1.Effect and compliance of cetirizine hydrochloride and pranoprofen eye drops on children with vernal keratoconjunctivitis

AIM: To observe the efficacy and compliance of cetirizine hydrochloride and pranoprofen eye drops on children with vernal keratoconjunctivitis.
● METHODS: A total of 63 children with vernal keratoconjunctivitis, from the outpatient department in our hospital between Jan. 2014 and Dec. 2015, were randomly divided into two groups including experimental group and control group. The cases in experimental group were treated with cetirizine hydrochloride, 2 times/d, 5 drops (0. 25ml, 2. 5mg) each time (≤6 years) or 10 drops (0. 5ml, 5mg) each time (>6 years), oral or oral with beverage and food, 1g/L pranoprofen eye drops, 4 times/d, 1 drop each time. The cases in control group were only treated with cetirizine hydrochloride, 2 times /d, 5 drops (0. 25ml, 2. 5mg) each time (≤6 years) or 10 drops (0. 5ml, 5mg) each time (>6 years), oral or oral with beverage and food for 14d. All the children were detected the scores of symptom and pathology before and after treatment. Compliance and side effect of the drug were surveyed by questionnaire.
● RESULTS: The scores of symptom, pathology and overall total after treatment were significantly lower than those before treatment in each group ( texperimental, symptom =10. 41, P < 0. 05; texperimental, pathology = 10. 05, P < 0. 05;texperimental, overal =10. 75, P<0. 05;tcontrol, symptom=8. 11, P<0. 05;tcontrol, pathology=8. 89, P<0. 05; tcontrol, overal =8. 41, P<0. 05). The scores of symptom, pathology and overall total in experimental group were lower than those in control group after the treatment (tsymptom=5. 27, P<0. 05; tpathology=3. 97, P<0. 05;toveral =4. 32, P<0. 05). The effective rate of experimental group ( 85%) was significantly higher than control group (60%) (χ2 =4. 92, P<0. 05). Most of the children ( 94%) had very good compliance. None of children had side effect such as sleepiness and thirst.
● CONCLUSION: Cetirizine hydrochloride and pranoprofen eye drops in combination may offer a safety and effective treatment on children with vernal keratoconjunctivitis and have good compliance.

The mitogen-actived protein kinase(MAPK)is one of important ways in eukaryote signal transmission network,and plays the key role in the gene expression regulation and the cytoplasm function activity.In the mammal organism,five kinds of different MAPK signal transduction passage.ERK1/2 signal transduction passage regulates cell growth and differentiation,JNK and p38 MAPK signal transduction passages play the vital role in the stress of inflammation and apoptosis etc.The activation of signal transduction passage is the typical characteristic of RA chronic synovitis,through this way to induct synoviocytes cytoplasm protein phosphorylation,then causes transcription factor and nucleoprotein c-Fos,c-Jun,AP-1 and NF-kappa B and so on phosphorylation,thus promotes cell proliferation and activation.It has the vital significance of the above-mentioned mechanism research to understand deeply and researche new medicine to RA.
5.Treatment of Level 2 Hypertension by Diagnosis and Treatment Program of Integrative Medicine: a Multi-centre, Randomized Controlled Trial.

OBJECTIVETo explore the therapeutic effect of diagnosis and treatment program of integrative medicine (IM) on level 2 hypertension in the young and middle-aged patients and their ambulatory blood pressure.
METHODSA randomized, placebo parallel and controlled, multi-center clinical trial was performed. Totally 199 young and middle-aged level 2 hypertension patients were randomly assigned to the treatment group (99 cases) and the control group (100 cases). All received combined hypotensive treatment program by taking Nifedipine Sustained Release Tablet and Hydrochlorothiazide as basic drugs. Patients in the treatment group additionally took Western medicine (WM) combined Jiangyabao serial drugs (0.31 g per tablet, 2 tablets each time, twice daily), while those in the control group additionally took WM combined simulative agents of Jiangyabao serial drugs (0.31 g per tablet, 2 tablets each time, twice daily). The treatment course was 8 weeks for all, and 24-week follow-ups performed. 24 h ambulatory blood pressure and casual blood pressure, and their efficacies were compared between the two groups, and safety assessed as well.
RESULTSCompared with before treatment in the same group, daytime and night casual blood pressure, as well as 24 h ambulatory blood pressure were all obviously improved in the two groups (P < 0.01). Average diastolic and systolic blood pressures at night decreased more in the treatment group than in the control group with statistical difference (P < 0.05). There was no statistical difference in total efficacies of daytime casual blood pressure or ambulatory blood pressure (P > 0.05).
CONCLUSIONJiangyabao serial drugs combined WM in treating young and middle-aged level 2 hypertension patients showed obvious effect in improving night blood pressure, especially for night diastolic blood pressure.

6.Risk factors of erectile dysfunction in patients with cardiovascular diseases.

OBJECTIVETo investigate the penile erectile function of hospitalized male patients with cardiovascular diseases, the incidence of erectile dysfunction (ED) in this cohort, and the relationship of ED with cardiovascular diseases and its risk factors.
METHODSUsing a self-designed questionnaire, we conducted an investigation among the hospitalized patients in the Department of Cardiovascular Diseases of the First and Second Affiliated Hospitals of Xi'an Jiaotong University. We measured their body height, body mass index (BMI), waist circumference, hip circumference, and blood pressure, obtained their personal data, past history, metabolic indexes, and erectile function scores by IIEF-5, and analyzed the risk factors of ED using univariate and multivariate logistic regression and OR analyses.
RESULTSTotally, 225 valid questionnaires were included in this investigation, which showed a 66.7% incidence of ED, 15.8% mild, 27.0% mild to moderate, 17.6% moderate, and 6.3% severe. The incident rates of ED in the 18-35 yr, 36-49 yr, 50-65 yr, and > 65 yr age groups were 13.6%, 39.1%, 89.2%, and 91.2%, respectively. Univariate logistic regression analysis manifested that the risk factors of ED in the patients with cardiovascular diseases included age (OR = 3.122, 95% CI 2.040-4.779), smoking (OR = 1.768, 95% CI 1.209-2.584), BMI (OR = 1.261, 95% CI 1.114-1.427), total cholesterol (OR = 1.77, 95% CI 1.339-2.340), TC/HDL (OR =1.715, 95% CI 1.349-2.181), hypertension (OR = 1.717, 95% CI 1.110-2.658), and coronary heart disease (OR = 2.235, 95% CI 1.169-4.275), while multivariate logistic regression analysis showed the risk factors to be age (OR = 4.99, 95% CI 2.264-10.998), financial condition, (OR = 2.804, 95% CI 1.127-6.976), smoking (OR = 2.109, 95% CI 1.179-3.772), BMI (OR = 1.414, 95% CI 1.136-1.760), and TC/HDL (OR = 2.001, 95% CI 1.016-3.943).
CONCLUSIONThe incidence of ED is high in hospitalized patients with cardiovascular diseases and rises with the increase of age. Age, smoking, financial condition, BMI, and TC/HDL are the risk factors of both ED and cardiovascular diseases, and financial condition is closely associated with ED.

7.Effect of early continuous renal replacement therapy on patients with severe sepsis

Objective To evaluate the effect of early continuous renal replacement therapy(CRRT)on patients with acute kidney injury(AKI)induced by severe sepsis. Methods A prospective study was conducted. 160 patients with AKI induced by severe sepsis admitted to intensive care unit(ICU)of Qingyuan People's Hospital between October 2009 and June 2013 were enrolled. According to the starting time of CRRT,the patients were randomly divided into two groups:an earl y treatment group and a regular treatment group(each,80 cases). All patients were treated in accordance with the principle of the cluster of severe sepsis. In early treatment group,the patents began to receive CRRT when the amount of urine output was<0.5 mL?kg-1?h-1 and had taken continuous conventional treatment over 6 hours,but the curative effect was not ideal. In the regular treatment group,the patients began to apply CRRT when the amount of urine output was<0.5 mL?kg-1?h-1 and had taken continuous conventional treatment persistently over 24 hours or difficultly corrected acidosis or heart failure developed. The changes in urine output,creatinine,saturation of blood oxygen,and albumin,and 28-day survival rate,length of ICU stay,time of organ support were measured and compared between the two groups to evaluate the therapeutic significance of the early CRRT. Results The mean length of ICU stay(day:6.5±1.7 vs. 8.2±1.6),length of organ support(day:5.3±1.8 vs. 6.0±1.4),the recovery time of urine amount(day:3.4±0.9 vs. 4.8±0.5)in early treatment group were significantly shorter than those in regular treatment group(all P<0.01);recovery ratio of urine in 1 week in early treatment group was significantly higher than that in regular treatment group(46.2%vs. 28.8%,P<0.05). The creatinine recovery ratio in 1 week in early treatment group was lower than that in regular treatment group(55.0%vs 78.8%),but there was no statistically significant difference(P>0.05). Early treatment group and regular treatment groups of creatinine recovery time were similar(day:5.1±1.3 vs. 5.2±1.2). 28-day survival rate in early treatment group was higher than that in regular treatment group(41.2%vs. 35.0%),but there was no statistically significant difference(P>0.05). Conclusion It is found that there is no evidence to suggest that early CRRT may improve the prognosis of the patients with AKI induced by severe sepsis,but it may be in favor of shortening the time of urine recovery,length of stay in ICU,length of organ support and length of dialysis.

Epicatechin content in Uncaria macrophylla Wall. was determined by RP-HPLC. As aresult, the epicatechin contents were 0.38% in the hook and stem, and 0. 820% in the leaf. Thus it seemedto be more worthwhile to produce epicatechin from the leaf rather than from the hook and stem.
