2.A Study on the Relationship among Family Functioning, Empathy, and Aggression by High School Students.

PURPOSE: The purpose of this study was to investigate the relationship among family functioning, empathy and aggression by high school students. METHODS: This was a descriptive study. The survey participants were 467 high school students from M city. Data were collected from November 29 to December 7, 2012 and self-report questionnaires including a Family Adaptability Cohesion Evaluation Scale, Interpersonal Reactivity Indexes, and an Aggression Questionnaire. The collected data were analyzed by descriptive statistics, independent-samples t-test, ANOVA, stepwise multiple regression. RESULTS: The majority of the subjects were 2nd graders (42.0%) and female (57.2%). 59.7% of the subjects had religion, 84.8% were living together with parents. The mean ages of their fathers and mothers were 49.90+/-0.20 and 46.85+/-0.19 respectively. Aggression was found to have a statistically negative correlation with family functioning and empathy. Stepwise multiple regression analysis revealed that the most powerful predictor of aggression was empathic concern. Empathic concern, family cohesiveness, perspective taking, personal distress and gender accounted for 16.7% of the variances. CONCLUSION: It is necessary to provide aggressiveness reduction programs for high school students that consider their family functioning and empathy.

3.Changes of Internal Jugular Venous Oxygen Content with Differences in Arterial CO2 Tension.

The routine management of head injury includes hyperventilation to produce hypocapnis with arterial CO2 tension 25~30 torr. But a decrease in cerebral blood flow with hypocapnia may result in cerebral ischemia. Our study was to evaluate the change of cerebral blood flow during hyperventilation in halthane anesthesia. The jugular venous oxygen saturation(SjvO2), arterio-venous oxygen content difference(CaO2-CjvO2), and oxygen extraction ratio(O2ER) were used as criteria of cerebral ischemia with reduced cerebral blood flow. The results are as follows: 1) SjvO2 was lower in group 2(PaCO2=22.8torr) than group 1(PaCO2=30.3 torr). 2) CaO2-CjvO2 and O2ER were higher in Group 2 than group 1. 3) No more increased possibility of cerebral ischemia with reduced cerebral blood flow was observed Group 2 than group 1.

4.Clinical Study of Status Epilepticus in Children.

PURPOSE: Convulsive status epilepticus(SE) is a serious, life-threatening neurological condition that requires immediate treatment to avoid significant morbidity and mortality. Despite improvements in the diagnosis and treatment of SE in the last two decades, SE in young infancy is still associated with high morbidity and mortality. Thus, understanding the varied etiology and clinical presentation and prognosis of SE is very important for improving the methods of evaluation and treatment of this major neurological condition. METHODS: Eighty-eight cases with 53 who have been admitted to the Department of Pediatrics, Fatima Hospital during the period of July, 1992 to June, 1997 were included. We described age distribution, etiologic classification according to age, seizure type, neurologic outcome, recurrence of SE and epileptic seizure. RESULTS: SE was frequent in young infant less than 3 years of age. Major etiology of SE was acute symptomatic(34.1%) and febrile(31.8%). In the seizure type, the majority(92.1%) was generalized convulsive, many cases(69.3%) of SE were first seizures. The neurologic sequelae were found in 15.9% and mortality rate in 5.7%. The neurologic sequelae and mortality were higher in acute symptomatic. In sixty-three follow-up cases, eleven cases were epileptic seizure, eight cases were recurred SE and two cases were recurred febrile SE. CONCLUSION: SE is a life-threatening neurological condition and occurrs mostly in young infants less than 3 years of age. It requires immediate detection of etiology in SE and aggressive treatment for reducing mortality and morbidity rates.

5.Vitamin D Deficiency Is Highly Concomitant but Not Strong Risk Factor for Mortality in Patients Aged 50 Year and Older with Hip Fracture.

BACKGROUND: The purpose of this study was to ascertain the prevalence of vitamin D deficiency and risk factors associated with mortality in patients > or =50-year-of-age with hip fractures. METHODS: A total of 489 patients > or =50-year-of-age who sustained a hip fracture from January 2010 to October 2014 were followed-up for a minimum of 1 year. Clinical and radiological outcomes were evaluated including prevalence of vitamin D deficiency. Crude mortality rates were calculated, and the effects of different risk factors on mortality were assessed. RESULTS: Vitamin D deficiency was present in 76.5% of cases (n=237). The prevalence of vitamin D insufficiency was 12.3%, and only 11.2% of patients had normal vitamin D levels. Accumulated mortality was 11% (54 patients) at 1 year. A univariate analysis showed that vitamin D deficiency (P=0.012), age (P<0.001), BMI (P<0.001), type of management (P<0.001), American Society of Anesthesiologists (ASA) score (P=0.009), pre-fracture ambulatory status (P<0.001), and osteoporosis (P<0.001) were associated with mortality. A multivariate analysis performed using a Cox proportional hazards model demonstrated that ASA score (P=0.001) and pre-fracture ambulatory status (P=0.011) were independently associated with mortality after hip fracture. CONCLUSIONS: We did not find a relationship between serum 25-hydroxy-vitamin D levels and mortality after hip fracture, although we observed a high prevalence of vitamin D deficiency and a significant association with mortality in the univariate analysis.

8.A Case of Coronary Artery Aneurysm.

A coronary artery aneurysm was first reported by Bougon in 1812, and was able to be diagnosed during life with advance of selective coronary arteriography. We have recently seen a 62-year-old woman who had intermittent anterior chest pain and dyspnea on exertion(NYHA functional class II) and diagnosed a coronary artery aneurysm by coronary arteriography. A saccular formed coronary artery aneurysm was located at left main coronary artery at the juction of the bifurcation of LAD and left circumflex artery and it's was 2cmx2cm and it's thickness of wall was 0.5cm. After the patency and consistency of coronary arteries were confirmed,resection of aneurysm with aneurysmorrhaphy was performed.

9.Retrospective Analysis of the Postoperative Patients Admitted to General Surgical-Medical Intensive Care Unit.

BACKGROUND: The present study was designed to examine the purpose of intensive care unit (ICU) admission and the prevalence of disease in postoperative patients admitted to general surgical-medical ICU. METHODS: Between 1 January 2007 and 31 December 2007, 646 cases of 612 patients admitted to a general postoperative patients admitted to general surgical-medical ICU were examined. The patients were classified into two groups, ICU treatment and ICU monitoring groups according to Knaus' suggestion which defines the kinds of treatment done exclusively in ICU. Patients' demographics, preoperative American Society of Anesthesiologists physical status classification (ASA) grade, prevalence of disease and emergent operation rate were analyzed. RESULTS: 255 patients (39.5%) were included in the ICU treatment group and 391 cases (60.5%) in the ICU monitoring group. The prevalence of respiratory, gastrointestinal, and central nervous diseases was higher significantly in the ICU treatment group. In addition, the average of ASA grade and the duration of operation were higher significantly in the ICU treatment group. CONCLUSION: Admission rate only for monitoring was higher than one for intensive treatment. An alternative strategy should be considered to care for postoperative patients who need just close monitoring.
