1.The specificity of anti-cyclic citrullinated peptide antibodies in the diagnosis of rheumatoid arthritis from a large cohort study in the Chinese

Objective To determine the sensitivity and specificity of anti-cyclic citrullinated peptide antibodies (anti-CCP antibodies) in the diagnosis of rheumatoid arthritis. Method A total of 1018healthy donors, 212 patients with rheumatoid arthritis, 435 patients with other connective tissue disease were recruited to this study. Anti-CCP antibodies and IgM-rheumatoid factor (RF) were determined by ELISA according to manufacturer instructions, with a cut-off of 20U. Result The frequency of positive anti-CCP antibodies in patients with rheumatoid arthritis is 48.1% (n = 102 ), higher than healthy donors (2.6%,n = 26) and patients with other connective tissue diseases (3.7%, n = 16). The specificity of anti-CCP antibodies is 97.4%. The titer of anti-CCP antibodies in patients with rheumatoid arthritis (429. 7 U) is much higher than that in healthy donors (29. 3 U ) and patients with other connective tissue diseases (36. 5 U). The frequency of positive IgM-RF in patients with rheumatoid arthritis is 94. 3% whilst only21.5% in healthy donors. The false positivity rate of IgM-RF is higher than anti-CCP antibody. Conclusion Anti-CCP antibodies is a highly specific autoantibody in the diagnosis of rheumatoid arthritis.
2.Effects of aerobic exercise combined resistance training on plasma oxytocin, arginine vasopressin and anxiety in male opioids-dependent addicts

Objective:To observe the changes of plasma oxytocin (OT), arginine vasopressin (AVP) and anxiety after exercise intervention in male opioids-dependent patients.Methods:Forty-five male opioids addicts who met the inclusion criteria and voluntarily participated in exercise rehabilitation were enrolled.According to stratified random sampling, all subjects were divided into exercise group ( n=22) and control group( n=23). Exercise group attended aerobic exercise combined with resistance training intervention, 5 times per week for 8 weeks.Aerobic exercise was mainly treadmill and elliptical, while resistance training was based on strength and endurance training.Subjects completed self-rating anxiety scale (SAS) and Chinese perceived stress perception scale (CPSS) before and after intervention, as well as physical fitness tests. Besides, plasma OT and AVP levels were detected.Independent sample t-test, paired sample t-test, and Chi-square test were conducted by using SPSS 20.0 software. Results:Before the intervention, there were no significant differences between the two groups in demography, drug history, SAS, CPSS, plasma OT and AVP(all P>0.05). The overall anxiety detection rate was 66.67%, the scores of SAS and CPSS were both higher than the Chinese norm, and the difference was significant(both P<0.01) . After the intervention, the levels of plasma AVP, the scores of SAS and CPSS in the exercise group (AVP(19.57±2.23)pg/ml, SAS(50.17±10.09), CPSS(36.59±6.36)) were significantly lower than those in the control group (AVP(22.53±2.56)pg/ml, SAS(57.12±12.00), CPSS(43.09±5.57), all P<0.05), and plasma OT((61.98±5.27) pg/ml) was significantly higher than that in the control group ((54.64±7.62) pg/ml)( P<0.01). Compared with baseline, maximal oxygen uptake(VO 2max), 1 min push-ups and sitting body flexion increased significantly in the exercise group (all P<0.05). Conclusion:Opioids drug addicts were prone to excessive stress and anxiety and other negative emotions in the process of drug withdrawal. Eight-week aerobic combined with resistance exercise changed the plasma OT and AVP levels of opioids addicts, effectively alleviated the perceived stress, improved the anxiety state and their health related fitness level.

Recombinant Fl-V (rFl-V) fusion protein is the main ingredient of the current candidate vaccine against Yersinia pestis infection, which has been under investigation in clinical trial in USA. We investigated the soluble expression conditions of rF1-V in Escherichia coli BL21 (DE3) that we constructed before. After scale-up and optimization of fermentation processes, we got the optimized fermentation process parameters: the culture was induced at the middle exponential phase with 50 µmol/L of IPTG at 25 °C for 5 h. Soluble rFl-V protein was isolated to 99% purity by ammonium sulfate precipitation, ion exchange chromatography, hydrophobic chromatography and gel filter chromatography. The protein recovery was above 20%. Protein identity and primary structure were verified by mass spectrometry and Edman sequencing. Results of purity, quality and western blotting analysis indicated that the target protein is a consistent and properly folded product. Furthermore, the immunogenicity of various antigens formulated with aluminum hydroxide adjuvant was evaluated in mice. Serum antibody titers of 4 groups including 20 µg rFl, rV and rFl-V and 10 µg rFl+10 µg rV, were assayed by ELISA after 2 doses. The antibody titers of anti-Fl with 20 µg rFl-V were obviously higher than titers with other groups; meanwhile there were no significant difference of anti-V antibody titers among them. These findings confirm that rFl-V would be the active pharmaceutical ingredient of the plague subunit vaccine.

5.Application value of fetal echocardiography in diagnosis and prognosis assessment of anomalous pulmonary venous connection (APVC)

Objective To explore the application value of fetal echocardiography for the diagnosis and prognosis assessment of anomalous pulmonary venous connection (APVC),by comparing the results of prenatal diagnosis of APVC by ultrasound and follow-up.Methods Eighty-one APVC fetuses diagnosed by fetal echocardiography were followed up regularly.These results were compared with postnatal ultrasound diagnosis,operation and anatomy.The main points of methodology and differential diagnosis were summarized.Results Eighty-one APVC fetuses included 68 cases of total APVC(31 cases of supracardiac type,15 cases of intracardiac type,22 cases of infracardiac type),13 cases of Partial APVC(2 cases of supracardiac type,10 cases of intracardiac type,1 case of infracardiac type);12 cases of isolated APVC,69 cases of complex APVC(combined with other cardiovascular abnormalities);6 cases were born(4 infants of isolated APVC survived after operation,2 infants of complex APVC died),2 cases were ongoing pregnancy,71 cases were aborted (30 cases were dissected),2 cases were lost to follow-up.Conclusions Fetal echocardiography has important application value in diagnosis and prognosis assessment of APVC and the ones with other cardiovascular abnormalities.

7.Development of biphasic drug-loading lipid emulsion of Salvia miltiorrhiza and its quality evaluation.

The feasibility of simultaneously loading both liposoluble and water-soluble components of Salvia miltiorrhiza in emulsion was discussed, in order to provide new ideas in comprehensive application of effective components in S. miltiorrhiza in terms of technology of pharmaceutics. With tanshinone II (A) and salvianolic acid B as raw materials, soybean phospholipid and poloxamer 188 as emulsifiers, and glycerin as isoosmotic regulator, the central composite design-response surface method was employed to optimize the prescription. The coarse emulsion was prepared with the high-speed shearing method and then homogenized in the high pressure homogenizer. The biphasic drug-loading intravenous emulsion was prepared to investigate its pharmaceutical properties and stability. The prepared emulsion is orange-yellow, with the average diameter of 241 nm and Zeta potential of -35.3 mV. Specifically, the drug loading capacity of tanshinone II (A) and salvianolic acid B were 0.5 g x L(-1) and 1 g x L(-1), respectively, with a good stability among long-term retention samples. According to the results, the prepared emulsion could load liposoluble tanshinone II (A) and water-soluble salvianolic acid B simultaneously, which lays a pharmaceutical foundation for giving full play to the efficacy of S. miltiorrhiza.

8.SSR information in Erigeron breviscapus transcriptome and polymorphism analysis.

OBJECTIVEThe SSR information in the transcriptome of Erigeron breviscapus was analyzed in this study, in order to further develop new functional genes SSR markers laid a solid foundation.
METHODSSR loci were searched in all of 52,060 unigenes by using est_timmer. Perl program and SSR primers were designed by Primer3. Furthermore, 36 pairs of primers were randomly selected for the polymorphism analysis on 13 Erigeron breviscapus plants collected from different places.
RESULTA total of 3639 SSRs were found in the transcriptome of Erigeron breviscapus, distributed in 3260 unigenes with the distribution frequency of 6.99%. Di-nucleotide repeat was the main type, account for as much as 34.41% of all SSRs, followed by mono-nucleotide (31.41%) and tri-nucleotide repeat motif (28.08%). The di-nucleotide repeat motifs of AT/AT and AC/GT were the predominant repeat types (28.71%). The tri-nucleotide repeat motifs of AAT/AT was the predominant repeat types (7.94%). For validation the availability of those SSR primers, we randomly selected 36 pairs of primers for PCR amplification. Among them, 34 pair primers (94.44%) produced clear and reproductive bands, 19 pair primers showed polymorphism (52.78%), and 13 Erigeron breviscapus plants were divided into 2 groups.
CONCLUSIONThere are numerous SSRs in Erigeron breviscapus transcriptome with high frequency and various types, this will provide abundant candidate molecular markers for genetic diversity study and genetic map in this plant.

9.Treatment of paraneoplastic pemphigus with Castleman's disease.

OBJECTIVETo discuss the clinical findings and treatment of paraneoplastic pemphigus (PNP) with Castleman's disease.
METHODSTo investigate the clinical, histopathologic and CT findings of 8 cases paraneoplastic pemphigus with Castleman's disease.
RESULTSAll of 8 patients were diagnosed PNP first and were found Castleman's tumor incidently during routine examination. All 8 cases showed severe erosion or ulcer of the oral mucosa with various skin lesions. Histopathologically, there were intraepidermal acantholytic vesicle, basal cell liquefaction, necrotic keratinocytes in the epidermis and lymphocyte infiltration in the upper dermis. CT scan appeared solitary mass in these patients. Some of them were attacked by bronchiolitis obliterans. All 8 patients were failed by use of predisone. Obvious relief of PNP and pulmonary lesion occurred after tumor was rescted.
CONCLUSIONSParaneoplastic pemphigus with Castleman's disease is a rare disease. The key step is to find and resect the tumor in abdomen. CT scan should be used to detect the tumor in patients with PNP, especially, when predisone was failed in treatment.

10.Icariine stimulates proliferation and differentiation of human osteoblasts by increasing production of bone morphogenetic protein 2.

BACKGROUNDIcariine is a flavonoid isolated from a traditional Chinese medicine Epimedium pubescens and is the main active compound of it. Recently, Epimedium pubescens was found to have a therapeutic effect on osteoporosis. But the mechanism is unclear. The aim of the study was to research the effect of Icariine on the proliferation and differentiation of human osteoblasts.
METHODSHuman osteoblasts were obtained by inducing human marrow mesenchymal stem cells (hMSCs) directionally and were cultured in the presence of various concentrations of Icariine. 3-(4, 5-dimethylthiazol-2-yl)-2, 5-diphenyltetrazolium bromide (MTT) test was used to observe the effect of Icariine on cell proliferation. The activity of alkaline phosphatase (ALP) and the amount of calcified nodules were assayed to observe the effect on cell differentiation. The expression of bone morphogenetic protein 2 (BMP-2) mRNA was detected by reverse transcriptase-polymerase chain reaction (RT-PCR).
RESULTSIcariine (20 microg/ml) increased significantly the proliferation of human osteoblasts. And, Icariine (10 microg/ml and 20 microg/ml) increased the activity of ALP and the amount of calcified nodules of human osteoblasts significantly (P < 0.05). BMP-2 mRNA synthesis was elevated significantly in response to Icariine (20 microg/ml).
CONCLUSIONSIcariine has a direct stimulatory effect on the proliferation and differentiation of cultured human osteoblast cells in vitro, which may be mediated by increasing production of BMP-2 in osteoblasts.
