4.The effect of hyperuricemia on inflammation and endothelin-1 production in hypertensive patients

Objective To investigate the effect of hyperuricemia(HUA) on inflammation and endothelin-1 (ET-1) production and treatment of Benzbnomanone in hypertensive patients.Methods 90 initial hypertensive patients were enrolled from the inpatient division and clinic of our hospital,60 patients of them were identified HUA,and 30 patients were normal in uric acid as control.All these hypertensive patients with HUA were treated with basic anti-hypertensive drugs,of them 30 patients were additionally treated with Benzbromarone table 50mg for 8 weeks.The levels of inflammation indices and ET-1 were compared between these hypertensive patients with HUA and hypertensive patients with normal serum uric acid,also hypertensive patients with HUA treated with or without Benzbromarone for 8 weeks.Results Compared with the hypertensive patients with normal serum uric acid,levels of ET-1,interleukin-1 (IL-1) and high-sensitive C reactive protein (hsCRP) were higher in the hypertensive patients with HUA.Also,the levels of these indices were positively correlated with the level of serum uric acid [(86.6 ± 4.8) pg/ml vs (82.4 ±6.9)pg/ml; (47.6 ±6.2)mg/L vs (19.1 ±4.1) mg/L; (3.4 ±0.8)mg/L vs (2.9 ± 1.1)mg/L,r =0.81,0.74,0.83,all P < 0.05].Benzbromarone could effectively decrease the levels of ET-1,IL-1and hsCRP in the hypertensive patients with HUA [(49.8 ± 5.0) pg/ml vs (87.5 ± 5.9) pg/ml ; (17.6 ±8.8) mg/L vs (48.2 ± 7.0) mg/L; (1.7 ± 0.7) mg/L vs (3.5 ± 0.9) mg/L,all P < 0.05].Conclusions HUA could increase the levels of inflammation and ET-1,while Benzbromarone effectivelv decreased these changes.Decreasing the level of serum uric acid would retard the process of atherosclerosis in the hypertensive patients with HUA.
5.CT features of solid-pseudopapfllary tumors of pancreas: comparison with clinical and pathologi-cal findings

Objective To evaluate the features of solid-pseudopapiHary tumors of pancreas (SPTP),and to compare with the clinical and pathological findings in order to improve the diagnostic accura-cy.Methods The CT appearances were analyzed retrospectively in 5 patients with pathologically proven SPTP.There were 4 women and 1 man,with the mean age of(16.20±7.49)years.Plain CT and contrasted CT were performed in all 5 cases.Results The tumors of 4 caseswerelocatedinthe head and neck of pan-creats and 1 case was in the body and tail of pancreas.Clear tumor margins were seen in all cases.The mean diameter of these tumors was 5.2 cm(3.0-9.0 cm).Two cases were predominantly composed of cystic por-tions,2 cases were mainly composed of solid portions,and 1 case was composed of similar proportions of solid and cystic portions.Inhomogeneous enhancement Was rewealed in contrasted CT scan,but Was lower in both phase than normal pancreas.Tumor border discontinuous ring-shape calcification was seen in 1 case,dilata-tion of the main pancreatic duct Was demonstrated in 1 case and no metastasis were seen.Conclusions Certain CT features may suggest the diagnosis of SPTP.CT combined with the clinical feature is helpful to the diagnosis.

8.Clinical characteristics of neuromyelitis optica

Objective To explore the clinical characteristics of neuromyelitis optica(NMO).Methods The clinical data of 18 patients with NMO were analyzed retrospectively.Results Of the enrolled patients,15 cases(83.3%) were relapsing form of NMO,16 cases(88.9%) were female and the mean onset age was 36 years old.Simultaneous bilateral optic neuritis(ON) or ON occured sequentially and horizontal impairent of spinal cord were the main clinic features in these 18 patients.Of the patients who received spinal cord MRI examinations,4 cases showed the lesions located in cervical cord,6 cases in thoracic cord and 5 cases in both cervical and thoracic cord.13 cases revealed lesions in spinal cord extended across 3 or more vertebral segments.Furthermore,brain lesions were detected by MRI in 6 patients.The mean onset number of per patient was five.Seven patients presented with mild disability,7 patients with median disability,2 patients with severe disability and death of 2 patients.Conclusions Most patients with NMO are female and relapsing form accounts for the majority type.The cardinal manifestations are bilateral ON and longitudinally extensive transverse myelitis(LETM).Some patients show brain lesions that may be detected by MRI examination.Relatively high relapsing frequency of clinical event and severe impairing are common in NMO.

10.Research progress on neuroendocrine prostate cancer induced by androgen depriva-tion therapy

Patients with metastatic prostate cancer are typically managed with androgen deprivation therapy. Most patients initially respond to treatment, but many eventually develop castration-resistant prostate cancer. Neuroendocrine prostate cancer (NEPC) is a highly aggressive subtype of castration-resistant prostate cancer, which often results from neuroendocrine differentiation of prostate cancer cells. NEPC has a dismal outcome with an average survival of less than 1 year and exhibits less response to radiochemotherapy. At least 25%of patients with lethal castration-resistant prostate cancer are predicted to eventually develop this type of highly-aggres-sive NEPC. However, research on the molecular mechanism of NEPC is limited;thus, further studies are needed to explore the develop-ment and application of anticancer drugs for NEPC.
