1.Correlation of Proliferating Cell Nuclear Antigen (PCNA) Expression and S-phase Fraction, Survival Rate in Primary Non-Small Cell Lung Cancer.

BACKGROUND: To study the prognosis of patients with lung cancer, many investigators have reported the methods to detect cell proliferation in tissues including PCNA, thymidine autoradiography, flow cytometry and Ki-67. PCNA, also known as cyclin, is a cell related nuclear protein with 36KD intranuclear polypeptide that is maximally elevated in S phase of proliferating cells. In this study, PCNA was identified by paraffin-embedding tissue using immunohistochemistry which has an advantage of simplicity and maintenance of tissue architecture. The variation of PCNA expression is known to be related with proliferating fraction, histologic type, anatomic(TNM) stage, degree of cell differentiation, S-phase fraction and survival rate. We analyzed the correlation between PCNA expression and S-phase fraction, survival. METHODS: To investigate expression of PCNA in primary lung cancer, we used immunohistochemical stain to paraffin-embedded sections of 57 resected primary non-small cell lung cancer specimen and the results were analyzed according to the cell type, cell differentiation, TNM stage, S-phase fraction and survival. RESULTS: PCNA expression was dMded into five group according to degree of staging(-, +, ++, +++,++++). Squamous cell type showed high positivity than in adenocarcinoma. Nonsignificant difference related to TNM stage was noticed. Nonsignificant difference related to degree of cell differentiation was noticed. S-phase fraction was increased wit advance of PCNA positivity, but t could not reach the statistic significance. The 2 year survival rate and median survival time were -50% 13 months, +75% 41.3 months, ++73% 33.6 months, +++67% 29.0 months, ++++25% 9 months with statistic significance (P<0.05, Kaplan-Meier, generalized Wilcox). CONCLUSION: From this study. PCNA expression was high positive n squamous cell cancer. And, there was no relationship between PCNA positivity and TNM stage, cellular differentiation or S-phase fraction. But, the patients with high positive PCNA staining showed poor survival rate than the patients with lower positive PCNA. It was concluded that PCNA immunostaining is a simple and useful method for survival prediction in paraffin embedded tissue of non-small cell lung cancer.

3.Rhinoplasty through the External Approach.

BACKGROUND: Rhinoplasty surgeons have a choice of several approaches to the nose. The approaches can conveniently be divided into external and intranasal techniques. We prefer the external approach to the intranasal approach because the former provides better operative fields and more complete correction. OBJECTIVES: The purpose of this report is to analyze the experiences of rhinoplasty through the external approach. MATERIALS AND METHODS: A retrospective study was performed in 119 patients who underwent rhinoplasty using the external approach from January of 1994 through March of 1997. RESULTS: This procedure has been used in all kinds of deformities even though most of them overlapped: 54 cases of crooked nose, 57 cases of saddle nose, 5 cases of hump nose and 3 cases of tip surgery. CONCLUSION: The exact procedures of cosmetic nasal tip surgery can be performed more easily by the external approach. The disadventages of this procedure, which is the columellar scar, longer operation time and post-operative edema, are neglible.

4.Impact of Solvent Exposure on the Occupational Hearing Loss.

OBJECTIVES: This study was to evaluate the impacts of simultaneous exposure to noise and mixed solvents on workers'hearing threshold level (HTL) over five-year period. METHODS: The study was conducted by interview and annual audiometric test on workers in ship building industry from 1994 to 1998. The cases(workers who exposed to noise and mixed solvent simultaneously) were 43 male workers and controls (workers who exposed to nolle) were selected by matching method with regard to age, sex, carrier, and noise exposure level. To assess the impacts of solvent exposure on hearing threshold level, with considering other factors, general liner model was used. RESULTS: 1. The audiogram of all subjects showed typical sensorineural hearing loss pattern. The mean HTLe were increased at high frequency (4000HB and 8000Hz) for study period. 2. The HTLs of cases were more increased than those of controls at high frequencies (4000Hz and 8000Hz), but there was not statlstical significance. 3. The impact of age on the HTL was statistically significant at 250Hz and 500Hz (p(0.05), and the impact of noise on the HTL was statistically significant at 250Hz, 2000Hz and 4000Hz (p(0.05), but the impact solvent exposure on the HTL was not significant. 4. The changes in HTLs of cases were higher than those of controls at high frequency, there were not statistical significance at 4000Hz, but only at 8000Hz (p=0.087). 5. Statistical analysis of the general linear model implicated that the changes in HTL was impacted by noise exposure bevel significantly (p=0.031) and Impacted by solvent exposure weakly (p=0.087) at 8000Hz. CONCLUSIONS: The results of this study suggest that workers who simultaneously exposed to noise and mixed solvent were at risk for more affected HTL than those exposed to noise exposure only, but we could not find definitive evidence. Further detailed studies must be made in large number of workers.

5.Correlation of Breast Tissue Density and Body Mass Index.

PURPOSE: In performing breast reconstruction, making symmetrical breast is still a challenge. A precise estimate of the volume of the breast specimen is necessary to reconstruct a symmetrical and aesthetically pleasing breast. This study aims at finding out the relationship with breast tissue density and body mass index designed to apply for breast reconstruction. METHODS: By using the Archimedes' principle, the authors calculated the volume of the breast specimen and drew a correlation between the density of breast specimen and BMI. From October 2002 to November 2004, this method was used on 197 patients to predict breast volume for TRAM flap reconstruction. RESULTS: The mean density was 0.9954g/cc and had no correlation with BMI (p-value=0.069). There was no difference between denstiy of breast tissue after skin spared mastectomy and that of breast tissue after nipple spared mastectomy. CONCLUSION: These data will be helpful to predict the needed volume for breast reconstruction.

6.Evaluation of Methods for Fetal Weight Estimates Using Ultrsound Formula at Term.

A total of 150women with singleton pregnancies who were delivered between 37 and 42weeks gestation had ultrasound scans on elective cesarean section day. The biparietal diameter(BPD), head circumference(HC), abdominal circumference(AC) and femur length(FL) were measured in all cases. Equations of estimated fetal body weight(BWT) for Korean term fetuses using AC alone, BPD/AC, AC/FL, HC/AC/FL, BPD/AC/FL, BPD/HC/AC/FL were made by stepwise multiple regression analysis and were compared with foreign equations such as Campbell(AC), Shepard(BPD/AC), Hadlock I(AC/FL), Hadlock II(HC/AC/FL), Hadlock III(BPD/AC/FL), and Hadlock IV(BPD/HC/AC/FL). The results were as follows. 1. When BPD, AC and FL were measured, the addition of HC didn,t affected to accuracy of EFW 3. The best results of all equations were obtained with our equation 5(Mean error=2.36gm, Mean absolute error=96.39gm, Mean deviation + SD(%)=0.07+0.37) and among foreign equations, the best results were with Hadlock III equation(Mean error=18.35gm, Mean absolute error=107.82gm, Mean deviation + SD(%)=-0.46+0.42) 4. The equations using AC/FL (our equation 3, Hadlock I) are more accurate than those using BPD/AC (our equatione 2, Shepard) when utilizing two parameters. Therefore, equation using AC/FL is recommended when the BPD is unobtainable at term or labor 5. The percentage of cases in which the EFW was within +5% and +200gm of the actual birth weight was highest at our equation 5 for 82% and 89%, Hadlock IV equation for 79%, 88%. 6. Our equations had relatively an equal tendency to under- and overestimate fetal weight, but foreign equations, especially Campbell and Hadlock II equations tended to underestimate. 7. Analysing according to actual birth weight, our all equations gave the most accurate estimates of fetal weigh for birth weight group between 3000-3499gm, but for birth weight group above 3500gm, that was not. At birthweight group above 3500gm, the equations depend on femur length are more accurate than the equations independ on fumr length.

7.A Case of Dedifferentiated Chondrosarcoms: It was Changed From a Hamartoma.

Extraosseous pulmonary chondrosarcoma is a rare neoplasm, which is characterized into two groups. One is termed a primary chondrosarcoma, and arise de novo (bronchial cartilage), the other is termed a secondary chondrosarcoma, and is superimposed on preexisting benign cartilagenous neoplasms, such as a chondroma or hamartoma. The preferred treatment is surgical resection. We recently experienced a secondary chondrosarcoma changed from a hamartoma.A 54-year-old woman was referred to our hospital becaused of an abnormal chest X-ray with mild dyspnea. We performed a percutaneous transthoracic needle biopsy and sputum examination. The abnormal mass had been diagnosed as a chondromatous hamortoma wit active pulmonary tuberculosis, which had been treated with anti-tuberculosis regimens. Despite her medication, and abnormal mass had grown. Therefore, we undertook a pneumonectomy with chest wall reconstruction.Histopathologically, the mass was grade II, dedifferenciated chondrosarcoma, with chronic granulomatous inflammation and necrosis.We suggest this case had changed from a chondromatous hamartoma to a dedifferentiated chondrosarcoma, with associated pulmonary tuberculosis. We report this case with a brief literature review.

9.Diastolic Time Intervals and Systolic Time Intervals in Hypertensive Patients.

For many years, interest in cardiac function has primarily centered around the systolic pump performance of the left ventricle. it is now recognized, however, that diastolic abnormalities may be just as important in the pathophysiology of certain cardiac disease states. To examine the left ventricular abnormalities (especially diastolic events) in hypertension, diastolic and systolic time intervals were measured from simultaneous high-speed recordings of a phonocardiogram, ECGs, apexcardiogram, echocardiogram and external carotid pulse in 35 hypertensive patients and were compared with those in 15 normal subjects. The hypertensive patients showed significantly prolonged preejection period (PEP) and shortened ejection time (ET), compared to those in normal control subjects (p<0.005, p<0.05 respectively). The PEP/ET ratio too was different from the control subjects in hypertensive patients (0.335+/-0.050 vs 0.422+/-0.666; p<0.005). The isovolumic relaxation time (IVRT) was increased to 81.3+/-15.0 msec, which was significantly longer (p<0.005) than in normal subjects (56.7+/-10.7 msec), in patients with hypertension. The active filling period (AFP) was also prolonged. In patients with hypertension there was no significant difference in rapid filling period and slow filling period compared with those in normal subjects. It is likely that in hypertensive patients the alterations of diastolic time intervals, reflecting disorders in elasticity and compliance, may occur in conjunction with abnormal systolic events.

10.A Case of Dorsal Nasal Cyst after Augmentation Rhinoplasty.

Augmentation rhinoplasty is one of the most popular aesthetic procedures for orientals. Various materials have been used for augmenting the nasal dorsum. Complications that may occur right after rhinoplasty are generally attributed to failure to obtain the desired aesthetic, functional and psychological results. Other complications include postoperative hemorrhage and infection including cellulitis, abscess formation and toxic shock. Serious life-threatening complications such as intracranial or general infection that occurred in the preantibiotic era are fortunately infrequent. Dorsal nasal cyst formation is also considered a rare complication of augmentation rhinoplasty. The cause of this complication is not well known untill now; however, the entrapment of nasal mucosa and subsequent growth through osteotomy sites are thought to be the basis for the formation of these mass. Dorsal nasal cyst formation of post-augmentaion rhinoplasty was reported in a few case around the world but not in Korea. However, we experienced one case of dorsal nasal cyst formation after augmentation rhinoplasty in our clinic. Therefore, we present the case report of dorsal nasal cyst formation as a rare post-augmentation rhinoplasty with inferior turbinate bone and cartilage.
