BACKGROUND: The consensus is that a bony Bankart lesion shorter than 25% of the length of glenoid does not affect the clinical result; hence, such lesions were often neglected. However, small bony Bankart lesions are associated with various types of capsulolabral lesions. METHODS: A total of 82 patients who had undergone arthroscopic capsulolabral lesion repair surgery for anterior shoulder dislocation were reviewed. The prevalence rates of early and late type of capsulolabral lesions were compared between a group of patients with and a group without small bony Bankart lesions. In addition, the types of accompanying capsulolabral lesion were analyzed according to the type of bony Bankart lesion. Finally, the clinical outcomes were evaluated (active range of motion, American Shoulder and Elbow Surgeons score and Rowe's score). RESULTS: Among the 13 patients who had small bony Bankart lesions, the prevalence rate of early and late type of capsulolabral lesions was 38.5% and 61.5%, respectively. Among the 69 patients without bony Bankart lesion, the prevalence rates of early and late type of capsulolabral lesions were 74% and 26%, respectively. Significantly worse clinical outcome was observed for the group of patients with both small bony Bankart lesions and late type of capsulolabral lesion. CONCLUSIONS: More severe type of small bony Bankart lesion appears to be associated with late type of capsulolabral lesion. The significantly worse clinical outcome for patients with both small bony Bankart lesion and late type of capsulolabral lesion indicates that small bony Bankart lesions cannot always be neglected.

This study was conducted in order to find out the attitude of housewives toward the home attendants system, the type of institute managing home attendants, the education for home attendants, the basic requisites for home attendants, and home nursing education, and identify the factors influencing on the housewives's attitude. The data were collected through questionnaires obtained from 401 housewives from March 1 to March 31, 1988. The results summarized were as follows; 1) 90.2 percent of total housewives favored the home attendants system, and housewives who had have their family member with health problem especially showed favorable response. 2) The favored types of institution managing home attendants appeared to be private social work agency(33.0%), half-private and half-public agency(32.2%), government agency(19.9%) and profit agency(2.2%). In particular, housewives who had experienced nursing favored half-private and half-public agency(37.6%). 3) As the basic requisites for home attendants, housewives wanted service attitude(37.2%), nursing education(34.4%) and nursing experience(28.4%). But, housewives with nursing experience favored nursing education rather than service attitude. 4) 91.1% of total housewives favored the home nursing education. In particular, housewives of high education level and nursing experience showed more favorable response to the home nursing education.

BACKGROUND: The proportion of patients with subacute cutaneous lupus erythematosus(SCLE) is considered to be 10% among the total LE population in western countries. The morphology of LE-specific skin lesions in SCLE : appear as papulosquamous erythema or annular erythema developing mostly on the exposed areas. Among Korean patients with LE the prevalence and the pattern of cutaneous eruption of SCLE may be different from those patients of western countries. OBJECTIVE: To obtain the prevalence and the morphological characteristics of cutaneous lesions in Korean patients with SCI E v;ere the main objectives. METHODS: With 7 cases of SCLE, clinical dermatologic examinations for the character, the distribution, and the clinical course of those cutaneous lesions, and for the extent of systemic symptoms and LE-related laboratory abnormalities in each patient were performed. Results/Coeclusion. The prevalence of SCLE among Korean patients with LE or latent LE was much lower than those seen in Caucasians. The pattern of cutaneous lesions were papular/ papulosquamous erythemas with no (or rare) cases of annular erytherna among those patients diagnosed as SCLE. ~Other cl~inica! features were similar to Caucasian patients. It seems that there are some racial differences in the cutaneous manifestat.ions of LE, which may be relevant to the dissimilarities in immunogenetic backgrounds between different racial groups.

Pituitary apoplexy, the sudden infarction, necrosis or hemorrhage into a pituitary adenoma may be threatening life and is difficult to diagnose. With an advent of CT, early diagnosis of hemorrhage into the pituitary adenomais possible if one has familiarity with its characteristic CT changes. Two cases of pituitary apoplexy are reported with brief review of the literatures. One case had a history of radiation therapy for pituitary adenoma 7 years ago, and the other had no symptoms or sings of pituitary tumor prior to onset of disease.

The purpose of this study was to assess the results of passive release of the pulley with needle under local anesthesia performed at the out patient department. There were 68 fingers of 54 patients. Fifteen patients(27.8 %) were male and 39 patients(72.2 %) were female. Involved fingers were 28 thumbs(41.2 %), 7(10.3 %) index fingers, 14(20.6 %) middle fingers, and 19(27.9 %) ring fingers. Ages were varied-between 41 years and 61 years old. Rheumatoid fingers were 12(17.6 %). The others had no underlying disease. An 18 gauge needle was introduced distal to the palpable nodule. The pulley was released passively when the PIP joint was extended. The follow-up period was 4.5 years on average(range; 1-8 years). At final follow-up local recurrence was noted in 1-2 weeks in 7 cases(10.3 %), which was due to technical fault in early trial cases. The patients complained of pain for a few days and mild tenderness for less than 2 weeks. In three cases adhesion occurred, which was resolved in a week by maniqulation. There was no difference between rheumatoid arthritis and the other underlying diseases as far as rate of recurrence was concerned. From these results it would be suggested that the passive release of pulley with needle is an effective method for the treatment of adult trigger finger.

The multistage maximal exercise test was performed to 135 cases with various heart disease and 207 control group between jan. 1980 and Dec. 1980 in KoreaGeneral Hospital. The change of heart rate, blood pressure and ST segment in various heart disease were compared with those in control group. The results were followings; 1. There were 130 male and 77 female of cases with control group, whose mean age were 50 years old. 2. General check up was the most common underlying disease of control group(67 patients) and the others were gastritis (28), neurosis (24), diabets mellitus (17), hepatitis (4) and peptic ulcer (4) frequency. 3. Change of heart rate, blood pressure and ST segment in control group during GXT were 89~154, 119/80~157/88 mmHg and 0.6~2 mm. 4. There were 81 male and 51 female of cases with various heart disease group, whose mean age were 50 years old. 5. Atherosclerotic heart disease was the most common underlying disease of various heart disease group (49 patients) and the others were hypertension (34), hypertensive cardiovascular disease (22), labile hypertension (14), angina (7), arrythmia (6) and others (3) in frequency. 6. Change of heart rate, blood pressure and ST segment in various heart disease group during GXT were 85~148, 140/93~178/102 mmHg and -0.7~1.3mm. ST segment in atherosclerotic heart disease, hypertension, hypertensive cardiovascular disease, valvular heart disease and myocardial infarction were significantly depressed(p<0.05) compared with ST segment in control group.
