1.Atrial Flutter in Children.

To analyse the clinical characteristics of children with atrial flutter(AF1), we reviewed(the medical records of the children who had been admitted and diagnosed as AF1 or 12 leads surface ECG during the period between February, 1986 and Octobe, 1990. The onset age of 25 children was evenly distributed between gestational period and upto 14 years of age. The sex ratio between boys and girls was 11:14. The cardiac abnormalities were detected in 21 children(dilated cardiomyopathy in 3, congenital heart defect in 18). In 4 children, AF1 occured in apparently normal heart. In 9 children, AF1 developed without relation to cardiac surgery(4 in normal heart, 3 in dilated cardiomyopathy, 2 in preoperative CHD). AF1 developed in 9 children within 30 days postoperatively(group A) and in 7, after 30 days postoperatively(group B). In group A, various types of operation were done. In 2 of them, AF1 persisted beyond the immediate postoperative period and 1 died of cardiac decompensation 3 months postoperatively. In group B, operative procedures before AF1 were Senning operation in 2, Fontan operation in 2, operation for tetralogy of Fallot, ventricular septal defect with severe tricuspid regurgitation, endocardial cushion defect in 1 each. Five children in group B( except 1 Senning operation and 1 Fontan operation) had significant residual atrioventricular regurgitation. The associated rhythm disturbance besides AF1 could be determined in 23 children. The sinus node dysfunction was associated in 8 ; the supraventricular dysrhythmia in 10; the ventricular dysrhythmia in 6; the atrioventricular conduction disturbance in 3, and no other associated rhythm disturbance in 6. The efficacy of various methods in conversion of AF1 to sinus rhythm were 57.1%(4/7) with DC cardioversion, 38.5%(5/13) with digoxin and other antiarrhythmic drug(amiodarone, beta blocker, quinidine, verapamil, flecainide), 25%(5/20) with pacing in esophagus or endocardium. The follow-up evaluation was possible in 24 children. Eight children were dead : 5 due to chronic decompensation, 2 due to sudden cardiac arrest, and 1 due to sepsis. Of the 16 alive children, 14 had recovered the normal sinus rhythm and the other 2 had persistent AF1. Twelve children without AF1 and 1 with persistent AF1 were on medication, Although there were none cardiac death in children without persistent AF1, 7 out of 9 children with persistent AF1 were dead. Of the 7 children with significant atrioventricular regurgitation, corrective procedures were done in 4. There were no death in these 4 children, despite of 2 cardiac death out of 3 children on whom corrective procedure were not done. In conclusion, the atrioventricular regurgitation was a significant factor in relation to the occurrence and persistence of AF1 and the prognosis was bad in children with persistent AF1. It is recommended to trat the children with AF1 associated with atrioventricular regurgitation promptly and more aggressively.

3.A Case of Lichen Planus Treated with Griseofulvin.

Improvement of lichen planus was achieved by 11 weeks of daily oral treatment with griseofulvin. The patient was 59-year-old male, has had hypertension and diabetes mellitus for 9 months, and history of various drug intake to these diseses for 6 months. Four months before first visit, symptome of lichen planus had developed suddenly. Because of treatment failure of oral antihistamine and topical steroid for 6 weeks, we began to use griseofulvin. Praspective studies are needed to better assess the affectiveness of griseafulvin in the treatment of lichen planus.

4.A Case of Familial Benign Chronic Pemphigus.

We present a case of familial benign chronic pemphigus in 50-year-old male patient who had had recurrent oozing, macerated and eroded skin lesions on the groin and scrotum for 20 years. At first, we diagnosed his case as scrotal eczema and/or intertrigo, but after repeated recurrance, it was confirmed as above disease by characteristic histopathologic findings.

5.A Case of Generalized Syringoma.

We present a case of generalized syringoma in a 12-year-old healthy girl. The patient has numerous skin colored or yellowish papules on the face, neck, anterior chest, axillae and abdomen. She has a family history of eyelid syringoma occuring in her mother and maternal grandmother. Diagnosis was confirmed by characteristic histopathologic findings.

7.Comparison of Nutrient Intakes Regarding Stages of Change in Dietary Fiber Increasing for College Students in Kyunggi-Do.

This study was conducted to compare nutrient intakes regarding stages of change in dietary fiber increasing behavior. Subjects were consisted of healthy 383 college students (250 females and 133 males) in Kyunggi-Do. Stages of change classified by an algorithim based on 6 items were designed each subjects into one of the 5 stages: precontemplation (PC), contemplation (CO), preparation (PR), action (AC), maintenance (MA). Nutrient intakes were assessed by 24-hr recall method. Regarding the 5 stages of changes, PR stage comprised the largest group (39.4%), followed by AC (33.7%), MA (14.6%), PC (7.6%), CO (4.7%). Female were more belong to either AC or MA. The higher stage of change in dietary fiber increasing behavior, the higher self-efficacy. In all male and female, there were no differences in energy, protein, monounsaturated fatty acids, polyunsaturated fatty acids and cholesterol intakes across the 5 stages. But, fiber, potassuim (K), vitamin A and vitamin C intakes of AC or MA were higer than those of PC, CO and PR. Energy% from fat of PR (25.4 - 26.5%) was higher than 20%, and those of AC and MA was lower than the other groups. Dietary P/S and omega6/omega3 ratios of AC and MA were similar to the recommended ratio. Female of PR had the most total saturated fat and palmitic acid and those of MA had the least. Male of PR had the least alpha-LNA (omega3) and total omega3 fatty acids and those of MA had the most. In male and female in AC or MA, fiber and K intakes from breakfast, dinner and snack and vitamin C intakes from all meals were higher than those of the other stages. These results of our study confirm differences in stages of change in fiber intake in terms of nutritional status. To have lower energy% from fat, higher intakes of K, fiber and vitamins, desirable ratio of dietary fatty acids, it needs consistent nutritional education leading to the AC or MA of fiber increasing behavior.

8.Therapeutic Experience with Clocortolone Pivalate.

Puraotix-C(0.1% Clocortolone Picalats Cream) is a new di-halogenated topical corticosteroid, and used in trial for treating several kinds of dermatoses. We treated 30 patients with Purantix-C who had one of the following dermatoses; atopic dermatitis, neurodermatitis, pustulosis palmaris et plantsris, seborrheic dermatitis, pityriasis rosea, nummular eczema, psoriasis vulgaris, pruritus ani and pompolyx. Among these 30 patients, Purantix-C showed excellent or good result (66.7%) either unoccluded or occluded application, 2~3 times daily for 7~10 days. Comparing with Lidex(0.05% fluorocinonide cream) and purantix-C, both of them had similar effect on eczema and psoriatic groups but Lidex was more effective than Purantix-C for treating the patients with the pustulosis palmaris et plantaris. Purantix-C without containing perfume and coloring substapce was less irritative.

9.A case of Vulvar Syringoma.

10.A case of Solitary Glomus Tumor.

A 72 years old man had an 1 x 1cm sized, solitary glomus tumor on the right knee joint and had suffered from sharp, episodic pain and tenderness, radiating to the right thigh and the buttock for about 20 years. Simple surgical excision was performed for removal of the tumor mass and the subjective symtoms. There is no recurrence until now.
