1.Correction of idiopathic scoliosis using internal fixation system implantation
Chinese Journal of Tissue Engineering Research 2007;0(30):-
The indication for treating adolescent idiopathic scoliosis (AIS) including development states, bending types, spinal rotation, as well as degree of deformity, but the orthosis occasionally involves in clinic. The third generation internal fixation system, such as CD, TSRH, and Chinese Greatwall system arose more attention with the advancement of spinal cube theory and internal fixation technique. Surgical treatment of scoliosis by fixation of the vertebral pedicle has superior effect in treating deformity at the sagittal/coronal plane or vertebra rotation, which can correct the three dimensional deformities of spinal column and improve patient's back appearance via revolving controlled rod.

Objective To study the application effect of ENCOR vacuum-assisted breast minimally invasive polypectomy system in the diagnosis and treatment of breast tumors. Methods Totally 86 breast cancer patients treated in the General Hospital of Yangmei Group from January 2017 to October 2017 were randomly divided into study group and control group, with 43 cases in each group. The study group was treated with ENCOR vacuum-assisted minimally invasive rotational atherectomy system, while the control group was treated by traditional breast excision. The operative conditions, postoperative complications and postoperative recurrence were observed and compared between the two groups. Results The intraoperative bleeding volume, operation time, scar length and postoperative recovery time in the study group were (5.1 ±1.2)mL,(15.9 ±3.5)min,(0.3 ±0.1)cm,(2.7 ±0.4)d,respectively, which in the control group were (12.7 ±2.3)mL,(26.8 ±4.6)min,(3.2 ±1.1)cm and (5.1 ±0.8)d,respectively, the differences between the two groups were statistically significant ( t=15. 579,14. 297,14. 108,13. 597,all P<0. 05). The total incidence rate of postoperative complications in the study group was 11. 63%,which was significantly lower than 30. 23% in the control group (χ2 =5. 187,P<0. 05). During the follow-up of 12 months,in the study group,2 patients (4. 65%) had residual tumor,1 patient (2. 33%) developed recurrence; in the control group, 3 patients (6. 98%) had residual tumor,2 patients (4. 65%) developed recurrence. There was no statistically significant difference between the two groups (χ2 =1. 013,P >0. 05). Conclusion ENCOR vacuum - assisted breast minimally invasive revolving system in the treatment of breast tiny tumors has advantages of less trauma and pain, rapid recovery, etc,it is safe and effective.

Objective:To monitor the liver oxygen saturation (rStO 2-liv) and the intestinal oxygen saturation (rStO 2-abd) in very low birth weight infants (VLBW) using near infrared spectroscopy (NIRS) and explore the difference between the two indicators and their clinical significance. Methods:This prospective study included newborns with a birth weight of less than 1 500 g at the Nanjing Women and Children′s Healthcare Hospital, from October 1, 2022 to March 31, 2023.On the 7 th day after birth, Gutcheck NEC scores were evaluated, followed by continuous NIRS measurement for 8 hours.Clinical data and NIRS measurements were collected and comparatively analyzed. The differences between groups were compared by two independent samples t-test and One-Way ANOVA. The diagnostic value of NIRS was analyzed using receiver operating haracteristic curves. Results:A total of 42 VLBW infants were enrolled in this study. There was no statistically significant difference between rStO 2-liv and rStO 2-abd ( P=0.117).According to the Gutcheck NEC score, there were 7 patients in the low-risk group, 29 in the medium-risk group, and 6 in the high-risk group. No statistically significant difference was observed in rStO 2-liv among the different risk groups ( F=2.145, P=0.131).The rStO 2-abd decreased significantly with increasing risk ( F=5.127, P=0.011).The Bland-Altman plot indicated no consistency between rStO 2-liv and rStO 2-abd ( P=0.024).The receiver operator characteristic curve showed that the area under the curve (AUC) for rStO 2-abd diagnosing the high-risk Gutcheck NEC score was 0.800, with a cutoff value of 41.41%, sensitivity of 85.70%, and specificity of 48.60%. Conclusions:Simultaneous measurement of rStO 2-liv and rStO 2-abd using NIRS is safe and feasible in VLBW infants, but the two measures can not be substituted for each other.Low rStO 2-abd (<41.41%) indicates a higher risk of necrotizing enterocolitis in infants.

Objective:To explore the selection of surgical methods for different sites of symptomatic Rathke's cleft cyst (RCC) and the clinical efficacies of these patients.Methods:Forty-seven patients with symptomatic RCC, admitted to our hospital from January 2016 to December 2019, were chosen in our study; 21 patients with intrasellar symptomatic RCC accepted surgery via unilateral nasal approach at the right side, 19 patients with intra-suprasellar symptomatic RCC accepted surgery via bilateral nasal approach, 3 patients with suprasellar symptomatic RCC accepted endonasal transsphenoidal surgery under endoscope, and 4 patients with suprasellar symptomatic RCC accepted craniotomy via pterion approach. The clinical efficacies and complications of patients accepted different surgical methods were compared. All patients were followed up for 3-36 months to observe the recurrence. Results:The postoperative symptoms of the patients were effectively improved, including headache relief ratio of 27/31, vision loss improvement ratio of 5/5, high prolactin relief ratio of 11/13, pituitary function improvement ratio of 9/18. Complications occurred in 6 patients, presenting as diabetes insipidus. Four patients recurred during follow-up.Conclusion:Intrasellar and intra-suprasellar symptomatic RCC accepted surgery via endoscopic transnasal transsphenoidal approach are safe and effective; selection of surgical methods for suprasellar symptomatic RCC should be determined according to the sizes and growth directions of cysts.
