Compared to traditional paper & pencil test,computer-based test has obvious advantages in improving reliability and validity of test.With the increasing popularity of computer-basedtest,making ICO examinations(International Basic Science and Clinical Sciences Assessment for Ophthalmologists) computerized can be realized in our country for the reason that we own hardware and software required by computerized exam,including standardized examination room,professional test personnel,examinee with computer skills,etc.Currently,we can rely on available resources to carry out pilots in the areas meeting the condition of computer-based test,take the form of paper and pencil and computer at the same time,and then promote the process of computer-based ICO examinations.

Objective To investigate the effect of glucocorticoid on ischemia-induced neuronal damage in gerbil hippocampus with forebrain ischemia model.Methods Twenty-one gerbils were randomly assigned to 3 groups (7 animals each). The animal was anesthetized and both common carotid arteries were exposed and separated. Silk threads were looped around these arteries. In group A and group B ,10 ?l saline was given and in group C dexamethasone-water soluble ( 3?g dissolved in 10 ?l saline ) was administered intracerebroventricularly. After a stabilization period of 60 min, transient forebrain ishcmia for 2.5 min was induced in group B and group C by pulling the arteries with 8g weights under the brain temperature of 37.5℃?0.2℃. Seven days after the ischemia, the brains were taken out and fixed with 10% buffered formalin. Brain slices, 5 ?m thick, were stained with hematoxylin and eosin. Rheumatoid arthritis(RA),a systemic,inflammatory autoimmune disorder characterized by inflammation of the lining or synovium of the joints,is a complex polygenic disease with a complicated inheritance mode.Both genetic and environmental factors determine the development and progression of RA.Study on susceptible genes of RA provides a theoretic basis for clinical diagnosis and treatment.As a novel high-throughput screening method,genome-wide association study(GWAS) is a powerful approach for mapping susceptible genes of polygenic disease like RA.GWAS can not only verify the well-established susceptible loci,but also identify novel genetic loci candidates.Great improvement has been made in using GWAS to screen for novel genes,which casts new lights on the mechanism and treatment of RA.This review summarizes the progress in using GWAS for screening of RA susceptible genes.

Objective To study the feasibility of endoscopic thyroidectomy with bipolar electrocoagulation. Methods Endoscopic partial thyroidectomy via precordial approach, mainly by using bipolar electrocoagulation and scissors, was accomplished in 21 patients with thyroid tumors with a diameter of 0.7～5.5 cm. Results The operation was accomplished endoscopically in 20 patients, whereas a conversion to open thyroidectomy was conducted in 1 patient. The mean incision length was 22 mm, the operation time was 69?47 min, and the blood loss was 55?41 ml. Postoperative analgesic treatment was required in 2 patients and no serious complications happened. Conclusions Endoscopic thyroidectomy with bipolar electrocoagulation is feasible in the absence of ultrasonic scalpel.

Objective To evaluate the feasibility of hand-assisted laparoscopic colorectal resection. Methods Clinical data of 21 cases of hand-assisted laparoscopic colorectal resection in this hospital from October 2003 to August 2004 were retrospectively reviewed. Results The hand-assisted laparoscopic resection was accomplished in 20 cases. The mean operation time was 144 min, the mean blood loss was 120 ml, the mean number of dissected lymph nodes was 8.5, and the mean time to intestinal function recovery, 69 h. Postoperative abdominal bleeding was found in 1 case, which was cured with conservative therapy. A conversion to open surgery was needed in 1 case. Conclusions Hand-assisted laparoscopic colorectal resection safe, feasible, and radical, with advantages of fewer complications and simplicity of performance.

Objective To discuss the molecular structures and functions of Alu repeats,and its relationships with malignant tumors. Methods Advances in molecular biology and oncology of Alu repeats in recent years had been reviewed. Results Homologous recombination between Alu repeats may result in various genetic exchanges,including duplications, deletions and translocations,cause a series of malignancy alteration by activating some oncogenes. Conclusion Alu repeats as a sort of multifunction regulatory sequences may play an important role during the carcinogenesis and progression of some tumors.

Objective Our research aimed to investigate the efficacy and the nursing characteristics of allogenic bone marrow mesenchymal stem cell (BM-MSCs) transplantation in HBV related acute-on-chronic liver failure (ACLF) patients.Methods A total of 91 HBV related ACLF patients were enrolled,among whom 45 patients received allogenic BM-MSCs transplantation via peripheral vein and other 46 patients received medical treatment only.We assessed the 12-week survival rates and the variation of model for end liver disease (MELD) score.Besides,we administrated comprehensive nursing for these patients in different stages of the transplantation.Results The cumulated survival of patients received BM-MSC transplantation was 75.6％,which was higher than the control group (54.5％).The level of total bilirubin and International Normalized Ratio (INR) had greatly improved.41 of 45 patients passed through perioperative period safely,without any severe adverse events.11 patients died due to exacerbation of liver failure,among whom 3 happened in perioperative period.But none was related with the operation.Conclusions Allogenic BM-MSCs transplantation can improve the survival rate of HBV related ACLF patients.The psychological nursing pre-operation,skilled operation,intensive care and observation post operation were beneficial to the patients' recovery after transplantation.
