5.A Medicolegal Consideration on Persistent Vegetative State in Korea.

This treatise summarizes the medical facts about the persistent vegetative state(PVS) and the associated issues. In August 1998, the opinions of the 380 neurologists and the 1007 neurosurgeons are inquired about the diagnostic criteria of PVS, the degree and the withdrawal of treatment on PVS patients. The response rate is 18.8%. The diagnostic criteria of PVS is determined by the items approved above 5007o of responses. (1) no evidence of cerebral function and an inability to interact with visual, auditory stimuli and follow commands. (2) maintaining the respiratory function with spontaneous respiration and normal respiratory pattern. (3) spontaneous eye opening and eye ball movements without sustained tracking. (4) variably preserved brain stem reflexes. (5) presence of sleep-wake cycles. (6) (1)-(5) symptoms should be persist for 6 months after the development of vegetative state. (7) the diagnostic criteria of PVS should be applied more than 3 times during the minimum observation period. (8) this diagnostic criteria of PVS could be applied to adults over 7 ages. As to the degree of treatment, nasogastric tube feeding and air-way keeping through the tracheostomy are approved over 9007b. Also antibiotics can be used in infection. As to the withdrawal of treatment, do not resuscitate order of patient s family is accepted by 46.1% in an emergency condition like cardiac arrest. But discharge against medical advice by the patient s family is accepted by 73.107o in non-emergency condition. In the case of neonates, parents decision is more determinative than any other thing.

6.Effects of Thera-Band(R) Resistance Training Including Isometric Exercise in Total Knee Replacement Patients.

BACKGROUND: The purpose of this study was to assess the effects of an exercise program using isometric and isotonic thera band exercises. METHODS: Data were collected from 30 total knee replacement patients hospitalized in the orthopedics department at 'B' hospital (control=15, experimental=15). The control group performed isometric exercises for one week while the experimental group performed three or more rounds of both isometric and isotonic exercises, including Thera-Band(R) exercises, everyday for four weeks. RESULTS: The experimental group improved significantly in muscular strength (Z=-1.99, P=0.047) and self-efficacy (Z=19.48, P=0.000), and showed lower levels of depression than the control group (Z=8.60, P=0.003). However, no difference was found in pain experience between the two groups. CONCLUSIONS: Exercise programs using the isometric and Thera-Band(R) resistance training improve muscular strength and self-efficacy and relieve depression in total knee replacement patients.

7.Computed tomography of cystic tumors of the mediastinum

Authors retrospectively analyzed the CT findings of cystic tumors of the mediastinum in surgically confirmed13 cases at Seoul National University Hospital during the recent 3 years from August 1980 to August 1983. Theresults are as follows: 1. Among 13 cases, bronchogenic cyst and cystic teratoma were 4 cases respectively,pericardial cyst were 2 cases, and thymic cyst and cystic hygroma was 1 case respectively. 2. 8.A Case of Ki-1 Positive Type B Lymphomatoid Papulosis.

The monoclonal antibody Ki-1(CD30) was first described in 1982 and was reported to react selectively with Reed-Sternberg cells in Hodgkins disease, highly activated B and T cells and large cell lymphomas of both T-and B-cell origin. Lymphomatoid papulosis(LyP) is characterized by recurrent erythematous papules or nodules that undergo spontaneous healing with hyperpigmentation and scarring. Histologically, two major types of LyP can be distinguished: type A is characterized by the presence of variable numbers of Ki-1+ large, atypical lymphocytes with some Reed-Sternberg like cells and type B is characterized by Ki-1- atypical cerebriform mononuclear cells similar to those in mycosis fungoides. We report a case of Ki-1+ lymphomatoid papulosis which was histopathologically recognized as a type B lesion but represented a Ki-1 positive reaction.

9.Hailey-Hailey Disease with a Family Histroy and Unique Nail Lesions.

Hailey-Hailey disease(benign familial chronic pemphigus) is a rare autosomal dominant disorder characterized by blisters at sites of friction such as the neck, axillae and groin which are caused by suprabasal epidermal acantholysis. We report two cases of Hailey-Hailey disease in the one family. One of the two cases has asymptomatic multiple longitudinal white bands in the fingernails associated with typical skin lesions. The nail lesions have not been described until reported by Burge in 1992 and it may be a characteristic finding in Hailey-Hailey disease.

10.Bilateral Wilms Tumor: A Case Report.

A male child of 9 months with bilateral Wilms tumor was admitted to our hospital. His chief complaint was abdominal mass. Clinical examination and IVP, USG and CT-scan disclosed bilateral Wilms tumor. Right nephrectomy and left partial nephrectomy were performed. Anuria was experienced to 24 hours and so exploratory-laparotomy was done. Renal autotranplantation was performed because of the thrombosis of renal artery. But six days after operation he died.
