1.Effects of estrogen and androgen on expression of metalloproteinase-2 in lachrymal gland in ovariectomized rats

Background The pathogenesis of dry eye is complicated,hormone level is thought to be one of impact factors in the development of dry eye.The regulation of the synthesis process of metalloproteinases(MMPs) in tissue has been reported.However,the effects of hormone on expression of MMP-2 in lachrymal gland is not clear.Objective This study was to investigate the effects of estrogen and androgen on the expression of MMP-2 in lachrymal gland in ovariectomized rats,and explore the role of MMP-2 in dry eye.Methods Sixty-four 3-monthold clean female Wistar rats were randomized into control group(8 rats),sham operation group(8 rats)and experiment group(48 rats).Ovariectomy(OVX) was performed on the rats of experiment group,and only fat tissue of abdominal cavity was cut off in the rats of the sham operation group.After 5 months of OVX,the experimental rats were subdivided into model control group,vehicle group,estrogen and androgen systemic or topical utilization groups and 8 rats for each group.Six weeks after administration of the drugs,the lachrymal gland was obtained.The expression of MMP-2 mRNA in the lachrymal gland was detected by reverse transcription PCR(RT-PCR),β-actin mRNA was used as an internal control,and the expression of MMP-2 protein was detected by Western blot,GAPDH was used as protein loading control.The use and care of the rats complied with the ARVO Statement.Results The expression of MMP-2 mRNA was strongest in the systemic estrogen group and was weakest in the systemic androgen utilization group.A significant difference in the MMP-2 mRNA expression was found among the 8 groups(F=18.60,P<0.01),and the MMP-2 mRNA was significantly higher in the model group than that of the normal control group(0.66±0.10vs.0.47±0.10)(q=3.01,P<0.05).In addition,the MMP-2 mRNA was significantly higher in the systemic estrogen group compared with the model group (0.83 ±0.10 vs.0.66-0.10) (q =2.79,P<0.05) ; while the expression of MMP-2 mRNA was significantly declined in the systemic androgen group in comparison with the model group(0.12±0.04 vs.0.66±0.10)(q=11.41,P<0.01).The MMP-2 protein presented with a strongest expression in the systemic estrogen utilization group and a weakest expression in the systemic androgen utilization groups.The expression level of the MMP-2 protein in the lachrymal gland was significantly different among the 8 groups(F =7.28,P<0.01).The MMP-2 in the model group was significantly higher than that of the normal group(0.55±0.13 vs.0.38±0.08) (q =2.39,P<0.05),and that in the systemic estrogen group was increased in comparison with the model group(0.69±0.12 vs.0.55±0.13) (q =1.85,P<0.05).However,the MMP-2 in the systemic androgen group was significantly lowed in comparison with the model group(0.27±0.07 vs.0.55±0.13) (q =4.32,P<0.01).Conclusions Estrogen may up-regulate the expression of MMP-2 in lachrymal gland,but the effect of androgen is opposite.Hormone level may play an important role in the regulation of the function of lachrymal gland.
2.Effect of hyperbaric oxygen on hypertensive cerebral hemorrhage

ObjectiveTo evaluate the clinical effect of hyperbaric oxygen (HBO) on hypertensive cerebral hemorrhage.Methods100 patients treated with HBO were taken as treatment group, and other 50 patients without HBO as control group. Their effectual rate were compared.ResultsThe total effectual rate of treatment group was 61%,and that of control group was 42%(χ2=4.86,P<0.05).ConclusionHBO can improve the recovery of nervous disorder after hypertensive cerebral hemorrhage.

5.Clinical characteristics analysis of Chlamydia psittaci pneumonia detected by metagenomic next-generation sequencing

Abstract: Objective To analyze the clinical characteristics of Chlamydia psittaci pneumonia and improve the diagnosis and treatment skills of clinicians on this disease. Methods The clinical data of thirty-nine Chlamydia psittaci pneumonia cases detected by metagenomic next-generation sequencing (mNGS) from September 2020 to January 2022 at the Affiliated Hospital of Southwest Medical University were retrospectively analyzed. Results There was a history of poultry exposure in 89.7%(35 cases) of the patients. The most common clinical manifestations were high fever (92.3%, 36), cough (76.9%,30), muscle soreness (48.7%,19), headache (38.5%,15), etc. Laboratory examinations showed 76.9% of patients had a normal leukocyte count, and 76.9% had decreased lymphocyte count, often accompanied by elevated C-reactive protein (100%), procalcitonin (97.4%), interleukin-6 (95.8%), interleukin-10 (95.8%), alanine aminotransferase (74.4%), and aspartate aminotransferase (84.6%). Univariate analysis indicated that there were statistically significant differences in the levels of aspartate transaminase, blood urea nitrogen, C-reactive protein, and procalcitonin between severe pneumonia patients and non-severe pneumonia patients(P<0.05). Multivariate logistic regression analysis showed that an elevated blood urea nitrogen (OR=4.899) had guiding significance for predicting the occurrence of severe pneumonia. Bronchoscopy examination showed no abnormalities in 53.6% of the patients. The imaging manifestations of pulmonary lesions were mainly lobar pneumonia (61.5%) and air bronchograms (94.9%). Therapeutically, it was sensitive to tetracyclines, macrocyclic lactones, and fluoroquinolones. A total of 84.6%(33 cases) of the patients were cured and discharged from the hospital at the end of the treatment. Conclusion Chlamydia psittaci pneumonia is a zoonotic disease that can be detected by mNGS. An elevated blood urea nitrogen level has guiding significance for predicting the occurrence of severe pneumonia. Empirically-selected regimens based on doxycycline are effective for the treatment of Chlamydia psittaci pneumonia.
6.The value of pulse-indicate continuous cardiac output in septic shock patients

Objective To investigate the value of pulse-indicate continuous cardiac output(PICCO) in septic shock patients.Methods In a retrospective study,58 patients who were diagnosed to be suffering from septic shock in the intensive care unit (ICU) were enrolled,from January 2010 to December 2011,and were divided into two groups:PICCO group(n =28) and the conventional group (n =30).We compared their 6 h-EGDT compliance rate,the level of lactate and central venous pressure (CVP),the oxygenation index,the characteristics of fluid intake and balance within 72 hours,duration of mechanical ventilation,and ICU stay of the two groups.The incidence of MODS after 72 hours in ICU,28-day mortality were recorded and compared.Results (1) Fluid intake((9565 ±1623) ml vs (12245±2253) ml,t=2.673,P=0.021) and balance ((3656 ± 1904) ml vs (5465 ± 2765) ml,t =2.357,P =0.012) were significantly lower in PICCO group compared the conventional group within 72 hours.(2) The oxygenation index ((252.6 ± 87.4) vs (226.8 ± 69.4),P < 0.05) in PICCO group increased significantly and duration of mechanical ventilation ((134.7 ±42.8) h vs (193.3 ± 92.4) h,t =1.356,P =0.023) reduced significantly compared with the conventional group after 72 hours.(3) There was no difference in the 6 h-EGDT compliance rate,the level of lactate and CVP,6 h EGDT,ICU stay,the incidence rate of MODS after 72 hours,28-day mortality in ICU(P >0.05).Conclusion In contrast with classic methods as CVP monitoring,PICCO catheter may assess more accurately the volume status and guide early fluid resuscitation in septic shock patients.
7.Conformal radiotherapy for elderly patients with advanced non-small-cell lung cancer

Objective To evaluate the effect of conformal radiotherapy for elderly patients with adva nced non-small-cell lung cancer. Methods From November 1998 to June 2001, 24 elderly patients with advanced non-small -cell lung cancer were treated with conformal radiotherapy. ResultsOne-year, two-year and three-year overall survival rate for the entire group are 91 67%, 54 16%, 43 33%. No patients developed acute radiation pneumonitis . According to the univariate analysis, significant prognostic factors included superior vena cava syndrome, Karnofsky performance status scores and gross tumo r volume. Conclusions Conformal radiotherapy improve the effect in elderly patients with advanced no n-small-cell lung cancer.
8.Clinical significance of preoperative imatinib therapy in stomach gastrointestinal stromal tumors

Objective Gastrointestinal stromal tumor( GIST) is a rare mesenchymal tumor from gastroin-testinal tract,and surgery remains the only curative treatment.In order to improve the outcome of surgical treat-ment,reduce the risk of surgery,and increase the quality of life,preoperative imatinib( IM) treatment for GIST is investigated.Methods We retrospectively studied the multidisciplinary team model treatments for 8 GIST cases receiving preoperative IM treatment.The cases were prescribed IM of 400 mg daily for 12 to 40 weeks and exten-sively followed until surgery was considered feasibleg.The clinical significance and safety profile were analyzed. Results Partial responsive rate was 62.5% in this study.There was no intolerable sever adverse effects by IM preoperative treatment.All the cases received R0 dissection,with no intraoperative tumor rupture.The postopera-tive recovery was satisfied.Conclusion IM preoperative treatment brings significant clinical benefit to large stomach GISTs and cardiac region GISTs.The preoperative treatment should be monitored carefully under a multi-disciplinary team.Preoperative IM treatment is an evocative treatment strategy for high risk GIST.

Objective: To construct attenuated salmonella typhimurium haboring an eukaryotic co-expression plasmid encoding TIP30 and human IFN-? gene and observing its effect on the growth of adenoid cystic carcinoma. Methods: The TIP30 and human IFN-? genes were amplified by PCR and inserted into eukaryotic expression vector pCI-neofor the construction of expression plasmids pCI-TIP30 and pCI-IFN, respectively. A co-expression plasmid pCI-TIP30/IFN was constructed by linking TIP30 and human IFN-? gene using the sequence of internal ribosome binding sequence (IRES). Three recombinant expression plasmids were transformed into an attenuated AroA'autotrophic mutant of salmonella typhimurium SL7207, the resultant bacteria were used to infected murine macrophage in vitro and the expressed products were detected by Western blot and ELISA, respectively. Tumor growth was observed by oral administration of the recombinant salmonella typhimuriums to the nude mouse with adenoid cystic carcinoma. Results: Murine macrophage infected with recombinant salmonella transformed with both plasmids pCI-TIP30 and pCI-TIP30/IFN could express TIP30 protein, and murine macrophage infected with recombinant salmonella transformed with pCI-IFN or pCI-TIP30/IFN could secret human IFN-? in the culture supernatant. Attenuated salmonella typhimurium and three constructed recombinant salmonella typhimuriums all had evident inhibition onthe tumor growth in nude mouse with adenoid cystic carcinoma. Conclusion: The recombinant attenuated salmonella typhimuriums haboring plasmid pCI-TIP30, plasmidpCI-IFN and co-expressing plasmidp-CI-TIP30/IFN were successfully constructed, which could inhibit the growth of adenoid cystic carcinoma in nude mouse.
