Hereditary anhidrotic ectodermal dysplasia is a rare condition characterized by underdeveloped ectodermal structure including the skin, teeth or skin appendages. The patient has characteristic feature of anhidrosis, hypotrichosis and defective dentition. We experienced a case of hereditary anhidrotic ectodermal dysplasia in a l-month-old male infant who had unexplained recurring fever, anhidrosis and characteristic facial feature, so we established the diagnosis with clinical feature and skin biopsy.

PURPOSE: To evaluate the efficacy of combined treatment of metallic biliary stent placement and radiotherapy in malignant biliary obstruction. MATERIALS AND METHODS: Between January 1992 and February 1994, 22 patients with malignant biliary obstruction were treated with metallic biliary stent placement and radiotherapy. The causes of the obstruction included cholangiocarcinoma(n=14), gallbladder carcinoma(n=4), colon cancer(n=2), pancreatic head cancer (n=1), and stomach cancer(n=1). According to the sites of obstruction level, patient were divided into 2 groups; hilar obstruction(n=18) and common bile duct obstruction(n=4). The patients received dose of 45 Gray/18 fraction by external radiotherapy and 20Gy/8f by high dose rate brachytherapy with iridium--192 source through percutaneous transhepatic biliary drainage(PTBD) catheter. The duration of stent patency after placement, survival period and survival rate by Kaplan Meier method were calculated in dead and alive patients, respectively. RESULTS: In all 22 patients, the duration of stent patency was 5.5(1.3--18.5)months. Survival periods after stent placement and PTBD were 5.3(2.0--8.5 )months and 7.9(4.0--14.7)months respectively in 9 dead patients and 7.7(1.3--21.0)months and 9.5(2.3--23.0)months in 13 alive patients. In all 22 patients, the survival rates in 6, 12 and 18 months were 78.9%, 47.5 % and 35.6 %, respectively. CONCLUSIONS: The results with combined metallic biliary stent placement and radiotherapy for the palliative treatment of malignant biliary obstruction in this study was more favorable, as compared with those of the other authors with only metallic biliary stent placement.

T-cell acute lymphoblastic leukemia (T-ALL) accounts for approximately 10-15% of entire ALL in children. The outcome of T-ALL has been improved through the intensified therapeutic strategy, however, it is still a more aggressive disease. In T-ALL a couple of transcription factor oncogenes are known to be relocated to the juxtaposition of T-cell receptor genes, potent promoter, by chromosome translocation. However the incidence of each chimeric gene formation in T-ALL is less than 5% and their clinical significance as a prognostic marker is lacking. A decade ago it was identified that activating mutations in NOTCH1 in about 60% of T-ALL. After then, activating NOTCH1 mutations present in T-ALL have been extensively investigated with regard to understanding its molecular pathogenesis, its prognostic significance, and developing molecularly tailored novel agents. Small molecule gamma-secretase inhibitor, blocking a proteolytic step required for creation of a fragment of NOTCH intracellular domain which actually act as a controller of its target gene expression, was tried as a target therapeutic drug for T-ALL. Although outcome of this drug was not satisfactory, challenges have been launched to develop new drugs which specifically act on the aberrant behavior of mutated NOTCH1 in T-ALL.

PURPOSE: To evaluate the usefulness of real-time ultrasound-guided biopsy as an aid in diagnosing pleural attached lung lesions. MATERIALS AND METHODS: Over a 2-year period (March 1992 through February 1994), 52 US-guided diagnostic biopsies were performed in 50 patients with peripheral lung lesions in contact with chest wall. 15 cases were performed with gun biospy and 37 cases with fine needles. Final diagnosis was confirmed by surgery and clinical follow-up. RESULTS: In 44 of 52 cases(84.6%), we obtained histologic or cytologic material by US-guided biopsy. Diagnostic accuracy and tissue type specific accuracy for malignancy were 83.9%(26/31) and 67.7% (21/31), respectively. Eighteen case% were reported to be benign and one of them was proven to be metastatic lesion. We could not find any complications such as hemoptysis or pneumothorax. CONCLUSION: Ultrasound-guided biopsy is a simple, safe and accurate diagnostic procedure for evaluation of peripheral lung lesion attached to the pleura.

PURPOSE: We investigated the impact of treatment on electroencephalogram (EEG) findings, and determined treatment efficacy according to EEG findings and antiepileptic drugs in children and adolescents with recurrent headaches. METHODS: We retrospectively analyzed the medical records of 131 patients, aged 5–18 years, with recurrent primary headaches and performed EEG study who visited the Chosun University Hospital Department of Pediatrics from January 2014 to December 2016. Headaches were classified according to the International Classification of Headache Disorders-III (ICHD-III, beta version), and EEGs were analyzed for changes after treatment for primary headache. RESULTS: Among 131 patients, we successfully collected completed all the data on 30 patients (18 boys, 12 girls). The frequency of abnormal EEG findings before treatment was not significantly different according to the type of primary headache (P= 0.390). The mean frequency (P=0.001), duration (P=0.002), and intensity of headaches (P < 0.001), and disability due to headache (P=0.003) were significantly decreased after treatment in patients with epileptiform discharges on EEG. The mean frequency and intensity of headaches and disability due to headache (P < 0.005) was also significantly decreased in the patients with both slow and normal EEG findings. The mean frequency (P=0.007), duration (P=0.01), and intensity (P < 0.001) of headaches, and disability due to headache (P=0.002) were significantly decreased after treatment with antiepileptic drugs in patients with epileptiform discharges. CONCLUSION: Abnormal EEG findings were no significant differences in terms of type of primary headaches. Our results suggest that antiepileptic drugs may alleviate headaches in patients with epileptiform discharges on EEG.

PURPOSE: The purpose of this study was to analyze the histologic concordance of sonographically guided core needle biopsy for phyllodes tumors or fibroepithelial lesions with cellular stroma of the breast by comparing this with the outcomes of excision biopsy and to identify any sonographic features that are helpful to predict phyllodes tumors. MATERIALS AND METHODS: We retrospectively reviewed 60 breast masses that were diagnosed as phyllodes tumors (n = 43) or fibroepithelial lesions with cellular stroma (n = 17) on a sonographically guided core needle biopsy. The tumors were all subsequently excised by surgery. The sonographic features were compared between the phyllodes tumors and the non-phyllodes tumors according to the results of excision biopsy. RESULTS: By the results on excision biopsy, there were 48 (80%) phyllodes tumors and 12 (20%) non-phyllodes tumors. Phyllodes tumors were diagnosed at a rate of 90.7% (39/43) for the nodules with phyllodes tumors on the sonographically guided core needle biopsy, and at a rate of 52.9% (9/17) for the nodules with fibroepithelial lesions with cellular stroma on the sonographically guided core needle biopsy. On sonography, heterogeneous internal echotexture (58% vs. 17%, respectively, p = 0.0239), clefts (56% vs. 17%, respectively, p = 0.0331) and horizontal linear striations (71% vs. 33%, respectively, p = 0.0221) were significantly more frequent in the phyllodes tumors than that in the non-phyllodes tumors. CONCLUSION: Identification of a heterogeneous-internal echotexture, clefts and horizontal linear striations on sonography might help differentiate phyllodes tumors from non-phyllodes tumors.

PURPOSE: To evaluate the ultrasonographic features of nonlactiferous breast abscess. MATERIALS AND METHODS: We retrospectively reviewed ultrasonograms of 21 cases with surgically and clinically proved nonlactiferous breast abscess. The cases included 17 cases of acute or chronic inflammation and 4 cases of tuberculosis. RESULTS: Location of the lesion was subareolar in 15 cases and peripheral in 6. Mean anteroposterior/transverse diameter ratio was 0.49. Internal echogenicitiy of the lesion was variable, with heterogeneous mixed-echoic echotexture in 18 cases and homogeneous hypoechoic in 3. Margin of the lesion was irregular in 18 cases(85.7%) and posterior sonic enhancement was observed in 17 cases(81%). There were also noted obliteration of adjacent superficial fascia, localized skin thickening, and sinus tract or ductal ectasia in 19(90.5%), 9 (42.9%), and 9(42.9%) cases respectively. CONCLUSION: Major ultrasonographic findings of nonlactiferous breast abscess was subareolar located, variable shaped mass with posterior enhancement. Additional findings were fistular formation, loss of superficial fascia, and axillary lymphadenopathy.
