This study aimed to evaluate the relationship between task performance, job satisfaction, and organizational contribution of dental hygienists, and to analyze the resulting impact on the organizational contribution of the dental hygienists. The study sample included 300 people working in a dental clinic during May 2016; data were collected through a self-questionnaire and an online questionnaire. Subsequently, 285 responses were analyzed. The results showed that a higher monthly income, business support services, and work satisfaction affect organizational contribution. The correlation of organizational contribution was the highest with business management in task performance and with work satisfaction in job satisfaction. In conclusion, a dental hygienist, who may have highest careers and is involved in the management of dental work, affects the organizational contribution of the dental clinic.

Subdural hematoma is a serious but rare complication of spinal anesthesia. A 70-year-old woman patient underwent elective total knee replacement under spinal anesthesia. At 4 days postoperatively, the patient complained of headache and vomiting. Brain computed tomography revealed an acute-on-chronic subdural hematoma with midline shift. The patient recovered completely after surgical decompression. We report a patient with an undiagnosed chronic subdural hematoma, who developed acute-on-chronic subdural hematoma after spinal anesthesia.

We report a first case of meningitis due to listeria monocytogenes after cardiac transplantation in Korea. This patient is a 40-year-old man with Dilated cardiomyopathy, he presented with intermittent dyspnea and abdominal distension for about 1 year. After cardiac transplantation, he was treated with azathioprine, cyclosporine and prednisolone for graft rejection. He was presented with intermittent fever, headache and lethargy for about 10days after cardiac transplantation. Listeria monocytogenes was isolated from blood culture and CSF culture. He was treated with intravenous penicillin G for 10days successfully and changed with ampicillin for 10days, took oral ampicillin for 10days without any complication after discharge.

Peripheral nervous system dysfunction is a rare complication in Henoch-Schonlein purpura, but it tends to recover spontaneously without treatment. A 78-year-old man who had ankylosing spondylitis presented with Henoch-Schonlein purpura associated with progressive sensorimotor polyneuropathy. He was diagnosed with chronic inflammatory demyelinating polyneuropathy, which did not improve despite intravenous immunoglobulin therapy. We describe a case of Henoch-Schonlein purpura, accompanied by chronic inflammatory demyelinating polyneuropathy in a patient with ankylosing spondylitis.

OBJECTIVES: The symptoms of fungal infections vary from minor to severe. According to recent reports, fungal infection prevalence is increasing due to increases in the aging population and in patients with compromised immune systems. However, the total prevalence of fungal infections in South Korea is unknown. We investigated the recent 5-year prevalence of each type of fungal infection in South Korea across age, sex, and regional groups. METHODS: Nationwide data from the National Health Insurance of South Korea were used. The target population included patients who had received treatment for a fungal infection that was listed as the main disease. For each patient, multiple treatments were counted as one case per year in the data analysis. RESULTS: The annual prevalence of all-type mycoses increased from 6.9% in 2009 to 7.4% in 2013. Among the specific groups, the prevalence of dermatophytosis was highest (5.2%), followed by that of opportunistic mycoses (1.7%) and superficial mycoses (0.2%). The prevalence of subcutaneous mycoses and generalized mycoses was very low (8 cases per 100,000 persons and 3 cases per 1 million persons, respectively). CONCLUSIONS: Every year, approximately 7.1% of the Korean population receives treatment for fungal diseases. Cases of opportunistic mycoses are assumed to increase each year. Therefore, further research is needed to understand and monitor the prevalence of mycoses to establish management policies to reduce the burden of fungal diseases.

Suicide is a major public health issue that causes over 800,000 deaths each year globally. Korea ranks high in suicide rates, in which around 24.3 per 100,000 individuals are reported to have died by intentional self-harm in 2017 according to Statistics Korea. The aim of this study was to examine the current status and trend of suicide ideation and attempt using data from the following five sources: Korean National Health and Nutrition Examination (KNHANES, ‘07–13, ‘15–17), Korean Community Health Survey (KCHS, ‘08–09, ’13, ’17), Korean Wealth Panel Study (KOWEPS, ‘12–17), Korea Health Panel Survey (KHP, ‘10–13), and Statistics Korea (1983–2017). Suicide ideation and attempts were also further examined based on equalized household income levels. Data published by Statistics Korea were used to show the updated suicide rate and number of deaths by intentional self-harm. The rate of suicide ideation at the recent year was 4.73% (KNHANES, ‘17), 6.96% (KCHS, ‘17), 1.63% (KOWEPS, ‘17), and 5.39% (KHP, ‘13). That of suicide attempts as recent year was 0.71% (KNHANES, ‘17), 0.32% (KCHS, ‘17), and 0.09% (KOWEPS, ‘17). Annual percentage change of suicidal ideation was −15.4% (KNHANES, ‘07–17), −2.5% (KCHS, ‘08–17), −8.6% (KOWEPS, ‘12–17), and −10.9% (KHP, ‘10–13). Annual percentage change of suicide attempts was −4.0% (KNHANES, ‘07–17), −4.4% (KCHS, ‘08–17), and −14.9% (KOWEPS, ‘12–17). Individuals with lower income levels were more likely to experience suicide ideation and attempts. Considering that Korea still shows a high suicide rate despite the continuously decreasing trend of suicide ideation and attempt, continuous observation and appropriate policy implementation regarding suicide related problems are necessary.

Background: Limited longitudinal evidence exists regarding the potential association between smoking status and hip fracture among individuals with type 2 diabetes. We investigated this association using large-scale, nationwide cohort data for the Korean population. Methods: This nationwide cohort study included 1,414,635 adults aged 40 and older who received Korean National Health Insurance Service health examinations between 2009 and 2012. Subjects with type 2 diabetes were categorized according to their smoking status, amount smoked (pack-years), number of cigarettes smoked per day, and duration of smoking. The results are presented as hazard ratios (HRs) and 95% confidence intervals (CIs) for the associations between smoking status parameters and risk of hip fracture in multivariable Cox proportional hazard regression analysis. Results: Compared with never-smokers, an increased adjusted HR (aHR) for hip fracture was observed in current smokers (1.681; 95% CI, 1.578 to 1.791), and a comparable aHR for hip fracture was found in former smokers (1.065; 95% CI, 0.999 to 1.136). For former smokers who had smoked 20 pack-years or more, the risk was slightly higher than that for never-smokers (aHR, 1.107; 95% CI, 1.024 to 1.196). The hip fracture risk of female former smokers was similar to that of female current smokers, but the hip fracture risk in male former smokers was similar to that of male never-smokers. Conclusion Smoking is associated with an increased risk of hip fracture in patients with type 2 diabetes. Current smokers with diabetes should be encouraged to quit smoking because the risk of hip fracture is greatly reduced in former smokers.

Background: Swallowing function deterioration is a common problem experienced by older adults worldwide. Many studies have been conducted to improve swallowing function in older adults; however, due to differences in intervention methods and study designs, it is difficult to draw a common conclusion. This study aimed to analyze trends and intervention methods in studies of swallowing function intervention for older adults conducted from 2010 to 2022, to establish a systematic approach for developing interventions to improve swallowing function in older adults and to provide evidence for this approach. Methods: Literature research was conducted for studies published between 2010 and 2022 that applied to swallow function interventions to adults aged 60 years or older. Databases including PubMed, Medline, RISS, Science On, KISS, and KCI were used. From a total of 1,164 articles searched using keywords, 20 articles were selected for final analysis. Results: The number of published articles steadily increased over time, and the intervention period was most commonly 6 or 8 weeks. The types of interventions included focused exercises to improve oral muscle strength in 12 articles and programs incorporating education, practice, and expert management in 8 articles. Among the focused exercises, tongue-strengthening exercises were most common in 4 articles. The evaluation variables for intervention effects were muscle strength evaluation, oral function evaluation, quality of life, and oral health and hygiene status. Muscle strength and oral function evaluations were statistically significant in focused exercise interventions, while the quality of life and oral health and hygiene status was significant in program interventions. Conclusion This literature review is meaningful as a study that can be used to select the intervention period and program contents when planning an elderly swallowing intervention program.

Suicide has been a long-standing problem for global public health, along with almost 800,000 deaths from suicide worldwide in 2016, accounting for 1.4% of all deaths. This study aimed to suggest up-to-date information about suicide-related indicators such as the rate of suicidal ideation, suicide attempt, and suicide death, and its trends by applying sampling weight to make it nationally representative. The rate of suicidal ideation as recent year was 4.73% (KNHANES, ‘17), 6.96% (KCHS, ‘17), 2.29% (KOWEPS, ‘18), and 5.39% (KHP, ‘13). That of suicide attempt as recent year was 0.51% (KNHANES, ‘18), 0.32% (KCHS, ‘17), and 0.15% (KOWEPS, ‘18). Annual percentage change (APC) of suicidal ideation was -15.4% (KNHANES, ‘07–13, ‘15, ‘17), -2.5% (KCHS, ‘08–09, ‘13, ‘17), -10.8% (KOWEPS, ‘12–18), and -10.9% (KHP, ‘10–13). APC of suicide attempt was -4.4% (KNHANES, ‘07–13, ‘15-18), -4.4% (KCHS, ‘08–09, ‘13, ‘17), and -13.6% (KOWEPS, ‘12–18). APC of death by intentional self-harm was -1.25% (Statistics Korea, ‘07–18). All suicide-related indicators were found to be decreasing in the overall from 2009. Individuals with lower income level were more likely to experience suicidal ideation and suicide attempts. Even though suicide rate had been continuously decreasing from its highest point in 2011 (suicide rate: 31.7 per 100,000 population) to 2017 (suicide rate: 24.3 per 100,000 population), it increased again in 2018 (suicide rate: 26.6 per 100,000 population). This information would be grounded on policy implementation for suicide prevention, thus continuous data observation is necessary.

BACKGROUND AND OBJECTIVES: Although it is well known that mitral valve repair provides a better postoperative outcome than valve replacement for mitral valve prolapse with mitral regurgitation, there haven't been much studied the nature of remnant regurgitation and the change of heart function and structure by remnant regurgitation after mitral valve repair surgery. We tried to research for it. METHODS AND RESULTS: Retrospective analysis of echocardiographic data and medical records was done in patients who underwent repair surgery for mitral valve prolapse with significant mitral regurgitation at the Keimyung University Dong-san Medical Center from February 1996 to February 2000. Patients who underwent echocardiography before and after the surgery were selected for the analysis. Of patients who underwent mitral valve repair surgery for mitral valve prolapse with mitral regurgitation during that period, 30 patients (male 15, female 15) were included in this study. The average age of them was 44.30+/-14.30 year old. After surgery, heart chamber size was decreased significantly in all patients (e. g. LVDd 6.25+/-1.06 cm vs. 5.14+/-0.62 cm, LV mass 272.46+/-107.36 gm vs. 197.30+/-75.16 gm). Remnant mitral regurgitation after repair surgery was found in 22 patients (73.3%). Remnant mitral regurgitation above trivial flow was found in 10 patients. Such case as involving ant. leaflet was found in 5 patients (16.7%), as involving post. leaflet, in 2 patients (6.7%), as involving both leaflet, in 3 patients (10%) among 10 ones. Eccentric flow among remnant regurgitation, was observed in 2 patients who underwent repair surgery for ant. leaflet, 1 patient who underwent repair surgery for both leaflet. CONCLUSION: Mitral valve repair surgery reduce the left ventricular chamber size and left ventricular mass. Postoperatively, patients who involve ant. leaflet were much remnant regurgitation above trivial flow more than others.
