1.Expression level of nuclear factor-kappa,substance P and aquaporin 4 gene in the joint synovium and the change of discharge frequency of pain sensitive neuron in the ventral posteriolateral thalamic nuclens of acute gout rats

Objective To investigate the expression level of nuclear factor-kappa(NF-κB),substance P(SP)and aquaporin 4(AQP4)gene and the change of discharge frequency of pain sensitive neurons (PSN)in the ventral posteriolateral thalamic nucleus(VPL)of acute gout(AG)rats.The neuro-genic inflammation of AG was explored.Methods Forty-eight rats were randomly divided into two groups:the control group and the AG group. According to the time interval after injection of monosodiumurate(MSU)into the the unilateral ankle joint,the AG group was subdivided into seven groups,ie.0.5 hours group,2 hours group,6 hours group,12 hours group,24 hours group,48 hours group and 72 hours group.There were 6 rats in each group.After recording of the discharge frequency of PSN in rats VPL nucleus,the expression level of NF-κB,SP and AQP4 gene in the rats joint synovium were evaluated at mRNA level by PCR in above mentioned time.Results In the 0.5 hours group,the discharge frequency of PSN in the rats VPL nueleus and the expression level of SP gene in the rats joint synovium had increased immediately after the injection of MSU(P<0.05).In the 6 hours group,they reached the peak level(P<0.05),and in the 12 hours group,they began to decrease gradually(P<0.05).In the 0.5 hours group,the expression level of NF-κB and AQP4 gene increased after the injection of MSU(P<0.05).However,their peak level presented at the 12 hours(P<0.05),and they decreaged after 24 hours(P<0.05).The statistical analysis of correlation had shown that there were positive correlation among the expression level of NF-κB,substance P.AQP4 gene and the change of discharge frequency of PSN in the rats VPL nucleus.Conclusion The discharge frequency of PSN in the rats VPL and the expression level of SP gene in the rats ioint synovium can be used to evaluate the Severity of pain in the AG rats.The expression level of NF-κB and SP gene can reflect the severity of neurogenic intlammation.We can know the severity of edema of the joint synovium by detecting the expression level of AQP4 gene.Pain and neurogenic inflammation play an important role in the pathogenesis of AG.Combingelectrophysiology and biochemical technique can shade light on the pathogenesis of AG from different aspects.In the meantime,it may provide a new method for developing new drugs and new approaches for clinical treatment.

Rheumatoid arthritis(RA),a systemic,inflammatory autoimmune disorder characterized by inflammation of the lining or synovium of the joints,is a complex polygenic disease with a complicated inheritance mode.Both genetic and environmental factors determine the development and progression of RA.Study on susceptible genes of RA provides a theoretic basis for clinical diagnosis and treatment.As a novel high-throughput screening method,genome-wide association study(GWAS) is a powerful approach for mapping susceptible genes of polygenic disease like RA.GWAS can not only verify the well-established susceptible loci,but also identify novel genetic loci candidates.Great improvement has been made in using GWAS to screen for novel genes,which casts new lights on the mechanism and treatment of RA.This review summarizes the progress in using GWAS for screening of RA susceptible genes.
3.Influence of glucocorticoid on ischemia-induced neuronal damage in gerbil hippocampus

Objective To investigate the effect of glucocorticoid on ischemia-induced neuronal damage in gerbil hippocampus with forebrain ischemia model.Methods Twenty-one gerbils were randomly assigned to 3 groups (7 animals each). The animal was anesthetized and both common carotid arteries were exposed and separated. Silk threads were looped around these arteries. In group A and group B ,10 ?l saline was given and in group C dexamethasone-water soluble ( 3?g dissolved in 10 ?l saline ) was administered intracerebroventricularly. After a stabilization period of 60 min, transient forebrain ishcmia for 2.5 min was induced in group B and group C by pulling the arteries with 8g weights under the brain temperature of 37.5℃?0.2℃. Seven days after the ischemia, the brains were taken out and fixed with 10% buffered formalin. Brain slices, 5 ?m thick, were stained with hematoxylin and eosin. Objective To investigate the effect of hyperuricemia (HUA) on nuclear factor-κB (NF-κB) in myocardial tissue,Mn superoxide dismutases (Mn-SOD) activity in mitochondria,cardiac function of rats with HUA.Methods Sixty SD male rats,including 30 young rats and 30 aged rats,were randomly divided into four groups:the young control group,the young HUA group,the aged control group and the aged HUA group.Fifteen rats were in each group.The HUA rat models were set up by lavage method with yeast extract and ethambutol.Left ventricular systolic pressure (LVSP),left ventricular end-diastolic pressure (LVEDP) and left ventricular pressure at the end of the rising and falling maximum velocity (+dp/dtmax) were determined by intubation in the left ventricle with cardiac catheter.Mn-SOD activity in the mitochondria of rat myocardial tissue was detected by chemical colorimetry.The expressions of NF-κB in rats myocardial tissue were measured by immunohistochemistry (IHC) and reverse transcription polymerase chain reaction (RT-PCR).Differences between groups were analyzed with One way analysis of variance,Student-Newman-Keuls q test was used to compare the mean of multiple samples.Results Hyperuricemia had no significant effect on the cardiac function of HUA groups (P>0.05).But the gene and protein expression of NF-κB in myocardial tissue and Mn-SOD activity of mitochondria were impacted by HUA.Both the gene and protein expression of NF-κB and the activity of Mn-SOD in HUA groups were increased (F=85.428 4,120.683 0 and 398.228 3,P<0.01).Furthermore,the increase of gene and protein expression of NF-κB and the Mn-SOD activity in rats myocardial tissue of young HUA group were higher than those in aged HUA group (q=6.818 6,10.693 6 and 18.877 9,P<0.05).Conclusion Hyperuricemia may cause inflammation and increase oxygen free radicals in myocardial tissue on one hand,and it may reduce inflammation and protect the myocardial tissue by increasing the activity of Mn-SOD in myocardial mitochondria of HUA rats,and removing the oxygen free radicals on the other hand.The response to uric acid in young HUA group is stronger than that in the aged HUA group.
5.Effects of simulated weightlessness on eNOS expression in pulmonary artery in rat

Objective To study the effects of simulated weightlessness on eNOS expression in the pulmonary artery in rat, and to investigate the mechanism of changed reactivity of pulmonary artery during simulated weightlessness. Methods The experiments were conducted in 16 male Wistar rats, and they were divided into 2 groups: suspension group and control group, 8 rats for each. Tail-suspension (-30?) was carried out for 14d to simulate weightlessness. eNOS protein expression and NO content in pulmonary artery were determined using Western blot analysis and nitrate enzyme-reduction method, respectively. Results Compared with control group, eNOS expression in the rat pulmonary artery increased significantly during simulated weigtlessness. Ojective To study the endothelial mechanism of simulated weightlessness on the reactivity of pulmonary artery. Methods Experiments were conducted using 32 Wistar male rats, which were randomly divided into 2 groups: tail-suspended group and control group, 16 rats for each group. Tail-suspension for 14 days was used to simulate the effects of microgravity. The changes in responses of pulmonary artery to several vascular active agents were determined by using in vitro vessel rings perfusion technique. And the changes were determined again after the endothelium of pulmonary artery was removed. Results Compared with the control group, the contractile responses of pulmonary artery to potassium chloride (68mmol/L) and phenylephrine (PE, 10 -7 ~10 -5 mmol/L) were markedly diminished after 14 days tail-suspension, while the dilatory responses to acetylcholine (Ach, 10 -8 ~10 -6 mmol/L) were markedly enhanced. The vascular reactivity of endothelium-removed pulmonary artery rings to PE and sodium nitroprusside (SNP) did not change after 14 days tail-suspension. Conclusion The contractile responsiveness is attenuated and the dilatory responsiveness is enhanced in the rats' pulmonary arteries after simulated weightlessness. That the pulmonary artery endothelium changes in structure or function after simulated weightlessness might be an important cause of changing in the reactivity of pulmonary artery.
7.Preoperative spleen-liver volume ratio predicts the risk of liver cancer recurrence after hepatectomy

Objective To explore the value of preoperative spleen-liver volume ratio (SLVR) for predicting recurrence of primary liver cancer after hepatectomy.Methods Clinical data of 86 cases of hepatocellular carcinoma undergoing hepatectomy were analyzed retrospectively from January 2009 to December 2014.According to the preoperative SLVR,these patients were divided into two groups:SLVR < 0.8 group (low SLVR group) and SLVR≥0.8 group (high SLVR group).Patients were followed-up until June 2015.Cox ratio risk pattern analysis was used for the recurrent correlative factors.Results The 1-,3-,and 5-year survival rates were 95.5%,88.6%,and 81.8% in the low SLVR group,and 73.8%,47.6%,and 40.5% in the high SLVR group,respectively.The difference were statistically significant between two groups (P <0.05).Cox regression analysis revealed that SLVR≥0.8,AFP-L3% ≥10%,the maximum diameter of the tumor > 5 cm,and hepatic or portal vein tumor thrombus were independent predictors of poor disease-free survival after hepatectomy for hepatocellular carcinomas (P < 0.05).Conclusions Preoperative SLVR≥0.8 is an independent adverse predictor of poor disease-free survival.
8.Safety of laparoscopic surgery in radical resection for different stage rectal cancer

Objective To explore the safety of the laparoscopic surgery in radical resection for different stage rectal cancer.Methods Clinical data of 200 cases with rectal cancer underwent laparoscopic radical resection(observation group) were analyzed retrospectively,including 52 cases of stage 0-Ⅰ,148 cases of stage Ⅱ-Ⅲ.Two hundred cases patients with rectal cancer underwent open radical surgery were selected as control group,including 44 cases of stage 0-Ⅰ,156 cases of stage Ⅱ-Ⅲ.The length to distal resection margin,the number of harvested lymph nodes,the incidence of postoperative complications and the prognosis of recent situation were observed,the safety of the laparoscopic surgery was analyzed.Results Whether rectal cancer of stage 0-Ⅰ or Ⅱ-Ⅲ,there were no significant differences in terms of the length of distal resection margin((2.5±0.9) cmvs.(2.4±1.1) cm,t=0.490,P=0.625;(3.1±1.0) cm vs.(3.2±1.3) cm,t=0.749,P=0.454),the number of harvested lymph nodes((12.3±4.2) vs.(12.7±3.9),t=0.480,P=0.632;(13.9±5.4) vs.(15.1±4.9),t=2.369,P=0.118),the incidence of postoperative complications (17.3% vs.18.2%,x2 =0.012,P=0.911;27.7% vs.28.8%,x2=0.049,P=0.825),the rate of local recurrence(1.9%vs.2.3%,x2 =0.014,P =1.000;4.1% vs.3.2%,x2=0.157,P=0.692),the distant metastasis (3.8% vs.2.3%,x2=0.195,P=1.000;5.4% vs.4.5%,x2=0.137,P=0.712) and 3-year survival rate (96.2%vs.95.5%,x2=0.010,P=0.808;83.8% vs.85.3%,x2=0.132,P=0.714) between the observation group and control group (P>0.05).Conclusion For rectal cancer of stage 0-Ⅰ,laparoscopic surgery has good safety,and worthy of popularization and application.For rectal cancer of stage Ⅱ-Ⅲ,due to its operation for a long time as well as the possible risk in postoperative anastomotic fistula,the implementation of laparoscopic surgery should be more cautious.
9.Evaluation of measurement uncertainty in routine coagulation assays in medical laboratory

Objective To explore the application of evaluation of measurement uncertainty in routine coagulation assays based on the data of internal quality control (IQC) and results of external quality assessment (EQA) .Methods Data of clinical coagulation assays of clinical laboratory of Xiangyang city hospital were collected ,Which came from six months of IQC and 3 times of EQA of ministry of health clinical laboratory center from 2012 to 2013 .The combined and expanded uncertainties of 3 measurements(PT , APTT and Fbg) were evaluated according to CNAS top-downapproach .Results When PT were in 12 .72 s and 21 .04 s ,its ex-panded uncertainty were 1 .06 s and 1 .96 s ,and APTT in 36 .6 s and 50 .6 s were 2 .59 s and 5 .30 s ,and Fbg in 3 .32 g/L was 0 .30 g/L .Conclusion Evaluation of measurement uncertainty by using internal quality control data and EQA results for routine coagula-tion detection index is economical and practical and acceptable way ,with good clinical practice .
10.Application of electronic bronchoscopy in diagnosis of recurrent tracheoesophageal fistula of type Ⅲ esophageal atresia

Objective To investigate the application of electronic bronchoscopy in diagnosis of recurrent tracheoe-sophageal fistula of type Ⅲ esophageal atresia. Methods 5 patients were reviewed who were suspected postopera-tive tracheoesophageal fistula recurrence of type Ⅲ esophageal atresia and examined through electronic bron-choscopy combined with injecting Methylene blue into stomach tube from Jan 2010 to Aug 2014. Male to female ra-tio was 4:1. The age was 2~15 months, median age was 7.4 months. Results The 5 cases were found trachea mem-brane fistula by electronic bronchoscope, recurrent tracheoesophageal fistula was diagnosed with Methylene blue in-jected into stomach tube and overflowed from trachea membrane fistula. Examination time is 3~7 minutes, the aver-age is 4.2 minutes. The 5 cases were confirmed by operation. Conclusion Electronic bronchoscopy combined with injecting Methylene blue into stomach tube to diagnose recurrent tracheoesophageal fistula is safe and feasible, the time is short and the diagnosis rate is high.
