1.Acute Cholangitis with Common Bile Duct Stone Caused by Lactococcus garvieae: A Case Report

Lactococcus garvieae is a Gram-positive cocci that has been known to be a fish pathogen, and considered as a low virulence organism rarely associated with human infection. We report a case of acute cholangitis with common bile duct (CBD) stone and bacteremia by L. garvieae bacteremia in a 70-year-old male. The patient presented with epigastric pain and was diagnosed with two CBD stones. Blood culture obtained prior to empiric antimicrobial therapy with ceftizoxime sodium showed growth with Escherichia coli and L. garvieae. The bacteria were confirmed by matrix-assisted desorption/ionization time-of-flight mass spectrometry. Initial attempt at endoscopic biliary drainage failed, and the patient underwent percutaneous transhepatic biliary drainage and subsequent stone removal. He occasionally ingested raw fish and had a history of gastric ulcer with acid suppression therapy, which could be possible risk factors for L. garvieae infection. This is the first case of L. garvieae bacteremia in acute cholangitis.

4.Colonoscopic Experience in 3 Cases of Primary Colonic Lymphoma.

Primary colonic lymphoma is a rare disease and open surgical biopsy is usually necessary for hietological confirmation. Despite the submucosal location of the tumors, histological confirmation is frequently made by endoscopic multiple punch biopsies. Recently, three patients were admitted to the hospital with right lower guadrant abdominal pain, mass and abnormalities in the right colon and cecum on the barium enema. We performed colonoscopy and found smooth surfaced polypoid mass with occasional shallow ulcerations without lobulation, so suspected lymphoma endoscopically but failed to confirm histologieal diagnosis by usual punch biopsy. Thereafter, all three cases were undergone to laparntomies, and two were confirmed to be non-Hodgkins lymphoma and the other was Hodgkins disease. Here, we present those three cases of endoscopically found colonic lymphoma with review of literatures.

5.Auditory-Perceptual and Acoustic Evaluation in Measuring Dysphonia Severity of Vocal Cord Paralysis.

BACKGROUND AND OBJECTIVES: The purpose of this study was to investigate the criterion-related concurrent validity of two standardized auditory-perceptual assessments and the Acoustic Voice Quality Index (AVQI) for measuring dysphonia severity in patients with vocal cord paralysis (VCP). MATERIALS AND METHODS: Total 210 patients with VCP and 236 normal voice subjects were asked to sustain the vowel [a:] and to read aloud the Korean text “Walk”. A 2 second mid-vowel portion of the sustained vowel and two sentences (with 26 syllables) were recorded. And then voice samples were edited, concatenated, and analyzed according to Praat script. Two standardized auditory-perceptual assessment (GRBAS and CAPE-V) were performed by three raters. RESULTS: The VCP group showed higher AVQI, Grade (G) and Overall Severity (OS) values than normal voice group. And the correlation among AVQI, G, and OS ranged from 0.904 to 0.926. In ROC curve analysis, cutoff values of AVQI, G, and OS were < 3.79, < 0.00, and < 30.00, respectively, and the AUC of each analysis was over .89. CONCLUSION: AVQI and auditory evaluation can improve the early screening ability of VCP voice and help to establish effective diagnosis and treatment plan for VCP-related dysphonia.

7.Histopathologic Changes of Cochlea and Hearing Threshold Changes according to Duration of Noise-Exposure in the Rat.

BACKGROUND AND OBJECTIVES: The degree of noise induced hearing loss was determined principally according to the level and duration of noise and patient's state. The purpose of this study was to investigate the cochlear histopathology and hearing threshold immediately after noise exposure according to duration of noise exposure, and finally to draw relationship between the cochlear pathology and hearing threshold. MATERIALS AND METHOD: Each group of animals (6 ears) has been exposed for 10 minutes, 1 hour, 2 hours, 3 hours, 4 hours, 24 hours and 48 hours to an octave band of noise with a center frequency of 4000 Hz and a sound pressure level of 110 dB. After noise exposure, the hearing thresholds of the subjects were determined with auditory brainstem response (ABR) and immediately their inner ear were fixed and observed with transmission electron microscopy (TEM). RESULTS: ABR thresholds were increased according to lengthening of duration of noise exposure. TEM findings of outer hair cells, Deiters' cells and ganglion cells showed more severe degeneration according to lengthening of duration of noise exposure. Damages of all kinds of cells appeared almost at the same time. CONCLUSION: Through the foregone study, cochlear pathology was proportioned to increased hearing threshold, and the damages of outer hair cells and ganglion cells appeared almost at the same time. It seems that not only damages of outer hair cells, but also damages of ganglion cells contribute to early hearing threshold shift during continuous intense noise exposure.

8.Venous Thromboembolism in Patients with Advanced Pancreatic Cancer Receiving Palliative Chemotherapy: Incidence and Effect on Prognosis

Background/Aims:
This study evaluated the incidence of venous thromboembolism (VTE) in patients with advanced pancreatic ductal adenocarcinoma (PDAC) at the authors’ institution and analyzed the risk factors associated with VTE and the overall survival (OS).
Methods:
One hundred and seventy patients with locally advanced or metastatic PDAC who received palliative chemotherapy at Daegu Catholic University Medical Center from January 2011 to December 2020 were included.
Results:
During a median follow-up period of 341 days, 24 patients (14.1%) developed VTE. Cumulative incidence values of VTE were 4.7% (95% confidence interval [CI], 2.39-9.22) at 90 days, 9.9% (95% CI, 6.14-15.59) at 180 days, and 16.9% (95% CI, 11.50-24.36) at 360 days. Multivariate analysis showed that a carbohydrate antigen 19-9 (CA 19-9) level over 1,000 U/mL (hazard ratio [HR], 2.666; 95% CI, 1.112-6.389; p=0.028) and a history of alcohol consumption (HR, 0.327; 95% CI, 0.109-0.981; p=0.046) were significant factors associated with VTE. Patients with VTE showed a shorter median survival (347 days vs. 556 days; p=0.041) than those without VTE. Multivariate analysis revealed VTE (HR, 1.850; 95% CI, 1.049-3.263; p=0.033) and CA 19-9 level over 1,000 U/mL (HR, 1.843;95% CI, 1.113-3.052; p=0.017) to be significant risk factors associated with OS.
Conclusions
The cumulative incidence of VTE in patients with advanced PDAC was 16.9% at 360 days. While a history of alcohol consumption was a protective factor, a high CA19-9 level was a risk factor for VTE. In addition, the occurrence of VTE was associated with poor prognosis.
9.Efficacy of 7-day Tailored Therapy for Helicobacter pylori Eradication based on Clarithromycin Resistance

Background/Aims:
Increasing resistance to clarithromycin (CAM) of Helicobacter pylori (H. pylori) is one of the main causes of recent decrease in eradication rate of standard triple therapy. The aim of this study was to evaluate the usefulness of 7-day tailored therapy based on the existence of CAM resistance.
Methods:
From January 2017 to May 2022, a total of 481 consecutive patients with H. pylori infection were recruited in Daegu Catholic University Medical Center. Treatment regimen was selected based on the result of CAM resistance test. Patients with CAM resistance (R group) were treated with bismuth-based quadruple therapy for 7 days. Patients without CAM resistance (S group) were treated with standard triple therapy for 7 days.
Results:
The overall H. pylori eradication rate was 89.4% (379 of 424) by per-protocol (PP) analysis. Patients with CAM resistance mutation included 166 patients (34.5%). The eradication rates of each group were 88.8% (135 of 152) and 89.7% (244 of 272) by PP analysis, for R and S group respectively. By intention-to-treat (ITT) analysis, the eradication rates were 81.3% (135 of 166) and 77.5% (244 of 315) for R and S group. CAM resistance was identified with a dual-priming oligonucleotide-based multiplex PCR.
Conclusions
In spite of this high CAM resistance (34.5%), the eradication rate of 7-day tailored therapy based on the existence of CAM resistance was 89.4%. The 7-day tailored therapy based on CAM resistance could be an acceptable treatment selection strategy for H. pylori eradication.
10.A Study on Usage of Classroom Desks and Chairs by Teenagers.

OBJECTIVES: To evaluate the compatability of high school students' desks and chairs to the fittness of their physique, to examine the factors related the posture and pain while they use the classroom desks and chairs, and to analyze the curvature of spine in sitting position. METHOD: Measurements of the dimensions of desks and chairs and a written survey to question students' habits regarding to the usage of classroom desks and chairs along with the physical examinations of spine including the analysis of spine curvature in sitting position with an electrogoniometer. RESULTS: Seven hundred forty nine among 831 male students, and 1,017 among 1,074 female students, complained of discomforts associated with the usage of classroom desks and chiairs. An examination of spine in sitting position using an electrogoniometer showed that both male and female subjects displayed the spine curvatures in the order of thoracic kyphosis, lumbar lordosis, and scoliosis. All subjects displayed pain in the order of low back(38.8%), posterior neck(23.9%), shoulder and elbow joints(15.8%), and buttock(15.7%). CONCLUSION: The study revealed an urgent need for the adaptation of classroom desks and chairs according to the growing physique of the adolescents. A majority of the teenaged subjects experienced the discomfort from the use of improper funiture and a poor posture. Authors highly recommend a formal education to the students regarding the importance of proper posture and the usage of adjustable desks and chairs.
