3.Cardiac Fibroma: A surgically excised case.

Primary cardiac tumors in infancy and childhood are rare, with fibromas being the second most common tumor after rhabdomyomas. Although cardiac fibromas are characteristically benign intramural tumors, they may exhibit exhibit expansile growth resulting in obstruction, valvular dysfunction, as well as other problems so early diagnosis and successful surgical excision are important. We report a case of cardiac fibroma in a 2 month-old male infant. He presented with generalized cyanosis from birth. Echocardiography showed oval round large mass filing the right atrium and ventricle which infiltrated into the lateral wall of the ventricle. Partial excision of the tumor was done after another echocardiogram showed a pericardial effusion and restriction of blood flow to the right ventricle due to the tumor. The resected tumor was ovoid, gray-tan, slightly firm and measuring 5x3x2.5 cm. Histologically, the tumor was composed of spindle-shaped fibroblasts and hyalinized fibrous tissue interdigitating with the surrounding myocardium.

6.Effect of Haloperidol on Alcohol Craving in Patients with Alcohol Dependence.

OBJECTIVES: This study was aimed at investigating the effect of haloperidol on alcohol craving in patients wih alcohol dependence. METHODS: Eighteen patients with alcohol dependence were divided randomly into two groups of nine patients each: one haloperidol group and the other, placebo group. The medication for each group was done for 14 days. Alcohol craving and difficulty in resisting drinking were measured on day 1 and day 14, each consisting of a series of four assessments. Assessment 0 was basal levels. Assessment 1 was made 3 hours after medication. Assessment 2 was made after alcohol intake in a dose of 0.4gm of 100% alcohol/kg body weight and assessment 3 was done after the second alcohol intake in the same amount. RESULTS: The results were as follows: 1) With acute treatment, placebo group showed a significant increase in alcohol craving whereas haloperidol group did not show any change after the first and second alcohol intake. 2) With chronic treatment, both groups showed significant increase in the alcohol craving after alcohol intake. 3) Haloperidol did not increase difficulty in resisting drinking after acute treatment, however, with chronic treatment, it resulted in a significant increase of the difficulty in resisting drinking. CONCLUSIONS: From these results, the authors suggest that an acute treatment of haloperidol lowers alcohol craving in patients with alcohol dependence, but the effect does not maintain itself with chronic treatment.

8.Pain Management for the Cancer Pain by Continuous Intravenous Infusion of Ketamine: A case report.

Although a major portion of terminal cancer patients have appealed pain, the management of their pains is not easy. We managed two terminal cancer patients. One subject was a 86-year-old female who, after pronounced lung cancer, suffered from shoulder pain. The other was a 28-year-old female who, after being subject to pronounced acute lymphocytic leukemia, suffered from hip joint and thigh pain. In our practice, their pain, which had not been controlled by various other sophisticated management strategies, was effectively alleviated by a continuous intravenous infusion of ketamine, a N-methyl-D-aspartic acid receptor antagonist. The initial dose was administered intramuscularly by injecting 10 mg. of ketamine into the patients and then an intravenous infusion was administered at the rate of 0.06 mg/kg/hr. After the continuous infusion of ketamine was initiated, the VAS decreased, respectively, from 9 and 10 to 4 and 3, without considerable side effects. In conclusion, intravenous infusion at the level of sub-anesthetic dose of ketamine is a good management to control totally bed-conditioned and intactable cancer pain.

9.Sleep and Alcohol.

Alcohol has been used as sedatives historically. The effect of alcohol on sleep is different according to its dose, timing of ingestion, and drinking frequency. Sleep problems may play a role in the development and course of alcohol-related disorders. Insomnia in alcohol-dependent patients is common and early treatment of insomnia may reduce the rate of relapse. Sleep apnea, restless legs syndrome, periodic limbs movement disorder, and altered circadian rhythm may be more frequent in this patients. Management of sleep and alcohol problems is important in treating alcohol-related disorder and sleep disorders, respectively.

Interest in the prevalence of patent foramen ovale(PFO) and its relation to embolic stroke has increased with the sophistication of methods for noninvasive cardiac assessment. The effect of foramen ovale closure on the risk for subsequent strokes is promised. A 22-year old woman was presented with sudden onset of cerebral infarct. She had a patent formen ovale, and right to left shunt during the Valsalva maneuver, which was diagnosed by transesophageal contrast echocardiography. Transcatheter closure of PFO was performed with Rashkind PDA umbrella under the guidance of transesophageal echocardiography. Transcatheter closure of PFO can be accomplised with little morbidity and may reduce the risk of embolic episode.
