2.Indirect measurement of blood pressure in neonates using an automatic noninvasive oscillometric monitor.

The measurement of blood pressure in neonates is an important diagnostic procedure. But the measurement of blood pressure has not been performed routinely because of difficulty in measuring blood pressure and variable normal range according to measuring apparatus. Recently some accurate and convenient apparatus of measuring blood pressure have been introduced in neonatal care, so the reference values of neonatal blood pressure may be obtainable. The authors measured systolic and diastolic blood pressure using a noninvasive oscillometric monitor instrument on 1,3,6,12,24,48,72 hours of life in 200 neonates born at Eulji General Hospital, Taejon. And we analysed the results according to birth weight, gestational age, delivery type, sex, meconium stain, preeclampsia and hypocalcemia. The following results were obtained: 1) On the 1st day of life, systolic and diastolic blood pressure were 65.611.7 mmHg and 36.7+/-5.8 mmHg in the normal birth weight neonates, and 56.2+/-6.7 mmHg and 34.14.2 mmHg in the low birth weight neonates, respectively. So the blood pressure of normal birth weight neonates were higher than that of low birth weight neonates. 2) On the 1st day of life, systolic and diastolic blood pressure were 65.5+/-11.8 mmHg and 36.6+/-5.8 mmHg in the fullterm neonates, and 57.6+/-5.2 mmHg and 35.6+/-3.8 mmHg in the preterm neonates, respectively. So the blood pressure of full term neonates were higher than that of preterm neonates. 3) The difference of blood pressure in analysis according to birth weight were wider than that according to gestational age. 4) The blood pressure of neonates were lowest on the 3 hours of life and increased gradually during 72 hours of life. 5) The blood pressure of neonates did not show any significant difference in analysis according to sex. delivery type, Meconium stain, preeclampsia, and hypocalcemia.

4.Accompanied Histopathologic Findings and Association of Serum beta-HCG Levels with Myosalpingeal Invasion in Ectopic Tubal Pregnancy.

Most ectopic pregnancies occur in the fallopian tubes. There have been numerous theories to explain the occurrence of ectopic pregnancy in fallopian tubes. The most commonly held view is that the passage of the fertilized ovum through the fallopian tube is delayed or hindered by chronic inflammation and its sequelae. We designed a study to evaluate the details of histopathologic changes and the location of implantation and how they relate to the clinical history. 182 fallopian tube specimens from patients who had undergone total or partial salpingectomy were examined. A high incidence of non-specific inflammation of plicae and wall of tube (31.9%) and salpingitis isthmica nodosa (12.6%) were observed. Other associated findings included acute salpingitis, complex plicae or complex hyperplasia of tubal epithelium, fibrous adhesion with ovary, endometriosis, and calcification. History of previous ectopic tubal pregnancy was found in 8 cases. The cases with serum beta-HCG value above 2,500 I.U./L (group I, n=97) were more frequently noted in those exhibiting myosalpingeal invasion of trophoblast (67 cases) than in those without invasion (30 cases). Of the 182 tubal pregnancies, 117 (64.3%) cases were found in the ampulla and 47 (25.8%) cases in isthmic location. In 117 ampullary pregnancies, the products of conception were found intraluminally in 71 cases (60.7%), and extraluminally in 34 (29.1%) cases, of which the products of conception were found entirely extraluminal. The products of conception, found both within and outside the tubal lumen, were found in 12 cases (10.2%). Of 47 tubes with isthmic pregnancies, 33 cases were intraluminal (70.2%), 12 cases were extraluminal (25.5%), and two cases were mixed (4.3%). In conclusion, significant histopathologic abnormalities accompany a majority of ectopic tubal pregnancy, and myosalpingeal invasion of trophoblast is correlated with high serum beta-HCG. Thus, it is necessary to confirm not only the ectopic placental tissue but also the accompanying details of the other histopathologic findings or the pathologic evaluation of ectopic tubal pregnancy.

5.Effects of Light on Daytime Sleep in 12 Hours Night Shift Workers: A Field Study

OBJECTIVES: Night shift workers suffer from sleep and daytime disturbances due to circadian misalignment. To investigate the role of environmental light in daytime sleep following 12 h-night shift work. METHODS: We enrolled 12 h-shift female nurses working at one university-affiliated hospital (n=10, mean age 26.6 years, shift work duration 3.8 years). This is a cross-over study to compare sleep between under light exposure (30 lux) and in the dark (<5 lux) following 12 h-night duty. Two sessions of experiments were underwent and the interval between sessions was about a month. Psychomotor vigilance test (PVT) had performed on awakening from sleep at each session and sleep-wake pattern had been monitored by actigraphy throughout the study period. Daytime sleep was also compared with night sleep of age-and gender matched daytime workers (n=10). RESULTS: Sleep parameters and PVT scores were not different between two light conditions. Activities during sleep seemed to be more abundant under 30 lux condition than in the dark, which was not significant. Compared to night sleep, daytime sleep of shift workers was different in terms of rapid eye movement (REM) sleep. Three shift workers showed sleep onset REM sleep and first REM sleep period was the longest during daytime sleep. CONCLUSIONS: Unexpectedly, daytime sleep of 12 h night shift workers was well-maintained regardless of light exposure. Early occurrence of REM sleep and shorter sleep latency during daytime sleep suggest that shift workers meet with misalignment of circadian rhythm as well as increased homeostatic sleep pressure drive.

7.The Occupational Health Status of the Small Scale Industries Participated in the 1997 Subsidiary Program on Health Care Management.

The small scale industries in Korea are important in occupational health care management due to high accident rate and poor working condition. The authors surveyed the 4,811 small scale industries participated in the 1997 subsidiary program on health care management. The contents of survey were the general characteristics of the factory, the results of workers' general and occupational health examination, the results of workplace environmental monitoring, and the number of occupational accident and injured person at 1995 and 1996. We classified all industries by 67 groups. The prevalence of the general disease was higher in 'Manufacture of other fabricated metal productsj. rManufacture of textiles', 'Manufacture of basic metalsj. In occupational disease, that was higher in FManufacture of metal products', 'Manufacture of basic metals', so in environmental monitoring in 'Manufacture of textiles', 'Manufacture of other fabricated metal products' 'Printfrig' in occupational injuries in 'Manufacture of chemical products', 'Manufacture of basic metals'. However the factories of 'manufacture of motor vehicle', 'manufacture of machine and equipment', 'manufacture of electric product' in the program are large in magnitude, but less important in occupational health management. We observed that the prevalence of general disease, occupational disease, workplace environmental monitoring, occupational injuries of subsidized industries in 1996 were increased compared with those in 1995. and were larger than those of nationwide statistics. These phenomenon shows that the subsidiary health program for participated industries is important and to be urgent. We observed that the major groups in district were different and so the approaching method of each district must be various and different. And the health program manager should focus on the C grade in health examination and the action level in workplace monitoring more than the D grade and the permissible level.

8.Two Cases of Convulsion Associated With Caprolactam.

Two young men were seen with nausea, vomiting, dermatitis, seizure after two-four days of occupational exposure to caprolactam, a nylon fiber precursor. There were no significant results in laboratory test, brain CT, EEG except leukocytosis, hyperglycemia. Caprolactam has been shown to induce convulsive disorder in experimental animal studios and Tuma et al (1981) described that one worker acutely exposed to caprolactam developed generalized tonic-clonic seizure with leukocytosis. The coincidence of typical skin lesion with otherwise unexplained generalized tonic-clonic seizure in those young man strongly suggests that caprolactam was causal agent.

9.Familial Juvenile Polyposis.

Familial juvenile polyposis is a rare intestinal polyposis characterized by the occurrence of multiple juvenile polyps in the gastrointestinal tract. We report a case of familial juvenile polyposis in a 17-year-old man with a review of the literature. This patient underwent total colectomy due to a 6 years history of rectal bleeding. Grossly, the colon showed 36 variable sized pedunculated polyps, measuring 2.5cm x 2cm from the largest size and 0.2cm x 0.2cm to the smallest size. Histologically, the polyps consisted of cystically dilated glands, lined by normal colonic epithelial cells, scattered in loose, edematous stroma containing inflammatory cell infiltration. There were no areas of tubular adenoma or malignancy in any of the polyp.

10.Detection of Helicobacter pylori by Polymerase Chain Reaction.

BACKGROUND: Helicobacter pylori has been implicated in the pathogenesis of active chronic gastritis and peptic ulcer disease in man. Thus, diagnosis and treatment of H. pylori infection are now of growing importance in ulcer management. A variety of non-invasive and invasive methods have been described for the detection of H. pvlori, but all of these techniques have disadvantages such as time consuming or insensitivity. So we describe the polymerase chain reaction(PCR) assay for the sensitive and specific detection of H. pylori. METHODS: Gastric biopsy specimens were obtained from 247 patients undergoing endoscopic examinations at Hanyang University Hospital. One half of the specimen was processed for routine culture and the other half for PCR. Bacterial genomic DNA from gastric biopsies was extracted by Instagene. Two sets of primer pairs derived from the nucleotide sequence of the urease A gene of H. pylori were used. RESULT: H. pylori was cultured in 100(40%) cases and PCR assay detected 179 (72%) cases (P<0.05, Chi-square test). Culture and PCR-positive cases totaled 100, and there were 68 cases negative by both methods. There were 79 culture-negative and PCR-positive cases, but none was culture-positive and PCR-negative. The assay was sensitive for detecting as little as 0.1 pg of DNA (1 bacterial cell). The specificity of detection was confirmed by ensuring that the primers did not amplify DNA extract from other bacteria. CONCLUSION: The PCR is a rapid, accurate, and sensitive method for the detection of H. pylori.
