2.Identification of a novel transcriptional factor HMGB34367 for activation of ?-smooth muscle actin gene in relation to pulmonary fibrosis

Objective: To demonstrate the influence of HMGB34367,a subfamily member of the high-mobility group protein B1(HMGB1),as a transcriptional factor of the ?-SMA gene expression.Methods: We cloned HMGB34367 cDNA from the fibrotic lung tissue of the Balb/C mouse treated with BLM by RT-PCR,sub-cloned it into a eukaryotic expression vector pcDNA 3.0 or pEGF-N2,and constructed a report plasmid,pDsRed-SMA,encoding red fluorescence(RFP) driven by ?-SMA promoter.Following the co-transfection of pDsRed-SMA and HMGB34367-pcDNA3.0/ pEGF-N2-HMGB34367 in the cultured 16HBE cells,we tested the expression of the RFP in the absence and presence of TGF?1 by fluorescence microscopy.After the transfection of HMGB34367-pcDNA3.0,the nucleus extracts from the transfected cells were subjected to electrophoretic mobility shift assay(EMSA) for the detection of the binding activity of HMGB34367 with ?-SMA promoter CarGB motif.RT-PCR was performed for the evaluation of the ?-SMA gene expression in the cells.Results: The over expression of HMGB34367 activated the ?-SMA promoter and enhances the expression of the ?-SMA gene.An increased binding activity of HMGB34367 with CarGB motif was detected by EMSA in the transfected cells.Conclusion:HMGB34367,a member of HMGB1 family,could act as a transcription factor for the transcriptional activation of the ?-SMA gene,which may play an important role in the development of lung fibrosis.
3.Role of epithelial-myofibroblast transdifferentiation under injury in the development of airway remodeling

Objective:To investigate if injury-related myofibroblast induction could occur in bronchial epithelial cells and the role of TGF-?_1(rhTGF-?_1) in epithelial-myofibroblast transdifferentiation. Methods:Cultured bronchial epithelial cells, 16HBE-14o, were exposed to poly-L-arginine(PLA). Using a immunohistochemical method, ?-smooth muscle actin (?-SMA) expression was detected in those cells after being treated with PLA . Relative activity of chloramphenicol acetyltransferase(CAT) was assessed in 16HBE-14o cells, which were transiently transfected with VSMP8 plasmid containing ?-SMA promoter linked to CAT gene, at different time points after rhTGF-?_1(10 ?g/L) treatment. Results:①?-SMA positive, spindle-shaped cells emerged among the regenerated 16HBE-14o epithelial cells following damage. The number of ?-SMA positive 16HBE-14o cells increased and cell repair was delayed under rhTGF-?_1 stimulation.② CAT activity increased significantly in VSMP8 plasmid transfected 16HBE-14o cells 24 h after TGF-?_1 stimulation. 5.Spiral CT Manifestations of Blunt Liver Trauma

Objective To investigate the spectrum of spiral CT imaging findings of blunt liver trauma.Methods Clinical data of 17 patients with blunt liver trauma were retrospectively collected. All patients underwent standardized spiral CT examination of the upper abdomen, which include plain scan, arterial phase and portal venous phase acquisition. The morphology, density and integrity of liver parenchyma and intrahepatic venous structures were carefully observed, as well as regions of porta hepatis, peritoneal cavity and retroperitoneal space.Results Twelve cases (70.6%) developed hepatic parenchymal laceration. There were 9 cases (52.9%) of traumatic hematoma, among which 5 were intraparenchymal and 4 were subcapsular. One case (5.9%) showed active bleeding within an intrahepatic hematoma, while two cases (11.8%) had injury (laceration) of hepatic veins. There were 7 patients (41.2%) who demonstrated the so-called “halo sign” around the intrahepatic portal branches. Thirteen patients were associated with peritoneal fluid (blood) collection, 3 with hematoma or hemorrhage of the right adrenal gland, 8 with plural effusion and 3 cases with rib fractures of right lower chest. Conclusion CT imaging findings of blunt liver trauma include parenchymal laceration, intraparenchymal and /or subcapsular hematomas, active hemorrhage, and tear of hepatic veins. Plain CT scan and contrast-enhanced dual-phase acquisition is very important for the comprehensive evaluation of patients with blunt liver trauma.

7.MRI evaluation of prognosis for hypoxic ischemic encephalopathy in term neonates

Objectives To explore the characteristics and value of magnetic resonance imaging (MRI) in predicting prognosis of hypoxic ischemic encephalopathy (HIE) in neonatal period, at 4 months and 4 years old. Methods Twenty-four patients with HIE were examined by MRI. Their MRI results in the neonatal period, at month 4 and year 4 and neurological functions at year 4 were compared. Results Periventricular signal alterations and deep gray matter involvement were common in HIE neonates. The neonates with deep gray matter involvement usually had neurological malfunction and poor prognosis. The patients with encephalomalacia and periventricular leukomalacia at 4 months and 4 years old also had poor prognosis. The abnormal MRI ifndings in HIE children at 4 month and 4 year old predicted the occurrence of neurological malfunction. Con-clusions The MRI of infant at 4 months old is important in prediction of neurological malfunction and provides guidance of clinical intervention for children with HIE.
8.Diagnostic Value of Contrast-Enhanced Volumetric Interpolated Breath-Hold Examination MR Sequence in Focal Hepatic Lesions

Objective To investigate the diagnostic value of a fast gradient-echo(GRE) three-dimensional contrast-enhanced volumetric interpolated breath-hold examination(3D-VIBE) MR sequence in evaluating focal liver lesions.Methods Conventional spin-echo T2W,2D GRE T1W plain scan and Gd-enhanced 3D-VIBE multi-phasic(early arterial,late arterial and portal venous phases) acquisitions were prospectively performed for 51 consecutive patients suspected of having focal liver lesions on CT or ultrasound imaging.Native T2W and 2D GRE T1W were acquired first,then 3D-VIBE fast scanning at early arterial,late arterial and portal venous phases respectively.The SNR and CNR of the liver lesions on plain scan and the enhancement patterns on contrast-enhanced 3D-VIBE images were carefully observed with correlation of the clinical and surgical pathological findings.Results There exited certain differences in SNR,CNR,and the enhancement patterns of different kinds of focal hepatic lesions in plain scan and Gd-enhanced multi-phasic 3D-VIBE acquisitions.Conclusion 3D-VIBE MR sequence is helpful in the detection and characterization of focal liver lesions.
9.Comparison study of readout-segmented EPI and single-shot EPI in DWI of orbit in healthy adults

Objective To explore the feasibility of the applicatoin of readout-segmented echo-planar imaging (RS-EPI) at 3.0T MR of the orbit DWI and compared with single-shot echo-planar imaging (SS-EPI).Methods Forty-two volunteers underwent both standard SS-EPI and RS-EPI DWI of the orbit at a 3.0T MR unit.Two sets of DWI images were independently qualitatively scored in the number of distinguishable normal structures,fat suppression,ghosting artifact and overall image quality.SNR of the vitreous body,geometric distortion ratio (GDR) in both anterior-posterior (AP) and right-left (RL) direction,and ADC value of the vitreous body and pons were also quantitatively calculated and compared between two sets of DWI images.The statistical analysis was performed.Results For qualitative assessment,the RS-EPI was superior to SS-EPI on the number of distinguishable normal structures (P=0.009 0),ghosting artifact (P<0.000 1),and overall image quality (P<0.000 1),while no significant difference was found on fat suppression (P=0.753 9).For quantitative assessment,RS-EPI had significantly lower GDR than the SS-EPI in both AP and RL direction (P=0.001 4,0.001 7).The SNR in RS-EPI was significantly lower than that of SS-EPI (P=0.004 0).Meanwhile,there was no significant difference of ADC on vitreous body and pons between RS-EPI and SS-EPI (P=0.143 8,0.126 2).Conclusion The RS-EPI can provide better image quality than SS-EPI protocol in orbital DWI at 3.0T MR imaging.
10.Analysis of drug resistance and the distribution of common pathogens of 342 cases of neonatal sepsis

Objective To analyze clinical common pathogenic bacteria distribution of neonatal sepsis and antimicrobial resistance ,in order to provide theoretical basis for clinical rational use of antibiotics .Methods 342 cases of neonatal sepsis were selected as the research objects ,through the case control study ,specimens were collected retro-spectively the clinical data and blood culture ,to understand the pathogenic bacteria children with neonatal septicemia culture,To understand the pathogenic bacteria in children with neonatal septicemia culture .Distribution and drug resistance of pathogenic bacteria were analyzed .Results 342 cases of neonatal septicemia infection rate was 5.4%, among the 136 cases of preterm infants with sepsis ,infection rate is 10.9%,206 cases of full-term infants with sep-sis,infection rate was 4.0%, Septicemia in premature infants infection than full -term sepsis (χ2 =88.07, P <0.01);No significant difference of septicemia in premature infants and full -term infants septicemia pathogen strain distribution(P>0.05).Coagulase negative Staphylococcus aureus and Staphylococcus aureus resistant to penicillin , drug resistance rates were 97.1% and 87.5%,secondly to erythromycin resistance ,respectively were 86.9% and 75.0%;Three kinds of common gram negative bacteria were sensitive toimipenem ,resistance to ampicillin and cefazo-lin,drug resistance rate was 100.0%,the second was piperacillin ,cefepime,cefotaxime and ceftazidime .Conclusion Neonatal septicemia of gram positive bacteria mainly coagulase negative Staphylococcus aureus and Staphylococcus aureus,gram negative bacteria mainly in grams of Klebsiella pneumoniae , Escherichia coli and Escherichia coli . Common pathogenic bacteria with drug resistance .Clinical treatment of neonatal sepsis should be rational use of anti -biotics,to strengthen the monitoring of drug resistance .
