6.Diagnosis and Clinical Features in Children Referred to Developmental Delay Clinic.

OBJECTIVE: To determine the phenomenologic, etiologic diagnosis and clinical features of children with developmental delay. METHOD: One hundred seventy-one children, referred to Developmental Delay Clinic which was multidisciplinary clinic for the evaluation of a suspected developmental delay, were prospectively enrolled. Diagnostic yield was ascertained after the completion of the questionnaire, clinical assessments and laboratory investigations by the physiatrist, pediatric neurologist and pediatric psychiatrist. RESULTS: One hundred fifty-one children met study criteria. The common phenomenologic diagnoses of children with developmental delay were mental retardation, delayed language disorder, autism and cerebral palsy. The etiologic diagnosis was determined in 44 (28.02%) children. The diagnoses were hypoxic-ischemic encephalopathy, malformation of cortical development, dysmorphic syndrome, chromosomal abnormalities, and neuomuscular disorders in the order of frequency. In many cases, the chief complaint of parent was not in accordance with final diagnosis. CONCLUSION: In the clinical assessment and management of children with developmental delay, the most important thing is integrative and comprehensive approach including all the developmental territories. And also, the settlement of paradigm for systematic evaluation of these children with other specialists will be needed.

7.An Anesthetic Experience with the Method of Continuous Intravenous Infusion of Thiopentone-D-Tuhoeurarine Mixture .

Since April 1972, we have used the method of continuous intravenous infusion of thiopentoned tubocurarine mixture in 74 cases which were not required fully muscular relaxation during operation such as orthopedic operation, laminectomy, and mastoidectomy etc. The results were as follows. 1. The average needs of thiopentone and d-tubocurarine were 430mg/hr and 6.5mg/hr. 2. There were no specific changes on blood pressure, pulse rate and respiration. 3. The average recovery time was 20 minutes. 4. This method was very simple, economic and no risk of explosion.

8.The Clinical Analysis of 100 cases of Coronary artery Bypass Grafting with the Right Gastroepiploic artery.

BACKGROUND: In an effort t enhance long term patency of coronary bypass grafts, utilization of arterial conduits have been on an icrease. With the same objective, we have been using the right gastroepiploic artery (RGEA) in coronary artery bypass procedures since 1998. The current paper has been undertaken with the aim of assessing the apropriateness, problems, and short term results of using the RGEA as an arterial graft conduit by studying the postoperative clinical results of 100 patients than received coronary artery bypass grafting (CARG) with this artery. MATERIAL AND METHOD: Between May of 1998 and May of 1999, an analysis of the mortality, postoperative myocardial infarction, and the need for IABP insertion as a result of low cardiac output were made between 100 consecutive patients undergoing CABG with the RGEA. There was one postoperative death due to cerebral infarction. Postoperative complications/morbidity comprised myocardial infarction in 2, cerebral infarct in 3, reoperation due to bleeding in 1, mediastinitis in 1, and low cardiac output syndrome necessitating IABP in 3 patients. Complicatons related to harvesting of the arterial grafts were not experienced in any of the patients. CONCLUSION: The results of the current data show that utilization of the RGEA in CABG is not associated with increased mortality/morbidity and demonstrates satisfactory short term results suggesting the usefulnessof this conduit as an arterial graft.

9.The usefulness of minimal F-wave latency and sural/radial amplitude ratio in diabetic polyneuropathy.

The possibility of whether minimal F-wave latency and a simple ratio between the sural and superficial radial sensory response amplitudes may provide a useful electrodiagnostic test in diabetic patients was investigated in this report. To evaluate the diagnostic sensitivity of minimal F-wave latency, the Z-scores of the minimal F-wave latency, motor nerve conduction velocity (MCV), amplitude of compound muscle action potentials (CMAP), and distal latency (DL) of the median, ulnar, tibial, and peroneal nerve were compared in 37 diabetic patients. For the median, ulnar, and tibial nerves, the Z scores of the minimal F-wave latency were significantly larger than those of the MCV. In addition for all four motor nerves, the Z scores of the minimal F-wave latency were significantly larger than those for the CMAP amplitude. Furthermore, 19 subjects showing abnormal results in the standard sensory nerve conduction study had a significantly lower sural/radial amplitude ratio (SRAR), and 84% of them had an SRAR of less than 0.5. In conclusion, minimal F-wave latency and the ratio between the amplitudes of the sural and superficial radial sensory nerve action potential are sensitive measures for the detection of nerve pathology and should be considered in electrophysiologic studies of diabetic polyneuropathy.

10.Clinical Characteristics of Psoriasis Patients with Elevated Total Serum Immunoglobulin E Levels: A Retrospective Single-Center Study

Background:
Psoriasis is a chronic T17 cell-driven immune-mediated inflammatory disease. However, patients with psoriasis may have elevated total serum immunoglobulin E (IgE) levels, which is a hallmark of Th2 inflammation.In previous case reports, psoriasis patients with elevated total serum IgE levels did not respond well to treatment or had exacerbated eczema lesions.
Objective:
We sought to investigate the clinical characteristics of psoriasis patients with elevated total serum IgE levels.
Methods:
This is a retrospective chart review of 130 patients with psoriasis who were tested for total serum IgE levels from November 1, 2009, to October 31, 2019. We compared the demographics, clinical characteristics, disease severity, and treatment regimen for each elevated IgE group (>214 U/mL) and normal IgE group (≤214 U/mL).
Results:
Among 130 patients with psoriasis, 41 (31.5%) had elevated total serum IgE levels. Elevated total serum IgE levels were positively associated with the severity of disease; psoriasis lesions including those on the scalp, hands, feet, and flexures, which are difficult to treat, were observed to be significantly higher in patients with elevated total serum IgE levels. There was no correlation between age, sex, disease duration, or presence of pruritus and total serum IgE levels.
Conclusion
This study identified the clinical characteristics of psoriasis patients with elevated total serum IgE levels in Korea.
