1.Expressions of apoptosis gene Bcl-2 and Bad in the lung tissue of mice with acute lung injury

Objective To explore the expressions of apoptosis gene Bcl-2 (B-cell leukemia-2) and Bad (bcl-xl/bcl-2-associated death promoter) in lung tissue of mice with acute lung injury (ALI). Method Twenty-four BALB/C female mice were randomly divided into control saline group (n = 6) and ALI group (n = 18). The ALI Group was further divided into 3 subgroups with 6 mice in each subgroup: ALI (4 h) ,ALI (6h) ,and ALI (8h) subgroups. Rats in the normal control group received injection of saline. The ALI models were produced by in-jection of oleic acid (0.9 mL/kg) via vena caudalis, and the criteria were met with the characteristically pathologi-cal changes in the lung tissue. Pathological changes of the lung tissue were examined and scored under light mi-croscopy 4 h,6 h and 8 hours after injury. The expressions of gene Bcl-2 and gene Bad were detected in lung tissue at above set intervals by using RT-PCR. Data of these assays were analyzed by using one-way ANOVA with SPSS version 13.0 software. Statistical significance was established at a P value of less than 0.05. Results The rela-tive magnitude of Bel-2 expression in ALI (4 h), ALI (6 h) and ALI (8 h) subgroups were significantly higher (58.00±5.31), (42.00±4.30), (32.51±10.40) as compared with the control group (24.30±1.00) (F =68.581, P < 0.05). The relative magnitude of Bad expressions in ALI (4 h), ALI(6 h) and ALI (8 h) sub-groups were signiticantly higher (29.32±1.19), (58.64±4.45), (95.12±4.34)as compared with control group (4.01±0.34) (F = 386.902,P < 0.05). The pathological scores ofhmg injury in ALI(4h), ALI (6 h) and ALI (8 h) subgroups were significantly higher (1.82±0.14), (2.52±0.25), (3.45±0.29) as compared with control group (0.27±0.03) (F = 260. 512, P <0.05). Comparisons between groups showed statistical signifi-cances (P < 0.05). Conclusions The aggravation of lung injury induced by oleic acid in mice related to the down-regulation of apoptosis gene Bcl-2 expression and up-regulating apoptosis gene Bad expression in lung tissue.
2.Application of ex-vivo liver resection combined liver autotransplantation in complex liver resection

ObjectiveTo evaluate the feasibility and efficacy of ex-vivo liver resection combined liver autotransplantation for patients with massive primary liver cancer who underwent complex liver resection.Methods The clinical data of 4 patients suffering from massive primary liver cancer who were admitted to the Beijing Chaoyang Hospital from January 2008 to May 2010 were retrospectively analyzed.Regular liver resection could not be carried out because the first,second and third hepatic hilum of the 4 patients were invaded by the tumors,so ex-vivo liver resection combined liver autotransplantation were performed.ResultsThe operation was successfully carried out for the 4 patients.The operation time,the duration of anhepatic phase and the volume of operative blood loss were 690-840 minutes,250-300 minutes and 400-1400 ml,respectively.Portacaval bypass operation was not performed.After ex-vivo liver resection,the inferior vena eava or hepatic vein and portal vein of the 4patients were repaired,and the allogenous blood vessels were kept to extend the superior vena cava of the remnant liver so as to facilitate the anastomosis of blood vessels and reconstruction of the first hepatic hilum. After operation,the hepatic function of 1 patient was back to normal; 1 patient who stfffered from abdominal hemorrhage received reoperation for hemostasia; 1 patient was found with hepatic dysfunction; 1 patient died of hepatorenal dysfunction at postoperative day 5.Compensatory hypertrophy was observed in the 3 patients who survived at postoperative months 1-2.Of the 3 patients,2 were found with multiple pulmonary metastases at postoperative months 8 and 9,and they died at postoperative mouths 13 and 15.Until April 2012,1 patient survived for 37 months with no tumor recurrence or metastasis. ConclusionsEx-vivo liver resection combined liver autotransplantation provides the technical feasibility for performing complex liver resection for patients. The incomplete compensation of liver function and the short-term recurrence of tumors after operation are still the main issues which hinder the development of this technique.
3.Reestablishment the roof of coronary sinus in treating unroofed coronary sinus syndrome with endocardial cushion defect and persistent left superior vena cava

Objective To summarize the surgical procedures of unroofed coronary sinus syndrome with endocardial cushion defect and persistent left superior vena cava.Methods From 2000 to 2008,753 cases of endocardial cushion defect underwent surgical repair.Among them 15 cages(2.0%)were associated with persistent left superior vena cava and unroofed coronary sinus syndrome.The drainage of the persistent left superior vena cava and coronary vein were corrected simultaneously during the operation by reestablishing the roof of the coronary sinus.The persistent left superior vena cava were drained to right atrium by intra-atrial tunnel with pericardial patch or left atrium folding in 14 cases and it was ligated in the remain one.Most of the coronary veins were drained into right atrium after the correction of cardiac defect.Results One patient died of severe pulmonary infection early after operation.Echocardiogram indicated that the velocity of left superior vena cava increased in the site of the roof of left atrium in one patient after operation.No obstruction of vena cava and pulmonary vein and remnant shunt between the atriums were found in the other 14 cases.There were no death and other complications during3 months to 6 years of follow-up.Conclusion The surgical procedures of unroofed coronary sinus syndrome with endocardial cushion defect and persistent left superior vena cava is difficult and good result can be expected by reestablishing the roof of the coronary sinus.
5.RADIOIMMUNOASSAY OF ENDOTHELIN

Endothelin (ET) is an endothelium-derived vasoconstrictive peptide. We have developed a sensitive and selective radioimmunoassay for porcine/human endothelin (ET-1). 126I-labled ET was perpared by lodogen method and was purified by HPLC. Its specific activity was about 64.75 TBq(1750Ci)/mmol. The assay has a detection limit of 0.17pg/tube and the assay range was 0.25-1000pg/tube. The procedure was developed for extraction of endothelin from human plasma using C18 Sep-pak extraction cartridges. Human plasma samples were extracted, assayed and the plasma ET values of 17 healthly volunteers was found to be 2.81 ?0.60pg/ml. Both patients with uraemia(n = 20) and acute myocardial infarction(n= 11) had significantly higher plasma values than normal subjects.
6.Sun's procedure for patients with acute type A aortic dissection

Objective To analyze the early outcomes of the Sun' s procedure,which is an approach integrating total arch replacement using a 4-branched graft with implantation of a special stented graft in the descending aorta,and observe the mortality,morbidity and mid-term clinical results of postoperative in patients with acute type A aortic dissection (AAAD).Methods Clinical data of 61 consecutive AAAD patients undergoing the Sun' s procedure were analyzed.There were 53 male and 8 female,aged (54 ± 12) years.61 patients had postoperative follow-up and the clinical effect of Sun' s procedure were retrospectively analyzed.Results Cardiopulmonary bypass time was (229 ± 46) min,aortic clamping time was (147 ± 37) min and unilateral selective cerebral perfusion time was (35 ± 9) min.Reoperation for excessive bleeding in 5 cases (8.2%),acute renal failure in 3(4.9%),tracheotomy patients for pulmonary infection in 3(4.9%),and spinal cord injury in 2 (3.3%),delayed thoracic incision healing in 1 (1.6%),and osteofascial compartment syndrome of the right lower limb in 1 (1.6%).The 30 day mortality is 8.2%,and the leading causes was low cardiac output syndrome in 2(3.3%),multi-organ failure in 1 (1.6%),pulmonary infection in 1 (1.6%) and ruptured of abdominal aortic dissection in 1 (1.6%).The average follow-up time is(5.1 ± 0.7) years.Postoperative 1 year survival rate was 91.8%,5 year survival rate was 83.6%.Conclusion The Sun' s procedure has generated a relatively lower mortality rate in 61 patients with AAAD.Postoperative survival rate is high,and the reoperation rate is low.
7.Exploration and practice of bilingual teaching in pediatrics of the undergraduates

To further explore the result of bilingual teaching in pediatrics,we randomly chose 200 undergraduates of 4 class and released students'questionnaires about bilingual teaching with teaching content before and after class to assess students'understanding of bilingual teaching and analysed appraisal result.We found no significant difference of student score between students accepting bilingual teaching and not accepting the bilingual teaching,but there was difference for English tests and expression level.So we think that students can fully accept the bilingual teaching of pediatrics under the premise with selecting appropriate teaching methods and means.

9.Urethral reconstruction using lingual mucosal graft for the treatment of complex hypospadias.

OBJECTIVETo review and summarize the experience of urethral reconstruction using lingual mucosal graft for the treatment of complex hypospadias.
METHODSWe evaluated the methods using lingual mucosal graft for the treatment of complex hypospadias. The dorsal inlay graft method and the tube graft method were used in our study.
RESULTSFrom Nov. 2010 to Oct. 2011,21 cases were treated. All the cases had at least one failed hypospadisa surgery before. The dorasl inlay grafting technique was used for 16 patients, the tube grafting technique was carried out in 5 patients(2 cases received Tube + Duply method). The follow-up period ranged from 3 months to 12 months (by average 6 months). The success rate of the methods was 61.9% with complications in 8 cases. Urethral fistula developed in 3 patients and urethral stricture developed in 5 patients. All cases who suffered stricture received the tube grafting technique. The fistula and urethral stricture were occurred at the site of the urethral termonoterminal anostomosis. Urethral stricture is the most common complication after the tube graft method for the treatment of the complex hypospadias.
CONCLUSIONSHarvesting the lingual mucosal graft is feasible and easy to perform. The complex hypospadias repair technique using lingual mucosal graft method provides good results. There fore it should be one of the preferred techniques for complex hypospadias repair. The dorsal inlay grafting technique has lower complication rate than the tube grafting technique. Early and regular urethral sounding should be carried out when patients received the tube graft method.

10.The antiandrogenic effect of dimethachlon and its mechanism.

OBJECTIVETo evaluate the antiandrogenic effect of heterocyclic fungicide dimethachlon and its mechanism.
METHODSA combination of in vivo and in vitro assays was selected. Hershberger assay was used to determine the antiandrogenic potential of dimethachlon in vivo. Six-week-old castrated male SD rats were administrated once daily for 7 days with testosterone propionate (TP, 100 micro g/d, sc) plus gavage doses of dimethachlon (50, 100 or 200 mg x kg(-1) x d(-1)), or procymidone (150 or 300 mg x kg(-1) x d(-1), positive control), or iprodione (100 mg x kg(-1) x d(-1), positive control), or flutamide (50 mg x kg(-1) x d(-1), positive control). Transcriptional activation assay in vitro was employed to determine the mechanism of antiandrogenic activity of dimethachlon. Human hepatoma liver cells HepG2 were transiently cotransfected with human androgen receptor (AR) expression plasmid and AR-dependent luciferase report plasmid. Transfected cells were exposed to various concentrations of dimethachlon or flutamide with or without dihydrotestosterone to induce the expression of luciferase gene.
RESULTSIn Hershberger assay, dimethachlon, as well as other known antiandrogens, caused decrease in weight of androgen dependent organs or tissues. In 200 mg/kg group, the weight of seminal vesicle, ventral prostate, dorsolateral prostate, Cowper's gland, and levator ani plus bulbocavernosus muscles decreased by 57.8%, 44.8%, 43.9%, 30.1%, and 34.1% respectively, but did not decrease in the vehicle control group. The order of their antiandrogenic potencies was: flutamide > procymidone > dimethachlon > iprodione. In transcriptional activation assay, dimethachlon could inhibit dihydrotestosterone-dependent AR activity in transfected HepG2 cells in dose-effect relationship. The inhibiting potency of dimethachlon was about 1/100 of that of flutamide.
CONCLUSIONDimethachlon has antiandrogenic effect, and acts as an AR antagonist. Its antiandrogenic potency is lower than flutamide and procymidone, but higher than iprodione.
