastic anemia 19 cases(10.5%) and I.T.P. 18 cases(9.9%). 2. By age distribution, 69.2% of the patients was less than 8 years of age. Male to Female ratio was 1.5:1. 3. The most common symptoms were purpura, fever, anemia and epistaxis. 4. In the laboratory findings; Platelet count was below 50,000/mm(3) in I.T.P.(66.7%), leukemia(68.0%), aplastic anemia(89.5%) but was above 150,000/mm(3) in allergic purpura(93.1%). Bleeding time was prolonged above 4min. in thrombocytopenic purpura but was within normal limits in non-thrombocytopenic purpura.

6.Normal Value of Fetal hemoglobin Concentration in Children.

A study of normal value of fetal hemoglobin concentration at each age in 1213 cases (852 patients, 361 normal children) who visited Chonnam university hospital from June 1977 to November 1978 was performed by alkali denaturation method, and following results were obtained. 1) The mean value of Hgb F at each age in normal children group was 65.5+/-1.8% in newborn infants during 24 hours, 40.5+/-1.6% in 1 month, 24.3+/-1.2% in 2 months, 15.3+/-1.4% in 3 months, 4.5+/-1.2% in 1 year 1.3+/-0.5% in 5 years, 0.9+/-0.7% in 10 years, 0.5+/-0.3% in 15 years. 2) The mean value of Hgb F at each age in patients group was 68.6+/-2.5% in newborn infants during 24 hours, 42.5+/-1.7% in 1 month, 22.9+/-1.1% in 2 months, 15.2+/-0.9% in 3 months, 5.5+/-1.0% in 1 year, 1.4+/-0.4% in 5 years, 0.9+/-0.6% in 10 years, 0.6+/-0.2% in 15 years. 3) The mean value of Hgb F at each age in both group was 67.5+/-2.0% in newborn infants during 24 hours, 41.8+/-2.1% in 1 month, 23.4+/-1.9% in 2 months, 15.2+/-1.0% in 3 months, 5.1+/-1.1% in 1 year, 1.4+/-0.8% in 5 years, 0.9+/-0.2% in 10 years, 0.6+/-0.2% in 15 years. 4) No significant correlation was found in the concentration of Hgb F at each age between normal children group and patients group, and either in the concentration of Hgb F at each sex of two groups.

7.Tussive Effect of Intravenous Fentanyl Administration and Antitussive Effect of Lidocaine.

BACKGROUND: Cough suppression is a well-known side effect of all opioid analgesics, but we experienced cough response after small dose of intravenous fentanyl. The goal of this study was to demonstrate the tussive effect of intravenous fentanyl. Method: 311 patients (ASA class 1, 2) were assigned randomly into three groups. Patient in group 1 recieved fentanyl 1 microgram/kg, patients in group 2 were given fentanyl 2 microgram/kg, and patients in group 3 recieved fentanyl 2 microgram/kg 2 min after lidocaine 1 mg/kg. We observed for 1 min after fentanyl injection to detect a cough response. RESULTS: Tussive response of fentanyl is produced by low dose (1~2 microgram/kg) fentanyl IV, and decreased by lidocaine pretreatment. This response is decreased with aging. All event is not related bronchospasm and anticholinergic premedication. CONCLUSION: We concluded that fentanyl causes tussive response and this response is inhibited by lidocaine and decreased with aging.

8.Clinical and Neuropsychological Characteristics of Adult Alcohol Use Disorder Patients with or without Attention-Deficit Hyperactivity Symptom.

OBJECTIVES: Alcohol use disorder (AUD) is often comorbid with adult attention-deficit hyperactivity disorder (ADHD) and other psychiatric illnesses. When associated with other mental problems, the prognosis of the AUD can be more serious. This study shows research on the clinical and neuropsychological characteristics according to whether or not ADHD symptoms and AUD were comorbid. METHODS: MethodsA total of 64 adult inpatients who completed AUD scales about adult ADHD, alcohol dependence, depression, anxiety, and impulsiveness. They also completed neuropsychological tests about attention and executive function. According to the Adult ADHD Self-Report Scale score, patients were categorized into two groups (ADHD symptom positive/negative group). RESULTS: Fourteen among the 64 subjects were part of the ADHD symptom positive group (21.9%). They had statistically significant shorter abstinence periods and a higher rate of history of 'rule violation during school' and 'physical damage in childhood' compared to the ADHD symptom negative group. CONCLUSION: The comorbidity rate with ADHD symptom in adult AUD is higher than the general population. The ADHD symptom positive group suffered from more severe and refractory AUD. Considering the more frequent history of rule violation during school in the ADHD symptom positive group, the association between ADHD and AUD may be mediated by conduct disorder. Therefore we suggest the necessity of careful evaluation and intervention in children and adolescents with ADHD and conduct disorder.

9.Congenital Hypothyroidism.

We made a clinical observation of 17 cases of congenital Hypothyroidism at the Pediatric department of Yonsei University hospital in Seoul, Korea during the period from January, 1969 to August, 1979. The results were as followings : 1) Male to female sex was about 1 : 1.1, and 6 cases (35%) of congenital hypothyroid-sm were diagnosed under 1 year old. 2) In etiological classificattion, aplastic or hypoplastic glands was 7 cases (41.2%) and normal or enlarged gland, 6 cases (35.3%) and ectopic thyroid tissue, 4 cases (23.5%). 3) In all cases, height and bone age retarded below normal range. 4) All cases were improved slightly after receiving dessicated thyroid and sodium-1 thyroxin therapy. Three cases had regalned normal growth and development after 6 months of treatment.
