Objective To evaluate the effectiveness of application of timing- work schedule mode in hu-man resource management according to the characteristc and work load of the outpatient department in island hospital. Methods General quality of care, nurses and patients' satisfaction degree were compared before and 12 months after the application of timing- work schedule. Results Total quality of care and nurses and pa- tients' satisfaction degree with nursing had statistical significance before and after the application of timing-work schedule mode. Conclusions To adoption of timing-work schedule mode in outpatient department in island hospital is feasible, it not only saves the nursing human resource, but also can guarantee the nursing quality and improve patients' satisfaction degree. So it is worthy of clinical application.

Objective:To detect the polymorphism of ALDH2 gene in Chinese and to guide the rational use of nitroglycerin. Meth-ods:The polymorphism of ALDH2 (Glu504Lys) was measured by colorful gene chip. Results:The polymorphism of ALDH2 from 1026 clinical samples in China was detected by gene chip. The results showed that there were three genotypes. The distribution of the three genotypes of wild type [ALDH2(504Glu)], heterozygotes [ALDH2(Glu504Lys)] and homozygous mutant [ALDH2(504Lys)] was 61. 7%(623 samples), 31. 6%(319 samples) and 6. 6%(67 samples), respectively. Conclusion:Approximately one third of Chinese patients can not be treated by nitroglycerin. The gene detection method in the study is suitable for the guide of rational drug use in hospi-tal.

Objective To study effect and significance of systemic inflammatory response syndrome(SIRS) score and APACHE Ⅱ score in prognosis in severe patients of emergency department.Methods Data of 114 patients were collected at admission.SIRS score and APACHEⅡ score were calculated in 24 hours.To analyze mortality of patients in different score,to estimate dependability of them.Result With SIRS score increasing,mortality rate increased as well and patients with SIRS score ≥2 or APACHE Ⅱ score ≥25 had significantly higher mortality rate.Conclusions SIRS score is a simple,useful predictor of outcome in patients of emergency department.It can be used in clinical generally.

Objective To explore the relationship between obese children with benign acanthosis ni-gricans and insulin-resistant and type 2 diabetes mellitus. Methods Levels of glucose, insulin, and glucose/ insulin ratio were measured on fasting blood specimens, and anthropometric parameters including waist/hip ratio, fat mass, body fat percentage and body mass index were examined in 42 obese children with benign acanthosis nigricans, 60 cases of simple obesity and 20 healthy children controls. Glucose tolerance tests were performed in groups of obese children with benign acanthosis nigricans and simple obesity, respectively. Results Two of 42 obese children with benign acanthosis nigricans were diagnosed as type 2 diabetes mellitus. OBJECTIVE To investigate the correlative factors that would affect the prognosis of geriatric lower respiratory infection.METHODS A total of 101 geriatric patients diagnosed as lower respiratory infection were enrolled in emergency ICU.The laboratory tests such as blood chemical tests,C-reactive protein,and troponin T(cTnT)were obtained twice 2 hours and 24 hours after admission,respectively.Other parameters including general condition,clinical outcomes as well as APACHE Ⅱ and SIRS scores within 24 hours were recorded.RESULTS The ages,cTnT and SIRS score were negatively correlated to clinical outcome.CONCLUSIONS The ages,SIRS scores and cTNT levels are independent prognostic factors in geriatric lower respiratory infection.

Objective To evaluate the usefulness of free angle M-mode echocardiography (FAM) in obtaining tricuspid annular plane systolic excursion (TAPSE) for assessing the fetal right ventricular function,and to compare the results of measurements of TAPSE by conventional M-mode(CM) and FAM.Methods Two hundred and forty-three normal fetuses in second and late trimester were divided into 5 groups by gestational age(GA):20-24 weeks,24+1-28 weeks,28+1-32 weeks,32+1-36 weeks,36+1-40 weeks.The TAPSE were measured by CM and FAM echocardiography,meanwhile multiple parameters for evaluating right ventricular function were obtained by using myocardial tissue Doppler imaging (TDI),and the correlation between TAPSE and other parameters were analyzed by linear correlation.ANOVA was used to compare CM-TAPSE,FAM-TAPSE,Em,Sm and right ventricular fractional shortening(RVFS)with different GA.Regression equation estimate was used to compare the relationship of FAM-TAPSE with GA.Independent sample t test was used to compare CM TAPSE with FAM TAPSE.Results There were significant differences in CM-TAPSE,FAM-TAPSE,Em,and Sm among 5 groups (P =0.000),the measured value of FAM-TAPSE was higher than that of CM TAPSE (P =0.000).Both FAM-TPASE and CM-TAPSE showed significant positive correlations with GA,Em and Sm,but FAM TAPSE demonstrated better correlation than CM-TAPSE.Conclusions By adjusting sample line of FAM,measuring the maximum displacement of tricuspid annular plane is feasible,and FAM may evaluate fetal right ventricular function more accurately and effectively.

Objective To evaluate the value of serum uric acid(UA)levels with reference to the age,waist circumference,and body mass index(BMI)in predicting the metabolic syndrome(MS)in obese children.Methods A total of 300 obese children,including 180 boys and 120 girls,were enrolled in this study.The height,BMI,waist and hip circumference,blood pressure,serum glucose,insulin and lipid profile in all participants were measured.Oral glucose tolerance test and insulin releasing test were performed.The boys or girls were divided into 4 groups according to the 4 quantile of UA level,respectively.The clinical characteristics and correlation of UA with the clinical indexes and MS components were compared.The binary Logistic regression analysis was applied in the risk of MS and its components for the 4 groups of obese children.The area under the receiver operating characteristic curve(ROC curve)of UA level,age,waist circumference and BMI were used to predict the MS.Results UA level was increased with the increase of age,waist circumference and BMI,and the UA level was significantly correlated with triacylglycerol,postprandial 2 h glucose(2 h PG)(r=0.196,0.174 in boys;r=0.291,0.179 in girls).In boys,the adjusted odds ratio and 95％CI of the highest quartile of UA for triglyceridemia was 2.71(95％CI:0.77-9.58);which in girls,the adjusted odds ratio and 95％CI of the highest quartile of UA for hyperglycemia,hypertension were 8.45(95％CI:1.76-40.52)and 3.93(95％CI:0.66-23.33),respectively,with significant differences.In boys,the area under the ROC curve of UA level,age,waist circumference and BMI which predict the MS were 0.652 0.626,0.621,0.62,respectively,and the differences were significant(all P<0.05).Conclusions The UA level is significantly correlated with the composition of MS,UA detection combining with reference to the age,waist circumference,and BMI is helpful for the identification of high risk groups of metabolic syndrome.
