1.INFLUENCE OF SB ON PHARMACOKINETICS AND THE ANTI-INFLAMMATORY ACTION OF ASP IN A LARGE DOSE

Influence of Sodium Bicarbonate (SB) in several doses on some pharmacokinetic parameters and the anti-inflammatory action of Aspirin (ASP) in a large dose was studied in rats of experimental inflammatory model. It was showed that SB significantly reduced the Tp, T_(1/2), AUC and increased the Ke, Cmax of plasma salicylic acid (SA). The anti-inflammatory effect of ASP was diminished as well. All of the changes, were enhanced as the doses of SB increased. The results in this paper suggested that combining ASP with SB in antirheumatic therapy might be an unreasonable regime.

Objective To examine serum lipid levels and to investigate the risk factors of dyslipidemia in children and to study the indices of screening test for dyslipidemia of children in Beijing.Methods Serum total cholesterol(TC),triglyceride(TG),high density lipoprotein-cholesterol(HDL-c) and low density lipoprotein-cholesterol(LDL-c) were detected by HITACHI 7060 automatic biochemical analyzer in Beijing students.Body height, weight,upper body height,waist circumference,upper arm circumference and blood pressure were measured.Body mass index(BMI) was calculated according to the measurement of body height and weight.Waist-to-height ratio(WHR) was also calculated.Then a Logistic regression analysis and ROC study were conducted to examine the relationship between the above indices and dyslipidemia.Results Of 962 students,107 were found dyslipidemia.The prevalence rate was 11.12%.The upper body height,upper arm circumference,BMI,WHR,systolic pressure and diastolic pressure in children with dyslipidemia were higher than those of normal subjects(P

5.Clinical and electrophysiological characteristics of Lambert-Eaton myasthenic syndrome: a retrospective study in forty-five patients

Objective To analyze the symptoms and signs in forty-five Lambert-Eaton myasthenia syndrome (LEMS) patients retrospectively. Characteristics of electrophysiological examinations were investgated. Methods Forty-five LEMS patients were reviewed and information gathered regarding clinical complains neurological symptoms, and other concomitant diseases. The records showed that repetitive nerve stimulation (RNS) and nerve conduction velocity (NCV) were performed in all patients. Needle electromyography (EMG) and skin sympathetic response (SSR) were performed in some patients. Results (1) The mean age of neurological clinical onset age was (51.2 ±6. 8) years old. The two most common symptoms were slight weakness of lower extremities ( n = 35 ) and upper extremities (n= 5). Dysarthria was found in 3 patients and neck weakness in 2 patients. Tendon reflex decreased and disappeared in 38 patients. Autonomic nervous system manifestations were presented in 30 patients. (2) RNS increasing was observed in all patients from 156% to 636%. Low frequency RNS abnormalities were found in 29 patients.Sensory nerve conduction velocity abnormalities or sensory nerve conduction velocity combined motor nerve conduction velocity abnormalities were found in 19 patients ( 42% ). Of the 30 patients who underwent a needle EMG examination, 20 had myogenic or neurological damage. Thirteen abnormal findings were observed in 25 patients who underwent SSR examination. Conclusion The most common manifestations were weakness in lower extremities and autonomic nervous system dysfunction. Many abnormal electrophysiological results were found in LEMS patients, including NCV and EMG abnormalities. These findings indicated that clinical manifestations exceed the neuromuscular junction and perhaps included the peripheral nerve and muscle.

Aim To investigate the inhibitory effect of hydrogen sulfide on expression of elastin in hypoxic pulmonary hypertensive (HPH) rats. Methods Twenty-four rats were randomly divided into three groups:control group (n=8), hypoxic group (n=8) and hypoxia+NaHS group (n=8). Pulmonary artery mean pressure (mPAP) of three group rats was measured via right ventricular catheterization. Right ventricle and left ventricle+septum of each rats were isolated from intact rats and weight of right ventricle/(weight of left ventricle+septum) ratio was calculated. The percentage of muscularized arteries of small pulmonary arteries was measured using a light microscope. The expression of elastin and TGF-? in pulmonary artery smooth muscle cells was observed with immunohistochemistry. AIM: To explore the regulatory effect of adrenomedullin (ADM) on pulmonary oxidative stress in the rats with pulmonary hypertension induced by high blood flow.METHODS: Healthy male SD rats (n=22) were randomly divided into control group, shunt group and shunt with ADM group.Abdominal aorta and inferior vena cava shunting was produced in the rats in shunt group and shunt with ADM group.After 8 weeks, ADM (1.5 μg·kg-1·h-1) was administered into the rats in shunt with ADM group subcutaneously by mini-osmotic pump for 2 weeks.Mean pulmonary artery pressure (mPAP) was evaluated by a right cardiac catheterization procedure.The ratio of right ventricular mass to left ventricular plus interventricular septal mass [RV/(LV+SP)] and relative medial thickness (RMT) in pulmonary muscularized arteries were calculated.The content of malonaldehyde (MDA), total antioxidative capacity (T-AOC), and activity of superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) in lung tissues were detected by colorimetry.The expression of NADPH oxidase 4 (NOX4) in the lung tissue was analyzed by Western blot.RESULTS: Compared with control group, the mPAP, RV/(LV+SP) and RMT in pulmonary muscularized arteries in shunt group were all significantly increased.The content of MDA and the expression of NOX4 in the lung tissues were significantly increased.The T-AOC, and activity of SOD and GSH-Px in the lung tissues were significantly decreased.However, mPAP, RV/(LV+SP) and RMT in pulmonary muscularized arteries in shunt with ADM group were significantly decreased as compared with shunt group.Meanwhile, ADM decreased the content of MDA and the expression of NOX4 in the lung tissues, but increased the T-AOC, and activity of SOD and GSH-Px in the lung tissue of shunt rats.CONCLUSION: ADM inhibits oxidative stress response in the development of pulmonary hypertension and pulmonary vascular structural remodeling induced by high pulmonary blood flow in the rats by down-regulating the NOX4 expression and strengthening the anti-oxidation response.
8.Progression from ocular to generalized myasthenia gravis in adults

Objective To investigate the time and rate of generalization developed from ocular symptoms in adults to generalized myasthenia gravis (GMG).Methods The data from 456 adult patients who started on ocular symptoms during Jan 2005 to Jan 2012 were retrospectively studied,with a follow-up period of 3 months to 17 years.The data included transfer time,Osserman type,difference between steroids group and no-steroids group.Results (1) During the follow-up period,197 (43.2%) patients developed to generalized myasthenia gravis,58.4% (115/197)in one year after onset of ocular symptoms,74.1% (146/197) in 2 years and 80.7% (159/197) in 3 years.86.8% (171/197) developed to Osserman Ⅱ a or Ⅱ b.(2) Of 233 patients untreated with steroids,134 (57.5%) developed to GMG,while 63 (28.3%) of 223 patients who were treated with steroids developed to GMG (x2 =38.57,P < 0.01).There were no difference between two groups in transfer time and Osserman type.Conclusion MG initiated from ocular syndromes in adults easily develops to GMG in 2 years.Patients treated with steroids have low chance of generalization.

10.Regulation of hydrogen sulfide on interleukins in rats acute lung injury

Objective To study the role of hydrogen sulfide(H2S)in the pathogenesis of acute lung injury (ALI)-induced oleic acid(OA)and its regulatory effects on the inflammatory msponse.Method The ALI models in SD rats were produced by intravenous injection of OA(0.1 ml/kg).Forty-nine male SD rats were divided into three groups randomly:control group,OA group and OA+sodium hydrosulfide(NaHS)group.Rats of OA group and OA+NaHS group were sacrificed at 2,4 and 6 hours after the administration of OA,respectively.Rats of control group were sacrificed at 6 hours after intravenous normal saline(0.1 ml/kg).PaO2,lung tissue wet/dry(W/D)ratio and IQA score were determined.The concentrations of H2S in lung tissue and plasma were tested by sensitive sulphur electrode.The IL-6.IL-8 and IL-10 levels were measured by doubleantibody sandwich ELSIA.Results Compared with control group,of lung tissues was observed in rats treated with OA induced ALI,which led PaO2 dropped,and lung tissue W/D ratio,PMN%in alvedolar lavage and IQA raised score in the lung at 2,4 or 6 hours after OA injection(P<0.01).In addition,IL-6,IL-8,and IL-10significantly increased,IL-6[plasma,control:(73.95±14.68)pg/ml,2 h:(186.70±23.85)pg/ml,4 h:(238.50±26.46)pg/ml,6 h:(215.95±25.86)pg/ml,P<0.01;lung tissue,control:(60.58±12.91)pg/ml,2 h:(160.32±24.57)pg/ml,4 h:(195.27±46.28)pg/ml,6 h:(185.66±17.42)pg/ml,P all =0.000)],IL-8[plasma,control:(80.69±20.42)pg/ml,2 h:(184.11±19.51)pg/ml,4 h:(286.20±53.34)pg/ml,6 h:(241.30±45.85)pg/ml,P<0.01;lung tissue,control:(69.14±15.96)pg/ml,2 h:(174.10±20.36)pg/ml,4 h:(249.02±31.17)pg/ml,6 h:(237.74±34.18)pg/ml,P<0.01] and IL-10[plasma,control:(39.78±8.97)pg/ml,2 h:(111.18±11.46)pg/ml,4 h:(115.60±13.91)pg/ml,6h:(102.41±9.93)pg/ml,P<0.01;lung tissue,control:(71.86±14.19)pg/ml,2 h:(126.96±18.72)pg/ml,4 h:(151.88±27.61)pg/ml,6 h:(137.28±14.22)pg/ml,P all=0.000] levels.in association with a decreased H2S content of plasma lung tissue decreased in[plasma,control:(36.58±6.80)μmol/L,2 h:(21.30±2.75)μmol/L,4 h:(20.63±1.26)μmol/L,6 h:(20.00±1.60)μmol/L,P<0.01;lung tissue,control:(27.61±2.20)μmol/L,2 h:(20.67±1.37)μmol/L,4 h:(20.79±1.10)μmol/L,6 h:(18.92±0.75)μmol/L,P<0.01]were observed in the plasma and lung tissue of OA-treated rats compared to controls.Administration of NaHS prior to OA treatment could lessen the lung pathologic changes induced by OA and elevate the concentrations of H2S in plasma[4 h:(26.67±3.44)μmol/L vs(20.63±1.26)μmol/L,P=0.042;6 h:(26.98±4.93)μmol/L vs(20.00±1.60)μmol/L,P<0.05]and lung tissue[4 h:(23.20±1.48)μmol/L vs(20.79±1.10)μmol/L,P=0.011;6 h:(21,43±1.79)μmol/L vs(18.92±0.75)μmol/L,P=0.016].At the same time,the levels of IL-6 in plasma(185.37±21.98)pg/ml vs(238.50±26.46)pg/ml,4 h,P=0.000;(124.22±21.84)pg/ml vs(215.95±25.86)pg/ml,6 h,P<0.01]and IL-8(199.40±34.56)pg/ml vs(286.20±53.34)pg/ml,4 h,P<0.01;(146.58±20.23)pg/ml vs(241.30±45.85)pg/ml,6 h,P<0.01]decreased significantly,but the level of IL-10(154.48±18.08)pg/ml vs(115.60±13.91)pg/ml,4 h,P=0.000;(138.06±20.01)pg/ml vs(102.41±9.93)pg/ml,6 h,P<0.01]increased significantly.Conclusions The levels of endogenous H2S drop during the course of ALI oleic acid induced in rats.Exogenous H2S could decrease the levels of inflammatory factors(IL-6and IL-8),but increase the level of anti-inflammatory factors(IL-10).It could change the ratio of inflammatory factors/anti-inflammatory factors and therefore play a protective role against oleic acid induced ALI in rats.
