3.Survival Prediction of Ganglio-thalamic Hemorrhage Accompanying Intraventricular Hemorrhage Using CT Scan Indices.

To develop a survival prediction model and to use it as a therapeutic guideline a series of 66 cases with ganglio-thalamic hemorrhage accompanying intraventricular hemorrhage were analyzed retrospectively. They were classified into the benign and fatal groups according to their final outcomes. Fourteen indices obtained from the initial CT scans were final outcomes. Fourteen indices obtained from the initial CT scans were subjected to multivariate discriminant analysis. The resultant discriminant function(Z) included the amount of hematoma in the parenchyme(AH), bicaudate cerebroventricular index(BCCI) maximum fourth ventricular width(FVW), and third cerebroventricular ratio(TCR) in an order of decreasing discriminating power and was as follows: Z = -3.2639 + 0.3508 X 10(-1) X AH + 6.8816 X BCCI + 0.1139 X FVW-5.7794 X TCR. This function predicted survivability with accuracy of 84.9% when it was applied to the original subjects. The conclusion is that AH, BCCI, FVW, and TCR are the potent predictors of the survival of patients with ganglio-thalamic hemorrhage accompanying intraventricular hemorrhage.

Dengue fever is an important health problem for international travelers to all endemic areas. The steadily increasing numbers of tourists visiting endemic areas raise the risk of exposure, and imported dengue cases are increasingly observed in nonendemic area. Dengue has a wide spectrum of clinical presentations, often with unpredictable clinical evolution and outcome. While most patients recover following a self-limiting, non-severe clinical course, a small proportion progress to severe disease such as dengue hemorrhagic fever or dengue shock syndrome. Therefore, it is important to suspect dengue fever in every febrile patient returning from the tropics. Whenever it is suspected, a quick diagnosis and adequate managements are essential to avoid complications. We report two cases of imported dengue fever in Korean children presenting with fever, headache, nausea, and rash.

6.The Occurrence of Retinopathy of Prematurity in Twins.

We compared 40 twin and control groups(birth weights <2500gm, gestational ages <37 weeks or oxygen therapy) to analysis twins as a risk factor of ROP. control groups were consisted of group 1(40 patients) and group 2(40 patients). The former birth weight was similar to twin group and the latter gestational age was similar to twin group. The results were as follows; 1. The incidence of ROP was 37.5% in twin group, 10% in group 1 and 27.5% in group 2, The incidence of ROP in twin group was highter than control groups, and there was statistically significant difference in incidence between twin group and group 1(p<0.05). 2. The incidence of stage 2 or worse ROP was 18% in twin group, 10% in group 1 and 23% in group 2, There were no statistically significant difference among each groups(p>0.05). 3. In case-control study of other associated possible risk factors, twin group had significantly higher rate of premature rupture of membrane than group 1 and C-section and hypoxia than group 2(p<0.05).

Adenomas of anogenital mammary-like glands have been rarely reported with various nomenclatures in the English literature. 8.Psoriasiform Sarcoidosis.

Sarcoidosis is a systemic granulomatous disease of undetermined etiology and pathogenesis that involves the skin and many of the internal organs. It involves any organ of the body; how-ever, the most common sites are the lung, lymph node, skin, and eye. Psoriasiform eruption is a rare morphologic manifestation of sarcoidosis. We report herein a case of 36-year-old man with psoriasiform papules on the elbows, forearms and knees of 5-year duration. Histopathologic examination revealed sarcoid granuloma throughout the dermis. This is the second case of psoriasiform sarcoidosis in Korea.

9.The Effect Intravitreal Dexamethasone and Antibiotic Therapy after Vitrectomy.

Bacterial endophthalmitis is an ocular emergency that requires rapid diagnosis and therapeutic decision making. The introdection of intravitreal injection of antibiotics has been a major advancement because it has resulted in a marked improvement in visual outcome. The intravitreal injection of steroids may be potentially useful in the treatment of endophthalmitis and other ocular inflammatory diseases. Forty eyes of pigmented rabbits were used, and divided into two groups. Group I was eyes without vitrectomy. In the right eye, 100 microliter of 1mgvancomycin, 400 microliter amikacin and 400 microliter dexamethasone injected was done. Group II was eyes with vitrectomy and lensectomy. At 2 weeks after lens and vitreous removal, rabbit eyes received an injection of a combination of 1mg vancomycin, 400 microliter amikacin and 400 microliter dexamethasone in right eye and BSS in left eye. The effect of combination injection was examined by light and transmission, scanning electron microscope at 3 days, 1 week, 2 weeks and 6 weeks following injection. The injection of combination without vitrectomy produced no toxicity. After injections of either combination or BSS after vitrectomy, macrophages were observed on the surface of retinal pigment epithelium and disorganized outer segments. This finding seems to be produced by vitrectomy procedure rather than drug toxicity. These results supports the hypothesis that the injection of these combinations is not toxic to aphakic/vitrectomized eyes.

10.Seizure Induced by Photosonic Device.

BACKGROUND & SIGNIFICANCE: Photosensitive epilepsy may occur by the diverse new implements. We experienced three patients suffered from the first seizure while using photosonic devices. The device is composed of light stimulating goggles and sound producing headphones. CASE: All the patients were teen-aged school girls and the types of seizure were generalized tonic-clonic seizure, focal motor seizure with secondary generalization, and complex partial seizure. They lost their consciousness while using the devices with the red colored light. Events began without aura in two but one patient felt continuous red flash light until after taking off the device. EEG revealed right frontal dominant spike-wave complexes in one but other patients showed no epileptiform discharges. During a year follow-up the patient who showed EEG abnormalities experienced another seizure that was focal motor seizure with secondary generalization but remained two did not. COMMENTS: Though the video game epilepsy is regarded as a kind of photosensitive epilepsy, the emotional excitement may contribute to the occurrence of seizure. But, this photosonic device is used for relaxation and our three patients showed normal EEG while photic stimulation during the tracing. We suggest that seizure induced by the photosonic lev genuine type of photosensitive epilepsy.
