2.A Case of Acardiac Twin.

Acardiac twinning affects 1 in 100 monozygotic twin pregnancies and 1 in 35,000 pregnancies overall. This condition is characterized by the absence or rudimentary development of fetal heart, and associated with various anomaly. The presence of an acardiac twin requires the normal (or "pump") twin to provide circulation for itself, as well as the acardiac sibling. The acardiac malformations are uniformly fatal in the affected twin, and mortality in the co-twin is as high as 55%. The principal perinatal problems associated with acardiac twinning are pump-twin congestive heart failure, maternal hydramnios, and preterm delivery. We recently experienced a case of acardius anceps associated with a normal male infant, so present with a brief review of the literature.

3.Intraarticular Anatomic Variants Associated with Discoid Meniscus

Discoid meniscus in the knee joint is common. From July 1990 through June 1994, the authors performed arthroscopy on 968 symptomatic knee joints. One hundred and six knees of 102 patients had discoid meniscus. Of the 106 knees, 14(13.2%) cases were associated with other intraarticular anatomic variants. There were 7(6.6%) cases with anomalous insertion of the anterior horn of the medial meniscus into the anterior cruciate ligament, and 7(6.6%) cases with anterior transposition of the insertion of the anterior cruciate ligament below the anterior tibial margin like the insertion of the posterior cruciate ligament. Of the 14 patients, the discoid meniscus were reshaped in 11 knees, partially resected in 2 knees, and one patient was treated with repair of a peripheral tear and reshaping. But had no treatment in associated intraarticular anatomic variants. The follow up was done from 24 to 54 months(average 37 months) after surgery. The results were excellent in 8 knees ; good in 4 knees and fair in 2 knees(Ikeuchi scale, 1982) The patients with excellent or good results were satisfied as were the authors, and these anatomical variants other than the discoid meniscus were not related to the patient's symptoms.

5.The comparative evaluation of clinical screening in combined tests cytology (ThinPrep(R)), HPV DNA test (Hybrid capture(R) II),cervicography for uterine cervical cancer.

OBJECTIVE: Since the accuracy of Pap smear for cervical neoplasm has been questioned, a number of adjunctive tests have been developed. The purpose of this study was to evaluate which protocol is the most effective screening test among cervical cytology (ThinPrep(R)), HPV DNA test (Hybrid capture(R) II) and cervicography. METHODS: We chose 252 patients who were underwent the biopsy among 829 patients who visited our hospital for cervical cancer screening test. These 252 patients were engaged in this study simultaneously. They underwent triple combined test [cervical cytology (ThinPrep(R)), HPV DNA test (Hybrid capture(R) II), cervicography] and colposcopic-directed biopsy or biopsy on operation for diagnostic evaluation. RESULTS: The triple combined test showed a sensitivity of 96.0%, while double combined test [cervical cytology (ThinPrep(R))+cervicography] showed a sensitivity of 89.0%, the other double combined test [cervical cytology (ThinPrep(R))+HPV DNA test (Hybrid capture(R) II)] showed a sensitivity of 86.7%. Cervicography showed a specificity of 75.4% (highest among the single test), positive predictability of 89.8% (also highest). CONCLUSION: The sensitivity of cervical cytology was markedly improved by combination with HPV DNA test and cervicography. So the triple combined tests which improved the high false negative rate of cervical cytology may be a new effective method as a cervical cancer screening test, if the effectiveness could be confirmed by mass screening study.

The incidence of Giant Condyloma Acuminata of the penis is unknown but considered rare. They are similar to condyloma acuminata, histologically and grossly similar to squamous cell ca. Most authorists consider that they get the real possibility of malignant change and they had been first described by Buschke in the German literatures in l896 as a variant of condyloma acuminata. A case of malignancy of the penis which has apparently transformed from Giant Condyloma Acuminata of the penis is herein presented with review of related literatures.
