We reported a new technique to repair of the posterior horn tear near the root of the medial meniscus. The radial posterior horn tear near the root showed total loss of the circumferential hoop tension and was similar to the total meniscectomized state. Efforts should be made to restore the circumferential hoop ten-sion and maintain stable bony insertion. We introduced a pull-out repair technique using ACL guide, suture repair set and two PDS No.1. This technique showed stable fixation and good hoop tension of the meniscus. Pull-out repair into the posterior root seems to be a useful method in the posterior horn tear near the root of the medial meniscus.

Background: Korea set up a new diagnosis-related group as a demonstration project in 2009. The new diagnosis-related group was reformed in 2016. The main purpose of the study is to identify the effect of reform on coverage of national health insurance. Methods: This study collected inpatient data from a hospital that contains medical information and cost from 2015 July to 2016 June. The dependent variable was the coverage of national health insurance. The dependent variable was divided by total, internal medicine partition, surgical partition, and psychiatric partition. To analyze the effect of the reform, this study conducted an interrupted time series analysis. The final sample included 23,695. Results: The health insurance coverage of internal medicine has the highest, followed by surgery and psychiatry. The health insurance coverage of bundle payment is higher than that of unbundled payment. The proportion of bundled payment and non-benefit decreased and the proportion of unbundled payment increased. The coverage of national health insurance significantly increased after policy reform in internal medicine partition (p-value=0.0356). Conclusion The results of the study imply that policy reform enhanced the coverage of national health insurance in internal medicine. The government needs to monitor side effects such as an increase of unbundled payment.

A study for seroprevalence of Hepatitis B virus markers (HBsAg, Anti-HBs, HBeAg and Anti-HBe) by EIA method and the Liver Function Test(SGOT and SGPT) using kinetic method was carried :out from January; to October 1995 :among male workers of a .major iron and steel manufacturing company, located,,in Pohang. The results were as follows; 1. The distribution of hepatitis B virus markers showed that the proportions of HBsAg(+)/Anti-HBs (-), HBsAg(-)/Anti-HBs (+) 'and HBsAg(+)/Anti-HBs (+) were 8.1%, 60.5% and 0.1%, respectively. The positive rate of HBsAg in all subjects was 8.2% whereas that of anti-HBs was 60.6%. 2. The proportions of HBsAg(+)/Anti-HBs (-) and HBsAg(-)/Anti-HBs (+) increased significantly with age. While the distribution of hepatitis B virus markers exhibited no significant differences among the groups classified by educational background or work type. 3. The proportion of drinkers in HBsAg (+)/Anti-HBs (-) (73; 9%) was significantly lower than those of HBsAg (-)/Anti-HBs (-) and HBsAg (-)/Anti-HBs (+) (84.6% and 85.7% respectively). The proportions of smoking for the three groups of HBsAg (-)/Anti-HBs(+),-HBsAg (+)/Anti-HBs (-) and HBsAg (-)/Anti-HBs (-) were 54.3%, 53.7% and 53.0%, respectively (p<0.01). The proportion of the subjects with abnormal liver function test in HBsAg (+)/Anti-HBs (-) was 40.1%, whereas those in HBsAg (-)/Anti-HBs (-) and HBsAg(-)/Anti-HBs (+) were 21.9% and 18.4%, respectively. 4. The distribution of e Ag markers among HBsAg (+) subjects showed that the proportions of HBeAg(+)/Anti-HBe(-), HBeAg(-)/Anti-HBe(+) and HBeAg(+)/Anti-HBe(+) were 31.3%, 55.0% and 1.6%, respectively. The proportion of HBeAg(+)/Anti-HBe(-) decreased significantly with age and those of HBeAg(-)/Anti-HBe(-) and HBeAg(-)/Anti-HBe(+) increased with age. The positive rate of HBeAg among HBsAg (+) subjects was 32.9%. In conclusion, as a screening method, EIA is. recommended to test hepatitis B virus markers, especially when examining Anti-HBs more sensitively. For the workers of the iron and steel manufacturing company studied, liver diseases caused by other than hepatitis B should be given a special care including an education avoiding alcohol drinking.

The purpose of this study was to determine the antioxidant effect of fucoxanthin. After rats were fed a normal fat diet (NF), high fat diet (HF), and high fat with 0.2% fucoxanthin diet (HF + Fxn) for 4 weeks, the markers of oxidative stress and antioxidant capacity like lipid peroxidation, plasma total antioxidant capacity (TAC), and activities of antioxidant enzymes (catalase, superoxide dismutase (SOD), and gluthathione peroxidase (GSH-Px)) were determined. mRNA expression of transcription factor, nuclear erythroid factor like 2 (Nrf2), and its target genes such as NAD(P)H quinone oxidoreductase1 (NQO1) and heme oxygenase-1 (HO-1) were also determined. Mean weight gain in the HF + Fxn group was lower, without statistical significance, and the total food intake in the HF + Fxn group was lower than that in the HF group (P < 0.05). The activity of GSH-Px (P < 0.05) in plasma was significantly higher in the HF + Fxn group than those in the HF group (P < 0.05). In the liver, the activities of catalase (P < 0.05) and GSH-Px (P < 0.05) in the HF + Fxn group were significantly higher than those in the HF group. Plasma TAC level was significantly higher in the HF + Fxn group than that in the HF group (P < 0.05). Lipid peroxidation in plasma tended to be lower without statistical significance. Fucoxanthin supplements were shown to have higher mRNA expression of Nrf2 and NQO1 than those in the high fat diet only group (P < 0.05). In conclusion, supplementation of fucoxanthin improved the antioxidant capacity, depleted by high fat diet, by activating the Nrf2 pathway and its downstream target gene NQO1. Therefore, supplementation of fucoxanthin, especially for those who consume high fat in their diet, may benefit from reduced risk of oxidative stress.

PURPOSE: Combined chemoradiotherapy is standard management for locally advanced non-small cell lung cancer (LA-NSCLC), but standard treatment for elderly patients with LA-NSCLC has not been confirmed yet. We evaluated the feasibility and efficacy of concurrent chemoradiotherapy (CCRT) for elderly patients with LA-NSCLC. MATERIALS AND METHODS: Among patients older than 65 years with LA-NSCLC, 36 patients, who underwent CCRT were retrospectively analyzed. Chemotherapy was administered 3-5 times with 4 weeks interval during radiotherapy. Thoracic radiotherapy was delivered to the primary mass and regional lymph nodes. Total dose of 54-59.4 Gy (median, 59.4 Gy) in daily 1.8 Gy fractions and 5 fractions per week. RESULTS: Regarding the response to treatment, complete response, partial response, and no response were shown in 16.7%, 66.7%, and 13.9%, respectively. The 1- and 2-year overall survival (OS) rates were 58.2% and 31.2%, respectively, and the median survival was 15 months. The 1- and 2-year progression-free survivals (PFS) were 41.2% and 19.5%, respectively, and the median PFS was 10 months. Regarding to the toxicity developed after CCRT, pneumonitis and esophagitis with grade 3 or higher were observed in 13.9% (5 patients) and 11.1% (4 patients), respectively. Treatment-related death was not observed. CONCLUSION: The treatment-related toxicity as esophagitis and pneumonitis were noticeably lower when was compared with the previously reported results, and the survival rate was higher than radiotherapy alone. The results indicate that CCRT is an effective in terms of survival and treatment related toxicity for elderly patients over 65 years old with LA-NSCLC.

BACKGROUND: Phadiatop test has been introduced as a single test for screening the atopics who were sensitized to common inhalant allergens. MATERIAL AND METHOD: We compared the clinical efficiency of Phadiatop test and total IgE level for defining presence of atopy in 136 asthmatic subjects. The presence of atopy was defined by skin prick test done with 10 common inhalant allergens. More than 2+ skin reactivity was defined as having atopy. Phadiatop test and total IgE level in serum were measured using Pharmacia CAP systems. RESULT: 109 out of 136 subjects had more than 2+ skin reactivity to at least one allergen and another 27 subjects had 1+ or negative skin reactivity to allergens. The performance characteristics of Phadiatop test for screening atopy was superior than total IgE level, with sensitivity(91.7% vs. 82.6%), positive predictive value(94.3% vs. 86.5%) and concordance rate(89.0 % vs. 75.7% ). Specificity and negative predictive value of the Phadistop test were acceptable and higher than that of total IgE level. CONCLUSION: These results suggested that Ph adiatop may be simple and useful for screening atopic status in Korean asthmatic subjects.

OBJECTIVE: The number of elderly patients with cerebral aneurysm has markedly increased. We investigated the clinical characteristics and the surgical outcomes of cerebral aneurysms among elderly patients more than 65 years of age compared to a control group including patients less than 65. MATERIALS AND METHODS: From March 2001 to May 2007, 590 patients with aneurysmal subarachnoid hemorrhage (SAH) were treated; among them, 88 patients (14.9%) more than 65 were candidates for this study. The variables included in the analysis were: age, gender, size and site of the aneurysm, the Hunt-Hess grade, size of the hematoma on the CT scan, and comorbidities. RESULTS: The mean age was 69 (range 65-84 years), 72 patients (81%) were female, 72 of the patients had a Hunt-Hess grade of I-III, 37 patients (42%) had anterior communicating artery bleeds, and 13 patients (16.7%) had multiple aneurysms. The treatments consisted of neck clipping for 77 aneurysms and endovascular therapy for 11 aneurysms. Fifty eight patients (65.9%) had a favorable outcome and the overall mortality was 11.4%. The main causes of unfavorable outcomes among the elderly patients included a poor Hunt-Hess grade on admission and a concurrent intracerebral hematoma. CONCLUSIONS: The results of this study showed that advanced age was not a contra-indication to aneurysm surgery and early craniotomy can lead to a better outcome in elderly patients.

The effects of retrograded rice on body weight gain, gut functions, and hypolipidemic actions in rats were examined. When the retrograded rice was produced by repetitive heating and cooling cycles, it contained significantly higher amounts of resistant starch (13.9 +/- 0.98%) than is found in common rice (9.1 +/- 1.02%) (P < 0.05). Sprague-Dawley rats were fed either common rice powder or retrograded rice powder, and mean body weight gain was significantly lower in the retrograded rice group (P < 0.05). The liver weight of the retrograded rice group (14.5 +/- 0.5 g) was significantly lower than that of the common rice group (17.1 +/- 0.3 g, P < 0.05). However, the weights of other organs, such as the kidney, spleen, thymus, and epididymal fat pad were not significantly affected by rice feeding. Intestinal transit time tended to be lower in rats fed retrograded rice when compared to rats fed the common rice, but the difference was not significant. The retrograded rice diet significantly increased stool output when compared to that in the common rice powder diet (P < 0.05), whereas fecal moisture content (%) was significantly higher in the retrograded rice group (23.3 +/- 1.2) than that in the common rice group (19.1 +/- 1.2) (P < 0.05). The retrograded rice group had significantly lower plasma cholesterol (P < 0.05), liver cholesterol (P < 0.05), and triacylglycerol contents in adipose tissue (P < 0.05) when compared to those in the common rice group. In conclusion, retrograded rice had higher resistant starch levels compared with those of common rice powder, and it lowered body weight gain and improved lipid profiles and gut function in rats.

OBJECTIVE: This study was performed to elucidate the technical and patient-specific risk factors for postoperative ischemia in patients undergoing temporary arterial occlusion (TAO) during the surgical repair of their aneurysms. METHODS: Eighty-nine consecutive patients in whom TAO was performed during surgical repair of an aneurysm were retrospectively analyzed. The demographics of the patients were analyzed with respect to age, Hunt and Hess grade on admission, Fisher grade of hemorrhage, aneurysm characteristics, timing of surgery, duration of temporary occlusion, and number of temporary occlusive episodes. Outcome was analyzed at the 3-month follow-up, along with the occurrence of symptomatic and radiological stroke. RESULTS: In overall, twenty-seven patients (29.3%) had radiologic ischemia attributable to TAO and fifteen patients (16.3%) had symptomatic ischemia attributable to TAO. Older age and poor clinical grade were associated with poor clinical outcome. There was a significantly higher rate of symptomatic ischemia in patients who underwent early surgery (p = 0.007). The incidence of ischemia was significantly higher in patients with TAO longer than 10 minutes (p = 0.01). In addition, patients who underwent repeated TAO, which allowed reperfusion, had a lower incidence of ischemia than those who underwent single TAO lasting for more than 10 minutes (p = 0.011). CONCLUSION: Duration of occlusion is the only variable that needs to be considered when assessing the risk of postoperative ischemic complication in patients who undergo temporary vascular occlusion. Attention must be paid to the patient's age, grade of hemorrhage, and the timing of surgery. In addition, patients undergoing dissection when brief periods of temporary occlusion are performed may benefit more from intermittent reperfusion than continuous clip application. With careful planning, the use of TAO is a safe technique when used for periods of less than 10 minutes.
