2.Clinical comparative study between Uniportal and single port video-assisted thoracoscopic surgery forⅠA stage non-small cell lung cancer

Objective To evaluate the clinical effect of Uniportal video -assisted thoracoscopic surgery and single utility port video-assisted thoracoscopic surgery for ⅠA stage non-small cell lung cancer .Methods A total of 81 patients with ⅠA stage non-small cell lung cancer was admitted to our hospital from January 2015 to November 2015 .The patients were divided into control group and observation group .51 patients of control group received single utility port video -assisted thoracoscopic surgery , while other 30 patients of observation group received Uniportal video -assisted thoracoscopic surgery .Results The incision length of observation group was(4.27 ±0.29) cm,operative time was (208.80 ±61.12) mins.The volume of blood intra -operation was (92.33 ±73.75)mL;the number of lymph nodes dissection was (15.62 ±5.12),post-operative drainage within 24 hours was(401.70 ±53.31)mL;drainage tube retention was (6.30 ±3.01)days;postoperative hospitalization time was(19.03 ±5.85)days.The incidence rate of postoperative complications was 13.33%.The incision length of control group was(3.86 ±0.23)cm.The operative time was(184.30 ±51.36)mins;The volume of blood intra-operation was(84.90 ±80.98)mL,the number of lymph nodes dissection was (15.84 ±5.66),post-operative drainage within 24 hours was(398.00 ±52.73)mL;drainage tube retention was(6.10 ±3.25)days;postoperative hospitalization time was(18.69 ±6.81) days;The incidence rate of postoperative complications was 25.49%, there were no significant difference between the two groups (P>0.05).VAS pain score of the observation group was(2.32 ±0.94)and it was(4.18 ±0.95)in control group,and observation group was significantly better than the control group(P<0.05).Conclusion Uniportal video -assisted thoracoscopic surgery has the benefit of less trauma and pain ,and it is worth using widely .
3.Effects of Roux-en-Y gastric bypass and sleeve gastrectomy on morbidly obesity

Objective To evaluate the effects of laparoscopic Roux-en-Y gastric bypass and sleeve gastrectomy on weight control in morbidly obese patients.Methods 48 patients with morbidly obesity underwent obese surgery (laparoscopic Roux-en-Y gastric bypass and laparoscopic sleeve gastrectomy) from Dec.2013 to Dec.2015,and 1-year follow-up study.The general situation of obesity and biochemical indicators were compared and analyzed.Results There were 20 cases in gastric bypass group and 28 cases in sleeve gastrectomy group.The excess weight loss percentage(%EWL)was 73±0.04 in gastric bypass group and 59±0.05 in sleeve gastrectomy group(P=-0.03).There was no statistical difference in weight change between the two groups.Low density lipoprotein cholesterol (LDL) level in gastric bypass group was significantly lower than that in sleeve gastrectomy group at 1 year after surgery (2.42±0.16 vs 3.29±0.20,P=0.006),while folic acid level was significantly higher than that in sleeve gastrectomy group(15.58±1.99 vs 7.59±1.59,P=0.022).Conclusions The one-year follow up shows there is no significant difference in the efficacy of weight control between the two groups.Gastric bypass group may be superior to gastrectomy group in nutrition indexes.However,it is just a short-term postoperative efficacy in morbidly obese patients,long-term weight control effects and obesity related diseases control need to be further explored.
4.Resveratrol inhibited hepatic fibrosis in mice with schistosomiasis japonica by modulating Th1 and Th2 responses

Aim To observe the antifibrogenic effect of resveratrol on mice with schistosomiasis japonica and its effect on Th1 and Th2 responses .Methods Forty-five mice infected with S.japonicum cercariae for 3 weeks were randomly divided into three groups named as infection group ( A) , resveratrol group ( B) and praz-iquantel group ( C) .Fifteen normal mice were taken as normal control group ( D) .In the 13th week post-infec-tion, all mice were sacrificed and the liver tissues were removed.Histopathological changes were observed in the liver of all groups .Splenocytes were prepared from spleens of mice with S.japonicum infection and the proportions of Th1 and Th2 cells in T cells were deter-mined by FACS respectively .RT-PCR was used to de-tect the relative IFN-γ,IL-13,TGF-βmRNA levels in liver tissue .Results After treatment , the degrees of liver fibrosis in groups B and C decreased in the 13th week post-infection ( P <0.01 ) .Compared to group A, the proportions of Th1 cells in group B significantly increased ( P<0.05 ) and the proportions of Th 2 cells in group B decreased significantly ( P <0.01 ) .The level of anti-SWA IgG 2 a in group B was significantly higher ( P<0.05) , while the anti-SEA IgG1 level in group B was lower ( P <0.01 ) than that in group A . The hepatic expression of IFN-γmRNA level in group B was higher than that in group A ( P<0.05 ) , and IL-13 ,TGF-βmRNA levels in group B were lower than in group A ( P<0.01 ) .Conclusion Resveratrol has an antifibrogenic effect through upregulating Th 1 cell re-sponse and downregulating Th 2 cell response in mice infected with Schistosoma japonicum.
5.The clinical value of " Push-pull traction-relax homing-Repeatedly confirmed" in the prevention of bile duct injury in LC

Objective To study the value of the technique of "Push-pull traction-relax homingrepeatedly confirmed" in the prevention of bile duct injury in LC. Methods From March 2001-August 2009, we applied this technique in 4800 cases of LC. The technique of "Push-pull traction" showed the structures of in the Calot's triangle. The technique of "relax homing" was to restore the cystic duct,hepatic duct and common bile duct to their original anatomical positions. The technique of "repeatedly confirmed" repeatedly identified the positions of the cystic duct, the common hepatic duct and the common bile duct. Results There was no bile duct injury. Conversion to open surgery happened in 118patients due to difficulties in identifying the Calot's triangle structures, bile duct stones, gallbladder cancer, and gallbladder-duodenal fistula. Conclusions The "Push-pull traction-relax homing-repeatedly confirmed" technique could effectively prevent bile duct injury in LC. The method is simple, easy to master and worthy of promotion.
6.Establishment and evaluation of a mouse model of sepsis

Objective The purpose of this study was to establish and evaluate a mouse model of sepsis for studying the mechanism of sepsis and development of anti-inflammatory drugs.Methods The sepsis in mice was induced by cecal ligation and puncture ( CLP) .The survival rates, microbial load, liver and kidney damages, cytokines and pathological changes were detected to evaluate the mouse models.Results The death of mice was closely related with the ligated sites. The mice with 50%cecal ligation displayed about 40% of 12-day survival rate, however, all the mice with 75% cecum ligation died within 4 days (P<0.01).Compared with the sham surgery group, the mice with 50% cecal ligation had a high microbial load in the blood and abdominal cavity.Leukopenia was also emerged (P<0.001).CLP mice demonstra-ted elevated levels of serum ALT, AST and BUN (P<0.01).The levels of IL1α, IL6, IL10, MIP1α, MIP1β, and TNFαwere increased a lot.The liver and lung showed obvious pathological injury at 48 h post CLP.Conclusions The established mouse model of CLP shows typical characteristics of sepsis and is an ideal tool for further study of anti-inflam-matory drugs.
7.Effect of Pien Tze Huang on bile excretion and its analgesic effect in experimental animals

ObjectiveTo investigate the effect of Pien Tze Huang on bile excretion and its analgesic effect in experimental animals. MethodsA total of 30 guinea pigs were randomly divided into Pien Tze Huang group, ursodeoxycholic acid group, and control group. According to body mass, the three groups were treated with Pien Tze Huang (140 mg/kg), ursodeoxycholic acid (22.5 mg/kg), and normal saline, respectively, by gavage for 4 consecutive days. Then bile drainage was performed to measure the volume of bile secretion and the change in bile composition. Related liver function parameters were also measured. A total of 30 mice were randomly divided into Pien Tze Huang group (360 mg/kg), ursodeoxycholic acid group (120 mg/kg), and control group. At one hour after the last administration, the mice were given intraperitoneal injection of 0.6% glacial acetic acid (0.15 ml/10 g), and the writhing test was performed to observe the analgesic effect of Pien Tze Huang. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsCompared with the control group, the Pien Tze Huang group and the ursodeoxycholic acid group had a significant increase in the volume of bile secretion (P=0.039 and 0.009). There were no significant differences between the three groups in cholesterol, bilirubin, total bile acid, and phospholipid in bile (all P>0.05) and liver function parameters (alanine aminotransferase, aspartate aminotransferase, gamma-glutamyl transpeptidase, total bilirubin, direct bilirubin, indirect bilirubin, and total bile acid) (all P>0.05). Compared with the control group, the Pien Tze Huang group had a significant reduction in the number of writhing times (P<0.001), suggesting that Pien Tze Huang had a marked analgesic effect. ConclusionAnimal experiments show that Pien Tze Huang has marked choleretic and analgesic effects, which provides theoretical and data support for the clinical application of Pien Tze Huang.

9.Management of hepatic artery stenosis after orthotopic liver transplantation

Objective To determine the timing for therapy and efficacy for different types of hepatic artery stenosis (HAS) after orthotopic liver transplantation(OLT). Methods From October 2003 to May 2007, a total of 21 patients had hepatic artery stenosis after OLT in this hospital. Of the 21 patients, 19 underwent stent placement in their narrowed hepatic arteries and 2 were regularly followed up. Liver function, clinical outcomes, and the hepatic artery potency were reviewed. Results The occurring rate of HAS was 3.43% (21/613) and its median time of diagnosis was 146 days (range, 2-515 days). Six patients with early HAS were treated with interventions and 2 of them died.For the 4 surviving patients, 2 received retransplantation. For the 15 patients with late HAS, 13 were treated with interventions and 4 of them died. Two patients received retransplantation. Seven surviving patients had abnormality in liver function. Another 2 patients had normal liver function because of hepatic portal form compensatory circulation. Conclusion The presence of ischemic bile duct lesion and whether or not favourable compensatory circulation exists or not should be considered before individualized therapeutic regimens adopted according to postoperative HAS types.
10.Analysis of patient experience at public hospitals in China

Objective To get an understanding of the patient experience in public hospitals nationwide, and to evaluate the implementation of the Action Plan to Improve Health Care. Methods Supported by the mobile technology, from September 6, 2017 to December 15, 2018, the authors conducted an online survey that measured the satisfaction of both inpatients and outpatients at secondary and tertiary hospitals across the country. 15 questions from six dimensions including registration experience, patient-doctor communication, nurse-patient communication, the healthcare signage system, responsiveness of care providers and privacy protection were prepared for outpatients, while 20 questions from nine aspects such as nurse-patient communication, patient-doctor communication, pain management, medication communication, admission and discharge information, responsiveness of care-givers, food service, friendliness to patient family, and the healthcare signage system were directed at inpatients. Descriptive statistical analysis was used to describe the basic features of the data. Results 9.18 million valid responses from outpatients and another 5.38 million from inpatients were obtained. The overall satisfaction rate with outpatient services had reached a score of 90.45 points where nurse-patient communication stands out as the top-rated dimension and privacy protection gets the lowest rating. On the other hand, the inpatient satisfaction stands at a score of 93.01 with friendliness to patient family receiving the top score and patient-doctor communication the lowest. Conclusions Despite the positive feedback Chinese patients give on the outpatient care they receive, we should make efforts to improve the outpatient care environment, the wayfinding system, privacy protection, and responsiveness of care-givers.
