2.Efficacy of endoscopic therapy in treatment of esophageal gastric varices bleeding in patients with hepatocellular carcinoma and portal vein tumor thrombus

Objective To compare the effect and survival period of endoscopic treatment group and non endoscopic treatment group of esophageal gastric varices bleeding in patients with hepatocellular carcinoma and portal vein tumor thrombus, in order to define the significance of endoscopic therapy for prolonging the survival period of patients. Methods 76 patients diagnosed as esophageal gastric varices bleeding with hepatocellular carcinoma and portal vein tumor thrombus from Jan. 2013 to Dec. 2015 were enrolled, of all these cases, 27 were non endoscopic treatment group and 49 were endoscopic treatment group. The medical records were analyzed retrospectively. Then compare the effect of emergency treatment, the main causes of death and survival time of the two groups. Results In the non endoscopic treatment group, 40.7% (11/27) patients died in one week after bleeding, 81.5% (22/27) patients died of esophageal gastric varices bleeding, the mean survival period was (42.03 ± 13.94) days; In the endoscopic treatment group, only 16.3% (8/49) patients died in one week (P < 0.05), 55.1% (27/49) patients died of esophageal gastric varices bleeding (P < 0.05), the mean survival time was (174.24 ± 34.42) days (P < 0.05). Conclusions Endoscopic therapy can effectively reduce the risk of death from acute hemorrhage and prolong the survival time in patients with hepatocellular carcinoma and portal vein tumor thrombus.

Objective To study the job demand of vocational medical laboratory technology in Jiangsu Province ,and as a basis to build curriculum system of vocational medical laboratory technology .Methods We surveyed 128 medical laboratory related institu‐tions in Jiangsu Province ,the survey included the demand for talent ,changing needs of reasons ,demand for vocational talent and job skill requirements ,data of the survey results were collected and analyzed .Results Demand for vocational talents for tertiary hospi‐tals were small ,while the primary health care institutions and relevant private medical institutions and companies were exuberant . The main jobs were clinical testing technology direction and pathological examination technical direction .In addition ,blood transfu‐sion and transfusion technology direction ,testing reagents and equipment marketing direction and technical for andrology laboratory direction also were needed .Conclusion According to the survey results ,we determine educational objectives ,basic requirement and specialty orientation of vocational medical examination major ,and propose clinical laboratory specializing in the basic platform+specialty orientation modulefor the curriculum system of vocational medical examination major .
4.The Clinical Value of Trophic Feeding in Preterm Infants

Objective To investigate the clinical effects of trophic feeding in preterm infants. Methods 93 preterm infants with asphyxia, prenatal asphyxia or respiratory distress syndrome who could not receive routine feeding were enrolled in this study. These patients were randomly divided into the treatment group and the control group. The hospitalized days, the time of tolerating milk feeding, regaining the birth body weight and discontinuing intravenous infusion were compared between the two groups. 5.Factors associated with recurrence of chronic hepatitis B after nucleoside analogs withdrawal: a meta-analysis

Objective To evaluate the factors associated with recurrence of chronic hepatitis B (CHB) after nucleoside analogs (NAs) withdrawal.Methods A literature search from PubMed,Wanfang data,CQVIP,CNKI,Duxiu and SinoMed was conducted to identify studies on the recurrence of CHB after NAs withdrawal.Meta-analysis was performed using RevMan 5.3.Random-effects or fixed-effects model was performed based on the heterogeneity.Weighted mean difference (WMD) was used to assess the continuous data,and odds ratio (OR) was used to assess the dichotomous data.Publication bias was evaluated with Egger' s regression test using Stata SE 11.0.Results A total of 20 case-control studies were included in this analysis.The recurrence rates were 21.0%,30.4%,33.2% in HBeAg-positive CHB patients,and 26.5%,34.1%,50.1% in HBeAg-negative patients after NAs withdrawal for 3 months,6 months and 1 year,respectively.For patients treated with lamivudine,the recurrence rates of CHB were 21.0%,28.0%,34.3% at 3-,6-and 12-month after NAs withdrawal.Meta analysis demonstrated that among HBeAg-positive CHB patients,the average age (WMD =7.36,95% CI:5.72-9.00,Z =8.81,P <0.01) and baseline HBV DNA level (WMD =0.26,95% CI:0.05-0.46,Z =2.44,P =0.01) were higher in recurrence group,while antiviral treatment duration was shorter in recurrence group (WMD =-3.12,95% CI:-4.56--1.68,Z =4.26,P < 0.01),and the rate of recurrence was higher in patients with liver cirrhosis (OR =2.59,95% CI:1.33-5.04,Z =2.79,P < 0.01).Among HBeAg negative patients,the average age of patients in recurrence group was higher than that in non-recurrence group (WMD =5.90,95% CI:1.57-10.23,Z =2.67,P < 0.01),and no difference was observed in other factors between recurrence and non-recurrence patients.Among patients treated with lamivudine,the average age (WMD =7.68,95% CI:5.02-10.34,Z =5.66,P <0.01) and baseline HBV DNA level (WMD =0.26,95% CI:0.05-0.46,Z =2.44,P =0.01) were higher,while antiviral treatment duration was shorter in recurrence group (WMD=-2.11,95%CI:-3.85--0.38,Z=2.39,P<0.01),and the rate of recurrence was higher in patients with liver cirrhosis (OR =2.59,95% CI:1.33-5.04,Z =2.79,P < 0.05).Conclusion Among HBeAg-positive and lamivudine-treated patients,age,baseline HBV DNA level,antiviral treatment duration and liver cirrhosis are associated with the recurrence of CHB after NAs withdrawal; while for HBeAg-negative patients,age is the only risk factor.
6. Comprehensive evaluation of suitable harvesting period of Acanthopanax Senticosus leaves by multiple indicators

Objective: The dynamic changes of eight active constituents and dry weight of Acanthopanax senticosus leaves in different periods were investigated, and the suitable harvesting period of A. senticosus leaves was discussed. Methods: The leaves of A. senticosus were collected at different times, and the dry weight of one hundred leaves was determined by electronic balance. The contents of L-phenylalanine, protocatechuic acid, syringin, chlorogenic acid, caffeic acid, rutin, hyperoside, and quercetin in the leaves of A. senticosus in different periods were determined by HPLC, and the total amount of accumulation of the eight active ingredients was calculated and combined with the analysis results of the principal component analysis method for comprehensive evaluation. Results: The results showed that the vegetative dry weight of A. senticosus increased during the period from S1 to S5, and increased most rapidly in S1-S2. The content of eight active constituents in the leaves of A. senticosus showed dynamic changes in different periods. The content of acid reached the maximum in S1 (June 3); The other seven components reached the maximum in S2 (July 3), and the results of principal component analysis also showed A. senticosus leaves collected in S2. The comprehensive scores of the eight active ingredients in leaves were the highest; The total accumulation of the eight active ingredients in different periods increased first and then decreased. During the period from S1 to S2, the total amount showed an upward trend, and reached the maximum at S2. Conclusion: According to the changes of dry weight and eight active ingredients in A. senticosus leaves in different periods, the best harvest time is around S2 stage (from late June to early July) which provides basic information for determining the suitable harvest time of A. senticosus leaves.
7.Study on challenge dose of pigeon paramyxovirus type 1 (Chuansha strain).

In order to determine the challenge dose of pigeon paramyxovirus type 1 (PPMV-1) inactivated vaccine (S-1 strain). The virus titer of PPMV-1 E5 allantoic fluid (Chuansha strain) was determined using SPF chicken embryos in this research. After inoculating 30-day-old and 120-day-old pigeons with low-HI antibody against PPMV-1 (HI antibody < or =2) with different doses of PPMV-1 (Chuansha strain), the clinical symptoms and histopathological lesions of the challenged pigeons were examined. The results showed that the minimal lethal dose (MLD) of PPMV-1 (Chuansha strain) was 102.5 ELD50, so we determined that 10(5.5) ELD50, which was 1000 times the MLD, could be taken as the challenge dose in the vaccine efficacy test for PPMV-1 inactivated vaccine (S-1 strain).

8.Value of MRS in the differential diagnosis of corpus callosum lesions

Objective To explore the value of multivoxel 1H-MRS in the differential diagnosis of corpus callosum lesions.Methods 17 patients with corpus callosum lesions confirmed by clinical or pathological method underwent MRS examination.These patients included 2 cases of lymphoma,4 cases of Marchiafava-Bignami disease,3 cases of glioma, 7 cases of infarction,and 1 case of multiple sclerosis.MRS features of lesions were analyzed.Results 2 cases with corpus callosum lymphoma revealed significantly increased Cho peak, decreased Cr and NAA peak,towering Lip peak.Among 4 cases with Marchiafava-Bignami diseases, 2 cases revealed increased Cho/Cr and decreased NAA/Cr,1 case showed normal,1 case revealed inverted Lac peak.For 3 cases with corpus callosum glioma,NAA peak decreased or disappeared,and Cho peak increased in varying degrees.7 cases with corpus callosum infarction revealed significant inverted Lac peak, and NAA peak decreased in varying degrees.1 case of multiple sclerosis revealed increased Cho peak,decreased NAA peak,and inverted Lac peak during acute period with enhancement,while these features returned normal during inactive period.Conclusion Multivoxel 1H-MRS plays an important role in the differential diagnosis of corpus callosum lesions.

10.Determination of curcumol and germacrone in Zedoary Turmeric Oil and Glucose Injections by RP-HPLC

AIM: To establish a HPLC method for determination of curcumol and germacrone in Zedoary Turmeric Oil and Glucose Injections. METHODS: RP-HPLC was applied for quantitative analysis of curcumol and germacrone. Hypersil ODS2 column (250 mm? 4.6 mm, 5 ?m) and gradient elution was used. Mobile phase A was acetonitrile and mobile phase B was water. Phase A time-percentage composition was as follows:0-3 min,45%;3-30 min,45%→65%;30-38 min, 65%; 38-45 min, 65%→90%;45-55 min,90%→100%;55-65 min,100%. The flow rate was at 1.0 mL?min -1 . Diode array detector was set at 214 nm and column temperature was at 25 ?C. RESULTS: The calibration curves of curcumol and germacrone were linear in the range of 0.26 - 6.5 ?g(r= 0.999 9 ) and 0.112 5 - 2.812 5 ?g(r=1), The average recoveries of them were 102.1% and 98.8% (n=5), respectively. CONCLUSION: Curcumol and germacrone can be determined by the method, and this will improve the quality control of Zedoary Turmeric Oil and Glucose Injections.
