1.Amblyopia and Sensory Fusional Anomaly in Intermittent Exotropia.

This study is designed to determine the frequency of amblyopia and sensory fusional anomaly in intermittent exotropia and the relationship between amblyopia and sensory fusional anomaly. Ninety eight intermittent exotropic patients were selected and using full corrected visual acuity, the selected patients were divided into amblyopia group and non-amblyopia group. Then, refractive errors, Bagolini lens test, Worth 4 dot test and stereoacuity test were measured and these values were statistically analyzed. The frequency of amblyopia among intermittent exotropia group was 14 patients[14.3%]. Anisometropia among amblyopia group was 6 patients[43.9%] and among non-amblyopia group was 6 patients [7.1%]. Anisometropia was statistically meaningful in amblyopia group compared with non-amblyopia group[p<0.002]. Sensory fusional anomaly among amblyopia group showed 50%at near distance and 64.3%at far distance, among non-amblyopia group 10.7% at near distance and 20.2%at far distance in Worth 4 dot test. Therefore, it appeared to be statistically meaningful in amblyopia group compared with non-amblyopia group[p<0.001, p<0.002]. Sensory fusional anomaly among amblyopia group showed 35.7%and among non-amblyopia group 14.3%in Bagolini lens test, it appeared to be statistically not meaningful[p>0.064]. In stereoacuity, it decreased statistically meaningful when amblyopia[p<0.003] or sensory fusional anomaly[p<0.019]were present.

2.Ret robulbar Optic Neuritis Associated with Multiple Sclerosis: Two Cases.

Optic neuritis is one of the most important clinical manifestations of multiple sclerosis. We experienced 2 cases of retrobulbar optic neuritis associated with multiple sclerosis. One was a 14-year-old female patient who presented with decreased visual acuity and visual field defect in right eye followed by paralysis of both lower extremities and the other was a 39-year-old female patient who presented with repeated paresthesia and motor weakness of upper and lower extremities then rapid decrease in visual acuity in both eyes. Both patients had relative afferent pupillary defect, color perception abnormality, variable visual field defect and abnormality in VEP. In fundus exam and fluorescene angiography, there was no specific abnormal finding. T2 weighted MRI showed multiple high signals corresponding to plaques. These two patients were diagnosed as multiple sclerosis. We should keep in mind that retrobulbar optic neuritis could be associated with multiple sclerosis as a presenting sign or as a manifestation during its clinical course.

4.A Case of Nonfunctioning Paraganglioma of the Posterior Mediastinum.

Paraganglioma is a tumor from the extra adrenal paraganglion system and is rarely observed in the mediastinum. The authors experienced a case of nonfunctioning paraganglioma of the posterior mediastinum. The patient was 34-years-old male in whom abnormal mass lesion was nites in chest radiograph with hemoptysis. His blood pressure and serologic examination were within normal range upon admission to our hospital. Chest CT revealed a tumor in the left lower lobe. Diagnostic thoracoscopy was performed and diagnosed a posterior mediastinal mass. Surgical resection was them performed. Posterior mediastinal mass was removed successfully and histological examination of the surgical specimen diagnosed paraganglioma. He received radiotherapy after surgery and was followed up. Related literature are reviewed.

5.Venous Irritation Incidence Associated with Vinorelbine Tartrate Injection Time.

PURPOSE: This study was to determine the incidence and severity of venous irriation in patients receiving vinorelbine tartrate (Navelbine ) in combination chemotherapy. MATERIAL AND METHODS: Twenty four patients histologically confirmed non-small cell lung cancer were enrolled in this study who receiving vinorelbine in combination chemotherapy through a peripheral vein from Oct. 1997 to Mar. 1999 with retrospective study design method. One group was 6~10 minutes infusion rate, the other was 10~20 minutes infusion rate with the same free-flow intravenous infusion. RESULTS: A total of 126 infusions were observed in this study. Sixty-two infusions were admi nistered at the 6~10 minutes, and 64 infusions were administered at the 10~20 minutes. The incidence of any venous irritation was 3.2% (2/62) in the group that received the infusion in 6~10 minutes and 10.9% (7/64) in 10~20 minutes (p=0.164), so we could not acquire any statistical significance. However the incidence of severe venous irritation (grade 3, 4) was 0% (0/62) in 6~10 minutes infusion group and 9.4% (6/64) in 10~20 minutes infusion group. There was a significant difference between two groups (p=0.028) CONCLUSION: Our results suggest that venous irritation associated with vinorelbine tartrate infusion can be reduced by shorter duration of administration and vinorelbine tartrate might be recom mended to administer at 6~10 minutes infusion in clinical practice.

6.The Feasibility of Short-term Dry Eye Animal Model Using Concanavalin A.

PURPOSE: To evaluate the possibility of short-term dry eye model in rabbits by injection of concanavalin A (conA) to the lacrimal and haderian gland of rabbits. METHODS: We injected conA (10 mg/ml, 0.05 ml) to the lacrimal and haderian gland of rabbits twice to induce inflammation of lacrimal gland and compared with saline-injected control by lacrimal gland biopsy with H&E staining for identification of inflammation. The ratio of lacrimal secretion was evaluated by Schirmer test (preinjection vs. postinjection of conA) for 10 days and the number of goblet cells was counted in 10 consecutive high power field using impression cytology with PAS staining. The corneal & conjunctival apoptotic cell deaths were investigated with TUNEL staining 10 days after injection. RESULTS: Infiltration of inflammatory cells and destructed normal architecture of lacrimal gland was found only in conA injected group till 10 days. The Schirmer test showed marked reduction (0.56+/-0.26) by 5days after injection compared with control group (1.07+/-0.35) (p=0.02) and its significant difference was maintained till 10days. The number of goblet cell was 9.70+/-5.03/x200HPF, which was statistically significant decreased compared to control (47.50+/-17.13/x200HPF) at 10 days (p=0.00). Apoptotic cells were increased in injected eye (26.20+/-4.27) compared with those in control (16.60+/-2.70). CONCLUSIONS: Injection of conA to lacrimal glands in rabbit shows decrease of lacrimal secretion and similar cytological changes of the cornea and conjunctiva in human dry eye patients. It suggests its possible feasibility of short-term dry eye animal model for the 10 days.

7.Isolation of Putative Limbal Epithelial Stem Cells by Fluorescein Activated Cell Sorting.

PURPOSE: To analyze the isolating pattern of slow cycling cells as putative limbal epithelial stem cells (PLESCs) using Hoechst exclusive cell sorting. METHODS: Rabbits were injected with 5-bromo-2-deoxyuridine (Brd U) 1 month prior to be sacrificed. After obtaining limbal tissues, fluorescence-activated cells were sorted on a Coulter EPICS 753 after they had been incubated with Hoechst 33342 and propidium iodide. Two different methods were applied to sort PLESCs. Side-population(Sp) cells were obtained using gates with dichroic mirror to detect low Hoechst blue and red after verapamil was treated. Hoechst negative cells were obtained using gates exhibiting low Hoechst blue with a 424/44 BP filter. Brd U-retaining cells were counted and their sizes were evaluated in each gated sample to compare isolating pattern of PLESCs in each method. RESULTS: The percentages of Sp cells and of the Hoechst negative fraction were 0.96 +/- 0.79% and 16.01 +/- 13.60%, respectively(p=0.021). Homogeneity and density of the small cells were higher in Hoechst negative fraction than in Sp cells. The percentage of Brd U-retaining cells was 47.36 +/- 10.34% and 47.14 +/- 14.94% in Sp cells and Hoechst negative fraction, respectively(p>0.05), and they were 10 times higher than in non-Sp and Hoechst positive fraction(p=0.000). CONCLUSIONS: Hoechst negative exclusion without verapamil more efficiently isolated PLESCs than Sp did.

8.Effects of Streptozotocin-Induced Type 1 Diabetes on Cell Proliferation and Neuronal Differentiation in the Dentate Gyrus; Correlation with Memory Impairment.

We examined the effects of steptozotocin (STZ)-induced type 1 diabetes on cell proliferation and neuroblasts in the subgranular zone of the hippocampal dentate gyrus (SZDG) of male Wistar rats. Change in memory function was also investigated using the passive avoidance test. In the SZDG, Ki67 (a marker for cell proliferation) positive nuclei were significantly decreased at 2 and 3 weeks and slightly decreased at 4 weeks after STZ treatment. Doublecortin (DCX, a marker for neuronal differentiation)-immunoreactive (+) neuroblasts with tertiary dendrites were significantly decreased in the STZ-treated group compared to those in the vehicle-treated group. However, DCX+ neuroblasts without tertiary dendrites were abundant at 4 weeks after STZ treatment. In addition, retention latency time in STZ-treated group was similar to that of vehicle-treated group at 2 and 3 weeks after STZ treatment. However, the retention latency time was significantly decreased at 4 weeks after STZ treatment. These results suggest that STZ significantly reduced cell proliferation and neuroblasts at 2~3 weeks after STZ treatment, but not at 4 weeks after STZ treatment although memory impairment was detected at 4 weeks after STZ treatment. The gradual reduction of DCX+ neuroblasts with tertiary dendrites may be associated with the impairment of hippocampus-related memory function.

9.Changes in Calcium-binding Proteins in the Rat Hippocampus after Involuntary Treadmill Exercise.

In this study, we investigated the effects of treadmill exercise on hippocampal levels of calcium-binding proteins - calbindin D-28k (CB), calretinin (CR) and parvalbumin (PV) - using immunohistochemistry and Western blot analysis. At 6 weeks of age, male Wistar rats were put on a treadmill with or without running for 1 h/day/5 consecutive days at a pace of 22 m/min for a period of 5 weeks. In sedentary and exercise groups, CB immunoreaction was detected in granule cells of the dentate gyrus, mossy fibers, and CA1 pyramidal cells. In addition, CB immunoreaction was observed in interneurons of the CA1-3 region. Exercise significantly increased CB immunoreactivity in dentate granule cells, CA1 pyramidal cells and CA1-3 interneurons. CR immunoreaction was mainly observed in interneurons of the dentate gyrus and CA1-3 regions. Similar number of CR-immunoreactive neurons was observed in the exercise and sedentary groups. PV immunoreaction was detected in interneurons of the dentate gyrus and CA1-3 regions. PVimmunoreactive fibers were significantly increased in all regions of the hippocampus in the exercise group, as compared to the sedentary group. Similar to the immunohistochemical findings, protein levels of CB and PV were also increased in the exercise group compared to the sedentary group. These increases in CB and PV in the hippocampus may induce neuronal plasticity after treadmill exercise and may be related to the enhancement of synaptic plasticity in the hippocampus by exercise.
