1.Percutaneous vertebroplasty for treatment of osteoporotic thoracolumbar vertebral compression fracture by filling auto-solidification calcium phosphate cement

Objective To explore the clinical effect of the percutaneous vertebroplasty(PVP) and filling auto-solidification calcium phosphate cement(CPC) in treatment of osteoporotic thoracolumbar vertebral fractures. Methods From January, 2004, 26 patients with osteoporotic thoracolumbar vertebral compression fracture (29 vertebral bodies) were treated with intraoperative manipulative reduction, PVP and filling auto-solidification CPC. Results PVP were successfully performed with unilateral or bilateral vertebral pedicles for the 29 vertebrae. The average injection dose of CPC was 4.6 mL. CPC leakage occurred during the procedure in 5 vertebrae of 5 cases. All the patients were followed up for 3 to 8 months (averaging 4.6 months) and experienced complete pain relief. There was no severe complication related to the treatment. Except in 3 patients who lost 15%of the vertebral body height, the anterior height of the injured vertebral body in the other 23 patients recovered averagely to 80%of the normal one. Conclusion PVP with filling auto-solidification CPC is safe and effective in the treatment of osteoporotic thoracolumbar vertebral compression fractures.
2.Construction and expression of human disulfide-stabilized Fv fragment gene to Rabies Virus

Objective To realize the construction and expression of human dsFv genes to rabies virus. Methods The genes of dsFv VH and VL were amplified by PCR-based point mutagenesis strategy. Then the genes were cloned into plasmid pET-22b (+). E. coli BL21(DE3) was transformed with the recombinant expression plasmid and the protein was induced in Luris-Bertani medium by addition of IPTG. The inclusion body proteins of VH and VL were denaturalized by GuHCl, and then re-naturalized in refolding solution to form dsFv fragments. Afterwards, we evaluated the antigen-binding activity of the dsFv and its stability as compared with its originative scFv. Results The fragment genes of dsFv to rabies virus were constructed successfully by PCR-based point mutagenesis strategy. Sequence analysis proved that cysteines were introduced into the position 44 aa of VH and position 100 aa of VL. The dsFv VH and VL genes were expressed in PET22b(+)/BL21(DE3). The VH and VL protein folded into the active dsFv antibody fragments which showed specific binding capability to rabies virus. Conclusion We succeeded in achieving the stabilization of the human scFv to rabies virus and obtaining the active human dsFv antibody fragments. This established a solid basis for further study of the biological activaty and clinical application of dsFv to rabies virus.
3.A controlled study of cortisol, thyroid hormone and autonomic function in anxiety disorder

ObjectiveTo investigate the features of the cortisol ( COR),thyroid hormone and autonomic function of anxiety disorder,and to observe the relationships between clinical subtypes,insomnia,gender factors and the neuroendocrine and autonomic function.Methods60 anxiety disorder patients and 60 healthy controls were randomly selected from a three-grade class A hospital over the same period.Blood at 8:00am to test the plasma COR,T3,T4,TSH level and examined the heart rate variability (HRV) on the same day,the anxiety disorder patients also finish the self-rating anxiety scale (SAS).ResultsThe plasma COR,TSH level and LF/HF index were significantly higher( (531.58 ± 218.59 ) nmol/L,P< 0.0l ; (3.50 ± 1.35 ) MIU/L,P< 0.05 ;3.87 ± 3.37,P<0.0l ),while the SDNN,LF,HF index were significantly lower in anxiety disorder than that in the control group (27.34±13.13,P<0.01;338.72 ±396.34,P<0.01;135.77 ±149.95,P<0.01).The plasma COR,TSH level and LF/HF index were significantly higher(P < 0.05,P<0.01,P<0.01 ),while the SDNN and HF index were significantly lower in panic disorder than that in the generalized anxiety disorder (P < 0.01 ).The plasma COR,TSH level and LF/HF index were significantly higher,while the SDNN,LF,HF index were significantly lower in panic disorder than that in the control group (P<0.01 ).The plasma COR was significantly higher,while the SDNN and HF index were significantly lower in generalized anxiety disorder than that of the control group (P<0.01 ).The plasma COR level and LF/HF index were significantly higher (P<0.01,P<0.05),while the SDNN,LF,HF index were significantly lower in anxiety disorder with or without insomnia than that in the control group (P< 0.01 ).The plasma TSH level was significantly higher in anxiety disorder with insomnia than that in the control group (P< 0.05 ).There were no significant difference of the neuroendocrine and autonomic function between anxiety disorder with or without insomnia (P > 0.05 ).The plasma COR,TSH level were significantly higher,while the SDNN,LF,HF index were significantly lower in the female group than that in the male group (P<0.01).The total SAS scores were significantly positively correhted with plasma COR levels,and was significantly negatively correlated with SDNN,HF index in the anxiety group.ConclusionThere are neuroendocrine and autonomic nervous system dysfunctions in anxiety disorder,and these dysfunctions are related with clinical subtypes,insomnia,and gender.

Objective:To investigate the relationship between TGF-? 1mRNA and acute rejection of small intestinal allografts in rats.Methods:Rats were divided into two groups.Group Ⅰ:Wistar→SD Small intestinal transplantation,Group Ⅱ:Wistar→SD Small intestinal transplantation+CsA.The pathologic changes of allografts at 3 rdday,5 thday,7 thday after operation investigated and the transcription of IL-2,IFN- ?,and TGF-? 1mRNA was detected by semi-quantity RT-PCR.Results:Pathologic change:There was acute rejection at 3 rdday,and became severe at 7 thday in group Ⅰ,and there was only slight rejection at 7 thday in groupⅡ.TIFN-?、IL-2mRNA obviously increased after operation in groupⅠ.But they increased only a little in group Ⅱ.It had statistical significance between the two groups(P
5.Comparison between vertebra fracture fixation plus calcium sulfate cement vertebroplasty and simple fracture fixation for treatment of thoracolumbar burst fractures

Objective To compare the clinical outcome between vertebra fracture fixation plus injectable calcium sulfate vertebroplasty and simple vertebra fracture fixation in the treatment of thoracolumbar burst fracture.Methods A total of 61 patients with thoracolumbar burst fracture treated from January 2005 to December 2008 were involved and divided into two groups,ie,Group A ( treated with three-level fixation at fractured vertebra and injectable calcium sulfate vertebroplasty) and Group B ( treated with three-level fixation at fractured vertebra alone).Group A had 22 males and 10 females,at mean age of 36.8 years (21-65 years).The mean follow-up period was 15.6 months (13-27 months).Group B had 18 males and 11 females,at mean age of 38.3 years (19-70 years).The mean follow-up period was 14.7 months (12-28 months).The ratio of anterior vertebral height,Cobb angle,VAS score were compared between the two groups.Results There were no statistical differences in the aspects of age,sex,fracture segments and preoperative neurological status distribution in the two groups( P >0.05 ).All patients with partial neurologic deficits initially improved for 1-2 grade at the final follow-up.Blood loss and operation time in Group A were less than that in Group B (P <0.05 ).The ratio of anterior vertebral height and the Cobb angle showed no statistical significance (P > 0.05 ),but the ratio of anterior vertebral height and the Cobb angle in Group A was less than those in Group B at the latest follow-up (P <0.05 ).The VAS score showed no statistical significance between the two groups at the latest followup (P > 0.05 ).There was one patient with screw breakage in Group B,while there was no implant failure in Group A.Conclusion The vertebra fracture fixation plus calcium sulfate cement vertebroplasty is a safe and effective method for the treatment of thoracolumbar burst fracture as it can restore the vertebral mechanical strength,achieve and maintain kyphosis correction,decrease the instrument failure rate and loss of vertebral height.
6.Transpedicular screw fixation for thoracolumbar fractures

Objective To evaluate the feasibility and outcome of transpedicular screw fixation in fracture reduction and maintenance of reduction for thoracolumbar fractures.Methods The clinical data of 43 patients with thoracolumbar fractures admitted to Orthopedic Trauma Hospital of Qingdao from January 2003 to December 2006 were retrospectively analyzed.The inclusion criteria were as follow;sin-gle vertebral fracture involving T_(12)-L_2, integrated pedicle and no inferior endplate fracture.All patients received operation within 10 days after injury.Three-level fixation at fractured vertebra plus pedicle screw fixation was performed in 23 patients (Group A) including 15 males and 8 females, at mean age of 45 years (19-77 years).Traditional two-level fixation was done on the other 20 patients (Group B) inclu-ding 11 males and 9 females, at mean age of 42 years (22-67 years).In Group A, the transpedicular screws fixing the fractured vertebra and cephal vertebra were distracted after locking the transpedicular screw of the fracture vertebra and caudal vertebra.Results The height of the anterior edge of the cau-dal disc in Group B was 0.035 ±0.042, greater than 0.061 ±.036 in Group A, with statistical differ-ence (P < 0.05).There was no statistical difference in the changes of Cobb angle after the operation be-tween (915.8 ±7.8)°in Group A and (13.1±5.2)°in Group B.34 patients were followed up for an average period of 15 months (11-24 months) , which showed smaller Cobb angle in Group A (5.4°compared with Group B (8.9°)(P <0.05).There was one patient with screw breakage in Group A and two in Group B.Conclusion Transpedicular screws can enhance the stability of the posterior short-segment instrumentation, avoid over-distraction of the caudal discs, improve stress distribution and lessen loss of correction for some thoracolumbar fractures.
7.CT characteristic findings of Meckel's diverticulum and its complications in children

Objective To investigate the CT manifestations of Meckel's diverticulum and its complications in children. Methods Retrospective analysis of Clinical and CT findings in 25 cases with pathologically proved Meckel's diverticulum. The unenhanced and contrast-enhanced CT were obtained in all patients. Results (1)The direct signs: the diverticulum cannot be shown in 5 cases; the diverticulums were found around the navel or in lower right abdomen in 20 cases. The blind-ending fluid-filled or gas-filled structures were found in 9 cases, with heterogeneous ring-enhancement. The tubercle-like structures were detected in 11 cases, which showed no enhancement in 4 cases, and homogeneous enhancement in 3 cases,and heterogeneous enhancement in 4 cases. (2) The indirect signs included intestine obstruction in 8 cases,swelling fat layer surrounding the diverticulum in 9 cases, free gas around the diverticulum in 3 cases,thickened mesentery in 8 cases, ascites in 4 cases, and intussusception due to inverted Meckel's diverticulum with "target sign" in 1 case. (3) CT classification: with diverticulitis or diverticular bleeding in 20 eases; with intestine obstruction or intussusception in 8 cases; bands-caused obstruction in 7 cases;intussusception in 1 case; with perforation in 3 cases. Conclusion Meckel's diverticulum and its complications have typical CT findings, and CT can clearly demonstrate diverticulum's shape, margin,internal components and surroundiug tissues.

10.Genetic polymorphisms of drug-metabolizing enzymes CYP2C19,NAT2, and TPMT in a Han Chinese population from He-nan area

AIM: To determine the distribution and frequency of functionally important allelic variants in the cytochromes P450 (CYP) 2C19, arylamine N-acetyltransferase 2 (NAT2), and thiopurine S-methyltransferase (TPMT) genes in the Han Chinese population and compare them with those of other ethnic populations.METHODS: Genotyping was carried out in a total of 210 unrelated Han Chinese volunteers derived from He-nan area. CYP2C19 variants ( * 2 and * 3), NAT2 variants ( * 6 and * 7), and TPMT variants ( * 3A, * 3B, and * 3C) were detected using polymerase chain reaction/restriction fragment length polymorphism (PCR-RFLP)assays. Detection of NA T2 * 5 and TPMT * 2 were performed using allele-specific polymerase chain reaction assays. RESULTS: Allele frequencies of CYP2C19 * 2 and * 3 occurred with 34.76 % and 6.4 %, respectively.Thirty-one persons ( 14.8 % ) carried two of these CYP2C19 alleles responsible for poor metabolizing activity.The frequencies of specif ic NAT2 alleles were 59.1%, 4.1%, 26.4 %, and 9.5 % for * 4 (wild-type), * 5(341C), * 6 (590A), and * 7 (857A), respectively. Genotyping of three different single nucleotide polymorphisms in the NA T2 gene revealed that the frequency of slow acetylators was 19.5 %. TPMT * 3C had an allelic frequency of 1.2 %. TPMT* 2, TPMT * 3A, or TPMT* 3B was not detected in the analysed samples. CONCLUSION: The overview of allele distribution for drug-metabolizing enzymes CYP2C19, NAT2, and TPMT among a Han Chinese population shows obvious difference to Caucasians. The data will be useful for clinical pharmacokinetic investigation and drug dosage administration to Han Chinese population.
